PURPOSE: To characterize the recovery of diagnostic cardiovascular procedure volumes in U.S. and non-U.S. facilities in the year following the initial COVID-19 outbreak. MATERIALS AND METHODS: The International Atomic Energy Agency (IAEA) coordinated a worldwide study called the IAEA Noninvasive Cardiology Protocols Study of COVID-19 2 (INCAPS COVID 2), collecting data from 669 facilities in 107 countries, including 93 facilities in 34 U.S. states, to determine the impact of the pandemic on diagnostic cardiovascular procedure volumes. Participants reported volumes for each diagnostic imaging modality used at their facility for March 2019 (baseline), April 2020, and April 2021. This secondary analysis of INCAPS COVID 2 evaluated differences in changes in procedure volume between U.S. and non-U.S. facilities and among U.S. regions. Factors associated with return to prepandemic volumes in the United States were also analyzed in a multivariable regression analysis. RESULTS: Reduction in procedure volumes in April 2020 compared with baseline was similar for U.S. and non-U.S. facilities (-66% vs -71%, P = .27). U.S. facilities reported greater return to baseline in April 2021 than did all non-U.S. facilities (4% vs -6%, P = .008), but there was no evidence of a difference when comparing U.S. facilities with non-U.S. high-income country (NUHIC) facilities (4% vs 0%, P = .18). U.S. regional differences in return to baseline were observed between the Midwest (11%), Northeast (9%), South (1%), and West (-7%, P = .03), but no studied factors were significant predictors of 2021 change from prepandemic baseline. HistoryA 43-year-old male patient with no known past medical history presented to the emergency department with new-onset bitemporal headache, dizziness, and bilateral lower extremity weakness for 1 day. The patient denied chest pain, shortness of breath, cough, or recent exposure to sick individuals. He was not on any medications and denied alcohol or illicit drug use. Vital signs were unremarkable. Physical examination was notable for a left-sided pronator drift and bilateral dysmetria that was more pronounced on the left. Results of routine laboratory workup, including complete blood count, metabolic panel, and high-sensitivity troponin level, were normal. An electrocardiogram revealed sinus tachycardia with a heart rate of 102 beats per minute, T-wave inversions in the inferior leads, left axis deviation, incomplete right bundle branch block, and frequent premature ventricular contractions. A radiograph of the chest was unremarkable. CT of the head without contrast enhancement demonstrated no acute intracranial abnormities. MRI of the brain without contrast enhancement revealed multiple acute infarcts involving left posterior inferior cerebellar artery distribution, right cerebellar hemisphere, right mesial temporal lobe, and right posterior limb of the internal capsule. CT angiography of the head and neck showed an occlusion of the right posterior cerebral artery near its origin, with a trace of distal flow.Given that these findings were concerning for a cardioembolic etiology of acute ischemic stroke, transesophageal echocardiography was performed. This showed mild left ventricular systolic dysfunction with an ejection fraction of 40%, mild global hypokinesis, and an additional finding also seen at subsequent cardiac CT and MRI that will be disclosed in part 2 of the case. The patient was started on systemic anticoagulation and guideline-directed medical therapy for heart failure with reduced ejection fraction. CT of the chest showed no evidence of lymphadenopathy or abnormalities in the lung parenchyma or interstitium. Coronary CT angiography was performed (Fig 1), followed by cardiac MRI (Fig 2).

41. Abstract 16341: Impact of a Resident Driven Multidisciplinary Multimodal Approach in Reducing Heart Failure Readmissions at a Community Based Teaching Hospital

- Abstract
Introduction:The Center for Medicare and Medicaid Services (CMS) defines heart failure (HF) readmissions as admissions to a hospital within 30 days of discharge. The Hospital Readmission Reduction Program under the Affordable Care Act imposes financial penalties for higher than expected risk-standardized 30-day readmission rates.Methods:Data on HF admissions for the hospital was obtained (CMS Codes 291, 292 and 293) for 2015-2019 and screened for readmissions.The following interventions were instituted from Mar 2018 - Mar 2019: (1) Establishment of a HF committee with cardiologists, chief residents, nurse managers, care coordinators, social workers and coders who implemented a Plan-Do-Study-Act (PDSA) cycle for quality improvement (2) Monthly chart review of readmitted cases by chief residents followed by peer feedback and education. (3) Resident and nursing implementation of a guideline-based HF checklist complemented by medical reconciliation by a clinical pharmacist and VNA and Telehealth home assistance (4) < 7-day clinic follow up for all HF discharges.Post-intervention readmission rates were compared to pre-intervention rates using the Chi-squared test.Results:570 patients were admitted with a diagnosis of HF from 2015-2019. The 30-day readmission rates prior to intervention were 22.1% (n=27), 21.9% (n=28) and 28.1% (n=39) for the years 2015, 2016 and 2017 respectively with a cumulative readmission rate of 24.16% (n=94).181 patients were admitted post-intervention (Mar 2018- Mar 2019) and had a 30-day readmission rate of 15.47% (n=28). The decrease in readmission rate (8.69%; 95% CI 1.5-15.1; p<0.02) was statistically significant and were lower than the CMS penalty benchmark of 21.3%.Conclusion:Implementation of our PDSA model targeting collaborated, checklist-based approach ensuring continuity care with systematic review and peer education results in lowering readmissions and promotes high-value care.

42. Abstract 13921: Coronary Graft Perforation Complicating Percutaneous Coronary Intervention With Excimer Laser Coronary Atherectomy in a Patient With Saphenous Vein Graft Occlusion

Introduction:There is limited data on coronary perforation occurring during percutaneous coronary intervention (PCI) in patients with a history of coronary artery bypass surgery (CABG) with an incidence estimated to be about 0.32-0.68%. There is even less data about such perforations occurring during PCI procedures with excimer laser coronary atherectomy (ELCA).Case:A 76-year old female with a history of coronary artery disease status post triple vessel CABG, peripheral arterial disease, hypertension, hyperlipidemia, diabetes and chronic kidney disease stage III was admitted for progressively worsening dyspnea on exertion. Peak troponin was 0.34. There was a decline in her ejection fraction on echo from 55% 4 months ago to 40%. Given her history of CAD and declining LV function, she underwent a coronary angioplasty which revealed occlusion of the saphenous vein graft (SVG). The culprit lesion was opened using ELCA and a drug eluting stent was being placed however during the procedure the area of the lesion perforated. Bleeding caused severe hypotension however the bleeding was contained, and the perforation was occluded with three bridging stents. The patient was transferred to the CCU for further care.Conclusion:Coronary graft perforation is a rare but devastating complication of PCI in patients with a history of CABG. Occlusion in an old SVG consist of diffuse plaque with thrombus, prone to distal embolization. ELCA is a safe option for predictable debulking during SVG-PCI and is quickly gaining popularity. It is known that procedure complexity increases risk of perforation in native vessels however reports of graft perforation during PCI with ELCA is not well reported which is what makes our case an important addition to the literature.

43. Abstract 13970: Thrombocytopenia Triggering Troponinemia: A Therapeutic Travesty

- Abstract
Introduction:First described by Amorosi, thrombotic thrombocytopenic purpura (TTP) is characterized by a pentad of symptoms: microangiopathic hemolytic anemia (MHA), thrombocytopenia, fever, renal failure and neurologic impairment. Cardiac symptoms are often overlooked as many patients with TTP are young and without risk factors. Only 3% of patients with TTP report chest pain. Retrospective analysis by Patschan et al identified myocardial infarction in 41% of patients. Most cases are only diagnosed at autopsy.Case:An 86-year-old female had been experiencing chest discomfort for 1 day. Transient numbness of the left arm prompted her to visit the ED. Cefuroxime had been prescribed for sinusitis 5 days ago after which she experienced bruising on her extremities. She was on amlodipine and rosuvastatin for hypertension and hyperlipidemia and was a nonsmoker. BP was 167/92 mm of Hg, hear rate 86 bpm and SpO296% on room air. She had scleral icterus, a normal cardiac exam and non-blanchable bruises on her extremities. Labs revealed- Hb: 10.4 g/dl, WBC: 5.1x103mm3, Platelet: 6x103mm3, BUN: 23 mg/dL, Cr: 0.72 mg/dL, Na: 125 mmol/L, alkaline phosphatase: 50 U/L, AST: 89 U/L, ALT: 33 U/L, bilirubin: 2.3 mg/dl and troponin: 5.19 ng/dl. EKG showed normal sinus rhythm with a 1stdegree AV block (PR interval of 212 msec) and no ST-T segment changes. She was treated for cefuroxime-induced thrombocytopenia with methylprednisone. Antiplatelet agents and anticoagulation for NSTEMI were held due to platelet count of 6x103mm3. 80 mg atorvastatin was administered. She became progressively obtunded and required intubation. CT head was negative. Peripheral smear showed schistocytes. TTP was diagnosed given the thrombocytopenia, MHA and neurological decline. Plasma exchange was planned. She went into sudden cardiac arrest and could not be resuscitated. ADAMTS13 assay revealed <3% activity, confirming TTP.Discussion:TTP is defined by <10% activity of the ADAMTS13 enzyme. Cardiac complications in TTP have been poorly outlined despite existing data attributing cardiac involvement to be a major cause of mortality. Severe thrombocytopenia may limit coronary intervention but should not preclude the use of mortality lowering drugs namely beta blockers, ACE inhibitors and statins.

Purpose: To characterize the recovery of diagnostic cardiovascular procedure volumes in U.S. and non-U.S. facilities in the year following the initial COVID-19 outbreak., Materials and Methods: The International Atomic Energy Agency (IAEA) coordinated a worldwide study called the IAEA Noninvasive Cardiology Protocols Study of COVID-19 2 (INCAPS COVID 2), collecting data from 669 facilities in 107 countries, including 93 facilities in 34 U.S. states, to determine the impact of the pandemic on diagnostic cardiovascular procedure volumes. Participants reported volumes for each diagnostic imaging modality used at their facility for March 2019 (baseline), April 2020, and April 2021. This secondary analysis of INCAPS COVID 2 evaluated differences in changes in procedure volume between U.S. and non-U.S. facilities and among U.S. regions. Factors associated with return to prepandemic volumes in the United States were also analyzed in a multivariable regression analysis., Results: Reduction in procedure volumes in April 2020 compared with baseline was similar for U.S. and non-U.S. facilities (-66% vs -71%, P = .27). U.S. facilities reported greater return to baseline in April 2021 than did all non-U.S. facilities (4% vs -6%, P = .008), but there was no evidence of a difference when comparing U.S. facilities with non-U.S. high-income country (NUHIC) facilities (4% vs 0%, P = .18). U.S. regional differences in return to baseline were observed between the Midwest (11%), Northeast (9%), South (1%), and West (-7%, P = .03), but no studied factors were significant predictors of 2021 change from prepandemic baseline., Conclusion: The reductions in cardiac testing during the early pandemic have recovered within a year to prepandemic baselines in the United States and NUHICs, while procedure volumes remain depressed in lower-income countries. 46. Anti-N-methyl-d-aspartate Receptor Encephalitis Related Sinus Node Dysfunction and the Lock-Step Phenomenon.

- Abstract
Described in 2007, anti-N-methyl-d-aspartate receptor encephalitis (ANMDARE) is a rare autoimmune limbic encephalitis affecting young adults (predominantly women of reproductive age) and is a paraneoplastic manifestation of ovarian teratoma in about half of the cases. ANMDARE is characterized by psychiatric changes, neurological changes, autonomic instability and cardiac dysrhythmias. In this report, we present a 36-year-old woman who was 16 weeks pregnant and brought to the hospital with confusion and subsequently had a seizure with Electroencephalography (EEG) demonstrated an extreme delta brush pattern consistent with ANMDARE. Patient developed sinus nodal dysfunction and was also found to have ovarian teratoma, a rather typical presentation for ANMDARE, that is considered a paraneoplastic syndrome for ovarian teratoma. In this report, we highlight the cardiac manifestation of ANMDARE, the pathophysiology associated with autonomic instability, and management strategies of this rare, and largely devastating illness.
