4. Allogeneic Stem Cell Transplant in Hematological Disorders: A Decade of Experience.

Abstract

Background: Allogeneic hematopoietic cell transplantation (allo-HCT) is a complex procedure with the potential to provide curative treatment for various hematological disorders. This study aims to evaluate the outcomes of allo-HCT in hematological diseases and identify significant complications in a single-center setting. Materials and Methods: We conducted a retrospective analysis of 180 patients with hematological diseases who underwent allo-HCT between January 2011 and December 2021. Key outcomes, including indications for transplantation, overall survival, engraftment time, relapse rates, graft-versus-host disease (GVHD), and transplant-related mortality (TRM) were assessed. Results: The most common indications for allo-HCT were benign hematological diseases, particularly aplastic anemia, and thalassemia major. Despite the majority of patients receiving fully matched transplants, acute GVHD was observed in 30% of the cohort. Graft failure occurred in 13 patients, with primary and secondary graft failure rates of 1.6% and 5.5%, respectively. Sepsis emerged as the primary cause of non-relapsed mortality at day 100 and beyond. The overall survival rate in this study was 62%, with 79% of patients disease-free on their last visit. Conclusion: This study provides valuable insights into the treatment strategies and patient care of allo-HCT for hematological disorders by offering a comprehensive overview of multiple relevant outcomes. The findings underscore the significance of addressing complications and risk factors associated with allogeneic transplantation, including GVHD and infections. Abstract

The future energy sector is expected to witness a substantial increase in the penetration of photovoltaics (PVs) and electric vehicles (EVs) due to the significant reduction of energy costs and emissions. While existing research studies have focused on assessing the individual impacts of grid integration of PVs and EVs, there's a notable lack of focus on comprehensively assessing the combined impacts of these technologies. To address this gap, this study offers an extensive review of how the large integration of PVs and EVs collectively will influence the future power system. For better understanding, first the individual impacts of PVs and EVs on power grid infrastructure are presented thoroughly, followed by the combined impact of PVs and EVs on grid stability, power quality, and energy economics. It has been observed that the combined impact of these technologies differs from their individual effects. Notably, the coordinated operation of PVs and EVs is key in addressing the discrepancies caused by their individual integration. Based on the literature reviewed and practical insights, this paper proposes a multifaceted perspective to consider techno-economic and environmental aspects for designing future power systems. The study concludes by offering recommendations and outlining potential directions for future research. The outcomes drawn in this paper will offer invaluable insights for policymakers and stakeholders as they work towards optimizing the integration of PVs and EVs into the evolving energy landscape. Abstract

Given the significant impact of the UN 2015 agenda on sustainable development, the search for eco-friendly and efficient energy solutions has intensified. The renewable based hybrid systems have emerged as feasible solution in this context. This paper explores the potential of utilizing a hybrid energy system (HES) from a distinctive perspective — addressing the challenges of load shedding at the distribution level. The HES configuration in this investigation includes photovoltaic (PV) array, wind turbines (WT), battery storage unit (BSU) and diesel generator system. The research involves simulations, optimization, and sensitivity analysis for a residential community in the southwestern part of Pakistan, which frequently experiences load shedding. Grasshopper optimization algorithm (GOA) is applied to simultaneously minimize the levelized electricity cost (LEC), payback period (PBP), and loss of power supply probability (LPSP). Simulations compare different HES configurations based on the estimation of local renewable energy potential and the load shedding schedule of the utility company. The optimal solution, with NPV =110, NWT =2, and NBSU =16, was selected, resulting in a minimum LEC of 6.64 cents/kWh and a PBP of 7.4 years. These results are validated using the particle swarm optimization algorithm (PSO). The performance of HES is benchmarked against conventional solutions, such as standalone diesel generators, battery-based uninterruptible power supplies (UPS), and combinations of generators and UPS. The findings demonstrate the significant superiority of HES over conventional solutions in terms of reduced LEC, shorter PBP and reduced carbon emissions. Finally, the sensitivity analysis examines the impact of varying component prices, feed-in tariff rates, meteorological conditions, and load demand variations on the LEC and PBP of the HES.

Abstract

Waldenström′s Macroglobulinemia (WM) is a B cell neoplasm characterized by infiltration of the bone marrow by a lymphoplasmacytic infiltrate and an IgM monoclonal gammopathy. We report a 15-year review of patients diagnosed with WM at our center. A total of 18 patients were diagnosed and treated at our center during the study period. Neurological symptoms were seen in almost 95% while B symptoms were present in almost 80% of patients. More than two-thirds of patients were anemic at the time of presentation and more than 90% showed bone marrow infiltration with lymphoplasmacytoid cells. Anemia, B symptoms, splenomegaly and neurological symptoms were the primary reasons in the majority of patients to initiate treatment. Chlorambucil was the primary treatment in more than half the patients followed by CVP. The median overall survival in all patients was 29 months (range 22-81 months). WM is a rare disorder and novel therapeutic modalities need to be identified to improve survival in these patients.

Abstract

Solubility is a vital factor for devloping drug delivery systems for poorly water soluble drugs. Several conventional approaches for enhancement of solubility have limited applicability, especially when the drugs are poorly water soluble. Nanosuspension technology can be used to enhance the solubilty, stability as well as the bioavailability of poorly water soluble drugs. Nanosuspensions are biphasic systems comperising of pure drug particles dispersed in an aqueous vehicle, stabilized by surfac active agents. Fabrication of nanosuspension is simple and more advantageous than other approaches. Techniques like high-pressure homogenization, wet milling, emulsification, solvent evaporation, bottom up technology and top down technology have been applicable in the fabrication of nanosuspensions. Nanosuspension delivery is possible by several routes, such as oral, pulmonary, parenteral and ocular routes. Nanosuspension not only solves solubility and bioavailability issue, but improve drug safety and efficacy. In this context, we reviewed the current techniques used to develop nanosuspensions and their recents studies application in drug delivery system.
