Das Gesundheitssystem steht aufgrund der alternden Bevölkerung vor großen medizinischen und gesellschaftlichen Herausforderungen. Ein wesentliches Hilfsmittel für die adäquate Vorsorge und Behandlung der zunehmenden chronischen Krankheiten ist das kontinuierliche Langzeitmonitoring von relevanten Vitalparametern. Während primäre Parameter wie der Herzschlag von großer Bedeutung sind, um akute Gesundheitsänderungen zu erkennen, weist ein daraus abgeleiteter Parameter, die Herzratenvariabilität (HRV), sowohl auf chronische als auch psychische Krankheiten hin. Etablierte Messgeräte wie der Elektrokardiograph haben den essentiellen Nachteil, dass ein permanenter Körperkontakt zur untersuchten Person erforderlich ist. Dies hat nicht nur einen immens hohen Personal- und Materialaufwand zur Folge, sondern schränkt vor allem beim Langzeitmonitoring die Selbstständigkeit und den Komfort der untersuchten Person ein. Wünschenswert wäre eine berührungslose und somit belastungsfreie Lösung. Die Radartechnologie als Verfahren der Abstandsmessung hat sich in den letzten Jahren als ein vielversprechendes Verfahren präsentiert. Es gab bislang jedoch nur unzureichende Untersuchungen zur Messbarkeit der HRV mittels Radar. Eine wesentliche Herausforderung stellt hierbei die benötigte Genauigkeit bei der Detektion einzelner Herzschläge dar, welche mit der bis dato analysierten Pulswellenkomponente nur schwer zu erreichen ist. In einer Vorarbeit konnte jedoch nachgewiesen werden, dass auch Herztöne mittels Radar messbar sind, welche durch ihre markante Charakteristik eine hohe Genauigkeit bei der Segmentierung versprechen. In dieser Arbeit wird erstmalig untersucht, ob eine berührungslose Messbarkeit der HRV mittels der mit Radar aufgezeichneten Herztonsignale möglich ist. Hierfür werden zunächst anhand von detaillierten theoretischen Vorüberlegungen die Aspekte ermittelt, welche für die Durchführung dieses Verfahrens notwendig sind. Dies umfasst neben der Erforschung eines Algorithmus für die automatisierte Qualitätsbestimmung des Herztonsignals auch ein Verfahren für die präzise Segmentierung und Detektion einzelner Herzschläge sowie eine nachfolgende Artefaktkorrektur und die Bestimmung der HRV-Parameter. Um die Umsetzbarkeit auch im praktischen Umfeld nachzuweisen, wurden zusammen mit dem Universitätsklinikum Erlangen umfangreiche Studien mit zahlreichen Probanden durchgeführt. Die daraus gewonnenen Messdaten, welche in ihrem Umfang und ihrer Art einzigartig sind, wurden anschließend für die Validierung der implementierten Verfahren und Algorithmen verwendet. Mittels Ensemble Learning und Gaußprozess-Regression konnten für die Qualitätsbestimmung der Herztöne F1-Scores von über 92 Prozent sowie Korrelationen von über 95 Prozent erreicht werden. Bei der Herztonsegmentierung konnte ein mehrschichtiges bidirektionales LSTM-Netzwerk die höchsten F1-Scores von 94 bis 98 Prozent erzielen. Im letzten Schritt wurden bei der HRV-Bestimmung mit dieser Architektur geringe Abweichungen von der Referenz im einstelligen Prozentbereich realisiert. Zudem wurde dies nicht nur in einem statischen Setting nachgewiesen, sondern es konnten auch die Verläufe bei sich dynamisch und abrupt ändernden Szenarien nachgebildet werden. Die in ihrer Art erstmaligen Untersuchungen in dieser Arbeit sowie die vielversprechenden Ergebnisse weisen auf ein enormes Potential der vorgestellten Technologie hin und stellen einen substantiellen Schritt in Richtung einer verbesserten Patientenversorgung mittels eines belastungsfreien Langzeitmonitorings dar. The healthcare system is facing major medical and societal challenges due to the aging population. The primary instrument for adequate prevention and treatment of the increasing number of chronic diseases is the continuous long-term monitoring of relevant vital parameters. While primary parameters such as heart rate are of great importance to detect acute health changes, a derived parameter, the heart rate variability (HRV), indicates chronic as well as mental diseases. Established monitoring devices such as the electrocardiograph have the essential disadvantage that permanent physical contact with the person being examined is required. This not only requires an immense amount of personnel and consumables, but also restricts independence and comfort of the person being examined, especially during long-term monitoring. A contactless and thus burden-free solution would therefore be desirable. Radar technology as a method of distance measurement has presented itself as a promising solution in recent years. However, there have been only insufficient studies on the measurability of HRV using radar. The major challenge here is the required accuracy in the detection of individual heartbeats, which is difficult to achieve with the pulse wave component that was utilized up to now. However, in a preliminary work, it could be proven that heart sounds can also be measured by radar, which promise a high accuracy during segmentation due to their distinctive characteristics. In this work, it is investigated for the first time whether a contact-free measurement of HRV is possible using the heart sound signals recorded by radar. For this purpose, the aspects that are necessary for the implementation are first determined on the basis of detailed theoretical preliminary considerations. This includes the research of an algorithm for the automated quality determination of the heart sound signal as well as a method for the precise segmentation and detection of single heartbeats and a subsequent artifact correction and calculation of the HRV parameters. In order to prove the feasibility in a real-life environment, extensive studies with numerous test persons were carried out in cooperation with the university hospital Erlangen. The resulting measurement data, which is unique in its scope and nature, was then used to validate the implemented procedures and algorithms. Using ensemble learning and Gaussian process regression, F1 scores of over 92 percent and correlations of over 95 percent were achieved for the quality determination of heart sounds. For heart sound segmentation, a multilayer bidirectional LSTM network was able to achieve the highest F1 scores of 94 to 98 percent. In the last step, low deviations from the reference in the single-digit percentage range were realized with this architecture during HRV analysis. Moreover, the efficiency was not only demonstrated in a static setting, but it was also possible to replicate the trends during dynamically and abruptly changing scenarios. The first-of-its-kind investigations in this work as well as the promising results indicate an enormous potential of the presented technology and represent a substantial step towards improved patient care by means of burden-free long-term monitoring.

Radar systems allow for contactless measurements of vital signs such as heart sounds, the pulse signal, and respiration. This approach is able to tackle crucial disadvantages of state-of-the-art monitoring devices such as the need for permanent wiring and skin contact. Potential applications include the employment in a hospital environment but also in home care or passenger vehicles. This dataset consists of synchronised data which are acquired using a Six-Port-based radar system operating at 24 GHz, a digital stethoscope, an ECG, and a respiration sensor. 11 test subjects were measured in different defined scenarios and at several measurement positions such as at the carotid, the back, and several frontal positions on the thorax. Overall, around 223 minutes of data were acquired at scenarios such as breath-holding, post-exercise measurements, and while speaking. Vital parameters are key indicators for the assessment of health. Conventional methods rely on direct contact with the patients&rsquo, skin and can hence cause discomfort and reduce autonomy. This article presents a bistatic 24 GHz radar system based on an interferometric six-port architecture and features a precision of 1 &micro, m in distance measurements. Placed at a distance of 40 cm in front of the human chest, it detects vibrations containing respiratory movements, pulse waves and heart sounds. For the extraction of the respiration rate, time-domain approaches like autocorrelation, peaksearch and zero crossing rate are compared to the Fourier transform, while template matching and a hidden semi-Markov model are utilized for the detection of the heart rate from sphygmograms and heart sounds. A medical study with 30 healthy volunteers was conducted to collect 5.5 h of data, where impedance cardiogram and electrocardiogram were used as gold standard for synchronously recording respiration and heart rate, respectively. A low root mean square error for the breathing rate (0.828 BrPM) and a high overall F1 score for heartbeat detection (93.14%) could be achieved using the proposed radar system and signal processing.
