Background: Adolescents with intellectual disabilities are insufficiently physically active. Where interventions have been developed and delivered, these have had limited effectiveness, and often lack a theoretical underpinning. Aim: Through application of the COM-B model, our aim is to explore the factors influencing adolescent physical activity within schools. Methods: A qualitative methodology, using focus groups with students who have mild/moderate intellectual disabilities, their parents'/carers' and teachers'. The COM-B model provided the lens through which the data were collected and analysed. Results: We identified of a range of individual, interpersonal, and environmental factors influencing physical activity, across all six COM-B constructs, within the context of the 'school-system'. Conclusion: This is the first study to use the COM-B model to explore school-based physical activity behaviour, for adolescents with intellectual disabilities. Identification of such physical activity behavioural determinants can support the development of effective and sustainable interventions.

Background: Adolescent mental illness is of increasing concern, with prevalence increasing especially in females in the United Kingdom. While service-related barriers can deter help-seeking, the role of family support and engagement in family activities, and attitudes towards seeking help from a doctor has received little attention. Studies exploring help-seeking from family and medical practitioners have done so without incorporating theories of behaviour change. This study aims to develop a model of adolescent help-seeking using the Theory of Planned Behaviour (TPB). Method: A cross-sectional survey of 1639 male and females aged 13-17 years in Northern Ireland. Structural Equation Modelling determined fit for TBP factors, with the addition of gender, experience of mental health issues, and engagement in family activities. Results: The Structural Equation Model suggests an adequate description of the relationships between TPB variables [X[superscript 2] (639) = 8302.36, p < 0.001; RMSEA = 0.09; SRMR = 0.06; CFI = 0.92; IFI = 0.92], with significant variance (R[superscript 2) of up to 61% ("Intentions to Seek Help from Family") and 24% ("Intention to Seek Help from Medical Professionals") explained. Males (compared to females) reported a more supportive family life, fewer mental health problems, more trust in doctors, less prosocial behaviour, more confidence discussing mental health with a doctor, and greater intentions to seek family or medical professional help when having emotional/ mental health problems. Conclusions: When supporting adolescent's gender, engagement with family and trust in doctors should be considered, especially where they have experienced mental health issues and may be less willing to seek help.

Sport provides a significant role in the lives of athletes; however, both positive and negative mental health effects may occur from sporting experiences, including burnout and/or well-being. A cross-sectional survey was conducted including 685 athletes (Mage = 23.39, SD = 6.22, 71% = male), testing multiple, complementary, self-determination theory hypotheses linked to well-being, and burnout. A multistage modeling approach encompassing confirmatory factor and path analysis was utilized, with results showing significant variance explained for well-being (R2 =.30) and burnout (R2 =.35). Several direct effects were found in line with self-determination theory, including between; needs-support and needs-satisfaction (β = 0.48), and needs-control and needs-frustration (β = 0.44); needs-satisfaction and motivational orientation (β = 0.25); needs-satisfaction and well-being (β = 0.37), and needs frustration and burnout (β = 0.25); and motivational orientation and burnout (β = −0.27), and motivational orientation and well-being (β = 0.18). Indirect effects were found for well-being and burnout via coach needs-support, needs-satisfaction, and motivational orientation in sequence (β = 0.24 and β = −0.22, respectively), in addition to burnout via coach needs-control, needs frustration, and motivational orientation in sequence (β = −0.12). To conclude, coach-based, sporting mental health interventions that promote the utilization of needs-supportive behaviors, while also highlighting the need to minimize needs-controlling behaviors, are recommended for the prevention of burnout and the promotion of well-being in athletes. Aim: A recent mental health in sport consensus statement advocates Keyes' two continua model with an associated Mental Health Continuum (MHC) instrument to assess mental health in athletes. However, there remains statistically inconsistent usage of the MHC in athletes, so further exploration of the MHC's psychometric factors is required. Methods: Athletes (N = 1,097) aged 32.63 (SD = 11.16) comprising 603 females (55.7%) and 478 males (44.3%), completed the 14-item MHC-Short Form, alongside validated measures of anxiety and depression. Five confirmatory factor analytic and bifactor models were developed based on extant research and theory. Results: Overall, a bifactor structure with a "general" positive mental health factor, and three specific factors ("hedonic well-being," "social well-being," and "psychological well-being") fitted the data well and was deemed the superior model. Conclusion: A bifactor model of the MHC-Short Form is recommended comprising a composite score alongside specific factors of hedonic, social, and psychological well-being. [ABSTRACT FROM AUTHOR]

Research has shown that childhood physical activity participation has a positive relationship with markers of wellbeing, such as self-esteem and quality of life, and physical activity participation may serve as protective mechanism against some mental illnesses including depression. The aim of the current study was to examine the relationship between gender, physical activity, screen time, body mass index and wellbeing in Irish school children (N = 705; mean age: 8.74 ± 0.52 years) from social disadvantage. In Northern Ireland, schools included in the 2010 Multiple Deprivation Measure (NIMDM) were invited to participate. Schools included for participation in the Republic of Ireland were from the Delivering Equality of Opportunity in Schools (DEIS) index. Data gathered included accelerometry (physical activity), self-report (screen time and wellbeing), and anthropometric measurements. Physical activity was objectively measured during eight consecutive days using Actigraph GT1M and GT3X devices, using stringent accelerometer protocol. Screen time activities were derived using questions adapted from the Health Promotion Agencies National Children's Survey in Northern Ireland. The KIDSCREEN-27 is a health-related quality of life measurement, and this tool was used by participants to self-report their health and wellbeing. Results suggest that boys accumulated more minutes of daily screen time than girls, however, boys were more physically active when compared to girls. Wellbeing scores for gender showed inverse associations with daily screen time. Standard multiple regression revealed that gender, physical activity, screen time and body mass index (combined) explained little variance in the prediction of wellbeing. Results indicate the importance of gender-based considerations for physical activity and screen time with children from social disadvantage. The inverse relationship found between overall screen time and wellbeing will help guide future healthy lifestyle interventions for Irish children of low-income communities. [ABSTRACT FROM AUTHOR]

The Jefferson Scale of Empathy (JSE) is a broadly used instrument developed to measure empathy in the context of health professions education and patient care. Evidence in support of psychometrics of the JSE has been reported in health professions students and practitioners with the exception of osteopathic medical students. This study was designed to examine measurement properties, underlying components, and latent variable structure of the JSE in a nationwide sample of first-year matriculants at U.S. colleges of osteopathic medicine, and to develop a national norm table for the assessment of JSE scores. A web-based survey was administered at the beginning of the 2017-2018 academic year which included the JSE, a scale to detect "good impression" responses, and demographic/background information. Usable surveys were received from 6009 students enrolled in 41 college campuses (median response rate = 92%). The JSE mean score and standard deviation for the sample were 116.54 and 10.85, respectively. Item-total score correlations were positive and statistically significant (p < 0.01), and Cronbach a = 0.82. Significant gender differences were observed on the JSE scores in favor of women. Also, significant differences were found on item scores between top and bottom third scorers on the JSE. Three factors of Perspective Taking, Compassionate Care, and Walking in Patient's Shoes emerged in an exploratory factor analysis by using half of the sample. Results of confirmatory factor analysis with another half of the sample confirmed the 3-factor model. We also developed a national norm table which is the first to assess students' JSE scores against national data.

Participation in sport can paradoxically be a source of psychological needs satisfaction and psychological needs frustration. Self-determination theory was applied to explain temporal relationships of athletes' psychological needs satisfactions and psychological needs frustrations with burnout through a two-wave longitudinal study. Participants included 184 athletes (Mage = 24.04 years, SD = 5.56, 67.9% male) representing a range of competitive levels. A latent difference score model specifying longitudinal relationships between burnout and needs satisfactions and needs frustrations was tested. Significant within-variable changes were observed for all needs-satisfaction and needs-frustration variables. Longitudinal associations were found in Models 3 (autonomy frustration) and 6 (relatedness satisfaction). Higher burnout at baseline predicted an increase in autonomy frustration (β = 0.13, p <.05), whereas higher relatedness satisfaction at baseline reduced burnout levels later in the season (β = −0.22, p <.001). To conclude, continuous tracking of athlete burnout levels and fostering of needs-supportive climates that minimize autonomy-controlling behaviors are recommended for the burnout prevention in athletes. Need-supportive environments have been shown to predict children's physical activity levels, and in a few cases to well-being. Grounded in self-determination theory (SDT), this cross-sectional study aimed to determine the influence of psychological need (competence and social relatedness) satisfaction on physical activity levels and well-being in children from areas of social and economic disadvantage. A total of 211 children aged eight and nine years from areas of low socio-economic status wore an accelerometer for one week, and completed a questionnaire assessing psychological need satisfaction and well-being. Confirmatory factor analysis and path analysis were conducted to assess the factor structure of the measures, and to test for theoretical relationships between psychological needs, physical activity and well-being. The factor structure of the instruments was supported, and a significant positive relationship was found between athletic competence and physical activity (ß = 0.19). Athletic competence (ß = 0.19), along with parental relatedness (ß = 0.32), positively predicted children's well-being. Physical activity alone did not predict well-being. Based on these findings, practitioners may consider components of SDT, reflective of need-supportive environments, when designing physical activity interventions. Interventions aimed at supporting children's perceptions of competence, and the involvement of parents, may offer the opportunity to increase well-being.

Background Physical activity (PA) can reduce young peoples' risk of depressive symptoms. Associations between PA and depressive symptoms are often investigated over timeframes spanning minutes to weeks. Less is known about whether childhood/adolescent PA can predict depressive symptoms in early adulthood. Methods Using a nationally representative sample from Ireland, latent growth mixture modelling was performed to investigate the extent to which different PA trajectories existed from ages 9–17, whether gender, weight status, and socio-economic deprivation at age 9, predicted PA trajectories from ages 9–17, and whether trajectory class membership predicted depressive symptoms at age 20. Results A 4-class solution was the best fit to the data (AIC = 52 175.69; BIC = 52 302.69; ssaBIC = 52 245.49; entropy = 1.00). Classes were labelled according to their baseline PA and slope of their trajectory: 'High-Decreasers'; 'Moderate-Decreasers'; 'Moderate-Stable'; and 'Low-Increasers'. A negative linear association existed between activity trajectory and the likelihood class members were female, overweight or socioeconomically deprived at age 9. The most active class (High-Decreasers) were significantly less likely to report depressive symptoms at age 20 than other classes. Conclusions Multiple PA trajectories exist throughout childhood and adolescence although differences in PA levels reduced over time. The most/least active children continued to be the most/least active throughout adolescence. Those most active were least at risk of depressive symptoms in early adulthood. Being female, overweight or experiencing deprivation at age 9 were all risk factors for inactivity throughout adolescence. Basic psychological needs theory is limited by variable-centered studies focused on linear relationships between perceived needs-supportive/controlling coach behaviors. Therefore, latent profile analysis was used to determine if heterogenous profiles emerged from the interactive effects of needs-supportive and -controlling coach behaviors and the subsequent association with sport-specific mental health outcomes (i.e., burnout and subjective vitality). A total of 685 athletes took part (age = 23.39 years, male = 71%), and the latent profile analysis revealed five novel, diverse profiles, labeled as "supportive-developmental," "needs-indifferent," "overly critical," "harsh-controlling," and "distant-controlling" coaches. The profiles predicted significant mental health variance (adjusted R2 = .15-.24), wherein the "supportive-developmental" profile scored most favorably on 90% of the outcomes. The largest mean differences were observed against the "harsh-controlling" (n = 5), "overly critical" (n = 3), and "distant controlling" (n = 2) profiles. Overall, latent profile analysis revealed substantial nuance in athletes' social contexts, predicting variance in mental health. Recreational athletes comprise the most prevalent population using illegal Anabolic Androgenic Steroids (AAS). Despite regulatory efforts, substances are widely accessible, and most users report the experience of harmful side effects. It remains unclear why few users seek professional medical help. The aim of this study was to determine AAS users' experience of side effects and help-seeking beliefs using an Interpretative Phenomenological Analysis of six interviews. Participants were from the United Kingdom (n = 5) and United States (n = 1), had all experienced side effects, with some reporting prolonged use of AAS (>10 years) and self-manufacturing the drugs from raw ingredients. Results showed that AAS users discredit medical professionals' competencies, and practice cognitive dissonance by avoiding challenging situations. A microculture for information-sharing has developed among AAS users who initially self-treat to counteract side effects, leaving them vulnerable to further harm. To conclude, there is an urgent need for educational interventions that outline the risky practice of unregulated AAS use and self-treatments, and the need to seek professional help. This study explored the perspectives of people with learning disabilities on relationships and supports in the Republic of Ireland. A national research network consisting of 21 researchers with learning disabilities, 12 supporters, and 7 university researchers conducted the study. Researchers with learning disabilities and their supporters ran 16 focus groups with peers with learning disabilities. A total of 97 adults with learning disabilities from across the country took part in the focus groups. Findings suggest that people with learning disabilities have a diversity of experiences and views on relationships and support needed to keep them. People with learning disabilities taking part in the focus groups identified that they need more support from friends, family, and services staff to develop new relationships and keep their existing ones. This support includes both emotional and systematic changes such as accessible transport, own housing, and changed laws. The article discusses how the research team used the findings to advocate for a change in the laws concerning sexuality and learning disability in the Republic of Ireland.
