Background: Previous epidemiological studies have been suggestive but inconclusive in demonstrating inverse associations of calcium, vitamin D, dairy product intakes with risk of colorectal cancer (CRC). We conducted a large population-based comparison of such associations in Newfoundland and Labrador (NL) and Ontario (ON). Methods: A case control study design was used. Colorectal cancer cases were new CRC patients aged 20-74 years. Controls were a sex and age-group matched random sample of the population in each province. 1760 cases and 2481 controls from NL and ON were analyzed. Information on dietary intake and lifestyle was collected using self-administered food frequency and personal history questionnaires. Results: Controls reported higher mean daily intakes of total calcium and total vitamin D than cases in both provinces. In ON, significant reduced CRC risk was associated with intakes of total calcium (OR of highest vs. lowest quintiles was 0.57, 95% CI 0.42-0.77, ptrend=0.03), total vitamin D (OR=0.73, 95% CI 0.54-1.00), dietary calcium (OR=0.76, 95% CI 0.60-0.97), dietary vitamin D (OR=0.77, 95% CI 0.61-0.99), total dairy products and milk (OR=0.78, 95% CI 0.60-1.00), calcium-containing supplements use (OR=0.76). In NL, the inverse associations of calcium, vitamin D with CRC risk were most pronounced among calcium- or vitamin D-containing supplement users (OR=0.67, 0.68, respectively). Conclusions: Results of this study add to the evidence that total calcium, dietary calcium, total vitamin D, dietary vitamin D, calcium- or vitamin D-containing supplement use may reduce the risk of CRC. The inverse associations of CRC risk with intakes of total dairy products and milk may be largely due to calcium and vitamin D. Key words: Calcium; vitamin D; dairy products; colorectal cancer

This study, conducted in Newfoundland and Labrador, assessed the level of awareness, perceptions and concerns of healthcare providers, health researchers, data managers and the general public about the collection, use and disclosure of personal health information (PHI) for research purposes. Data collection involved surveys and follow-up focus groups with participants. Results indicate a poor understanding generally with regard to privacy rights and responsibilities. Many professionals are unfamiliar with the legislative environment for PHI, particularly as it pertains to the access and use of PHI for research purposes. Lack of familiarity with basic requirements for patient-based research, coupled with heightened sensitivity to privacy issues owing to various federal and provincial regulatory initiatives, could have a chilling effect on health research. Importantly, our results indicate that the public is much less concerned about the use of their PHI for health research purposes than are professionals who collect, store and share it.

HLA typing was performed on 384 individuals of an isolated population of 1,500 people with a.familial aggregate of lymphoma and immunodeficiency cases. Eighty-five % of the total population were descendants of the founding couple. First cousin marriages were common. There was a three-fold or higher increase of the following haplotypes as compared to the frequencies in Sheffield: HLA-A28, Bw35, HLA-A28, B18, HLA-A10, B18, HLA-A2, B18, HLA-A11, Bw40 and HLA-A11, B7. The frequency of HLA-A1, B8 was low (5.4%). The most common genotype was HLA-A2, B12/A2, B12 followed by HLA-A2, B12/A28, Bw35. We found 30 HLA homozygous individuals, of these 15 were HLA-A2, B12/A2, B12. There were two possible HLA cross-overs which may be confirmed and three postulated cross-overs which can never be confirmed as one or both parents of the individuals in question are deceased. Some of the haplotypes could be traced back to the first, second and third generations, i.e. to the first half of the nineteenth century. No single haplotype or antigen was shared by the patients.

RÉSUMÉLes étapes utilisées pour l'officialisation des préférences en matière de soins en fin de vie et les facteurs en rapport avec ces étapes sont flous dans la documentation. À l'aide des données de la troisième phase de l'Étude sur la santé et le vieillissement au Canada (ESVC-3), nous avons examiné les relations entre les prédicteurs démographiques et en matière de santé, et les trois résultats (à savoir si les participants avaient réfléchi à leurs préférences de fin de vie, s'ils en avaient discuté, ou s'ils les avaient officialisées), et s'il y avait des relations entre les trois résultats. La région de résidence au Canada, le sexe féminin, et davantage d'années de scolarité étaient des facteurs associés à des personnes ayant réfléchi à leurs préférences. La région de résidence, le sexe féminin, et le manque de déficience cognitive étaient associés aux discussions en matière de préférences, et la région de résidence et le fait de ne pas avoir de conjoint étaient associés au dépôt de documents officiels. Les résidents de l'Ontario étaient les plus enclins à avoir réfléchi à leurs préférences, à en avoir discuté, et à les avoir officialisées, tandis que les résidents des Maritimes étaient les moins enclins à y avoir pensé et à avoir agi en conséquence. Background Primary care practitioners are familiar with the frail elderly and commonly have to deal with their multi-morbidity and their functional decline, both physically and mentally. However, there are well elderly with high quality of life and very few co-morbidities who seldom seek medical care. Objective To determine if a nurse-based program of home-delivered care, linked directly with the primary care practitioner or primary care team, would improve quality of life, symptoms, satisfaction with care and utilization of community and medical services, in independent community living old elderly. Design Randomized controlled trial. Setting St. John's, Newfoundland, Canada. Participants Two hundred and thirty-six independent, community-dwelling, cognitively functioning, people aged 80 years and older. Intervention A nurse-based program of care, carried out in the patients home, that involved a detailed assessment of needs, the development of a plan to meet the needs, and up to eight visits to the patients home during a 1-year period to facilitate the meeting of those needs. Control group Usual care Main outcome measurements Quality of Life measured using the SF-36 and the CASP-19 scales; symptomology using the Comorbidity Symptom Scale; patient satisfaction using the PSQ-18; and assessment of health care services (community services, emergency room visits, hospitalizations, use of diagnostic services and family doctor visits) through patient recall, family physician chart review and assessment of hospitalization records. Results There were no statistical or meaningful differences between the intervention and control groups in any of the outcomes measured. Conclusion The intensive, home-delivered, program of care for the well old elderly did not have an impact on the outcomes measured.

Background. The Argentia region of Newfoundland and Labrador, Canada, was home to a US naval base during a 40-year period between the 1940s and the 1990s. Activities on the base resulted in contamination of the soil and groundwater in the region with chemicals such as heavy metals and dioxins, and residents have expressed concern about higher rates of cancer in their community. This study investigated the rate of cancer diagnosis that is disproportionately high in the Argentia region.Methods. Cases of cancer diagnosed between 1985 and 2011 were obtained for the Argentia region, two comparison communities, and the province of Newfoundland and Labrador. Crude and age-standardized incidence rates of cancer diagnosis were calculated and compared. The crude incidence rate was adjusted for differences in age demographics using census data, and age-standardized incidence rates were compared.Results. Although the Argentia region had a higher crude rate of cancer diagnosis, the age-standardized incidence rate did not differ significantly from the comparison communities or the provincial average. Argentia has an aging population, which may have influenced the perception of increased cancer diagnosis in the community.Conclusions. We did not detect an increased burden of cancer in the Argentia region.

Newfoundland has the highest rate of colorectal cancer (CRC) of any Canadian province. In order to investigate the factors, especially genetic components, responsible for CRC we established the Newfoundland Colorectal Cancer Registry. In a 5-year period we examined every case of CRC diagnosed under the age of 75 years and obtained consent from 730 cases. Careful analysis of family history was used to assign a familial cancer risk, based on established criteria. We observed that 3.7% of CRC cases came from families meeting the Amsterdam II criteria and a further 0.9% of cases involved familial adenomatous polyposis (FAP). An additional 43% of cases met one or more of the revised Bethesda criteria and 31% of all cases had a first-degree relative affected with CRC. We compared the Newfoundland data with data from the province of Ontario, where the same recruitment and risk-assessment criteria were used. In all categories, the indicators of familial risk were significantly higher in Newfoundland. These data were also compared to results published from 13 other population-based studies worldwide. In every category the proportion of Newfoundland cases meeting the criteria was higher than in any other population. The mean differences were: 3.5-fold greater for FAP, 2.8-fold higher for Amsterdam criteria, 2.0-fold higher for Bethesda criteria and 1.9-fold higher for the number of affected first-degree relatives. We conclude that the high incidence of CRC in Newfoundland may be attributable to genetic, or at least familial, factors. In the high-risk families we provide evidence for the involvement of founder mutations in the APC and MSH2 genes.

In the fall of 2001, the Canadian Cancer Society – Newfoundland and Labrador Division (CCS), a non-governmental patient advocacy organization, approached us to request a study of out-of-pocket costs for patients who travel to access cancer treatment. Anecdotal evidence suggests that these costs influence the decisions cancer patients make about their care (Maynard 2001). The CCS had previously embarked on advocacy efforts on this issue, but the provincial Department of Health and Community Services had cited the need for more research. The Canadian Institutes for Health Research (CIHR), the Newfoundland and Labrador Centre for Applied Health Research and the Canadian Breast Cancer Foundation – Atlantic Division funded the study, with data collection beginning in fall 2002. We surveyed 484 cancer patients, interviewed 21 cancer care providers and gathered information about medical travel subsidy programs across Canada. We found substantial variation in out-of-pocket costs for patients travelling to access cancer care. One in three rural patients (those living more than one hour away from a cancer clinic) paid more than $200 for travel and lodging during a single trip to an oncologist, with 9% incurring costs of more than $1,000 per trip. A significantly larger proportion of rural residents had higher costs than their urban counterparts and said that expenses for travel, drugs, child care and travel time were important considerations in their care decisions (Mathews and Basky 2004). Patients’ cost-cutting strategies (including rationing medications, lengthening follow-up times, or choosing inpatient palliative care) may compromise quality of care. We recommended that the restrictive eligibility criteria of the provincial Medical Travel Assistance Program (the subsidy that reimburses patients for medical care-related travel costs) be reviewed, and that cancer care be provided in smaller communities wherever possible. The primary audiences for our study were the CCS, the Newfoundland Cancer Treatment and Research Foundation (NCTRF, the provincial cancer care provider) and the Department of Health and Community Services. We also wanted to communicate our findings to study participants, politicians, the public, cancer advocates and care providers in Canada and other researchers.

In May 2003, a survey questionnaire was distributed to all licensed primary care physicians in Newfoundland and Labrador. The objective was to examine the attitudes, self-reported practices, and continuing medical education (CME) preferences of primary care physicians as they pertain to prostate cancer screening.Data was obtained from 485 primary care physicians using self-reports of prostate cancer screening practices, attitudes towards prostate cancer screening, and CME preferences. Respondents' characteristics were also collected (eg, gender, years of experience).A majority of respondents screen asymptomatic male patients for prostate cancer. Screening behaviour was related to high volume practice settings, fee-for-service and increased with patient age. Most common reasons for screening were family history, age of patient, and patient request. Majority of physicians agreed that prostate screening should be routinely performed on all men beginning at age 50, however half of physicians believe there is lack of evidence to support digital rectal examination (DRE) and one-third of physicians do not believe the prostate-specific antigen (PSA) nor DRE are accurate screening tests. Areas of greatest interest for CME included topics related to prostate cancer screening effectiveness, strategies for prevention, sexual dysfunction, available treatments and their side effects, and management options.Physicians are supportive of the value of screening, however the reliability of and evidence to support DRE and PSA as prostate cancer screening tests are in question. CME which addresses issues surrounding prostate screening and areas related to patient education and counselling are of greatest need.

SeveralN-nitroso compounds (NOC) have been shown to be carcinogenic in a variety of laboratory animals, but evidence of their carcinogenicity in humans is lacking. We aimed to examine the association between NOC intake and colorectal cancer (CRC) risk and possible effect modification by vitamins C and E and protein in a large case–control study carried out in Newfoundland and Labrador and Ontario, Canada. A total of 1760 case patients with pathologically confirmed adenocarcinoma and 2481 population controls were asked to complete a self-administered FFQ to evaluate their dietary intakes 1 year before diagnosis (for cases) or interview (for controls). Adjusted OR and 95 % CI were calculated across the quintiles of NOC (measured byN-nitrosodimethylamine (NDMA)) intake and relevant food items using unconditional logistic regression. NDMA intake was found to be associated with a higher risk of CRC (highestv.lowest quintiles: OR 1·42, 95 % CI 1·03, 1·96;Pfor trend = 0·005), specifically for rectal carcinoma (OR 1·61, 95 % CI 1·11, 2·35;Pfor trend = 0·01). CRC risk also increased with the consumption of NDMA-containing meats when the highest tertile was compared with the lowest tertile (OR 1·47, 95 % CI 1·03, 2·10;Pfor trend = 0·20). There was evidence of effect modification between dietary vitamin E and NDMA. Individuals with high NDMA and low vitamin E intakes had a significantly increased risk than those with both low NDMA and low vitamin E intakes (OR 3·01, 95 % CI 1·43, 6·51;Pfor interaction = 0·017). The present results support the hypothesis that NOC intake may be positively associated with CRC risk in humans. Vitamin E, which inhibits nitrosation, could modify the effect of NDMA on CRC risk.

Objective To examine the association between dietary patterns and colorectal cancer (CRC) survival. Design Cohort study. Setting A familial CRC registry in Newfoundland. Participants 529 newly diagnosed CRC patients from Newfoundland. They were recruited from 1999 to 2003 and followed up until April 2010. Outcome measure Participants reported their dietary intake using a food frequency questionnaire. Dietary patterns were identified with factor analysis. Multivariable Cox proportional hazards models were employed to estimate HR and 95% CI for association of dietary patterns with CRC recurrence and death from all causes, after controlling for covariates. Results Disease-free survival (DFS) among CRC patients was significantly worsened among patients with a high processed meat dietary pattern (the highest vs the lowest quartile HR 1.82, 95% CI 1.07 to 3.09). No associations were observed with the prudent vegetable or the high-sugar patterns and DFS. The association between the processed meat pattern and DFS was restricted to patients diagnosed with colon cancer (the highest vs the lowest quartile: HR 2.29, 95% CI 1.19 to 4.40) whereas the relationship between overall survival (OS) and this pattern was observed among patients with colon cancer only (the highest vs the lowest quartile: HR 2.13, 95% CI 1.03 to 4.43). Potential effect modification was noted for sex (p value for interaction 0.04, HR 3.85 for women and 1.22 for men). Conclusions The processed meat dietary pattern prior to diagnosis is associated with higher risk of tumour recurrence, metastasis and death among patients with CRC.

RÉSUMÉBut: Comparer au moyen d'un plan d'étude transversale les caractéristiques démographiques et fonctionnelles des utilisateurs âgés et des non-utilisateurs de services de soins à domicile dans les centres urbains de la Nouvelle-Écosse (n = 533) et de la Terre-Neuve (n = 449).Résultats: Plus de Néo-Écossais et Néo-Écossaises (24%) que de Terre-Neuviens et Terre-Neuviennes (11%) reçoivent des services de soins à domicile. Dans les deux provinces, la plupart des gens dépendants par rapport aux activités de la vie quotidienne (AVQ) ne reçoivent pas des services de soins à domicile, et la plupart des utilisateurs de soins à domicile n'ont pas de dépendence par rapport aux AVQ. À Terre-Neuve, la dépendence et la démence sont plus fréquentes chez les utilisateurs de soins à domicile, contrairement aux utilisateurs en Nouvelle-Écosse ou le fait d'avoir 85 ans ou plus, de ne pas avoir de soutien dans la même habitation, de ne pas être marié et d'être limité par rapport aux activitiés instrumentales de la vie quotidienne (ATVQ) a pour conséquence une augmentation importante de la probabilité d'utilisation de soins à domicile. À Terre-Neuve, la tendance à recourir à des soins à domicile est plus forte chez les gens ayant une dépendence par rapport aux AIVQ.Conclusions: Si les soins à domicile doivent se substituer à des soins en établissement, il faut être prêt à adapter à un nouveau type d'utilisateur les programmes existants de soins à domicile.

Background Diet is regarded as one of the most important environmental factors associated with colorectal cancer (CRC) risk. A recent report comprehensively concluded that total energy intake does not have a simple relationship with CRC risk, and that the data were inconsistent for carbohydrate, cholesterol and protein. The objective of this study was to identify the associations of CRC risk with dietary intakes of total energy, protein, fat, carbohydrate, fiber, and alcohol using data from a large case-control study conducted in Newfoundland and Labrador (NL) and Ontario (ON), Canada. Methods Incident colorectal cancer cases (n = 1760) were identified from population-based cancer registries in the provinces of ON (1997-2000) and NL (1999-2003). Controls (n = 2481) were a random sample of residents in each province, aged 20-74 years. Family history questionnaire (FHQ), personal history questionnaire (PHQ), and food frequency questionnaire (FFQ) were used to collect study data. Logistic regression was used to evaluate the association of intakes of total energy, macronutrients and alcohol with CRC risk. Results Total energy intake was associated with higher risk of CRC (OR: 1.56; 95% CI: 1.21-2.01, p-trend = 0.02, 5th versus 1st quintile), whereas inverse associations emerged for intakes of protein (OR: 0.85, 95%CI: 0.69-1.00, p-trend = 0.06, 5th versus 1st quintile), carbohydrate (OR: 0.81, 95%CI: 0.63-1.00, p-trend = 0.05, 5th versus 1st quintile) and total dietary fiber (OR: 0.84, 95% CI:0.67-0.99, p-trend = 0.04, 5th versus 1st quintile). Total fat, alcohol, saturated fatty acids, monounsaturated fatty acids, polyunsaturated fatty acids, and cholesterol were not associated with CRC risk. Conclusion This study provides further evidence that high energy intake may increase risk of incident CRC, whereas diets high in protein, fiber, and carbohydrate may reduce the risk of the disease.

The impact of micronutrient intake and colorectal cancer (CRC) risk is poorly understood. The objective of this study was to evaluate the associations of selected micronutrients with risk of incident CRC in study participants from Newfoundland, Labrador (NL) and Ontario (ON), Canada.We conducted a population-based study among 1760 case participants and 2481 age- and sex-matched control participants. Information on diet and other lifestyle factors were measured using a food frequency questionnaire and a personal history questionnaire. Odds ratios (OR) and 95% confidence intervals (CI) were calculated using unconditional logistic regression, controlling for covariables.Highest compared to lowest quartile intakes of certain micronutrients were associated with lower risk of CRC, including: calcium (from food and supplements (FS), OR=0.59; 95% CI=0.45-0.77, and from food only (FO): OR=0.76, 95% CI=0.59-0.97), vitamin C (FS:OR=0.67; 95%CI:0.51-0.88), vitamin D (FS: OR=0.73; 95% CI: 0.57-0.94, FO: OR=0.79, 95% CI=0.62-1.00), riboflavin (FS: OR=0.61; 95% CI=0.47-0.78, and folate (FS: OR=0.72; 95% CI=0.56-0.92). Higher risk of CRC was observed for iron intake (highest versus lowest quintiles: OR=1.34, 95% CI=1.01-1.78).This study presents evidence that dietary intake of calcium, vitamin D, vitamin C, riboflavin and folate are associated with a lower risk of incident CRC and that dietary intake of iron may be associated with a higher risk of the disease.

We aimed to describe the perceptions of health care providers concerning patient and health care provider strategies to limit out-of-pocket costs for cancer care. We conducted semi-structured interviews with 21 cancer care providers (nurses, social workers, oncologists, surgeons, pharmacists, and dieticians) in Newfoundland and Labrador. Patients try to minimize costs by substituting or rationing medications, choosing radical treatments, lengthening the time between follow-up appointments, choosing inpatient care, and working during treatment to minimize loss of income. Providers respond to the financial concerns of patients by helping them to access financial assistance programs, by changing chemotherapy and supportive drug prescriptions, and by shortening radiation treatment protocols. They admit patients to hospital and arrange follow-up with physicians closer to a patient&rsquo, s home. Out-of-pocket costs resulting from cancer care are incurred at all phases of treatment and follow-up. These costs are substantial concerns for some patients and their health care providers. Encouraging communication between patients and their providers is needed to identify individuals at risk and to safely modify care plans. Tele-oncology and public drug, medical travel, and leave programs are needed to ensure that patients are better able to afford the costs related to cancer care.

Although a large body of epidemiological research suggests that red meat intake increases the risk of colorectal cancer, little is known regarding how such an association varies across populations and types of red meat. The objective of this study was to assess whether an association exists between the intakes of total red meat and pickled red meat and the risk of colorectal cancer in study subjects residing in Newfoundland and Labrador. This case–control study of 1,204 residents of Newfoundland and Labrador was part of a larger study on colorectal cancer. Personal history food frequency questionnaires were used to collect retrospective data from 518 individuals diagnosed with colorectal cancer and 686 controls. Intakes were ranked and divided into tertiles. Logistic regression was used to examine the possible association between meat intakes and colorectal cancer diagnosis while controlling for possible confounding factors. A positive, but non-statistically significant, association between total red meat intake and CRC was observed in this study. Pickled red meat consumption was found to be significantly associated with an increased risk of CRC (men, OR = 2.07, 95% CI 1.37–3.15; women, OR = 2.51, 95% CI 1.45–4.32), the odds ratios increasing with each tertile of consumption, suggesting a dose–response effect. Intake of pickled red meat appears to increase the risk of colorectal cancer in Newfoundland and Labrador.

We examined the importance of 5 items (stage of illness, personal feelings, travel costs, drug costs and child care costs) in the cancer treatment decisions of urban and rural residents after they had started treatment for their cancer.We surveyed 484 adults who presented for care at cancer clinics in Newfoundland and Labrador from September 2002 to June 2003. Respondents rated the importance of each of the 5 items in their cancer care decisions on a 5-point Likert scale, which was later collapsed into 2 categories, "important" and "not important." We used chi2 tests and multiple logistic regression to compare the responses of urban and rural residents.In our sample of 484 respondents, there were 258 (53.3%) urban and 226 (46.7%) rural residents. After controlling for other significant predictors, we found that rural residents were more likely to report that travel costs (odds ratio [OR] 1.79, 95% confidence interval [CI] 1.21-2.63), drug costs (OR 1.69, 95% CI 1.13-2.23) and child care costs (OR 2.33, 95% CI 1.09-4.96) were "important" in cancer treatment decisions compared with urban residents. Stage of disease and personal feelings were equally important to urban and rural residents.Financial impediments disproportionately affect rural residents' decisions about cancer care and highlight the need to ensure that centralized specialist care, such as cancer treatment, is accessible.

Objectives: To evaluate whether the participating controls represented the underlying population in a colorectal cancer case-control study in a geographically isolated North American population. Methods: The characteristics of the study controls recruited through Random-digit-dialing (RDD) were compared with those in the corresponding target population estimates. Results: Participating controls were more likely to have higher levels of education and income and were less likely to be smokers. Conclusion: Study controls recruited through RDD tend to have higher socioeconomic status, which may lead to overestimation of a number of risk factors in this study.

Background While substantive epidemiological literature suggests that alcohol drinking and obesity are potential risk factors of colorectal cancer (CRC), the possible interaction between the two has not been adequately explored. We used a case-control study to examine if alcohol drinking is associated with an increased risk of CRC and if such risk differs in people with and without obesity. Methods Newly diagnosed CRC cases were identified between 1999 and 2003 in Newfoundland and Labrador (NL). Cases were frequency-matched by age and sex with controls selected using random digit dialing. Cases (702) and controls (717) completed self-administered questionnaires assessing health and lifestyle variables. Estimates of alcohol intake included types of beverage, years of drinking, and average number of alcohol drinks per day. Odds ratios were estimated to investigate the associations of alcohol independently and when stratified by obesity status on the risk of CRC. Results Among obese participants (BMI ≥ 30), alcohol was associated with higher risk of CRC (OR: 2.2; 95% CI: 1.2-4.0) relative to the non-alcohol category. Among obese individuals, 3 or more different types of drinks were associated with a 3.4-fold higher risk of CRC relative to non-drinkers. The risk of CRC also increased with drinking years and drinks daily among obese participants. However, no increased risk was observed in people without obesity. Conclusion The effect of alcohol of drinking on CRC seems to be modified by obesity.

We studied four families with prolactin-secreting pituitary tumors as part of multiple endocrine neoplasia syndrome type I (MEN I). The propositus in family A had galactorrhea-amenorrhea and an enlarged sella turcica at the time of parathyroidectomy. Family studies showed that her mother had prolactinoma and primary hyper-parathyroidism, one of three uncles had primary hyperparathyroidism, and two sisters had prolactin-secreting adenomas, All affected family members shared the HLA haplotype: A1, B8, Cw-, DR3; linkage with HLA could not be excluded. The proband in family B had prolactinoma, primary hyperparathyroidism and primary hypothyroidism associated with a high-lying vestigial thyroid. The proband's father had primary hyperparathyroidism and thymic carcinoid, and two uncles had primary hyperparathyroidism. The proband and father shared the haplotype A2, B8, Cw-, DR3. In family C the proband had primary hyperparathyroidism and prolactinoma. Three of her sisters had primary hyperparathyroidism. One sister, who also had carcinoid tumor of the lung and a prolactinoma, married an affected member of family D; a daughter had prolactinoma and metastatic thymic carcinoid. Both proband and sister were 46 xx/xo and shared the haplotype A1, B8, Cw-, DR3. The fourth propositus, a member of family D reputed to have familial primary hyperparathyroidism, was found to have a prolactinoma four years following parathyroidectomy. One of the patient's affected uncles also had a pituitary tumor. The haplotype involved was Aw24, B35, Cw-. We conclude that (1) prolactinomas are an integral part of MEN I; (2) the syndrome may be associated with aberrant organ migration and chromosomal separation; (3) MEN I is associated with A1, B8; evidence for genetic linkage being inconclusive; (4) patients may harbor other tumors including thymic carcinoid.

A familial aggregate of seven cases of Hodgkin's disease (HD) has been investigated by HLA typing. Over 600 people in the immediate population (i.e. about half) have been HLA typed and haplotypes have been obtained for 95% of them. It was expected that the cases would share a particular HLA haplotype or at least that they would have one or two HLA antigens of the same series in common. However, this was not the case so no simple idea of association of HLA with HD cases was upheld. When antigen frequencies were examined in the whole population, it was found that HLA B18 increased progressively in incidence from 0.08 to 0.4 in successive groups of individuals each one more closely related to the HD cases. Similarly the community with the highest incidence of HD also had the highest incidence of B18. Thus B18, which in the world figures carries the highest relative risk, emerged as important in this study. Of four proposed interpretations of the data, we are most interested in the idea that the important HLA association is at a population level rather than at the level of the individual patient. A hypothesis, based on the concept of a "healthy carrier" for the HD agent, explains how such an association might operate. It is possible that B18-linked complement deficiency could be the basis for such a carrier state.

Previous reports of increased numbers of rhesus-negative individuals in series of Hodgkin's disease (HD) patients have received a unique type of confirmation. In an extended family of over 1,200 people in which an aggregate of 7 cases of HD has occurred, there were twice the usual number of rhesus-negative individuals. Thus, these HD patients also arose in a population containing an excess of rhesus negatives. Detailed analysis of rhesus antigens and haplotypes with the aid of five antisera and blood samples from 1,133 community members revealed that the patients and their families carried R1 and r but that there was exclusion from those groups of Ro and ra. Ra may have been excluded from the patients but was found in members of their immediate families. No other haplotypes were identified. The differences between HD families and members of the remainder of the extended family are statistically significant (p < 0.0005). The data are consistent with two interpretations which are not mutually exclusive. First, certain rhesus complexes, notably r, may confer susceptibility to the development of HD, probably by virtue of a closely linked gene, conversely other complexes such as Ro may confer resistance. The second interpretation is that the postulated HD agent may flourish better in populations of certain rhesus make-up than it does in others. Individuals of a particular rhesus genotype may be ‘carriers’ or ‘spreaders’. Further investigations are suggested which should contribute to the solution of this epidemiological puzzle.

Background Mobility disability is a major adverse health outcome associated with aging and an impediment to older adults’ well-being and behaviors in social and leisure activities. It has been shown that lifestyle factors, including smoking and alcohol consumption, have been used as coping strategies to deal with the negative impact of disability. The aim of this study was to determine the prevalence of smoking and alcohol consumption among older Canadians with different levels of mobility disabilities and to examine factors associated with these two lifestyle patterns among those with disabilities. Methods Secondary data analysis was performed using individuals (n = 6,038) aged 65 years and older from both the 2001 Participation and Activity Limitation Survey and the 2003 Canadian Community Health Survey. Multivariate logistic regressions examined the relationship between disability severity and smoking as well as alcohol consumption while controlling for potential confounding socioeconomic factors. Results The proportion of current smokers among seniors with less-severe and more-severe mobility disabilities and those in the general population was comparable with 12.55%, 11.57% and 11.93%, respectively. Forty-eight percent of seniors in the general population consumed alcohol regularly, compared to only 12.85% with more-severe mobility disabilities. No significant association was shown between the severity level of mobility disabilities and smoking (odds ratio = 0.90, 95% confidence interval: 0.75, 1.08). However, seniors having more-severe disability were less likely to consume alcohol regularly (odds ratio = 0.76, 95% confidence interval: 0.65, 0.89). Other variables including age, gender, income, living status, and social participation also impacted these lifestyle patterns among the study population. Conclusions Smoking and alcohol patterns present different associations with the severity level of mobility disabilities. Compared with the general population, elderly Canadians with mobility disabilities had similar smoking prevalence but differ significantly in terms of alcohol consumption. Results from this research will be relevant to decision makers involved in program planning, health education, and policy development as it pertains to the prevention and management of age-related disability.
