15. Long-term survival of a patient with amyotrophic lateral sclerosis (ALS) who received autologous adipose-derived mesenchymal stem cells.

Abstract

OBJECTIVE: Amyotrophic lateral sclerosis (ALS) is a progressive debilitating neurodegenerative disease with a life expectancy of 3-5 years from initial symptoms. We report a case of ALS who received autologous adipose-derived mesenchymal stem cells (ADSC) and was followed up for 7 years. CASE REPORT: A 46-year-old man noticed weakness of his legs, difficulties on going down the stairs and coughing during eating in 2009. After complete workout, a diagnosis of ALS was confirmed. His ALS Functional Rating Scale-R (ALSFS-R) was 43. Symptoms rapidly progressed and he coughed and choked during eating. Starting in 2013, the patient received a total of six intravenous infusions of autologous ADSC. Changes in electromyogram, nerve conduction, and ALSFS-R were assessed. RESULTS: Soon after the administration, he noticed that he did not cough during conversation or eating food. Although he had difficulty in walking down the stairs, he remained well without coughing, dysarthria, or dysphagia. His ALSFS-R increased up to 45. Fascicular potentials were not detected in any muscles examined including trapezius muscle and rectus femoris muscles. The patient was well for 7 years after ADSC therapy by the time of this report and more than 10 years from the time of onset. CONCLUSIONS: The present case suggests that autologous ADSC can be administered safely and may be potentially useful in patients with ALS. Further investigations are warranted in order for the results to be generalized to other ALS patients. [ABSTRACT FROM AUTHOR]

Abstract

The production systems of today are required to meet changing market conditions. To date, most conventional flexible systems depend on human flexibility. However, the demand for flexible automation systems is increasing owing to rising labor costs and quality requirements, even in emerging countries. To implement automation systems in the unpredictable market, cost reduction is essential to reduce the investment risk. However, there is no systematic design method for reducing the cost of automation systems. Abstract

To study a possible relationship between inhibition of axonal flow and amyloidogenesis, the authors examined amyloid precursor protein (APP) immunoreactivity in rat brain treated with colchicine. After intraventricular injection of colchicine, the proximal axons of exposed neurons became swollen and showed a large increase in APP immunoreactivity, whereas the cytoplasm and dendritic processes showed lesser increases. These changes were seen in ipsilateral neurons of the hippocampus, lateral septal nucleus, amygdala, and entorhinal, parietal and temporal cortices, as well as bilaterally in the periventricular hypothalamic nucleus. The increase of APP immunoreactivity appeared as early as 3 hours after the injection. It peaked at around 24 hours, and began to clear after about 4 days. A few strongly APP-positive dystrophic neurons remained. In serial sections at these later time periods, some strongly argentophilic neurons and Alz-50 positive neurons, each with abnormal neurities, could be demonstrated. The result suggests that APP may undergo fast axoplasmic flow in rat brain and that argentophilic changes of Alz-50 immunoproduction may follow APP accumulation caused by inhibition of axoplasmic flow.

Abstract

Using immunoperoxidase methods, 94 human adrenal tumors were examined for evidence of immunoreactivity and receptor expression of insulinlike growth factor I (IGF-I) and insulin. The frequency of IGF-I in adrenocortical carcinomas was significantly higher than that in adenomas of the adrenal glands. The adrenocortical carcinomas showed strong intensity of staining for IGF-I, IGF-I receptors, and insulin receptors. A significant correlation between immunoreactivity and receptor expression of both IGF-I and insulin was found only in the adrenocortical carcinomas. The adrenocortical adenomas with Cushing's syndrome and pheochromocytomas, more than adrenocortical adenomas with Conn's syndrome, also stained strongly for insulin receptors. Thus the IGF-I and insulin probably play a role in the growth of adrenocortical carcinoma tissues, possibly through autocrine mechanisms. The expression of insulin receptors in adrenocortical adenomas in the presence of Cushing's syndrome and pheochromocytomas may be associated with functions.
