33. Physiotherapy at the intersection between standardization and individual adaptation

Abstract

Drawing on the phenomenological tradition, contemporary physiotherapy practice may be understood as an embodied experience; one that requires standardized approaches to evidence-based examination, treatment, and evaluation while simultaneously requiring adaptation of each of these elements of practice to individual patients. Drawing on material from a study of encounters between physiotherapists and patients, this chapter addresses how the work of reconciling the needs for standardization and adaptation is accomplished in physiotherapy practice. The research approach and analysis of the material draws on a number of scholars in the phenomenological tradition, beginning with Maurice Merleau-Ponty. The study illuminated the ways in which both explicit and tacit forms of knowing are brought together in the physiotherapy encounter. Physiotherapists use explicit knowledge primarily as a framework for thinking about assessment and intervention; however, it is the inter-subjective, communicative practice of being-with-another from which the therapeutic encounter draws its power. It is this being-withanother that enables the physiotherapist to adapt abstract, explicit knowledge for the individual with whom they co-construct physiotherapy care. The study revealed how the knowledge put into play in physiotherapy depends on the sensitivity and reflective embodied knowledge of the physiotherapist, and their bodily style and developed professional skills. This is the bodily style that in some way transforms the therapist’s relation to the world, and especially to various and creative ways of knowledge translation at different moments in the process of practicing physiotherapy.

Abstract

Objective: Developing compassionate and humanistic practitioners is an enduring imperative in health professions education. Engaging in epistemic reflexivity, or the ability to question the ways in which we practice, and their association with organizational and social structures, creates the ability to interrogate embedded assumptions. This insight allows for disruption of patterns of automaticity, which allows us to re-imagine how disciplinary practices could be otherwise. Methods: In this interpretivist inquiry, we used qualitative methods to explore experiences with an educational initiative to develop knowledge and skills in epistemic reflexivity among learners enrolled in the final year of a Master's-level entry-to-practice physical therapy training program. We used observations, a focus group and a face-to-face interview to deepen our understanding of the socially-constructed and iterative processes that underpin reflexivity. Results: Our data illuminated key dimensions of study participants' experiences that suggest a transformation in students' orientation and ability to question takenfor-granted aspects of physiotherapy practice; however, skillful nurturing of the process by facilitators with expertise in epistemic reflexivity was important to allow these efforts to bear new positive possibilities rather than negative impacts. Conclusions: Developing capacity in epistemic reflexivity must be carefully facilitated and nurtured. Future research could elaborate on the relevance of our findings in other novice groups within the health professions and how our provisional step-wise framework may be adapted for learners other than novices. Abstract

Healthcare management, entrepreneurship, and innovation involves multiple actors spanning from individuals to the larger system level. For example, patients, providers, entrepreneurs, managers, scientists, and investors/funders are nested and interconnected in collective partnerships, teams, organizations, communities, and political systems. These relationships are multifaceted and result in layers of complexity as well as a range of opportunities for the stakeholders involved. In healthcare, stakeholders oftentimes have differing goals and the goods/services provided can be less tangible, with credence qualities or with longer time horizons around outcomes, compared to other areas of business management. Tasks are complex and interdependent while co-production among numerous actors from different backgrounds is common. Therefore, a better understanding of the managerial, entrepreneurial, and innovative processes across healthcare can contribute to our research progress, practitioner application, and broader social impact. In this symposium, we will bring together scholars from across disciplines and geography to explain how to advance healthcare innovation and entrepreneurial activity. The papers consider a range of stakeholders, leverage various theoretical and methodological approaches, and focus on multiple levels in healthcare, such as the political economy of a region, venture capital investment across counties, temporary teams in hospitals, business model innovation for firms, and individual entrepreneurial mindsets.
