Abstract

Adaptive optimization plays a pivotal role in federated learning, where simultaneous server and client-side adaptivity have been shown to be essential for maximizing its performance. However, the scalability of jointly adaptive systems is often constrained by limited resources in communication and memory. In this paper, we introduce a class of efficient adaptive algorithms, named $FedAda^2$, designed specifically for large-scale, cross-device federated environments. $FedAda^2$ optimizes communication efficiency by avoiding the transfer of preconditioners between the server and clients. At the same time, it leverages memory-efficient adaptive optimizers on the client-side to reduce on-device memory consumption. Theoretically, we demonstrate that $FedAda^2$ achieves the same convergence rates for general, non-convex objectives as its more resource-intensive counterparts that directly integrate joint adaptivity. Empirically, we showcase the benefits of joint adaptivity and the effectiveness of $FedAda^2$ on both image and text datasets.

2. VERCEL: Verification and Rectification of Configuration Errors with Least Squares

Abstract

We present Vercel, a network verification and automatic fault rectification tool that is based on a computationally tractable, algorithmically expressive, and mathematically aesthetic domain of linear algebra. Vercel works on abstracting out packet headers into standard basis vectors that are used to create a port-specific forwarding matrix $\mathcal{A}$, representing a set of packet headers/prefixes that a router forwards along a port. By equating this matrix $\mathcal{A}$ and a vector $b$ (that represents the set of all headers under consideration), we are able to apply \textit{least squares} (which produces a column rank agnostic solution) to compute which headers are reachable at the destination. Reachability now simply means evaluating if vector $b$ is in the column space of $\mathcal{A}$, which can efficiently be computed using least squares. Further, the use of vector representation and least squares opens new possibilities for understanding network behavior. For example, we are able to map rules, routing policies, what-if scenarios to the fundamental linear algebraic form, $\mathcal{A}x=b$, as well as determine how to configure forwarding tables appropriately. We show Vercel is faster than the state-of-art such as NetPlumber, Veriflow, APKeep, AP Verifier, when measured over diverse datasets. Vercel is almost as fast as Deltanet, when rules are verified in batches and provides better scalability, expressiveness and memory efficiency. A key highlight of Vercel is that while evaluating for reachability, the tool can incorporate intents, and transform these into auto-configurable table entries, implying a recommendation/correction system.

6. A Survey on Segment Routing with Emphasis on Use Cases in Large Provider Networks

Abstract

Segment routing is heralded as important technology innovation in large provider networks. In the domain of large telecom service providers, segment routing has the potential to be in the same league as MPLS and IPv6 as well as pave a way towards the successful implementation of SDNs. In this regard, this paper is a survey on the existing segment routing work in the community. We begin by describing how segment routing works, inclusive of the various building blocks. The paper describes the segment routing architecture in terms of its positioning vis-a-vis existing networking technologies, as well as the data-plane and the control plane. Label encoding techniques that are central towards implementing segment routing are then discussed. In order to make segment routing relevant to the current domain of providers, we postulate various use cases, their working and issues of interoperability. The paper also reviews the current set of segment routing implementation cases in the industry. Thereafter we position segment routing among the various provider manifestations.

Abstract

Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk. Among COVID-19-positive patients, those who were older, male, with renal or pulmonary disease had a significantly higher 30-day mortality risk.

27. Proximal Interphalangeal Joint Arthroplasty Using the Wide-Awake Local Anesthesia No Tourniquet Technique.

Abstract

Background: Proximal interphalangeal joint (PIPJ) osteoarthritis is a common condition that results in pain, stiffness, and loss of function in the affected hand. Proximal interphalangeal joint arthroplasty is an effective treatment option when conservative methods have failed. The wide-awake local anesthesia no tourniquet (WALANT) technique to perform surgery carries advantages such as lack of tourniquet discomfort, reduces the staffing and costs associated with anesthesia and sedation, and allows faster recovery. We aimed to determine whether the WALANT technique was safe and effective in the context of PIPJ arthroplasty. Methods: Patients were enrolled retrospectively from January 2015 to October 2020 by examining operating theater records and surgeon logbooks. Electronic patient records were examined to obtain patient data. Disabilities of the Arm, Shoulder, and Hand (DASH) questionnaires and Visual Analog Scale (VAS) for pain were sent by post to patients—with a separate DASH and VAS for each digit operated on. Results: Twenty-nine PIPJ arthroplasties were carried out using WALANT technique by 3 different surgeons all using the dorsal approach. All cases were successfully carried out as day-case procedures. There was a significant correlation with increasing VAS and increasing DASH score. Proximal interphalangeal joint arthroplasty improved range of motion from 28.9 ± 5.5° to 79.4 ± 13.3° (P <.0001). Two cases developed complications related to surgery. Conclusions: Our study is the first to report the use of WALANT to perform PIPJ arthroplasty, and shows comparable results with traditional methods. Larger, multicenter prospective trials are required to determine the efficacy of this technique and to quantify its economical benefit. [ABSTRACT FROM AUTHOR]

Abstract

Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk.

Abstract

Background: The aim of the study is to understand the association of Nucleotide excision repair (NER) inter‑genetic polymorphic combinations with overall survival (OS) in lung cancer as well as its histological subtypes in the North Indian population. Materials and Methods: Genotyping was performed using polymerase chain reaction‑restriction fragment length polymorphism. For survival analysis, the univariate Kaplan–Meier and multivariate Cox regression model were applied. Recursive partitioning method, survival analysis tree was applied to study unfavorable genotypic combinations in NER single‑nucleotide polymorphisms. Results: Combinatorial studies suggested no association between polymorphic combinations of NER genes and OS in lung cancer patients. When stratified as per lung cancer histological subtypes, adenocarcinomas patients with XPG 670 and XPC 499 polymorphism a significant increase in OS in combined heterozygous and mutant genotype with a lower hazardous ratio (Hazard Ratio¹ (HR) = 0.20; P = 0.004). Small‑cell lung carcinoma (SCLC) patients with XPF 11985A>G and XPD Arg156Arg polymorphism showed a 4‑fold hazard ratio among heterozygous genotype (HR¹ = 4.84; P = 0.007) where no significant results are obtained in patients suffering from squamous cell carcinoma histological subtypes. STREE showed XPG Asp1104His (W), XPD Lys751Gln (H + M), XPF Arg415Gln (H + M) genotype was associated with a lower hazard ratio (P = 0.0007) showing survival of 11.6 months when compared with the reference (median survival time = 3.52). Conclusion: It can be concluded that SCLC patients with polymorphic combinations of the NER pathway were associated with a higher risk of mortality. STREE depicted the association of NER polymorphic combinations with a lower hazard ratio predicting them to be a good prognostic factor for lung cancer. [ABSTRACT FROM AUTHOR]

Abstract

Radial flow turbines are extensively used in turbocharging technology due to their unique capability of handling a wide range of exhaust gas flow. The pulsating flow nature of the internal combustion engine exhaust gases causes unsteady operation of the turbine stage. This paper presents the impact of the pulsating flow of various characteristics on the performance of a radial flow turbine. A three-dimensional computational fluid dynamic model was coupled with a one-dimensional engine model to study the realistic pulsating flow. Applying square wave pulsating flow showed the highest degree of unsteadiness corresponding to 92.6% maximum mass flow accumulation due to the consecutive sudden changes of the mass flow rates over the entire pulse. Although saw-tooth showed a maximum mass flow accumulation value of 88.9%, the mass flow rates entailed gradual change resulted in the least overall mass flow accumulation over the entire pulse. These two extremes constrained the anticipated performance of the radial flow turbine operates under realistic pulsating flow. Such constraints could develop an operating envelop to predict the performance and optimize radial flow turbines' power extraction under pulsating flow conditions.

Abstract

Objectives: This study was planned to define the clinical profile of pulmonary tuberculosis (PTB) patients having renal amyloidosis, to identify the factors responsible for development of amyloidosis, to detect the time period between onset of amyloidosis and PTB, and analyze clinical features of amyloidosis in PTB patients for early diagnosis and timely assessment. Materials and Methods: Patients of PTB having pedal edema, proteinuria, and grossly diseased kidneys on ultrasound abdomen were subjected to renal biopsy and appropriate biochemical investigations. Clinical profile of biopsy proven amyloidosis cases was analyzed. Results: There were 43 patients (32 males, 11 females, age range 20-65 years) having PTB with pedal edema, proteinuria, and renal medical disease on abdominal ultrasound where amyloidosis was confirmed by renal biopsy. The total duration of illness ranged from two months to seven years (mean 2.25 years) and was less than five years in 93% patients. All patients had significant proteinuria. Nephrotic syndrome was seen in 23, hypertension in 19, hypoalbuminemia in 33, hypercholesterolemia in 29, and deranged renal functions in 32 patients. Ninety percent patients had moderate to far advanced pulmonary lesions on chest radiography with smear positivity in 21 patients. Conclusions: Renal amyloidosis is an important complication of PTB and should be suspected clinically in patients presenting with a triad of pedal edema, proteinuria, and medical renal disease on ultrasound. Contrary to general belief, renal amyloidosis may occur in PTB patients having disease for relatively shorter duration, and even if adequately treated.

Abstract

Drug-induced respiratory diseases are difficult to diagnose and therefore usually not identified, probably underestimated and under-reported. We report a case of diphenylhydantoin/phenytoin-induced chronic pulmonary disease in a 62-year-old male patient presenting with progressive dyspnea, eosinophilia, and pulmonary abnormalities. The importance of drug history in clinical history-taking and early diagnosis of drug-induced respiratory diseases is emphasized so as to prevent permanent pulmonary damage.

Abstract

The case of a 35-year old man who presented with massive left sided haemothorax as a complication of an asymtomatic pancreatic pseudocyst is descibed. The diagnosis was confirmed by very high amylase content of the pleural fluid. The complications of pancreatitis and pancreatic pseudocyst are also briefly discussed. Haemothorax represents an unusual pulmonary complication of pseudocyst pancreas and should be considered in the differential diagnosis of pleural fluid collection in pancreatitis.

Abstract

Adenoid cystic carcinoma (ACC) is an uncommon tumor that very rarely affects the lower respiratory tract and lung parenchyma. Fine needle aspiration cytology (FNAC) of primary pulmonary ACC has been rarely described. We report here a 32 year-old female with peripheral ACC, diagnosed by percutaneous transthoracic FNAC. The cytological diagnosis was further confirmed by histological examination of the biopsy specimen.
