5. Chemotherapy followed by surgery compared with surgery alone for localized esophageal cancer

Abstract

Preoperative chemotherapy does not appear to improve the prognosis of patients with esophageal cancer. Researchers randomly assigned 440 patients with esophageal cancer to have surgery only or to have chemotherapy two to four weeks before surgery. Survival rates at one and two years were about 60% and 35% in both groups. Chemotherapy also did not reduce the risk of recurrence.

6. Hepatic resection versus transplantation for hepatocellular carcinoma

Abstract

Many types of advanced liver disease are now treated by liver transplant. However, the role of liver transplant in the treatment of hepatocellular cancer has not yet been defined. Hepatocellular carcinoma, a malignant tumor of the liver, occurs most often in Africa and the Far East, and has a poor prognosis. Surgical excision of the tumor has been the only treatment found to be effective. Previous studies have reported a high rate of recurrence for hepatocellular cancer patients treated by transplant. However, a small number of patients have been cured by transplant. A study was undertaken to compare the 10-year outcome of patients who received a liver transplant (105 subjects) and those who underwent subtotal liver resection (76 subjects). The overall survival rate at one year was 71.1 percent for resection, and 65.7 percent for transplant; at two years, 55.0 percent, and 49 percent; at three years, 47.2 percent, and 39.2 percent; at four years 37.2 percent, and 35.6 percent; and, at five years 32.9 percent, and 35.6 percent. In both treatment groups survival correlated with tumor staging, a system for describing the extent of the spread of cancer. If the patient had liver cirrhosis (degenerative disease of the liver), survival after transplant was significantly better than after resection. Cancer recurrence rate was high in both groups; at 50 after resection, and 43 percent after transplant. A better than five year survival rate was achieved by 12 patients who underwent resection, and by 13 patients who received a liver transplant. Improvements in survival will probably come from advances in nonsurgical anti-cancer therapy given before and/or after surgical removal of the tumor. Abstract

Null mutations that cripple TCR antigen recognition explain rare primary immunodeficiencies, but it is not understood why more common single nucleotide substitutions that create subtle TCR signaling defects are paradoxically associated with autoimmunity in some instances but not others. Here we analyse how a set of Zap70 gene variants, which alter individual catalytic cleft residues to produce stepwise decreases in TCR signaling, impact upon opposing TCR functions in different T cells and upon regulation of the immune system as a whole. One Zap70 variant, murdock, that moderately decreases TCR signaling and thymic selection is tolerated at the system level, while a more severe Zap70 defect, mrtless, abolishes thymic positive selection and causes immunodeficiency. At a threshold between these two states, negative selection and Foxp3+ regulatory T cell formation are compromised to a greater degree than positive selection and memory/effector cell formation. This cellular imbalance can no longer be controlled at the system level, resulting in formation of autoantibodies and hyper-IgE. The pleiotropic functions of Zap70 and their differential response to graded genetic variation provide a paradigm for understanding common genetic variation in humans.

Abstract

Null mutations that cripple T cell receptor (TCR) signaling explain rare primary immunodeficiencies, but it is not understood why more common polymorphisms that lead to subtle TCR signaling defects are paradoxically associated with autoimmunity. Here we analyzed how a series of Zap70 variants with step-wise decreases in TCR signaling impacted upon opposing TCR functions of immunity and tolerance. One Zap70 variant, murdock, moderately decreased TCR signaling and thymic selection without compromising immunological tolerance, whereas a more severe Zap70 defect, mrtless, abolished thymic-positive selection and led to immunodeficiency. Signaling capacities between these two thresholds disproportionately compromised negative selection and Foxp3+ regulatory T cell formation, creating a cellular imbalance between immunogenic and tolerogenic functions that resulted in the excessive production of autoantibodies and immunoglobulin E (IgE). Abstract

Summary: A methodology for characterizing the acoustical properties of a port environment, namely Port Everglades, has been proposed and carried out. This approach includes both a port-wide analysis of how the basic oceanographic features within the port impact the acoustic properties, and also a more focused sampling methodology within a small region of Port Everglades, allowing for the acoustic characteristics, including ambient noise, and an approximate signal absorption to be computed. The results documented through the duration of this research indicate that the temperature variation throughout the port is the principal contributor to the characteristics of the sound velocity profile. Ambient noise measurements have revealed high levels of background noise within the sub-5 kHz region, owing likely to consistent port traffic. The calculation of absorption indicates that high frequency systems, i.e. > 100 kHz, may encounter problems when transmitting over a considerable distance. 41. Colonic Ganglioneuroma

Abstract

• Ganglioneuromas are neuroectodermal tumors that are found in diverse anatomic sites, but they are very uncommon in the colon. We observed a patient with partial colonic obstruction whose roentgenogram had a napkin-ring construction that was thought to be carcinoma. On resection, pathologic examination showed a ganglioneuroma. Mesenchymal neoplasms of various histologic types occur throughout the gastrointestinal tract. Neurogenic tumors are not uncommon in Von Recklinghausen's neurofibromatosis and multiple mucosal neuroma syndrome, but solitary lesions unassociated with these conditions are rare. To our knowledge, this complication in the colon is only the second such reported case. We believe that this case is noteworthy because of its clinical and roentgenographic resemblance to carcinoma.(Arch Surg 115:75-77, 1980)
