Abstract

BackgroundThe surrounding environment influences the constitution of illness perceptions. Therefore, local research is needed to examine how Libyan diabetes patients perceive diabetes and how their perceptions influence their medication adherence.MethodsA cross-sectional study was conducted at the National Centre for Diabetes and Endocrinology in Tripoli, Libya, between October and December 2013. A total of 523 patients with type 2 diabetes participated in this study. A self-administered questionnaire was used for data collection; this included the Revised Illness Perception Questionnaire and the eight-item Morisky Medication Adherence Scale.ResultsThe respondents showed moderately high personal control and treatment control perceptions and a moderate consequences perception. They reported a high perception of diabetes timeline as chronic and a moderate perception of the diabetes course as unstable. The most commonly perceived cause of diabetes was Allah's will. The prevalence of low medication adherence was 36.1%. The identified significant predictors of low medication adherence were the low treatment control perception (p=0.044), high diabetes identity perception (p=0.008), being male (p=0.026), and employed (p=0.008).ConclusionDiabetes illness perceptions of type 2 diabetic Libyans play a role in guiding the medication adherence and could be considered in the development of medication adherence promotion plans.

7. Exploratory analysis of traditional risk factors of ischemic heart disease (IHD) among predominantly Malay Malaysian women

Abstract

Abstract Background The risk factors of ischemic heart disease (IHD) specific for women are less well studied. However, knowing the risk factors of IHD for women will empower women themselves to be better informed and thus can help them in decision making concerning their health condition. The objective of this study is to explore the commonly studied risk factors of ischemic heart disease (IHD) among a group of Malaysian women. Methods A case control study was conducted among 142 newly diagnosed IHD women patients registered in government hospitals in Terengganu, Malaysia and their 1:1 frequency matched population controls. Data on sociodemographic and socioeconomic profile, co-morbidities, lifestyle factors related to physical activities, dietary fat intake, stress, passive smoking history, anthropometric measurements and biochemical markers were obtained. Results Middle aged women were recruited with women diagnosed with diabetes (aOR = 1.92, 95% CI: 1.11–3.31), having low HDL-C (aOR = 3.30, 95% CI: 1.28–8.27), those with positive family history of IHD (aOR = 1.92, 95% CI:1.13–3.26) and passive smokers (aOR = 2.99, 95% CI:1.81–4.94) were at higher odds of IHD. Conclusions The findings are useful for public health interventions and policy making focusing on specific women population.

Abstract

Sugar-sweetened beverages (SSBs) are associated with cardiometabolic diseases and social inequities. For most nations, recent estimates and trends of intake are not available; nor variation by education or urbanicity. We investigated SSB intakes among adults between 1990 and 2018 in 185 countries, stratified subnationally by age, sex, education, and rural/urban residence, using data from the Global Dietary Database. In 2018, mean global SSB intake was 2.7 (8 oz = 248 grams) servings/week (95% UI 2.5-2.9) (range: 0.7 (0.5-1.1) in South Asia to 7.8 (7.1-8.6) in Latin America/Caribbean). Intakes were higher in male vs. female, younger vs. older, more vs. less educated, and urban vs. rural adults. Variations by education and urbanicity were largest in Sub-Saharan Africa. Between 1990 and 2018, SSB intakes increased by +0.37 (+0.29, +0.47), with the largest increase in Sub-Saharan Africa. These findings inform intervention, surveillance, and policy actions worldwide, highlighting the growing problem of SSBs for public health in Sub-Saharan Africa.

Abstract

Animal-source foods (ASF) provide nutrition for children and adolescents’ physical and cognitive development. Here, we use data from the Global Dietary Database and Bayesian hierarchical models to quantify global, regional and national ASF intakes between 1990 and 2018 by age group across 185 countries, representing 93% of the world’s child population. Mean ASF intake was 1.9 servings per day, representing 16% of children consuming at least three daily servings. Abstract

The global burden of diet-attributable type 2 diabetes (T2D) is not well established. This risk assessment model estimated T2D incidence among adults attributable to direct and body weight-mediated effects of 11 dietary factors in 184 countries in 1990 and 2018. In 2018, suboptimal intake of these dietary factors was estimated to be attributable to 14.1 million (95% uncertainty interval (UI), 13.8–14.4 million) incident T2D cases, representing 70.3% (68.8–71.8%) of new cases globally. Largest T2D burdens were attributable to insufficient whole-grain intake (26.1% (25.0–27.1%)), excess refined rice and wheat intake (24.6% (22.3–27.2%)) and excess processed meat intake (20.3% (18.3–23.5%)). Across regions, highest proportional burdens were in central and eastern Europe and central Asia (85.6% (83.4–87.7%)) and Latin America and the Caribbean (81.8% (80.1–83.4%)); and lowest proportional burdens were in South Asia (55.4% (52.1–60.7%)). Proportions of diet-attributable T2D were generally larger in men than in women and were inversely correlated with age. Diet-attributable T2D was generally larger among urban versus rural residents and higher versus lower educated individuals, except in high-income countries, central and eastern Europe and central Asia, where burdens were larger in rural residents and in lower educated individuals. Compared with 1990, global diet-attributable T2D increased by 2.6 absolute percentage points (8.6 million more cases) in 2018, with variation in these trends by world region and dietary factor. Abstract

A study was conducted with 154 Malaysian medical students to assess the extent to which their attitudes and knowledge about smoking changed over the course of their training. The results showed that women's knowledge and attitudes had improved. Male non-smokers knowledge improved but not their attitude while smokers did not improve on either scale. Women and male non-smokers improved their knowledge and attitudes about environmental tobacco but smokers did not. Attitudes were influenced by peers but not by family or teachers.

Abstract

This study examined the family context for knowledge, attitudes and efforts regarding exposure to environmental tobacco smoke (i.e. passive smoking) among Malaysian medical students. Women, non-smokers, and never smokers scored better on all three scales. Students whose brothers did not smoke scored better on knowledge and attitude scales. Although knowledge, attitudes and effort scores were not higher among those whose friends did not smoke, there were consistent correlations between those scores and friends' pressures not to smoke, particularly for men and non-smokers. Friends' pressures not to smoke were correlated only with attitude scores among smokers (the more the pressure from friends, the better the attitude about protecting others from exposure to environmental tobacco smoke.) Women, men and non-smokers received consistent pressures from others not to smoke. That is, for these three groups, where a family member or friend admonished the student not to smoke, there was also likely to be pressure from others. This was not the case for smokers. There was a consistent pattern of correlations between knowledge, attitudes and efforts for men, women and non-smokers, but there was a link only between attitudes and efforts among smokers. So even among these who smoked, pro-health attitudes were linked with pro-health efforts. Of special interest were the relationships between the family context and the extent to which medical students felt a commitment to inform patients about the hazards of environmental cigarette smoke. Never smokers and those whose brothers did not smoke had better scores on the patient responsibility to inform variable. Scores on this variable were correlated with friends' but not other family members' pressures not to smoke. While physician responsibility to inform variable was related to scores on knowledge, attitudes and efforts for the general sample, and for men, women and non-smokers, this was not true for smokers. Abstract

Introduction The socioeconomic impact of aging population can be reduced if majority of people achieve successful aging (SA). SA is having good health along with opportunities for participation and security as people age. This study aimed to determine the prevalence of researcher's defined successful aging (RDSA) and self-rated successful aging (SRSA) among pre-retirement public servants and their predictors. Methods The sample included 1,064 pre-retirement public servants (50 to 60 years old) from nine government agencies. Data was analyzed using Multiple Logistic Regression to test for the association between the studied factors and SA. Foreigner and those on long medical leave were excluded from this study. Results The prevalence of RDSA and SRSA was 37.5% and 98.7%, respectively. Results showed four (4) significant factors with higher odds of having RDSA were not obese, good social support, being physically active and younger age. Meanwhile, five (5) factors highly selected by respondents as predictors for SRSA were having good spiritual or religious practice, happy family, good psycho cognitive function, social support and good physical function. Conclusions The prevalence of SRSA was higher despite the presence of self-reported chronic diseases and physical limitation among respondents. The discrepancy in both prevalence of SA reflects the differences that exist between the criteria for SA perceived by respondents and researchers. Misperception among respondents of their aging process as 'successful' despite having disease may worsen their health status because they continue practicing unhealthy lifestyles without action to improve it. Abstract

Abstract Background This cross-sectional study assesses factors that contribute to discontinuing breastfeeding among employed mothers in Malaysia. Methods A structured questionnaire was used in conducting this study involving all government health clinics in Petaling district between July and September 2006. Respondents were Malaysian women with children between the ages of six to twelve months who were formally employed. Factors studied were selected socio-demographic and work-related characteristics. Results From a total of 290 respondents, 51% discontinued breastfeeding. The majority (54%) of mothers who discontinued breastfeeding had breastfed their babies for less than three months. Compared to Malay mothers, the risk of breastfeeding discontinuation were higher among Chinese (AOR 3.7, 95% CI: 1.7, 7.8) and Indian mothers (AOR 7.3, 95% CI 1.9, 27.4). Not having adequate breastfeeding facilities at the workplace was also a risk factor for breastfeeding discontinuation (AOR 1.8, 95% CI: 1.05, 3.1). Conclusion It is important that workplaces provide adequate breastfeeding facilities such as a room in which to express breast milk and a refrigerator, and allow mothers flexible time to express breast milk.

34. Influence of an intervention program promoting voluntary fasting practices and its perceived barriers among overweight or obese Muslim women working in the public sector, Malaysia

Abstract

Introduction: This article aims to describe the effect of an intervention to promote the practice of voluntary Islamic fasting and its barriers among a group of overweight and obese Muslim women working in the public sector in Malaysia. Materials and methods: In this quasi-experimental study, an intervention to encourage voluntary fasting was delivered in a half day seminar, supported by relevant booklets prepared. A self-administered questionnaire was used to capture data on voluntary fasting practices and its perceived barriers before and four months after the intervention. Data was analysed using Mc Nemar test to compare the proportion before and after the intervention. Significant level was set at p<0.05. Results: A total of 56 women were recruited. Their mean age was 36.65 years, most were married, had formal education up to secondary level and median income of RM 3000 per month. At baseline, 60.7% of them have practiced voluntary fasting. After intervention, the number of participants who practices voluntary fasting increased with a significant increase in three types of voluntary fasting (i.e. Mondays only, Thursdays only and 6 days in Shawal). The highest barrier to practice voluntary fasting was having no motivation to do so, followed by work commitment, health problems and family commitment. Discussion and Conclusion: Most of the respondents could practice voluntary fasting regularly if motivated to do so and backed by social support both at work and at home.

Methodology: This was a cross-sectional study using data from the electronic birth census. The independent variables were mothers’ age and height, parity, birth weight and birth length. All term newborns, both alive and stillbirth, with 37–41 completed gestational weeks, and a birth weight of at least 500 g was extracted from the census. Results: A total of 26,503 newborns fulfilled the inclusion criteria (13,655 males, 12,840 females). The mean head circumferences for male and female newborns were 32.93 cm (± SD 1.32) and 32.56 cm (± SD 1.31). The average head circumference for Malaysian newborns was found to be smaller than the World Health Organization Standard Growth Chart for Term Infant. A total of 17.6% (n=4,669) of the total samples were observed to have microcephaly. Among them, 73.2% (n=3,419) were non-proportionate microcephaly with normal birth weight of 2.5kg and above. Bivariate analyses showed that all independent variables were significant predictors of microcephaly. Both simple and multiple logistic regressions demonstrated that low birth weight was the most significant predictors for microcephaly (adjusted OR 12.14, 95% CI 10.80, 13.65). Conclusion: There is an increasing trend of microcephaly across the years and the low birth weight was noted as the main predictor of microcephaly. The respondent’s level of physical activity prior to Ramadan was determined by using the metabolic equivalent score (METs) calculated by using the international physical activity questionnaire (IPAQ). METs score >600 were categorized as ‘high to moderate level’ while METs score ≤600 as ‘low level’ physical activity. BMI, blood pressure and blood biochemical levels were measured before and during Ramadan. Results: 140 respondents were recruited and 57.14% were in the ‘high to moderate level’ physical activity group. At baseline systolic and diastolic pressure, TC, LDL-C, TG, FBS, TC/HDL-C ratio was lower but HDL-C was higher in the ‘high to moderate level’ physical activity group. During Ramadan, HDL-C reduced and TC/HDL-C ratio increased significantly in both groups (P<0.01), TG reduced significantly only in the ‘high to moderate level’ physical activity group (P=0.02) while FBS reduced significantly only in the ‘low level’ physical activity group (P=0.04). Comparing changes between the two groups showed no significant difference. Conclusions: The positive blood biochemical changes during Ramadan were more prominent in the group who were more physically active prior to Ramadan.

44. Food consumption among overweight and obese working Malay women in urban settings

Abstract

Background: The Malaysian Dietary Guideline 2010 (MDG) helps Malaysian to objectively estimate their food consumption. This study looks into the type of food consumed in a group of overweight and obese Malays women and the influences of their socio demographic characteristics. Methods:This cross-sectional study was conducted among 140 Malay women with body mass index (BMI) ≥25.0 kg/m2, working in public offices in two urban areas in Malaysia. Their food consumption was monitored using a 7-day self-administered food diary developed based on MDG 2010. The mean numbers of servings were compared to the servings recommended in the MDG 2010 for women who were not physically active, and then the association with socio demographic characteristics was tested using independent t test. Results: Their food consumption compared to MDG recommendations were as follows: 79.8% for carbohydrate, 24.8% for vegetables and fruits, 112.5.0% for protein and 124.0% for milk and dairy products. The consumption of fruits and vegetables was higher in the higher education group (P=0.03), of protein was higher in the high income group (P=0.02) and of milk and dairy product was higher in the younger age group (≤40 year old) (P=0.05). Conclusions:Their consumption of protein and dairy product was higher but the consumption of carbohydrate and vegetables and fruits, was lower than the recommended quantity. The type of food consumption was influenced by respondents’ education level, income and age group.

Abstract

Parenting behaviour is culturally sensitive. The aims of this study were (1) to translate the Parental Bonding Instrument into Malay (PBI-M) and (2) to determine its factorial structure and validity among the Malaysian population.The PBI-M was generated from a standard translation process and comprehension testing. The validation study of the PBI-M was administered to 248 college students aged 18 to 22 years.Participants in the comprehension testing had difficulty understanding negative items. Five translated double negative items were replaced with five positive items with similar meanings. Exploratory factor analysis showed a three-factor model for the PBI-M with acceptable reliability. Four negative items (items 3, 4, 8, and 16) and item 19 were omitted from the final PBI-M list because of incorrect placement or low factor loading (0.32). Out of the final 20 items of the PBI-M, there were 10 items for the care factor, five items for the autonomy factor and five items for the overprotection factor. All the items loaded positively on their respective factors.The Malaysian population favoured positive items in answering questions. The PBI-M confirmed the three-factor model that consisted of care, autonomy and overprotection. The PBI-M is a valid and reliable instrument to assess the Malaysian parenting style. Confirmatory factor analysis may further support this finding.Malaysia, parenting, questionnaire, validity.
