The role of prophylactic central lymph node dissection (pCLND) for papillary thyroid cancer (PTC) remains contentious, and the impact of pCLND on long-term patient outcomes is unclear.A retrospective analysis of data from the Surveillance, Epidemiology, and End Results (SEER) database was performed. Patients diagnosed with PTC who did not undergo pCLND between 2004 and 2015 were included in this study, and patients with pN0 PTC who underwent CLND were included as the control group. The researchers calculated the subdistribution hazard ratio (SHR) using the Fine-Gray model and the hazard ratio (HR) using the Cox proportional hazards regression to compare Thyroid cancer-specific survival (TCSS) and overall survival (OS) of the different groups.A total of 38,205 T1-2cN0 PTC patients without pCLND were eligible for the study entry, and 24,157 patients with T1-2pN0 PTC patients who had received CLND were included as the control group. The actuarial 10-year TCSS and OS rates of patients without pCLND were 99.53% and 92.77%, respectively. Patients without pCLND had similar TCSS compared with the control group after adjusting for age, sex, race, tumor stage, multifocality, thyroid surgery, and radiation (SHR = 1.35, 95% CI: 0.95 - 1.93). However, patients without pCLND had a significantly poorer OS than the control group (HR = 1.38, 95% CI: 1.26 - 1.51).Patients without pCLND had similar TCSS compared with the control group after adjusting for confounders but had significantly poorer OS. Whether the OS disparities were attributed to pCLND or other factors still needs further study.

Obesity is one of the most serious global health problems, with an incidence that increases yearly and coincides with the development of cancer. Adipose tissue macrophages (ATMs) are particularly important in this context and contribute to linking obesity-related inflammation and tumor progression. However, the functions of ATMs on the progression of obesity-associated cancer remain unclear. In this review, we describe the origins, phenotypes, and functions of ATMs. Subsequently, we summarize the potential mechanisms on the reprogramming of ATMs in the obesity-associated microenvironment, including the direct exchange of dysfunctional metabolites, inordinate cytokines and other signaling mediators, transfer of extracellular vesicle cargo, and variations in the gut microbiota and its metabolites. A better understanding of the properties and functions of ATMs under conditions of obesity will lead to the development of new therapeutic interventions for obesity-related cancer.

Background: The aim of this study was to develop and validate nomograms to predict the survival in patients with papillary thyroid cancer (PTC). Patients and methods: Adult patients who were surgically treated for PTC were selected from the Surveillance, Epidemiology and End Results (SEER) program (2004-2013). A multivariate analysis using the Cox proportional hazards regression was performed, and nomograms for predicting 10-year overall survival (OS) and cancer-specific survival (CSS) were constructed. The discrimination and calibration plots were used to measure the accuracy of the nomograms. Results: The records of 63,219 patients with PTC were retrospectively analyzed. Nine independent factors including age, race, sex, marital status, tumor size, extrathyroidal extension, radioactive iodine, T stage, and M stage were assembled into the OS nomogram. A nomogram predicting CSS was constructed based on eight factors (age, sex, marital status, tumor size, extrathyroidal extension, T stage, N stage, and M stage). With respect to the training set, the nomograms displayed improved discrimination power compared to the TNM staging system (6th edition) in both sets. The calibration curve for the probability of survival showed agreement between the predictive nomograms and the actual observation. Conclusion: We have successfully developed prognostic nomograms to predict OS and CSS for PTC with excellent discrimination and calibration.

The etiology of granulomatous lobular mastitis (GLM) remains unknown. This study aimed to detect bacteria in GLM using Nanopore sequencing and identify the relationship between GLM andThe bacterial detection on fresh samples (including breast pus and tissue) of 50 GLM patients using nanopore sequencing and culture methods. The bacterial detection rate of participants with different stages were compared and analyzed. Formalin-fixed and paraffin-embedded (FFPE) tissues from 39 patients were performed on Gram staining to identify Gram-positive bacilli (GPB) within lipid vacuoles. Moreover, the clinicopathological characteristics of GLM patients in different bacterial subgroups were also conducted.In 50 GLM patients, the detection rate of bacteria was 78% using nanopore sequencing method, especially in the early stage of GLM (over 80%), which was significantly higher than that using culture methods (24%,Nanopore sequencing showed rapid and accurate bacteria detection over culture method in GLM patients. Purpose: Metabolic syndrome (MetS) is a risk factor for differentiated thyroid cancer (DTC). Whether MetS impacts the aggressiveness of DTC is still unclear. We carried out this study to clarify this issue.Methods: We evaluated 455 consecutive DTC patients treated with surgery. The patients were divided into three groups based on their number of MetS components: patients without any MetS components, patients with one to two MetS components, and patients with three to five MetS components. The clinical features and histological aggressiveness of DTC at the time of diagnosis were evaluated.Results: A total of 455 patients were included in this study. Eighty-five patients had three or more metabolic components and were diagnosed as having MetS. Those patients with one or two metabolic components had higher risks for bilaterality (OR = 1.90, 95% CI:1.04–3.46) and capsule invasion (OR = 1.68, 95% CI: 1.05–2.70). MetS was a risk factor for larger tumors (OR = 2.25, 95% CI: 1.25–4.06), more lymph node metastasis (OR=3.14, 95% CI: 1.68-5.87), multifocal tumor (OR = 2.38, 95% CI: 1.30–4.35), bilateral disease (OR = 3.39, 95% CI: 1.67–6.90), capsule invasion (OR = 2.26, 95% CI: 1.15–4.45), and extrathyroid invasion (OR = 4.44, 95% CI: 1.67–11.81) after correction for age, sex, and the thyroid-stimulating hormone level (TSH).Conclusions: In our hospital-based cohort study, MetS was associated with the aggressiveness of DTC. This association was still significant after being adjusted for age, sex, and TSH.

Objective It has been reported that papillary thyroid carcinoma (PTC) patients with lymph node metastasis (LNM) are largely associated with adverse outcomes. The present study aimed to assess the correlation between the number of metastatic lymph nodes (NMLNs) and clinical prognosis in patients with PTC. Methods We retrospectively reviewed the medical records of patients with PTC who underwent initial thyroid cancer surgery in Renmin Hospital of Wuhan University between 2017 and 2019. A total of 694 patients with PTC and cervical lymph node dissection as well as a total checked number of lymph nodes ≥ 5 were involved in this study. The clinicopathological characteristics of patients were compared according to NMLNs, the number of central cervical lymph nodes (CLNs) and the number of lateral lymph nodes (LLNs). Results NMLNs > 5, CLNs > 5 and LLNs > 5 were 222 (32.0%), 159 (24.3%) and 70 (10.1%) seen in the analyzed samples, respectively. Young patients, patients with larger tumor diameter, bilaterality, multifocality and gross extrathyroidal extension (ETE) were more inclined to NMLNs > 5, CLNs > 5 and LLNs > 5 (P P = 0.001; LLNs ≤ 5/5: P 10 mm (OR = 2.131), bilaterality (OR = 1.889) and tumor gross ETE (OR = 2.759) were independent predictors for high prevalence of total NMLNs > 5 (P 10 mm (OR = 2.006), and tumor gross ETE (OR = 2.520) were independent predictors for high prevalence of CLNs > 5 (P 5 (OR = 6.733) and tumor gross ETE (OR = 4.737) were independent predictors for high prevalence of LLNs > 5 (P Conclusions In conclusion, it is evident that NMLNs is related to the invasive clinicopathological features and adverse outcome of patients with PTC which should be correctly evaluated to provide an appropriate guidance for reasonable treatment and careful follow-up.

Results: The LNM and NMLNs>5 were seen in 694 (68.0%) and 222 (21.7%) cases, respectively. Young patients, patients with larger tumor diameter, bilaterality, multifocality and gross extrathyroidal extension (ETE) were more inclined to LNM and NMLNs >5 (PP5 were negatively associated with advanced tumour stage (P5: 5/222, 97.7%; P=0.002). With our improved understanding of the biological behavior of breast cancer, minimally invasive intervention is urgently needed for personalized treatment of early disease. Intraductal therapy is one such minimally invasive approach. With the help of appropriate tools, technologies using the intraductal means of entering the ducts may be used both to diagnose and treat lesions in the mammary duct system with less trauma and at the same time avoid systemic toxicity. Traditional agents such as those targeting pathways, endocrine therapy, immunotherapy, or gene therapy can be used alone or combined with other new technologies, such as nanomaterials, through the intraductal route. Additionally, relevant mammary tumor models in rodents which reflect changes in the tumor microenvironment will help deepen our understanding of their biological behavior and heterogeneity. This article reviews the current status and future prospects of intraductal therapy in breast cancer, with emphasis on ductal carcinoma in situ.

Epidemiological and animal studies indicate an association between high levels of dietary fat intake and an increased risk of breast cancer. The multifaceted role of autophagy in cancer has been revealed in previous years. However, the mechanism of this role remains unknown. In the present study, the two most common free fatty acids, palmitate acid (PA) and oleic acid (OA), were used to determine the effect on human breast cancer MDA-MB-231 cells, and the possible role of autophagy was investigated by detecting light chain 3 (LC3)-II/I. Bafliomycin A1 was used to detect autophagy flux. High palmitate acid condition-induced MDA-MB-231 cell death and invasion were mitigated by 3-methyladenine pretreatment or transfection with shRNA against autophagy protein 5. By contrast, high oleic acid condition induced MDA-MB-231 cell proliferation, migration and invasion were mitigated using rapamycin. The present results suggest that autophagy has an important role in the effects of PA and OA on breast cancer growth and metastasis in vitro.

The protein tyrosine phosphatase 1B (PTP1B) is an important regulator of metabolism. The relationship between PTP1B and tumors is quite complex. The purpose of this study is to explore the expression pattern and role of PTP1B in breast cancer. The expression of PTP1B was detected in 67 samples of breast cancer tissue by Western blot. Cell growth assay, Transwell migration assay, and Scratch motility assay were used to examine the proliferation and migration of MCF-7 with and without PTP1B. The total levels and phosphorylated levels of signal transduction and activator of transcription 3 (STAT3) and the expression of C-C motif chemokine ligand 5 (CCL5) were also examined by Western blot. PTP1B was overexpressed in over 70% of breast cancer tissues, correlating with patients with estrogen receptor (ER)-negative, progesterone receptor (PR)-negative, and human epidermal growth factor receptor 2 (HER2)-positive tumors. The data also showed that both tumor size and lymph node metastasis were significantly higher in patients with a higher level of PTP1B. The proliferation and migration of MCF-7 cells were found to be inhibited after knocking down the gene of PTP1B. Our data also showed that PTP1B could up-regulate the dephosphorylated level of STAT3, which could increase the expression of CCL5. These phenomena indicated that PTP1B may play a crucial role in the development of breast cancer.

Objective: To study the effect of the application of inflation fluid containing ropivacaine hydrochloride in endoscopic thyroidectomy on stress response and pain mediators. Methods: 128 patients receiving endoscopic thyroidectomy in our hospital between November 2015 and November 2016 were selected and randomly divided into two groups (n=64), observation group of patients received the inflation fluid containing ropivacaine hydrochloride as the inflating medium during endoscopic thyroidectomy and control group of patients received saline inflation fluid as the inflating medium during endoscopic thyroidectomy. Serum was collected before and after operation respectively to determine the levels of stress responserelated hormones and pain-related mediators. Results: Serum adrenocorticotropic hormone (ACTH), cortisol (Cor), norepinephrine (NE), epinephrine (E), renin (PRA), angiotensin-2 (ANG-2), aldosterone (ALD), dynorphin (DYN), β-endorphin (β-EP), nitric oxide (NO) and substance P (SP) levels of both groups after operation were significantly higher than those before operation (P

The neutrophil-to-lymphocyte ratio (NLR) has been reported to possess significant prognostic value in multiple types of cancer. We conducted a meta-analysis to evaluate the prognostic value of pretreatment NLR in soft tissue sarcoma (STS).A systematic literature search through April 2018 was conducted to identify studies evaluating the prognostic value of the pretreatment NLR in STS patients. The end points were overall survival (OS), disease-free survival (DFS), progression-free survival (PFS), and clinicopathological parameters. All statistical analyses were conducted with Stata 13.0.Fourteen cohorts with 2820 patients were analyzed. Elevated NLR was significantly correlated with worse OS [hazard ratio (HR): 1.59, 95% confidence interval (95% CI): 1.28-1.97, P .001] and DFS/PFS (HR = 1.28; 95% CI = 1.12-1.47; P .001). In addition, elevated NLR was highly correlated with age (≥ 65 years), tumor size (5 cm), tumor depth (deep), Grade (G3), and TNM stage (III-IV).Overall, pretreatment NLR could be an adverse prognostic biomarker for STS.

Background Breast cancer is the most common malignant tumor in females worldwide. Many differences exist in clinico-pathological characteristics of breast cancer patients between China and Western countries. This study aimed to analyze clinico-pathological characteristics of breast cancer from central China. Methods Clinico- pathological information on breast cancer from three hospitals in central China was collected and analyzed. Results From 1994 to 2012, 2,525 patients with a median age 50 years were included in this study. The 45-49-year age group and invasive ductal carcinoma not otherwise specified accounted for the highest proportions (19.1%, 480/2,525 and 81.0%, 1,982/2,446). Stages 0-I, II and III accounted for 28.0% (682/2,441), 48.4% (1,180/2,441), and 23.7% (578/2,441), respectively. Distribution of N stage showed that N0 accounted for 53.2% (1,344/2,525), and proportion of N0 rose from 51.1% (157/307) in 30-39-year age group to 64.3% (110/171) in ≥ 70-year age group, with an average increase of 2.1% in each age group. Modified radical mastectomy, radical mastectomy, breast-conserving surgery and simple mastectomy were performed for 71.8% (1,812/2,525), 18.0% (454/2,525), 5.2% (131/2,525) and 2.6% (66/2,525), respectively. Proportions of breast-conserving surgery in age ≤ 44-year group (68/132, 51.5%) and simple mastectomy in age ≥ 60-year group (57/89, 64.0%) were higher than in the other age groups. Breast cancers positive for estrogen receptor accounted for 53.0% (1,107/ 2,112). The comparisons among this study and other reports showed higher proportion of younger patients, lower proportion of breast- conserving surgery and positive estrogen receptor patients in China than western countries. Conclusions Clinico-pathological characteristics in this study demonstrated clear differences between the center of China than Western countries. Additional classification systems should be developed to guide grading of early breast cancer more accurately, especially for N0 patients. Invasive ductal carcinoma is a focus for intensive research.

Type 2 diabetes mellitus (T2DM) has contributed to advanced breast cancer development over the past decades. However, the mechanism underlying this contribution is poorly understood. In this study, we determined that high glucose enhanced proteasome activity was accompanied by enhanced proliferation, migration and invasion, as well as suppressed apoptosis, in human breast cancer MCF-7 cells. Proteasome inhibitor bortezomib (BZM) pretreatment mitigated high glucose-induced MCF-7 cell growth and invasion. Furthermore, high glucose increased protein kinase C delta (PKC?)-phosphorylation. Administration of the specific PKC? inhibitor rottlerin attenuated high glucose-stimulated cancer cell growth and invasion. In addition, PKC? inhibition by both rottlerin and PKC? shRNA significantly suppressed high glucose-induced proteasome activity. Our results suggest that PKC?-dependent ubiquitin proteasome system activation plays an important role in high glucose- induced breast cancer cell growth and metastasis.

Objective: To evaluate the relationships between lymph node ratio (LNR, the ratio of positive lymph nodes in excised axillary lymph nodes) and disease-free survival (DFS) by comparing with traditional absolute positive lymph node number (pN classification) for prediction of breast cancer (BC) progrnosis. Methods and Patients: We retrospectively reviewed patients who received comprehensive therapy in Department of Breast Surgery, Hubei Cancer Hospital, China from Jan 2002 to Dec 2006 (Group A), and Department of Breast and Thyroid Surgery, Renmin Hospital of Wuhan University, China from Jun 2008 to May 2012 (Group B). Patients were allocated to low-risk (≤0.20), intermediate-risk (> 0.20 but ≤ 0.65), high-risk (>0.65) groups by LNR. The primary endpoint was 5-DFS. Results: A total of 294 patients were included in our study. LNR was verified as a negative prognostic factor for DFS (P= 0.002 in Group A, P< 0.0001 in Group B). Since the rate of persistence to adjuvant endocrine therapy such as 5-year aromatase inhibitors (AI) would decrease over time in patients with hormone-sensitive breast cancer, it is necessary to investigate if a patient support program could modify patients’ beliefs and improve their persistence to AI treatment. This was a prospective, multicenter, controlled, observational study to evaluate the efficacy of a patient support program in improving postmenopausal patients’ persistence to adjuvant AI medication for early stage breast cancer (NCT00769080). The primary objective was to compare the rates of 1-year persistence to upfront adjuvant AI for patients in the two observational arms (standard treatment group and standard treatment plus patient support program group). In this study, 262 patients were enrolled in the standard treatment group and 241 patients in the standard treatment plus patient support program group. The mean 1-year persistence rates were 95.9 and 95.8 % for the standard treatment group and the standard treatment plus patient support program group, respectively (P = 0.95). The mean times to treatment discontinuation were 231.2 days in the standard treatment group and 227.8 days in the standard treatment plus patient support program group, with no statistically significant difference between the two groups (P = 0.96). There was also no statistically significant difference in the reason for treatment discontinuation (P = 0.32). There was a significant relationship between the patient centered care questionnaire and poor persistence (odds ratio = 3.9; 95 % CI, 1.1–13.7; P = 0.035), suggesting that the persistence rate of patients with whom the doctor always or usually spends time is greater than that of patients with whom the doctor sometimes or never spends time. Patients’ persistence to adjuvant AI medication for postmenopausal, early stage breast cancer is relatively high in the first year and is not significantly increased by adding a patient support program to standard treatment.

To investigate the effect of suppression of CCL5 ligand gene on the proliferation of human breast cancer cells.A lentiviral vector carrying a short interfering RNA (siRNA) targeting CCL5 was transfected into human breast cancer cell line MCF-7 and MDA-MB-231 cells. The expression of CCL5 mRNA in the cells was detected by real-time PCR. The proliferation of MCF-7 and MDA-MB-231 cells was assessed by MTT assay and FACS assay, and the colony formation ability of both cell lines were measured, respectively.Real time PCR showed a good knockdown effect of CCL5 in both cell-lines. Colony-forming assay showed that the ability of colony formation of MCF-7/CCL5-siRNA and MDA-MB-231/CCL5-siRNA was decreased markedly. The colony number of MCF-7/CCL5-siRNA group was (0.34 ± 0.08), significantly lower than 0.81 ± 0.12 in the MCF-7/CCL5-N group and 0.92 ± 0.12 in the MCF-7 group (P0.05). The colony number of MDA-MB-231/CCL5-siRNA group was 0.33 ± 0.10, significantly lower than 0.97 ± 0.09 in the MDA-MB-231/CCL5-N group and 1.04 ± 0.07 in the MDA-MB-231 group (P0.05). However, MTT assay revealed that the proliferation of MCF-7/CCL5-siRNA cells was not significantly different from that of MCF-7/CCL5-N or MCF-7 cells, respectively (P0.05), and the same result was found in MDA-MB-231 cells. FACS assay showed that the proliferation index (PI) of groups MCF-7/CCL5-siRNA, MCF-7/CCL5-N and MCF-7 were 0.48 ± 0.03, 0.43 ± 0.01 and 0.45 ± 0.02. The PI of groups MDA-MB-231/CCL5-siRNA, MDA-MB-231/CCL5-N and MDA-MB-231 cells were 0.48 ± 0.02, 0.44 ± 0.05 and 0.47 ± 0.02. There was no statistical difference among them (P0.05).The down-regulation of CCL5 gene in human breast cancer cells may significantly suppress their colony formation ability, rather than affecting their population doubling time to some extent.

In this paper, a thermal tomography method, which can deduce the temperature distribution of the inner heat source from the surface thermal map, is applied to the clinical breast diagnosis. With the comparison to the conventional mammography and ultrasound imaging, the clinical value of thermal tomography on breast diagnosis is discussed. Moreover, with the help of the infrared thermal imaging, the different rules of the temperature response on normal or cancerous mammary tissue by activating the sympathetic nervous system with a cold stimulus on hands is studied preliminarily.

In this paper, a new approach of analyzing and concluding the information of inner heat source in the human tissue is proposed. By using the infrared technology, the breast surface temperature distribution can be got. With the temperature distribution the inner heat source can be deduced. This information may help Doctors make the clinical diagnosis.

To evaluate the efficacy and safety of capecitabine as first-line therapy in patients with advanced and recurrent colorectal cancer.From December 2000 to November 2001, sixty patients with advanced and recurrent colorectal cancer received first-line capecitabine treatment given at a dose of 1250 mg/m(2) twice daily, on days 1 - 14 every 21 days. At least 2 cycles were administered.The overall response rate was 23.3% with 14 PR, 24 SD (40.0%) and 15 PD. The median survival time was 14.7 months. The survival rate was 63.9% at 12-months and 33.4% at 24-months. Grade III-IV adverse effects were diarrhea in 4 patients (6.6%), anemia in 2 (3.3%) and hand-foot syndrome (HFS) in 1 (1.7%); Grade I-II adverse effects were hyperpigmentation in 20 (33.3%), HFS in 18 (30.0%) and diarrhea in 10 (16.7%).Capecitabine is an efficacious and better-tolerated alternative treatment for the patients with advanced and recurrent colorectal cancer.

The invasive behaviors of cancer nests are the main reflects of invasive ductal carcinoma(IDC)'s malignant biological behavior.In present work,the authors adopted the image processing method to extract mathematical parameters of IDC cancer nests which were specifically stained by immunohis to chemical method.The relationship between cancer nests' parameters and patients' disease free survival was studied.There were 8 mathematical parameters extracted.Multivariate analysis showed that total perimeter(P=0.043) and circularity(P=0.015) of cancer nests were independent impact factors of patients' disease free survival.The hazard risk ratios of total perimeter and circularity were 1.518(95% CI [1.013-2.273]) and 1.682(95% CI [1.107-2.555]) respectively,which were higher than that of lymph node staging(1.361,95% CI [1.085-1.706]) and lower than that of histological grading(4.685,95% CI [2.865-7.660]).Results of this study indicated that analysis on mathematical parameters of cancer nest is helpful to predict the prognostic information of early stage breast cancer patients without node metastasis.
