6. Labour Analgesia for Obstetricians: Epidurals and Beyond

Abstract

Epidurals remain the gold standard for labour analgesia, especially during the current COVID-19 pandemic. They provide superior analgesia and lower the physiological and chemical responses to labour stress. However, they are associated with higher rates of instrumental delivery, maternal pyrexia and hypotension. Less invasive substitutes include nitrous oxide and parenteral opioids such as patient-controlled analgesia (PCA) with remifentanil. Non-pharmacological methods include hypobirthing and relaxation techniques, giving the mother a less medicalised labour experience. Newer regional techniques such as erector spinae and quadratus lumborum blocks are promising alternatives in those who cannot have or do not want an epidural. This review will outline the indications and contraindications to labour epidural use, and potential alternatives to an epidural.

Abstract

Abstract Introduction The Australia & New Zealand Academy for Eating Disorders (ANZAED) recently developed general principles and clinical practice standards recommended for mental health clinicians and dietitians providing treatment for people with eating disorders. Separate mental health practice and training standards were then devised as a foundation for strengthening the workforce and providing guidance to professional training programs and service providers on the minimal standards required for practice in the eating disorder field. Recommendations The present recommendations for mental health professionals providing eating disorder treatment describe the following practice and training standards: eating disorder treatment foundations (including co-ordination of services, establishing a positive therapeutic alliance, professional responsibility and knowledge of levels of care), assessment, diagnosis, intervention (including evidence-based intervention, managing psychiatric risk and managing co-morbid mental health problems), and monitoring and evaluation. Conclusions Further work is required to disseminate these standards to clinicians providing services across Australia to people with eating disorders, and to support adherence in the clinic room where they can translate to improved outcomes for clients. Pathways to supporting adherence include expert supervision of practice, incorporation in training and supervised practice in university settings, and support with checklists that can be used by consumers and referring professionals.

Abstract

This paper describes how the Saturn V F-1 engine could be resurrected for potential use in a new heavy lift launch vehicle (HLLV). Performance, history, and reliability of the F-1 will be discussed and a 1990s F-1 engine configuration presented. The status of residual hardware, production capability, test-stand availability, and a general program plan to accomplish this restart will be presented. Similarities between restarting the F-1 program and restarting the Atlas and Delta engine programs will be discussed.

Abstract

The optical properties of undoped and modulation-doped A1GaN/GaN single heterostructures (SHs) grown by metalorganic chemical vapor deposition are investigated at low temperature using photoluminescence measurements. The formation of a two-dimensional electron gas at the heterojunction is verified by temperature-dependent Hall mobility and 300 K capacitance-voltage measurements. Radiative recombination is observed between the electrons in two-dimensional quantum states at the heterointerface and the holes in the flat-band region or bound to residual acceptors both in undoped and modulation-doped A1GaN/GaN SHs. These peaks disappear when the top A1GaN layer is removed by reactive ion etching. In addition, the photoluminescence results under different laser excitation intensity and lattice temperature are also described for undoped and modulation-doped A1GaN/GaN SHs with various A1 compositions and growth interrupt times. c 2001 American Institute of Physics. Abstract

Genes associated with increased susceptibility to multiple sclerosis (MS) have been identified, but their functions are incompletely understood. One of these genes codes for the RNA helicase DExD/H-Box Polypeptide 39B (DDX39B), which shows genetic and functional epistasis with interleukin-7 receptor-α gene (IL7R) in MS-risk. Based on evolutionary and functional arguments, we postulated that DDX39B enhances immune tolerance thereby decreasing MS risk. Consistent with such a role we show that DDX39B controls the expression of many MS susceptibility genes and important immune-related genes. Among these we identified Forkhead Box P3 (FOXP3), which codes for the master transcriptional factor in CD4+/CD25+ T regulatory cells. DDX39B knockdown led to loss of immune-regulatory and gain of immune-effector expression signatures. Splicing of FOXP3 introns, which belong to a previously unrecognized type of introns with C-rich polypyrimidine tracts, was exquisitely sensitive to DDX39B levels. Given the importance of FOXP3 in autoimmunity, this work cements DDX39B as an important guardian of immune tolerance.

31. Psychosocial outcomes of a non-dieting based positive body image community program for overweight adults: A pilot study.

Abstract

Background: The limited success of traditional diet focused obesity interventions has led to the development of alternative non-dieting approaches. The current study evaluated the impact of a community based non-dieting positive body image program for overweight/obese people on a range of psychosocial outcomes. The characteristics of this real-world sample presenting for a non-dieting weight management intervention are also described. Method(s): Overweight and obese participants enrolled in the eight week 'No More Diets' (NMD) group program completed self-report questionnaires assessing disordered eating thoughts and behaviours, body image, motivation for exercise and psychopathology pre- and post-treatment. Result(s): Participants (n = 17; 16 female) were aged between 19 and 78 years, with a BMI ranging from 25.2 kg/m2 (Overweight) to 55.9 kg/m2 (Severely Obese). They reported elevated levels of eating disorder pathology, body shape preoccupation, depression, anxiety and stress compared to community norms (p < .05). Following treatment there were significant improvements in reported body shape preoccupation, shape concern and eating attitudes (p < .05), and clinically significant changes (small to medium effect sizes; 0.3-0.35) for improvements in reported weight concern, eating competence, stress and health evaluation. There were no changes in reported dietary restraint, emotional eating and uncontrolled eating, or eating concern (p > .05). Conclusion(s): Individuals presenting for the NMD program demonstrated increased eating disorder pathology and more generalised psychopathology compared to community norms. The NMD program was particularly beneficial for body image and shape concern. Addressing these body image factors may help to address some of the perpetuating factors of obesity and disordered eating, which are often not addressed in the traditional diet-based weight loss interventions.Copyright © 2013 Bloom et al.; licensee BioMed Central Ltd.

Abstract

Vehicles capable of lifting very large and heavy payloads into low earth orbit (LEO) will be needed to support certain large-scale space missions being considered for the late 1990-2000 time period. These missions include the support of strategic national commitments, large space power systems, and interplanetary and lunar exploration. Nearer term missions will utilize the Space Shuttle and its direct derivatives, the Shuttle-Derived Vehicles (SDV), currently being defined under NASA and DOD sponsored studies. A conceptual design of a Heavy Lift Launch Vehicle (HLLV) that will lift a half million pounds to low earth orbit is presented. Design alternatives and key system design problems and issues are discussed. The new developments required, such as a new large liquid booster engine and Space Shuttle Main Engine (SSME) modifications, are identified. New candidate launch locations and launch facility requirements are also discussed. The commonality and utilization of the booster stage 'elements' as a liquid booster for a 'growth' Space Shuttle are conceptually defined.

Abstract

This paper identifies some of the advanced transportation systems and their associated propulsion systems being considered by MSFC for near-term missions (1980-1990), future missions (1990-2000), and far-term missions (post 2000). The near-term launch-vehicle considerations center around the growth Shuttle and the Shuttle-derived Heavy-Lift Launch Vehicle (HLLV) systems. The future and far-term considerations support the development of larger more advanced transportation systems. In such cases, the changing nature of the propulsion requirements needed by the launch vehicle are identified. The evolvement of chemical propulsion launch vehicles into the far future is prognosticated, and where applicable from a launch vehicle or propulsion viewpoint, orbit transfer vehicles are discussed.
