FIX replacement therapy using plasma-derived protein or recombinant protein has significantly reduced bleeding and disability from hemophilia B, particularly when used in a prophylactic fashion. Although modern factor replacement has excellent efficacy and safety, barriers to the broader use of prophylaxis remain, including the need for intravenous (IV) access, frequent dosing, variability in individual pharmacokinetics, and cost. To overcome the requirement for frequent factor dosing, novel forms of recombinant FIX have been developed that possess extended terminal half-lives. Two of these products (FIXFc and rIX-FP) represent fusion proteins with the immunoglobulin G1 (IgG1) Fc domain and albumin, respectively, resulting in proteins that are recycled in vivo by the neonatal Fc receptor. The third product has undergone site-specific PEGylation on the activation peptide of FIX, similarly resulting in a long-lived FIX form. Clinical trials in previously treated hemophilia B patients have demonstrated excellent efficacy and confirmed less-frequent dosing requirements for the extended half-life forms. However, gaps in knowledge remain with regard to the risk of inhibitor formation and allergic reactions in previously untreated patient populations, safety in elderly patients with hemophilia, effects on in vivo FIX distribution, and cost-effectiveness. Additional strategies designed to rebalance hemostasis in hemophilia patients include monoclonal-antibody-mediated inhibition of tissue factor pathway inhibitor activity and siRNA-mediated reduction in antithrombin expression by the liver. Both of these approaches are long acting and potentially involve subcutaneous administration of the drug. In this review, we will discuss the biology of FIX, the evolution of FIX replacement therapy, the emerging FIX products possessing extended half-lives, and novel “rebalancing” approaches to hemophilia therapy. Central nervous system Tumors, including metastasis, are a considerable source of morbidity and mortality. Currently, treatment options such as surgery, radiotherapy, and chemotherapy have been introduced to prevent the progression of the disease, but still, these patients do not have a good prognosis. Stereotactic radiosurgery (SRS) reduces the damage to the surroundings by focusing the radiation on the tumor tissue. In this paper, we aim to investigate the outcomes of SRS on patients with gastrointestinal-originated brain metastases. A systematic review and meta-analysis used the PRISMA guideline from inception until 27th March 2024, utilizing the relevant key terms. Records were screened and included based on pre-defined inclusion and exclusion criteria. Demanding data was extracted and analyzed using STATA v. 17. This meta-analysis of 29 studies examining SRS for brain metastases from gastrointestinal cancers revealed several significant findings. The pooled distant intracranial disease rate was 33% (95% CI: 0.21-0.45). Local tumor control rates were high, with an overall pooled rate of 88% (95% CI: 0.83-0.92). Survival outcomes showed a 6-month overall survival (OS) rate of 47% (95% CI: 0.42-0.52), decreasing to 32% at one year and 11% at two years. The 5-year OS rate was 2% (95% CI: 0.01-0.03). Subgroup analyses revealed variations in outcomes based on primary tumor site, with gastric cancer patients showing better short-term survival (73% at six months) compared to hepatic primaries (31% at six months). The 6-month progression-free survival (PFS) rate was 67% (95% CI: 0.12-1.22). Tumor control outcomes showed complete regression in 11% of cases, partial regression in 44%, stable disease in 30%, and progression in 20%. The overall mortality rate was 84% (95% CI: 0.75-0.93). This meta-analysis supports the efficacy of SRS in managing brain metastases from gastrointestinal cancers. SRS offers effective local control and may improve quality of life despite poor long-term outcomes. The high rates of distant intracranial progression underscore the need for comprehensive management strategies addressing both local and systemic disease. Background and Objectives: This study assessed the efficacy and safety of stereotactic radiosurgery (SRS) in comparison with watchful waiting for managing Koos grade II vestibular schwannomas (VS)., Methods: A retrospective, multicentric analysis was conducted, focusing on patients with Koos grade II VS who either received SRS (SRS group) or were observed (observation group). To ensure comparability between groups, propensity score matching was used, including factors such as demographic characteristics, tumor dimensions, and hearing assessments. The primary end points examined were tumor control, maintenance of serviceable hearing, and neurological outcomes., Results: A total of 92 patients were equally matched across both cohorts, with a median follow-up of 37 months for the SRS group and 27.5 months for those observed. The SRS cohort exhibited superior tumor control over observation across 3, 5, and 8 years, achieving a 100% control rate vs 47.9%, 40.1%, and 34.3% for the observation group at these time intervals, respectively (P < .001). Serviceable hearing preservation rates were comparable between the 2 groups throughout 3, 5, and 7 years (72.9% for SRS vs 65.4% for observation at 3 years; P = .86). Moreover, SRS management correlated with a lower incidence of vestibular symptoms (odds ratio = 0.11, P = .002), with no significant disparity in the deterioration of cranial nerve (CN) V or CN VII functions. Background: Regarding the newly diagnosed vestibular schwannomas (VSs), active surveillance, microsurgical resection (MS), and stereotactic radiosurgery (SRS) are the leading treatment options. Although SRS is an effective intervention with a low incidence of complications, failure may occur occasionally. Several options, including repeat SRS, are considered salvage treatment after failure of the SRS. In this systematic review and meta-analysis study, we aimed to evaluate the efficacy and outcomes of repeat stereotactic radiosurgery (SRS) in progressive VS following the failure of the initial SRS., Method: The electronic databases of PubMed/Medline, Scopus, Embase, and Web of Science (WOS) were searched from inception to August 23rd, 2024. Studies that evaluated the role of repeat SRS in the setting of VS were included. The risk of bias was assessed using the Risk of Bias in Non-Randomized Studies of Interventions tool. The R program performed the meta-analyses, sensitivity analysis, publication bias, and meta-regression., Results: A total of 11 studies encompassing 260 VS patients with repeat SRS were included in our study. The median time interval between initial and repeat SRSs ranged from tumor volume ranged from 43 to 62 months. Our analysis revealed a pooled tumor control of 91 % (95 % CI: 86 %-94 %). Regarding the radiological response, the pooled regression rate was 59 % (95 % CI: 52 %- 65 %), while the pooled progression rate was 9 % (95 % CI: 6 %- 14 %). Regarding the clinical outcomes, the pooled serviceable hearing preservation (SHP) rate was 36 % (95 % CI: 22 %-53 %), while worsened fifth cranial nerve (CN) and seventh CN rates were 12 % (95 % CI: 7 %- 19 %) and 8 % (95 % CI: 5 %- 12 %), respectively. Background: Hemangioblastomas (HBs) are rare, benign central nervous system (CNS) neoplasms that rarely occur in the supratentorial. Resection with the goal of gross total resection (GTR) is often considered the primary treatment. Stereotactic radiosurgery (SRS) has been utilized more commonly in unresectable or partially resected cases. In this study, we aimed to evaluate SRS's effectiveness and clinical outcomes in supratentorial HBs., Methods: A retrospective analysis of multi-centers from 1993 to 2022 was conducted on patients with supratentorial HB treated with SRS., Results: The study included 13 patients with 30 supratentorial HBs (24 von Hippel-Lindau (VHL), 6 sporadic). Median age was 26 years. Most tumors were solid (86.2 %). SRS was primary treatment in 86.7 % of cases and adjuvant in 13.3 %. VHL lesions were significantly smaller than sporadic ones (0.2 vs. 3.7 cc, p = 0.009). Median margin dose was higher in VHL cases (17 vs. 13.0 Gy, P = 0.031). For VHL cases, local control (LC) was 100 % at 6 months, 96 % (95 % CI: 88 %, 100 %) from 12 to 60 months post-SRS. For sporadic cases, LC was 100 % at 6 months, 80 % (95 % CI: 52 %, 100 %) from 12 to 60 months post-SRS (p = 0.39). No adverse radiation events or mortality occurred., Conclusion: SRS demonstrated a promising role in the clinical course of supratentorial HBs. Introduction: The radio-surgical literature increasingly uses biological effective dose (BED) as a replacement for absorbed dose to analyze outcome of stereotactic radiosurgery (SRS). There are as yet no studies which specifically investigate the association of BED to local tumor control in para-sellar meningioma., Methods: we did a retrospective analysis of patients underwent stereotactic radiosurgery (SRS) for para-sellar meningioma during the period of 1995-2022. Demographic, clinical, SRS parameters, and outcome data were collected. The target margin BED with and without a model for sub-lethal repair was calculated, as well as a ratio of BED at the target margin to the absorbed dose at the target margin. Factors related to local control were further analyzed., Results: The study was comprised of 91 patients, 20 (22.0%) and 71 (78.0%) of whom were male and female, respectively. The median age was 55.0 (interquartile range Q1, Q3:47.5,65.5years). 34 (37%) patients had a resection of their meningioma prior to SRS. The median interval from SRS to last clinical follow up or progression was 89 months. 13 (14.3%) patients were found to have progression. 3-, 5- and 10-years local tumor control were 98%, 92% and 77%, respectively. In cox univariate analysis, the following factors were significant: Number of prior surgical resections (Hazard Ratio [HR] = 1.82, 95% CI = 1.08-3.05, p = 0.024), BED (HR = 0.96, 95% CI = 0.92-1.00, p = 0.03), and BED/margin (HR = 0.44, 95% CI = 0.21-0.92, p = 0.028). A BED threshold above 68 Gy was associated significantly with tumor control (P = 0.04)., Conclusion: BED and BED /margin absorbed dose ratio can be predictors of local control after SRS in parasellar meningioma. Purpose: The Journal of Neuro-Oncology (JNO), established in 1983, plays a key role in publishing research on brain and spinal cord tumors. This study examines JNO's publication trends, focusing on country and gender representation to highlight its global impact., Methods: Statistical analyses were conducted using R. Gender of the first authors was predicted using a gender-guesser, and author affiliations were used to determine publication countries. We introduced a novel Country-Related Diversity (CRD) index to assess the JNO's representativeness, comparing a country's JNO publications to its overall neurosurgical output. An index value of 1 indicates proportional representation., Results: The JNO corpus, spanning from 1983 to 2024, comprises 8,154 documents with an average document age of 14.4 years. The average citation count per document is 28.71, with a rate of 2.16 citations per document per year. JNO's scientific output has grown significantly, peaking at 397 articles in 2011, with a long-term annual growth rate of 3.7%. The keyword analysis highlights "glioblastoma" as the most frequent term, reflecting the journal's neuro-oncological focus. Geographically, the U.S. led with 2,535 articles (40.1%), followed by China and Germany. International collaboration rose steadily, with multi-country publications increasing from 4.76% in 1983 to 20.98% in 2024. Analyzing contributions from different countries showed a converging CRD index toward 1 (P < 0.01), with U.S. and non-U.S. countries trending similarly. Upper-middle-income countries displayed fluctuating CRD patterns, whereas lower-middle-income countries lagged behind. Authorship analysis showed an increasing trend in co-authorship (P < 0.01), with the average number of authors per paper reaching 10.4 by 2024. Gender representation revealed a growing number of female first and senior authors, although males still dominate. By 2024, 32.9% of first authors and 21.6% of senior authors were female, signaling a gradual trend toward gender parity (P < 0.01)., Conclusions: The CRD index offers a standardized measure of country-specific research representation in the JNO. The convergence towards 1 reflects balanced international representation. Brainstem cerebral cavernous malformations (CCM) are clinically more aggressive compared to superficial CCMs. Due to their location, resection can be challenging, making stereotactic radiosurgery (SRS) an attractive alternative for symptomatic patient. Brainstem CCM patients (n = 170) were treated with Gamma Knife SRS at 11 radiosurgical centers. Hemorrhagic risk reduction, risk factors of post-SRS hemorrhage, and clinical outcomes were retrospectively analyzed. Most patients had a single (165/170 patients) brainstem CCMs treated; the majority of CCMs (165/181) presented with bleeding. Single-session SRS decreased the risk of repeat hemorrhage in patients with hemorrhagic brainstem CCM (HR: 0.17, p < 0.001) using recurrent multivariate analysis. The annual hemorrhage rate decreased from 14.8 per 100 CCM-years before SRS to 2.3 after treatment. Using univariate Cox-analysis, the probability of a new hemorrhages after SRS was reduced for patient older than 35 years (HR = 0.21, p = 0.002) and increased with a margin dose > 13 Gy (HR = 2.57, p = 0.044). Adverse radiation effect (ARE) occurred in 9 patients (5.3%) and was symptomatic in four (2.4%). At a median follow-up of 3.4 years (Inter-quartile range: 5.4), 13 patients (8.0%) had a worsened clinical status, with the treated CCM being the cause in 5.6% (10) of the patients. Single-session SRS decreased the risk of repeat hemorrhage in patients with hemorrhagic brainstem CCM and conveyed this benefit with a low risk of advrse radiation effects (ARE) and worsening clinical status., (© 2024. Background and Objectives: Better local control but higher rates of adverse radiation events (ARE) have been reported when combining American Society for Radiation Oncology (ASTRO)-guideline-suggested dose (SD) stereotactic radiosurgery (SRS) with immunotherapy or targeted therapy for melanoma brain metastases. The objective of this study is to explore the efficacy and safety of lower prescription doses compared with ASTRO guidelines for single-fraction SRS for patients with melanoma metastases who are concurrently receiving immunotherapy or targeted therapy., Methods: We conducted a retrospective, single-center study on 194 patients who underwent SRS between 2009 and 2022. After propensity score matching, 71 patients with 292 metastases were included in the ASTRO-SD (20-24 Gy for <2 cm, 18 Gy for ≥2 to <3 cm) group and 33 patients with 292 metastases in the reduced dose (RD, <20 Gy for <2 cm, <18 Gy for ≥2 to <3 cm) group., Results: The median diameter (5.4 vs 5.2 mm, P = .6), prescription volume (0.2 vs 0.2 cm3, P = .2), and radiographic follow-up (11 vs 12 months, P = .2) were similar in the 2 groups. The cumulative incidence of progressing metastases was significantly higher in the SD compared with the RD group (P = .018). Higher prescription volumes and ASTRO-suggested radiation doses were associated with local progression in multivariable analysis. Radiographic AREs were significantly more common in the SD compared with the RD group (8.6% vs 3.1%, P = .005). BRAF and other tyrosine kinase inhibitors' concurrent use, higher prescription volumes, and ASTRO-suggested radiation doses were associated with an increased risk of radiographic ARE., Conclusion: This study provides evidence that RD SRS could offer reduced toxicity rates, while maintaining high local control as compared with the current guideline-SDs for the treatment of melanoma brain metastases., (Copyright © Congress of Neurological Surgeons 2024. Objective: The International Stereotactic Radiosurgery Society aims to establish evidence-based guidelines for single-fraction stereotactic radiosurgery (SRS) in treating intracranial cavernous malformations., Methods: We conducted a systematic review following Preferred Reporting Items for Systematic Reviews and Meta-Analyses and Meta-analysis of Observational Studies in Epidemiology guidelines, searching electronic databases up to January 2024 to assess SRS's impact on post-treatment hemorrhage rates. Pooled risk ratios (RRs) and confidence intervals were used to quantify this effect, along with assessments of lesion volume changes, seizure outcomes, and SRS-related adverse effects., Results: Our meta-analysis included 32 studies with 2672 patients. A significant decrease in annual hemorrhage rates was observed post-treatment (RR = 0.17), with rates of RR = 0.29 in the first 2 years and RR = 0.11 thereafter. Hemorrhage rates significantly differed before and after 2 years post-SRS (RR = 0.36). Among epileptic patients, 20.2% had epilepsy pretreatment, and 49.9% were seizure-free post-SRS, while 30.6% experienced reduced seizure frequency. Lesion volume changes showed a reduction in 46.9%, stability in 47.1%, and an increase in 6.7%. Symptomatic radiation effects affected 8% of patients. Subgroup analysis revealed symptomatic change rates of 6% at doses ≤13 Gy compared to 9% at doses >13 Gy. Permanent clinical deficits were rare (2%)., Conclusions: This meta-analysis suggests SRS is an effective intervention for intracranial cavernous malformations, significantly reducing hemorrhage rates and improving seizure outcomes. Objective: Intracranial arteriovenous malformations are the most common cause of intracranial hemorrhages in pediatric patients. Stereotactic radiosurgery (SRS) has been used extensively to treat these lesions. The authors conducted a systematic review and meta-analysis to report treatment outcomes and long-term complications., Methods: This study follows the PRISMA and MOOSE guidelines, with the search spanning electronic databases up to February 6, 2024. The outcome measures included obliteration rate, hemorrhage in the latency period, symptomatic radiation-induced changes (RICs), cyst formation, and radiation-induced tumorigenesis., Results: A total of 1493 patients across 24 studies were included. The pooled complete obliteration after single-fraction SRS was 64.7% (95% CI 58%-69%). The pooled post-SRS hemorrhage rate at the final follow-up was 6.2% (95% CI 5%-8%). The overall incidence rate of RIC was 31.3% (267/854 patients), and the incidence of symptomatic RIC was 8.8% (114/1289 patients). For permanent symptomatic RIC, the pooled incidence was 4.8% (62/1283 patients). At final follow-up, 17 cases of radiation-induced necrosis were documented among 654 patients (2.6%). Similarly, cyst formation was reported in 1.3% of cases (17/1265 patients) and radiation-induced tumors occurred in 0.15% of cases (2/1342 patients)., Conclusions: SRS can be considered an effective intervention for appropriately selected pediatric patients with arteriovenous malformations. Long-term complication rates appear to be low but additional longitudinal studies are required to better define the long-term outcomes.

Background: Nonfunctioning pituitary adenomas (NFPAs) are a significant subtype of pituitary tumors, accounting for 30% of all pituitary tumors and 10-20% of intracranial tumors. The primary treatment for NFPAs is resection, but complete resection is often challenging due to the tumor's proximity to critical structures, leading to frequent recurrences. Stereotactic radiosurgery (SRS) has emerged as a viable treatment option for recurrent or residual NFPAs, but its long-term efficacy and safety profile require further investigation., Methods: This systematic review followed PRISMA guidelines and included studies published up to February 2024. We searched MEDLINE, Embase, and Cochrane databases for studies evaluating SRS for recurrent/residual NFPAs. Inclusion criteria focused on studies reporting outcomes and complications of SRS, while exclusion criteria omitted case reports, case series, and non-English studies. Data extracted included demographic details, dosimetry parameters, and follow-up durations. The risk of bias was assessed using the ROBINS-I tool, and statistical analyses were performed using single-arm meta-analyses., Results: A total of 24 studies involving 3,781 patients were included. The mean follow-up duration was 60 months. Tumor control was achieved in approximately 92.3% of patients. The risk of developing hypopituitarism post-SRS was 13.62%, while the risk for panhypopituitarism was 2.55%. New visual field deficits occurred in 3.94% of patients. Cranial nerve deficits were rare, with event rates below 1% for CN III, CN V, and CN VI., Conclusion: SRS is effective in managing recurrent or residual NFPAs, achieving high tumor control rates. However, the risk of hypopituitarism remains a significant concern, necessitating regular endocrinological monitoring. While generally safe, the potential for new visual field deficits and other cranial nerve deficits must be considered. SRS remains a valuable treatment option, but clinicians should be aware of its potential complications., (© 2024. Background and Objectives: Studies comparing neurological and radiographic outcomes of repeat to initial stereotactic radiosurgery (SRS) intracranial arteriovenous malformations are scarce. Our aim was to perform a retrospective matched comparison of patients initially treated with SRS with those undergoing a second radiosurgical procedure., Methods: We collected data from arteriovenous malformations managed in 21 centers that underwent initial and repeated radiosurgery from 1987 to 2022. Based on arteriovenous malformations volume, margin dose, deep venous drainage, deep, and critical location, we matched 1:1 patients who underwent an initial SRS for treatment-naive arteriovenous malformations and a group with repeated SRS treatment., Results: After the selection process, our sample consisted of 328 patients in each group. Obliteration in the initial SRs group was 35.8% at 3 and 56.7% at 5 years post-SRS, while the repeat SRS group showed obliteration rates of 33.9% at 3 years and 58.6% at 5 years, without statistically significant differences (P = .75 and P = .88, respectively). There were no statistically significant differences between the 2 groups for obliteration rates (hazard ratio = 0.93; 95% CI, 0.77-1.13; P = .5), overall radiation-induced changes (RIC) (OR = 1.1; 95% CI, 0.75-1.6; P = .6), symptomatic RIC (OR = 0.78; 95% CI, 0.4-1.5; P = .4), and post-SRS hemorrhage (OR = 0.68; 95% CI; P = .3)., Conclusion: In matched cohort analysis, a second SRS provides comparable outcomes in obliteration and RIC compared with the initial SRS.
