Results of treatment of 204 elderly and senile patients who underwent cystprostatectomy or anterior pelvic exenteration are analyzed. A comparative analysis of two groups of patients whose operation ended with the traditional drainage through the anterior abdominal wall (n = 100), and bilateral perineal drainage (n = 104) is presented. Bilateral perineal drainage after operations on the pelvic organs, accompanied by cystectomy and extended lymphadenectomy in conjunction with the restoration of the peritoneum lateral pelvic walls, improves postoperative recovery of intestinal peristalsis, promotes an earlier reduction in the intensity of pain and morbidity in the early postoperative period. Perineal installation of drains is a simple in design and safe procedure. We recommend bilateral perineal drainage after operations on the pelvic organs, accompanied by cystectomy and extended lymphadenectomy.

Organizational concepts of improvement of urological aid given to military men with erectile dysfunction. The reasons for insufficiency of urological aid rendered to military men with erectile dysfunction were established. The algorithm of its diagnostics was developed. Complete and reduced volume of urological aid with reference to the existing medical and diagnostic capacities of military medical and preventive treatment facilities has been developed. Organizational concepts of improvement of urological aid given to the patients with erectile dysfunction have been determined.

The physico-chemical characteristic of medicinal preparations used for the treatment of various bladder diseases is provided including their electrophoretic activity and the ability to penetrate into bladder tissues, e.g. during electrophoresis. The optimal parameters of intracavitary electrophoresis are considered.

The present study is devoted to improving quality of life of patients in elderly and senile age after operation of Bricker by finding the optimal method of forming ureterointestinal anastomosis. From 2007 to 2009 103 patients of elderly and senile age with diseases requiring removal of the bladder were treated in the Lenigrad Regional Oncology Centre. All the patients were made cystectomy. Patients were divided into two groups: In 1st group, the ureterointestinal anastomosis was formed a classical way "end to side" described Bricker, in the 2nd group ureterointestinal anastomosis was performed by the method of Wallace - "common area". Pathological conditions developed in patients in late postoperative period were as follows: hydronephrosis in early and later stages, obstructive pyelonephritis, frequent attacks of chronic pyelonephritis, chronic renal failure, urinary fistula. Formation of ureterointestinal anastomosis by Wallace during surgery reduces the amount of later postoperative complications. Quality of life was better after the formation of ureterointestinal anastomosis by Wallace.

The investigation was undertaken to study changes in the structure of the vitreoretinal complex in experimental chlamydial infection. Six rabbits were inoculated with Ch. pneumonia (6 eyes) and Ch. trachomatis (6 eyes) via instillations, subconjunctivally and intravitreally. Clinical and pathomorphological study was conducted during 128 days, by using immunohistochemical techniques. All modes of inoculation resulted in the development of an intraocular infectious process as uveitis, choriorenitis, and vitreitis (endophthalmitis) of varying degrees--from subclinical to severe. Immunohistochemical study revealed the pathogen in the structures of the ocular posterior segment and in venous blood in all cases. When locally inoculated, chlamydial infection becomes disseminated, by afflicting the ocular posterior segment, no matter what the mode of inoculation is applied. The clinical picture of the lesion widely varies from subclinical manifestations to a severe process.

For the last few years there were enlarged possibilities of completing of troops by military servicewomen. Coincidently with this, there were increased requirements for military servicewomen. On the base of postulate of three levels of professional health, determining effectiveness of professional activity (social-psychological, psychological and physiological) there was realized a research of morbidity and functional state of organism of military servicewomen, of relations of these indexes with effectiveness of discharging of professional responsibilities. Was marked, that military servicewomen in other equal conditions of health care achieve the required indexes by high personal adaptation potential, neuro-psychoic tolerance, adherence to collective forms of military work, adequate orientation in concrete situation, rapid elaboration of strategy of behavior and socialization. The got data permitted to make a system-structure analyze of professional health of military servicewomen and on it's base to determine directions of improvement of their medical maintenance.

The complexity of flying work and human factor significance increase the responsibility of aviation physician for health and high working capacity of the flying staff. The early diagnosis of potentially dangerous urological pathology is of particular importance because the renal and urinary tract diseases are revealed unexpectedly among the flying staff. The incidence of surgical pathology including the urologic one takes the third place in the RF Armed Forces. That's why the main directions to improve the urologic care in the Armed Forces medical service are the optimization of urologists' numerical staff, the development of new organizing-and-staff structure and the number of specialized urologic departments (centers, rooms, and wards) on the basis of central, district and garrison hospitals, the increase in urologists' professional training quality, the improvement of treatment-and-prophylactic work. It is necessary to conduct the quarterly, half-yearly and yearly analysis of urological diseases, connected with flying incidents.

Modern methods for diagnosis of chlamydial infection are compared. In order to detect the total systems' manifestations in patients with chlamydial infection, the authors suggest to extend the list of clinical materials to be collected in patients with suspected chlamydial infection and to use direct immunofluorescent test, polymerase chain reaction, and ligase chain reaction in diagnostic studies.
