Abstract Background While the treatment guidelines have been established for pure urothelial carcinoma (pUC), patients with variant type urothelial carcinoma (vUC) face limited effective treatment options. The effectiveness of immune checkpoint inhibitors (ICI) in patients with vUC remains uncertain and necessitates additional research. Method We conducted a retrospective, multicenter study to explore the effectiveness of ICI in patients with pUC or vUC in Taiwan. We evaluated the overall response rate (ORR) through univariate logistic regression analysis and examined the overall survival (OS) and progression-free survival (PFS) using Kaplan-Meier analysis. Additionally, we employed univariate and multivariate Cox proportional hazards models to analyze the data. Result A total of 142 patients (116 pUC, 26 vUC) were included in our final analysis. The ORR was marginally higher in patients with pUC compared to those with vUC (34.5% vs. 23.1%, p = 0.26). Among all patients, 12.9% with pUC achieved a complete response (CR) after ICI treatment, while no vUC cases achieved CR (p = 0.05). There were no significant differences in PFS (median 3.6 months vs. 4.1 months, p = 0.34) or OS (median 16.3 months vs. 11.0 months, p = 0.24) when comparing patients with pUC or vUC. In the subgroup analysis, patients with pUC who underwent first-line ICI treatment exhibited significantly improved OS compared to those with vUC (24.6 months vs. 9.1 months, p = 0.004). Conclusion The use of ICI as monotherapy is a feasible and effective treatment approach for patients with metastatic vUC.

Abstract Symptoms have been used to diagnose conditions such as frailty and mental illnesses. However, the diagnostic accuracy of the numbers of symptoms has not been well studied. This study aims to use equations and simulations to demonstrate how the factors that determine symptom incidence influence symptoms’ diagnostic accuracy for disease diagnosis. Assuming a disease causing symptoms and correlated with the other disease in 10,000 simulated subjects, 40 symptoms occurred based on 3 epidemiological measures: proportions diseased, baseline symptom incidence (among those not diseased), and risk ratios. Symptoms occurred with similar correlation coefficients. The sensitivities and specificities of single symptoms for disease diagnosis were exhibited as equations using the three epidemiological measures and approximated using linear regression in simulated populations. The areas under curves (AUCs) of the receiver operating characteristic (ROC) curves was the measure to determine the diagnostic accuracy of multiple symptoms, derived by using 2 to 40 symptoms for disease diagnosis. With respect to each AUC, the best set of sensitivity and specificity, whose difference with 1 in the absolute value was maximal, was chosen. The results showed sensitivities and specificities of single symptoms for disease diagnosis were fully explained with the three epidemiological measures in simulated subjects. The AUCs increased or decreased with more symptoms used for disease diagnosis, when the risk ratios were greater or less than 1, respectively. Based on the AUCs, with risk ratios were similar to 1, symptoms did not provide diagnostic values. When risk ratios were greater or less than 1, maximal or minimal AUCs usually could be reached with less than 30 symptoms. The maximal AUCs and their best sets of sensitivities and specificities could be well approximated with the three epidemiological and interaction terms, adjusted R-squared ≥ 0.69. However, the observed overall symptom correlations, overall symptom incidence, and numbers of symptoms explained a small fraction of the AUC variances, adjusted R-squared ≤ 0.03. In conclusion, the sensitivities and specificities of single symptoms for disease diagnosis can be explained fully by the at-risk incidence and the 1 minus baseline incidence, respectively. The epidemiological measures and baseline symptom correlations can explain large fractions of the variances of the maximal AUCs and the best sets of sensitivities and specificities. These findings are important for researchers who want to assess the diagnostic accuracy of composite diagnostic criteria.

Background Coronavirus disease 2019 (COVID-19) has caused an enormous loss of life worldwide. The spike protein of the severe acute respiratory syndrome coronavirus 2 is the cause of its virulence. Bamlanivimab, a recombinant monoclonal antibody, has been used alone or in combination with etesevimab to provide passive immunity and improve clinical outcomes. A systematic review and meta-analysis was conducted to investigate the therapeutic effects of bamlanivimab with or without etesevimab (BAM/ETE) treatment. Methods Our study was registered in PROSPERO (registry number CRD42021270206). We searched the following electronic databases, without language restrictions, until January 2023: PubMed, Embase, medRxiv, and the Cochrane database. A systematic review and meta-analysis was conducted based on the search results. Results Eighteen publications with a total of 28,577 patients were identified. Non-hospitalized patients given bamlanivimab with or without etesevimab had a significantly lower risk of subsequent hospitalization (18 trials, odds ratio (OR): 0.37, 95% confidence interval (CI): [0.29–0.49], I2: 69%; p < 0.01) and mortality (15 trials, OR: 0.27, 95% CI [0.17–0.43], I2: 0%; p = 0.85). Bamlanivimab monotherapy also reduced the subsequent risk of hospitalization (16 trials, OR: 0.43, 95% CI [0.34–0.54], I2: 57%; p = 0.01) and mortality (14 trials, OR: 0.28, 95% CI [0.17–0.46], I2: 0%; p = 0.9). Adverse events from these medications were uncommon and tolerable. Conclusions In this meta-analysis, we found the use of bamlanivimab with or without etesevimab contributed to a significantly-reduced risk of subsequent hospitalization and mortality in non-hospitalized COVID-19 patients. However, resistance to monoclonal antibodies was observed in COVID-19 variants, resulting in the halting of the clinical use of BAM/ETE. Clinicians’ experiences with BAM/ETE indicate the importance of genomic surveillance. BAM/ETE may be repurposed as a potential component of a cocktail regimen in treating future COVID variants.

Bladder cancer is a common cancer with well-established therapeutic strategies. However, recurrence occurs in 50% of patients with non-muscle-invasive bladder cancer, and 20% of patients progress to muscle-invasive bladder cancer. The 5-year survival rate for muscle-invasive bladder cancer patients is disappointingly low, ranging from 36% to 48%. A molecular marker of interest is chitinase 3-like-1 (CHI3L1), which is elevated in various cancers, including bladder cancer. In addition to its role in cancer cells, CHI3L1 also has regulatory abilities in immune cells. Neutrophil infiltration has been shown to positively correlate with overall survival, progression-free survival, and relapse-free survival in bladder cancer patients. However, the relationship between CHI3L1 and neutrophils remain poorly understood. Therefore, this study investigated the relationship between CHI3L1 level and protumor neutrophil infiltration in bladder cancer. We analyzed the GSE128959 dataset and the data of a bladder cancer cohort undergoing chemotherapy. We observed higher expression of CHI3L1 in bladder cancer patients with invasive or chemotherapy-resistance. Our results revealed a positive correlation between CHI3L1 expression and protumor neutrophil infiltration. Elevated CHI3L1 expression was associated with genes which were related to the recruitment and infiltration of neutrophils. Consequently, CHI3L1 may serve as a novel evaluation factor for the degree of neutrophil infiltration in advanced bladder cancer in those scheduled for chemotherapy.

In Taiwan, COVID-19 was relatively controllable, and the reduction in routine vaccinations was not profound. The impact of the pandemic on vaccination remained unclear. We collected vaccination uptake data at our hospital and analyzed the weekly trends of different vaccines. Urothelial carcinoma (UC) is characterized by a high incidence of TP53 mutation, and overcoming resistance to cisplatin-based chemotherapy in UC is a major concern. Wee1 is a G2/M phase regulator that controls the DNA damage response to chemotherapy in TP53-mutant cancers. The combination of Wee1 blockade with cisplatin has shown synergistic efficacy in several types of cancers, but little is known regarding UC. The antitumor efficacy of the Wee1 inhibitor (AZD-1775) alone or in combination with cisplatin was evaluated in UC cell lines and a xenograft mouse model. AZD-1775 enhanced the anticancer activity of cisplatin by increasing cellular apoptosis. AZD-1775 inhibited the G2/M checkpoint, improving the sensitivity of mutant TP53 UC cells to cisplatin by enhancing the DNA damage process. We confirmed that AZD-1775 combined with cisplatin reduced tumor volume and proliferation activity and increased the markers of cell apoptosis and DNA damage in the mouse xenograft model. In summary, the Wee1 inhibitor AZD-1775 combined with cisplatin elicited a promising anticancer efficacy in UC, and constitutes an innovative and promising therapeutic strategy.

BackgroundMental illness diagnostic criteria are made based on assumptions. This pilot study aims to assess the public’s perspectives on mental illness diagnoses and these assumptions.MethodsAn anonymous survey with 30 questions was made available online in 2021. Participants were recruited via social media, and no personal information was collected. Ten questions focused on participants’ perceptions regarding mental illness diagnoses, and 20 questions related to the assumptions of mental illness diagnoses. The participants’ perspectives on these assumptions held by professionals were assessed.ResultsAmong 14 survey participants, 4 correctly answered the relationships of 6 symptom pairs (28.57%). Two participants could not correctly conduct the calculations involved in mood disorder diagnoses (14.29%). Eleven (78.57%) correctly indicated that 2 or more sets of criteria were available for single diagnoses of mental illnesses. Only 1 (7.14%) correctly answered that the associations between symptoms and diagnoses were supported by including symptoms in the diagnostic criteria of the diagnoses. Nine (64.29%) correctly answered that the diagnosis variances were not fully explained by their symptoms. The confidence of participants in the major depressive disorder diagnosis and the willingness to take medications for this diagnosis were the same (mean = 5.50, standard deviation [SD] = 2.31). However, the confidence of participants in the symptom-based diagnosis of non-solid brain tumor was significantly lower (mean = 1.62, SD = 2.33, p < 0.001).ConclusionOur study found that mental illness diagnoses are wrong from the perspectives of the public because our participants did not agree with all the assumptions professionals make about mental illness diagnoses. Only a minority of our participants obtained correct answers to the calculations involved in mental illness diagnoses. In the literature, neither patients nor the public have been engaged in formulating the diagnostic criteria of mental illnesses.

BackgroundFrailty is associated with major health outcomes. However, the relationships between frailty and frailty symptoms haven't been well studied. This study aims to show the associations between frailty and frailty symptoms.MethodsThe Health and Retirement Study (HRS) is an ongoing longitudinal biannual survey in the United States. Three of the most used frailty diagnoses, defined by the Functional Domains Model, the Burden Model, and the Biologic Syndrome Model, were reproduced according to previous studies. The associations between frailty statuses and input symptoms were assessed using odds ratios and correlation coefficients.ResultsThe sample sizes, mean ages, and frailty prevalence matched those reported in previous studies. Frailty statuses were weakly correlated with each other (coefficients = 0.19 to 0.38, p < 0.001 for all). There were 49 input symptoms identified by these three models. Frailty statuses defined by the three models were not significantly correlated with one or two symptoms defined by the same models (p > 0.05 for all). One to six symptoms defined by the other two models were not significantly correlated with each of the three frailty statuses (p > 0.05 for all). Frailty statuses were significantly correlated with their own bias variables (p < 0.05 for all).ConclusionFrailty diagnoses lack significant correlations with some of their own frailty symptoms and some of the frailty symptoms defined by the other two models. This finding raises questions like whether the frailty symptoms lacking significant correlations with frailty statuses could be included to diagnose frailty and whether frailty exists and causes frailty symptoms.

RationaleAtrial fibrillation is a critical health burden. Targeting calcium (Ca2+) dysregulation and oxidative stress are potential upstream therapeutic strategies. Fibroblast growth factor (FGF) 1 can modulate Ca2+ homeostasis and has antioxidant activity. The aim of this study was to investigate whether FGF1 has anti-arrhythmic potential through modulating Ca2+ homeostasis and antioxidant activity of pulmonary vein (PV) and left atrium (LA) myocytes.MethodsPatch clamp, western blotting, confocal microscopy, cellular and mitochondrial oxidative stress studies were performed in isolated rabbit PV and LA myocytes treated with or without FGF1 (1 and 10 ng/mL). Conventional microelectrodes were used to record electrical activity in isolated rabbit PV and LA tissue preparations with and without FGF1 (3 μg/kg, i.v.).ResultsFGF1-treated rabbits had a slower heart rate than that observed in controls. PV and LA tissues in FGF1-treated rabbits had slower beating rates and longer action potential duration than those observed in controls. Isoproterenol (1 μM)-treated PV and LA tissues in the FGF1-treated rabbits showed less changes in the increased beating rate and a lower incidence of tachypacing (20 Hz)-induced burst firing than those observed in controls. FGF1 (10 ng/mL)-treated PV and LA myocytes had less oxidative stress and Ca2+ transient than those observed in controls. Compared to controls, FGF1 (10 ng/mL) decreased INa−L in PV myocytes and lowered Ito, IKr−tail in LA myocytes. Protein kinase C (PKC)ε inhibition abolished the effects of FGF1 on the ionic currents of LA and PV myocytes.ConclusionFGF1 changes PV and LA electrophysiological characteristics possibly via modulating oxidative stress, Na+/Ca2+ homeostasis, and the PKCε pathway.

The traditional electrospinning needles are easily blocked and the processing is not suitable for mass production. In order to address the problems, this study uses a rotary linear electrode for electrospinning for the production of polyvinyl alcohol (PVA)/chitosan(CS)/graphene(Gr) nanofibrous membranes. The membranes are observed for micro-structure and tested for thermal stability, surface resistivity, and hydrophilicity, thereby examining the influence of the content of chitosan. The test results show that the presence of chitosan improves the hydrophilcity as well as affects the morphology of PVA/CS/Gr nanofibrous membranes where the nanofiber diameter is smaller. Moreover, FTIR results suggest that chitosan and PVA interact to generate hydrogen bonds that stabilize the thermal properties of nanofibrous membranes. In particular, the maximum thermal decomposition temperature of PVA/CS/Gr nanofibrous membranes composed of PVA/CS ratio being 9:1 is 297.7 ℃. Finally, using chitosan also increases the electrical conductivity and decreases the surface resistivity. This fabricated technique provides the possibility of mass-production of PVA/CS/Gr nanofibrous membranes in the future. Abstract In this article, we report a facile and simple approach for tuning graphene nanosheet structures (GNS) with different ions in the electrolytes through cathodic plasma exfoliation process in electrochemical reactions. We obtained sheet- and onion-like GNS when aqueous electrolyte NaOH and H2SO4, respectively, were present during plasma exfoliation in the electrochemical reactions, as evidenced from scanning electron microscopy and transmission electron microscopy images. Moreover, the onion-like GNS exhibited a specific surface area of 464 m2 g−1 and a supercapacitive performance of 67.1 F g−1, measured at a scan rate of 5 mV s−1 in 1 M NaCl; these values were much higher than those (72 m2 g−1 and 21.6 F g−1, respectively) of the sheet-like GNS. This new approach for efficiently generating tunable stacked graphene structures with different ions, in the cathodic plasma exfoliation process, has promising potentials for use in energy storage devices.

Under extreme low-lighting conditions, images have low contrast, low brightness, and high noise. In this paper, we propose a principal component analysis framework to enhance low-light-level images with decomposed luminance–chrominance components. A multi-scale retinex-based adaptive filter is developed for the luminance component to enhance contrast and brightness significantly. Noise is attenuated by a proposed collaborative filtering employed to both the luminance and chrominance components that reveal every finest detail by preserving the unique features in the image. To evaluate the effectiveness of the proposed algorithm, a simulation model is proposed to generate nighttime images for various levels of contrast and noise. The proposed algorithm can process a wide range of images without introducing ghosting and halo artifacts. The quantitative performance of the algorithm is measured in terms of both full-reference and blind performance metrics. It shows that the proposed method delivers state-of-the-art performance both in terms of objective criteria and visual quality compared to the existing methods.

Enterovirus infection is endemic in many areas, especially in Southeast Asia. Enterovirus infection with severe complications (EVSC) is life-threatening, and timely diagnosis and management are crucial for successful management. Here, we report on a 2-year-old boy with hand, foot, and mouth disease. Myoclonic jerks developed and left abducens nerve palsy followed. Brain magnetic resonance imaging (MRI) showed rhombencephalitis. Pulmonary edema and cardiopulmonary failure developed, and intravenous immunoglobulin and extracorporeal membrane oxygenation were administered. He had a tracheostomy with home ventilator use after 64 days of hospitalization. At a 5-year follow-up, his neurodevelopment was normal with complete recovery from the abducens nerve palsy. The progress of EVSC may be rapid and fulminant, and timely diagnosis is critical for patient prognosis and outcomes. The presence of abducens nerve palsy is an indicator of enteroviral rhombencephalitis, and immediate and appropriate management is suggested.

This study constructs a flexible gradient foaming structure by changes the maleic anhydride-grafted styrene-ethylene/butylene-styrene (SEBS-g-MAH) content on the basis of a two-step foaming technique. The effects of SEBS-g-MAH content and its ratio in the first and second foaming steps on microstructure, compression, quasi-static burst, dynamic cushion, and thermal performance of flexible polyurethane foam composites are investigated. Results show that, SEBS-g-MAH content affects microstructure, compressive stress, burst strength, dynamic cushion and thermal performance. Gradient foaming improves compressive stress and quasi-static bursting strength by 3.81 and 1.56 times, respectively. The absorbed energy under dynamic impact of gradient foaming can be increased by 87% relative to that in one-step foaming. This study provides a new way approach to SEBS application, and serves as a new reference for designing of energy-absorbing foam composites by filler addition. Glucagon-like peptide 1 receptor agonists (GLP-1RAs) and sodium-glucose cotransporter-2 inhibitors (SGLT2is) are antihyperglycemic agents with cardioprotective properties against diabetic cardiomyopathy (DCM). However, the distinctive mechanisms underlying GLP-1RAs and SGLT2is in DCM are not fully elucidated. The purpose of this study was to investigate the impacts of GLP1RAs and/or SGLT2is on myocardial energy metabolism, cardiac function, and apoptosis signaling in DCM. Biochemistry and echocardiograms were studied before and after treatment with empagliflozin (10 mg/kg/day, oral gavage), and/or liraglutide (200 μg/kg every 12 h, subcutaneously) for 4 weeks in male Wistar rats with streptozotocin (65 mg/kg intraperitoneally)-induced diabetes. Cardiac fibrosis, apoptosis, and protein expression of metabolic and inflammatory signaling molecules were evaluated by histopathology and Western blotting in ventricular cardiomyocytes of different groups. Empagliflozin and liraglutide normalized myocardial dysfunction in diabetic rats. Upregulation of phosphorylated-acetyl coenzyme A carboxylase, carnitine palmitoyltransferase 1β, cluster of differentiation 36, and peroxisome proliferator-activated receptor-gamma coactivator, and downregulation of glucose transporter 4, the ratio of phosphorylated adenosine monophosphate-activated protein kinase α2 to adenosine monophosphate-activated protein kinase α2, and the ratio of phosphorylated protein kinase B to protein kinase B in diabetic cardiomyocytes were restored by treatment with empagliflozin or liraglutide. Nucleotide-binding oligomerization domain, leucine-rich repeat and pyrin domain-containing 3, interleukin-1β, tumor necrosis factor-α, and cleaved caspase-1 were significantly downregulated in empagliflozin-treated and liraglutide-treated diabetic rats. Both empagliflozin-treated and liraglutide-treated diabetic rats exhibited attenuated myocardial fibrosis and apoptosis. Empagliflozin modulated fatty acid and glucose metabolism, while liraglutide regulated inflammation and apoptosis in DCM. The better effects of combined treatment with GLP-1RAs and SGLT2is may lead to a potential strategy targeting DCM.

BackgroundChronic kidney disease (CKD) increases the occurrence of atrial fibrillation and pulmonary vein (PV) arrhythmogenesis. Calcium dysregulation and reactive oxygen species (ROS) enhance PV arrhythmogenic activity. The purposes of this study were to investigate whether CKD modulates PV electrical activity through dysregulation of calcium homeostasis and ROS. Methods and ResultsBiochemical and electrocardiographic studies were conducted in rabbits with and without CKD (induced by 150 mg/kg per day neomycin sulfate and 500 mg/kg per day cefazolin). Confocal microscopy with fluorescence and a whole‐cell patch clamp were applied to study calcium homeostasis and electrical activities in control and CKD isolated single PV cardiomyocytes with or without treatment with H89 (1 μmol/L, a protein kinase A inhibitor) and MPG (N‐[2‐mercaptopropionyl]glycine; 100 μmol/L, a ROS scavenger). The ROS in mitochondria and cytosol were evaluated via intracellular dye fluorescence and lipid peroxidation. CKD rabbits had excessive atrial premature captures over those of control rabbits. Compared with the control, CKD PV cardiomyocytes had a faster beating rate and larger calcium transient amplitudes, sarcoplasmic reticulum calcium contents, sodium/calcium exchanger currents, and late sodium currents but smaller L‐type calcium current densities. CKD PV cardiomyocytes had a higher frequency and longer duration of calcium sparks and more ROS in the mitochondria and cytosol than did controls. Moreover, H89 suppressed all calcium sparks in CKD PV cardiomyocytes, and H89‐ and MPG‐treated CKD PV cardiomyocytes had similar calcium transients compared with control PV cardiomyocytes. ConclusionsCKD increases PV arrhythmogenesis with enhanced calcium‐handling abnormalities through activation of protein kinase A and ROS.

In this article, expanded vermiculite (EV)-enhanced polyurethane foam bionic composites inspired by pomelo peel is proposed. The columnar lattice structure mold is employed to constitute the periodic interface structure and gradient foam structure, and the nylon nonwoven fabric is combined as the surface layer. The effects of EV content on the thermal, compression, and dynamic cushion properties of bionic composites are investigated. Results show that residual char increases with EV content, which conduces to decrease the release of heat flow. The proposed bionic composite with columnar lattice structure has optimal compressive modulus, energy absorption and dynamic cushion efficacy when 1 wt% EV is added. However, its performance decreases slowly when EV fillers are continuously added because the cell morphology is changed from round to irregular shape and the interfacial adhesion of filler−matrix is weakened. Owing to their unique bionic structure, composites can absorb 99% of the energy impacted by flat impactor within a smaller deformation and achieve 97% absorption efficiency for a hemispheric impactor in cushion test.

In this study, a self-made continuous needleless dynamic linear electrospinning technique is employed to fabricate large-scale graphene (Gr)/carbon nanotubes (CNT)/polyvinyl alcohol (PVA)/sodium alginate (SA) nanofibrous membranes. The synergistic effect of Gr and CNT fillers in the PVA/SA membrane is explored in depth by changing the volume ratio (v/v) of Gr and CNT as 10:0, 8:2, 6:4, 4:6, 2:8, and 0:10. Microstructure, functional group, conductivity, and hydrophilicity of PVA/SA/Gr/CNT membranes was characterized. Results show that the linear electrode needleless electrospinning technique can be spun into 200-nm diameter fibers. The PVA/SA/Gr/CNT fibrous membrane has good hydrophilicity and thermal stability. A Gr/CN ratio of 6:4 possessed the optimal synergistic effect, which showed the lowest surface resistivity of 2.53 × 103 Ω/m2. This study will provide a reference for the large-scale preparation of nanofibrous membrane used as a artificial nerve conduit in the future.
