18. Resection and Stump Stabilization in Giant Cell Tumor Distal Ulna: A Case Report

Abstract

Introduction: Giant cell tumors (GCTs) of distal ulna are extremely rare accounting for 0.45%–3.2% of all the cases of GCTs. These are locally aggressive and have a higher rate of recurrence of up to 40% with conservative modality of treatment. Proximity to carpus and diminished range of motion makes their treatment a challenge. Case Report: A 27-year-old male presented to us with swelling right distal ulna. X-ray and MRI were suggestive of GCT. The diagnosis was confirmed by core needle biopsy. The patient was managed by wide resection ulna with extensor carpi ulnaris tenodesis. Conclusion: GCT ulna although very rare in presentation is a possible diagnosis. Wide resection of ulna is a viable treatment option to achieve disease free status. Extensor carpi ulnaris tenodesis helps stabilization of ulnar stump.

19. Acetabulum Reconstruction with TantalumCup and Augments in Dysplastic Hip Type 4 using 3D Printing Technology

Abstract

Introduction: Acetabular reconstruction in a patient with neglected DDH during complex primary total hip replacement is a challenging procedure to do. We are reporting assessment of acetabular defect in one such patient with pre-operative 3D printing, followed by reconstruction with trabecular metal shell and augments. Case Report: A 32-year-old female with neglected DDH (Crowe’s type 4) reported to us with pain, limp, and shortening of affected limb. Operated somewhere else with THR, she had a missing acetabular component and coral stem in situ at the time of presentation. Acetabular defect was assessed by 3D printing of pelvis following which acetabular reconstruction with tantalum cup and augments along with femoral stem revision was done. Conclusion: This study confirmed the role of 3D printing pelvis model in meticulous pre-operative planning in patients with complex hip deformities. Reconstruction of acetabular defects with tantalum cup, and augments is a reasonable solution to achieve better function.

Abstract

Human resource capacity building is a key strategy in the design, delivery, sustainability and scale up HIV treatment and prevention programmes. The review aims to present human resource capacity building initiatives undertaken by the National AIDS Control Organisation (NACO) and to discuss the available opportunities in India.There was minimal emphasis on human resource capacity building in National AIDS control programme (NACP)-I. The focus of capacity building in NACP-II was on strengthening the capacity of partners implementing various HIV/AIDS interventions. NACP-III (2007-2012) focussed on capacity building as a priority agenda. Other than short-term training programmes, NACP-III is strengthening the capacity of partners through the State Training and Resource Centre, Technical Support Unit, District AIDS Prevention Control Unit, Fellowship Programme and Network of Indian Institutions for HIV/AIDS Research.Various opportunities to enhance and consolidate capacity building responses in HIV/AIDS in India may include mainstreaming of capacity building, appropriate management of knowledge and resources, effective delivery of training, measuring and documenting impact,accreditation of programmes and institutes,use of information technology, identifying and implementing innovations and working for sustainability.Growing demand for capacity-building in HIV/AIDS needs substantial efforts to ensure that these are implemented effectively and efficiently. NACO had made significant strides in these regards, but at the same time there are arduous challenges like measuring impact, quality, documentation, operational research, and sustainability. NACO is formulating Phase-IV of NACP. This review will provide feedback to the NACO for strengthening its strategic document for human resource capacity building.

Abstract

The success of cancer chemotherapy has increased greatly the potential areas for its inclusion in clinical trials and therefore has made the experimental design considerations more complex in these trials. New drugs which are launched yet go through the three classic phases of clinical study beginning with clinical pharmacology (Phase I), efficacy screening (Phase 2) and role delineation (Phase 3). The Phase 2 and 3 trials now need to be considered within the overall therapeutic strategies which are required for each of the many diseases which are called cancer. Drugs consumed in chemotherapy, such as cytarabine, work in different ways to stop the growth of cancer cells, either by killing the cells or stopping them from increase exponentially. may stop the growth of cancer cells by blocking some of the enzymes needed for cell growth. It is not yet known whether cytarabine is more effective in treating acute myeloid leukemia. The motive of this study is to assess the efficacious and eternal side effects of using combined tretinoin and arsenic trioxide in treating patients with newly detection of acute promyelocytic leukemia. Chemotherapy was identical, except for the addition of DTIC (300 mg/m2) for each course. A new drug may have required a lot of Phase 2 and 3 trials in a variety of tumors before it can be considered appropriately estimated. Essential aspects of valid clinical trials include an adequate protocol which details the study which is viable not for the current status but also for the future aspects. [ABSTRACT FROM AUTHOR]
