Chronic prostatitis / chronic pelvic pain syndrome is a common urological disease that is the most common clinical form of chronic prostatitis. The combination in its pathogenesis of chronic inflammation of the prostate and chronic pain creates difficulties in diagnosis and treatment. This review aims to highlight modern approaches and diagnostic tools most commonly used in the practice of urology. The article presents the main clinical manifestations, modern classification and methods of research of patients with chronic prostatitis / chronic pelvic pain syndrome. Clinical and laboratory signs of certain forms of prostatitis with emphasis on the joint manifestations of chronic bacterial prostatitis and chronic prostatitis / chronic pelvic pain syndrome are considered. Diagnostic criteria, bases of laboratory and differential diagnosis of chronic prostatitis are considered. Particular attention is paid to the difficulties of laboratory diagnosis of chronic prostatitis / chronic pelvic pain syndrome, in particular, evaluation of the results of repeated microscopic and cultural examinations of the secretion of the prostate, ejaculate and post-massage urine. The actual questionnaire of patients for objectification and monitoring of their complaints is presented; the possibility of individualization of treatment according to the patient’s phenotype and the implementation of a multimodal approach to the provision of medical care for prostatitis. The peculiarities of communication with the patient and the most common scales for assessing the condition of patients as a tool for objectifying complaints are presented. The place of instrumental methods in the diagnostic algorithm for chronic prostatitis is presented.

Третій рік поспіль в межах Гамовської конференції працює Біологічна секція: «Важливість ідей Г.А. Гамова для біології 21-ого століття», організація якої викликана великою повагою до особистості вченого Георгія Антоновича Гамова, наукові інтереси якого об’єднали астрофізику, космологію та молекулярну біологію. Цього року Біологічна секція працювала в режимі on-line 17.08.2021. Роботу секції розпочали з виступу професора Тобіуса Дельбрюка (Institute of Neuroinformatics – ETH and University of Zurich, Zurich, Switzerland), сина видатного фізика, Нобелевського лауреата Макса Дельбрюка (в певний період товариша Г.А. Гамова). Професор Тобіус Дельбрюк назвав свою доповідь – "Out of this world: Recounting Max's Delbruck memories of George Gamow”. Ця доповідь, присвячена феноменальній особистості Г.А. Гамова, придала засіданню біологічної секції емоційну атмосферу наближення до великих ідей, що надали і зараз надають поштовх для розвитку молекулярної біології. Значну зацікавленість учасників секції викликала доповідь Dr. V.N. Korzun (KWS SAAT SE & Co. KGaA (м. Айнбек, Німеччина) «Applications of genetic and genomic research in cereals», що продемонструвала впровадження в селекційний процес сучасних молекулярно-генетичних технологій. З доповіддю «DNA-protein interactions as a tool of synthetic biology», що присвячена високо технологічним розробкам зі створення біосенсорів науково-виробничою фіромою Explogen LLC (EXG) (м. Львів, Україна) виступив к.б.н. Ю. Ребець. Наступна пленарна доповідь «Using the G.A. Gamow’s ideas for molecular genetic diagnostics of infectious and somatic human diseases at the current stage of medical development» була представлена білоруськими вченими, а саме професор С.А. Касцьюк розповіла про молекулярно-генетичні дослідження, що виконуються в Білоруській медичній академії післядипломної освіти. Dr. Yu. Monchak з McGill University (м. Монреаль, Канада) також представив доповідь присвячену впровадженню ДНК-технологій в діагностику патології людини ˗ «Targeted therapy, DNA sequence and the race against neoplasia». Ця доповідь викликала велику зацікавленість учасників біологічної секції. Молоді науковці Іщенко О.О., Жарікова Д.О., Роман І.І., Доля Б., Рошка Н.М., Чубик І.Ю., Попович Ю.А., Топораш М.К., Пидюра М.О. – доктори філософії з біології, кандидати наук, аспіранти, що займаються дослідженнями в галузі молекулярної біології представили дев’ять доповідей, що відбивають результати виконаних досліджень у низці провідних університетів нашої країни, а саме у Львівському національному університеті ім. І. Франка, у Одеському національному університеті імені І.І. Мечникова, у Чернівецькому національному університеті ім. Юрія Федьковича та ДУ «Інституті харчової біотехнології та Геноміки» (м. Київ). Представлені доповіді викликали жвавий інтерес, а формат on-line дозволів об’єднати у роботі секції понад 35 учасників з різних країн ˗ України, Білорусі, Швейцарії, Німеччини, Канади і Казахстану.

Extracorporeal shock wave therapy (ESWT) is a non-pharmacological, non-invasive procedure for the treatment of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). The objective: to evaluate the dynamics and relationship of clinical symptoms and ejaculate cytokine concentrations during the use of ESWT in the treatment of patients with CP/CPPS. Materials and methods. The study included 37 patients aged 18–45 years with CP/CPPS who received ESWT in the projection of the prostate and seminal vesicles. Symptoms of prostatitis were assessed by the NIH-CPSI scale, and ejaculate cytokines were assessed by ELISA before and after treatment. To analyze the results of treatment, patients with a significant reduction in the activity of prostatitis symptoms (by 6 or more points on the NIH-CPSI scale) were grouped into subgroup A. Patients with insufficient efficacy were grouped into subgroup B. Results. After treatment, 21 (57%) patients significantly reduced the intensity of prostatitis symptoms (subgroup A). Melasma is a chronic cutaneous condition that not only affects the appearance of patients but also has an immense psychosocial impact on patient quality of life. Although many treatment options are currently available, there is no single universal treatment regimen that works for all patients. Significant attention has been recently focused on the development and use of topical treatments that can lighten the dark patches associated with melasma. One such ingredient that has received growing attention in recent years is cysteamine. It works through several mechanisms to influence the melanogenesis pathway. This commentary reviews the information on this topical ingredient and offers relevant clinical insights. (

Falls in non-COVID-19-related hospital admissions during the pandemic affected the reasons for dermatology consultation and mode of consultation delivery. In order to assess the impact of the pandemic and the effects of telehealth on the inpatient dermatology service, we compared consultations completed between March 17, 2020 and October 31, 2020 with the same period of 2019. Dermatology received fewer consultations for management assistance during the pandemic, possibly due to patients with chronic dermatoses not meeting admission criteria or avoiding hospitalization. Consultations were also less likely to require laboratory work, imaging, and biopsies in 2020, potentially due to frequent consultation of benign conditions enabled by telehealth and stricter enforcement of only completing the biopsies necessary for acute inpatient management. Despite the shift toward remote consultations in 2020, the impact on diagnosis and management remained unchanged compared to 2019. Providers were less likely to document clinical improvement in 2020, potentially attributable to inferior communication regarding management recommendations or an increase in diagnoses not expected to improve. While remote consultations allowed dermatologists to provide comparable care during the pandemic, further research on clinical outcomes of remote consultations is required to maximize its benefits to patients and the healthcare system.
