Abstract

The objective of this study is to identify the effects of statins and risk factors for thrombosis in patients with new onset of systemic lupus erythematosus (SLE) with or without antiphospholipid antibodies (aPL). Consecutive patients with SLE without history of thrombotic events were retrospectively enrolled from April 1997 to February 2014. The development of first thrombosis and death caused by thrombosis were defined as the study endpoint. Risk and protective factors for developing thrombosis were analyzed. A total of 152 patients, 80 positive and 72 negative for aPL, were included. In aPL-positive patients, 15 developed arterial (n=6) and venous (n=9) thrombosis (median follow-up period 69 months). Cox's proportional hazards model showed that older age at SLE onset and IgG-anticardiolipin antibodies (aCL) were statistically significant risks for thrombosis. Statin therapy was identified as a statistically significant protective factor against thrombosis (hazard ratio 0.12, 95% confidence interval 0.01-0.98). In aPL-negative patients (median follow-up period 46 months), seven patients developed thrombosis (five arterial and two venous). No risk factors for thrombosis were found in this group. In aPL-positive patients with SLE, the late disease onset and the presence of IgG-aCL represented additional risk factors for thrombosis. Statin treatment appeared as a protective factor for thrombosis.

Abstract

One of the valuable methods of cardiac valve diagnosis can be performed by auscultation. We proposed a signal processing and extensive characterization method based on wavelet analysis to investigate important characteristics of heart sounds of normal and pathologic systolic murmur human subjects. Time-scale maps yielded by wavelet transform calculation were solved using magnitude thresholding operation and centre of gravity to restrict temporal and frequency-related of valvular activities. From our experimental results, temporal and frequencyrelated parameters of S1, S2, and their components coidd be characterized precisely. Application of our method was adequate to characterize the heart sounds objectively, clearly, systematically, and comprehensively. The method was considered valuable to explain mechanisms of cardiac valves functions. We expected that the method would be helpful for clinical diagnosis as well as developing of heart sound modelling and educational purpose. 16. CRM1, an RNA transporter, is a major species-specific restriction factor of human T cell leukemia virus type 1 (HTLV-1) in rat cells

Abstract

Rat ortholog of human CRM1 has been found to be responsible for the poor activity of viral Rex protein, which is essential for RNA export of human T cell leukemia virus type 1 (HTLV-1). Here, we examined the species-specific barrier of HTLV-1 by establishing rat cell lines, including both adherent and CD4+ T cells, which express human CRM1 at physiological levels. We demonstrated that expression of human CRM1 in rat cells is not harmful to cell growth and is sufficient to restore the synthesis of the viral structural proteins, Gag and Env, at levels similar to those in human cells. Gag precursor proteins were efficiently processed to the mature forms in rat cells and released into the culture medium as sedimentable viral particles. An HTLV-1 pseudovirus infection system suggested that the released virus particles are fully infectious. Our newly developed reporter cell system revealed that Env proteins produced in rat cells are fully fusogenic, which is the basis for cell–cell HTLV-1 infection. Moreover, we show that the early steps in infection, from post-entry uncoating to integration into the host chromosomes, occur efficiently in rat cells. These results, in conjunction with reports describing efficient entry of HTLV-1 into rat cells, may indicate that HTLV-1 is unique in that its major species-specific barrier is determined by CRM1 at a viral RNA export step. These observations will enable us to construct a transgenic rat model expressing human CRM1 that is sensitive to HTLV-1 infection.

Abstract

PURPOSE: To prospectively evaluate if findings on paired inspiratory and expiratory thin-section computed tomographic (CT) scans in patients with silicosis correlate with pulmonary function test results. MATERIALS AND METHODS: Institutional review board approval and patient consent were obtained. Thirty-seven men (mean age, 71 years; range, 53-88 years) with silicosis were included. All patients had undergone inspiratory and expiratory thin-section CT and spirometry. Silicotic nodules, large opacity, emphysema, reticular opacities, bronchiectasis, and air trapping were graded subjectively on CT images. Emphysema was quantified on these images with built-in software. CT numbers were correlated with spirometric findings by using Spearman rank correlation analyses. Ten healthy volunteers (three men and seven women; mean age, 58 years) served as control subjects. RESULTS: After exclusion of three patients with inadequate image quality, 34 patients (mean age, 70 years; range, 53-88 years) were enrolled in the study group. Spirometric values did not differ significantly between patients with simple (n = 20) and patients with complicated (n = 14) silicosis but were significantly lower in patients than in control subjects. CT findings included air trapping (n = 33), emphysema (n = 26), nodules (n = 32), bronchiectasis (n = 22), large opacity (n = 19), and reticulation (n = 5). The extent of both air trapping and emphysema correlated negatively with spirometric values; the air trapping score showed the strongest correlation (ratio of forced expiratory volume in 1 second to forced vital capacity [FVC]: rho = -0.632, P < .001; forced expiratory flow at 50% of the FVC: rho = -0.576, P = .001). Silicotic nodule, large opacity, and bronchiectasis scores did not correlate with obstructive functional impairments.
