Extrapulmonary tuberculosis (TB) represents 10%–15% of TB cases, with lymph nodes being the most frequent site in India. Genital TB is uncommon, and testicular TB is rarer, still accounting for only 3% of genital TB cases. Isolated testicular TB is extremely rare and often mimics other testicular conditions, such as malignancy, complicating diagnosis. We present a case involving a 49-year-old male with diabetes and a history of smoking, who experienced bilateral scrotal swelling and pain over 3 months, accompanied by weight loss. He had no history of respiratory complaints, fever, TB, or significant medical history. Physical examination revealed bilateral scrotal swelling (3 cm × 3 cm) and multiple inguinal lymph nodes. Scrotal ultrasound demonstrated bilateral heterogeneously hypoechoic masses, while magnetic resonance imaging of the scrotum and testis identified solid cystic lesions with heterogeneous enhancement. Serum alpha-fetoprotein and lactate dehydrogenase levels were within normal limits. Fluorodeoxyglucose positron emission tomography–computed tomography revealed metabolically active lesions in both testes and several lymph nodes. Chest X-ray was normal. With a strong suspicion of malignancy, the patient underwent bilateral high inguinal orchiectomy. Histopathological analysis of the excised tissue showed dense epithelioid granulomas with caseous necrosis, indicative of TB. The patient was commenced on antitubercular therapy, with plans for continued monitoring. Although rare, isolated testicular TB should be included in the differential diagnosis of testicular masses. Prompt recognition and appropriate treatment are essential to prevent unnecessary surgical procedures and ensure optimal management.

Skeletal tuberculosis (TB) is a type of extrapulmonary TB affecting bony structures. Surprisingly, rib involvement is rare, comprising only 0%–5% of bone TB cases. Multifocal tuberculous osteomyelitis of the rib is a very rare condition, affecting only 0.1% of individuals with TB. We present here the case of a 28-year-old male who is an accountant in a travel agency with no comorbidities presented with complaints of cough with expectoration and swelling over the left posterolateral chest wall for 3 months’ duration. Imaging by computed tomography and magnetic resonance imaging of the chest showed features of multifocal tuberculous osteomyelitis of ribs with active pulmonary TB. Pus was aspirated from chest wall swelling detected mycobacterium TB in Catridge based nucleic acid amplification test, and the patient was started on antitubercular therapy. This case emphasizes skeletal TB, despite being the prevalent form of extrapulmonary TB, and presents an intriguing aspect with rib involvement remaining a relatively rare phenomenon.

Introduction: Mobile health (mHealth) technologies, including smartphone apps and wearables, have improved health care by providing innovative solutions for monitoring, education and treatment, particularly in mental health. Method: This review synthesises findings from a series of reviews on mHealth interventions in psychiatry. Publications were systematically searched in PubMed, MEDLINE, PsycINFO, ScienceDirect, Scopus, Web of Science and Cochrane Library. Results: Out of 2147 records, 111 studies from 2014 to 2024 focusing on anxiety and depression were included. These studies highlight the effectiveness of mHealth interventions in reducing symptoms through cognitive–behavioural therapy, mindfulness and psychoeducation, benefitting adolescents, perinatal women and marginalised groups. Additionally, mHealth shows promise in managing substance use disorders and severe mental illnesses like schizophrenia, bipolar disorder and psychosis. Conclusion: Despite positive outcomes, challenges such as data privacy, user engagement and healthcare integration persist. Further robust trials and evidence-based research are needed to validate the efficacy of mHealth technologies.

Abstract The global crisis of antimicrobial resistance (AMR) necessitates the development of broad-spectrum antibacterial drugs effective against multi-drug resistant (MDR) pathogens. BWC0977, a Novel Bacterial Topoisomerase Inhibitor (NBTI) selectively inhibits bacterial DNA replication via inhibition of DNA gyrase and topoisomerase IV. BWC0977 exhibited a minimum inhibitory concentration (MIC90) of 0.03–2 µg/mL against a global panel of MDR Gram-negative bacteria including Enterobacterales and non-fermenters, Gram-positive bacteria, anaerobes and biothreat pathogens. BWC0977 retains activity against isolates resistant to fluoroquinolones (FQs), carbapenems and colistin and demonstrates efficacy against multiple pathogens in two rodent species with significantly higher drug levels in the epithelial lining fluid of infected lungs. In healthy volunteers, single-ascending doses of BWC0977 administered intravenously ( https://clinicaltrials.gov/study/NCT05088421 ) was found to be safe, well tolerated (primary endpoint) and achieved dose-proportional exposures (secondary endpoint) consistent with modelled data from preclinical studies. Here, we show that BWC0977 has the potential to treat a range of critical-care infections including MDR bacterial pneumonias.

Background: Artificial Intelligence (AI) can assist healthcare professionals with various aspects of patient care and healthcare systems. This systematic review aims to assess the role of AI in healthcare today, emphasizing how it may impact medical diagnosis and physiotherapy treatment. Methods: The electronic databases searched included PubMed, Scopus, Embase, and Web of Science. A thorough search of relevant literature published in peer-reviewed journals is part of the review, focusing on research examining AI's application in clinical trials, medical imaging, drug development, physiotherapy, and disease management. The search initially identified 255 studies. After screening, 30 studies met the inclusion criteria. Results: This review provides evidence that AI systems can help with these aspects of medical diagnosis and therapy, especially in medical imaging, where they can precisely detect anomalies and lesions in images. However, the review also highlights the possible drawbacks and restrictions of using AI in healthcare. These include the risk that AI algorithms may exacerbate pre-existing health disparities and the necessity of ensuring that AI is implemented fairly, safely, and effectively for every patient. Conclusion: AI systems can assist with disease management, medication development, medical imaging, clinical trials, medical diagnosis, and physiotherapy. The assessment finds that caution must be exercised in their application to ensure that potential advantages are maximized while possible dangers and drawbacks are minimized.

AbstractThis article attempts to explain how the parliamentary left interacted with the social terrain of Kerala, which has a strong presence of organized social pressure groups. Kerala has a unique political landscape where radical left politics coexist with reactionary community forces. The early twentieth-century social reform movement has the credit of producing two parallel but antagonistic traditions that influenced modern Kerala politics more or less equally. The scholars have been polarised into two groups to explain what dominates Kerala politics, class, or caste. This work explicates these complexities by analyzing how the Communist Party engaged with the communities with the help of the literature on the democratic approach to social diversity. It tracks the evolution of the communist movement in Kerala in establishing and broadening its social base. It employs primary and secondary resources to examine the subject in question thoroughly. The primary resources used in the article are interviews with significant party leaders and ideologues. In doing so, this work argues that the Communist Party carefully balanced the ideological commitment to an integrationist approach to diversity on the one hand and a practical requirement for social accommodation and recognition on the other hand.

Thermo hydraulic analysis of nano-fluid flow phenomena through backward-facing channel has currently gained attention of the researchers, and is applied in a variety of engineering applications. Hydro-thermal properties of water/FMWCNT nano-fluid flow in a backward-facing channel embedded with an obstacle under uniform heat flux have been studied numerically with the variations in the values of Reynolds numbers [Formula: see text] weight percentage of carbon nano-particle [Formula: see text] and differently shaped obstacles. The governing equations (continuity, x-momentum, y-momentum, and energy) have been solved using the finite volume approach, and fluent software has been used to visualize the simulation results. Impact of various forms of obstacle (plane, elliptical, and triangular), the ratio of obstacle height to width (A/B), the ratio of obstacle pitch to width [Formula: see text], [Formula: see text], and the values of [Formula: see text] on the different rheological behaviour of nano-fluid flow have been investigated. Medical and dental professionals should continue to adhere to preventive measures after COVID-19 vaccination due to their increased risk of exposure to the virus, particularly as new variants emerge that may heighten their risk perception and susceptibility. Therefore, this study aimed to explore the effects of COVID-19 vaccination on complacency to adherence to COVID-19 preventive behavioral measures and mental health among medical and dental professionals. In this cross-sectional study 410 medical and dental professionals were recruited from different medical and dental hospitals in Islamabad, Pakistan. The data was collected using a valid and reliable questionnaire comprising of three sections (socio-demographic, information of preventive behaviors performance against COVID-19 after vaccination, mental health status). A chi-square test and ordinal logistic regression were used for analysis. ObjectiveThe objective of the study is to compare and investigate the combined and individual effects of workstation ergonomics, physiotherapy and patient education in improving CgH headaches and work ability in office workers.Methods96 eligible CgH participants were divided into the ergonomics modifications group (EMG; n = 24), physiotherapy group (PTG; n = 24), and ergonomics modifications combined with physiotherapy group (EPG; n = 24) and education control group (CNG; n = 24), the participants received the respective treatment for 4 weeks. Primary (CgH frequency) and secondary (CgH pain intensity, CgH disability, flexion rotation test (right and left), neck disability index and work ability) scores were measured. The effects of treatment at various intervals were analyzed with a 4 × 4 linear mixed model analysis (LMM) between treatment groups and time intervals.ResultsFour weeks following training EPG group showed more significant changes in primary outcome CgH frequency; 4.6 CI 95% 3.63 to 5.56 when compare to control group. The same gradual improvement was noticed at 8 weeks 8.2 CI 95% 7.53 to 8.86 and at 6 months follow up 11.9 CI 95% 11.25 to 12.54 when compare to other groups (p = 0.001) which is statistically 52.97% improvement. Abstract Several population-level studies have described individual clinical risk factors associated with suboptimal antibody responses following COVID-19 vaccination, but none have examined multimorbidity. Others have shown that suboptimal post-vaccination responses offer reduced protection to subsequent SARS-CoV-2 infection; however, the level of protection from COVID-19 hospitalisation/death remains unconfirmed. We use national Scottish datasets to investigate the association between multimorbidity and testing antibody-negative, examining the correlation between antibody levels and subsequent COVID-19 hospitalisation/death among double-vaccinated individuals. We found that individuals with multimorbidity ( ≥ five conditions) were more likely to test antibody-negative post-vaccination and 13.37 [6.05–29.53] times more likely to be hospitalised/die from COVID-19 than individuals without conditions. We also show a dose-dependent association between post-vaccination antibody levels and COVID-19 hospitalisation or death, with those with undetectable antibody levels at a significantly higher risk (HR 9.21 [95% CI 4.63–18.29]) of these serious outcomes compared to those with high antibody levels.

Early kidney stone detection is essential for the diagnosis and treatment of people who have kidney stones. The objective of this study is to employ deep learning algorithms for renal stone detection, addressing the critical need for early, accurate diagnosis, which can significantly improve patient outcomes and reduce healthcare costs. The paper thoroughly assesses a variety of models, including ResNet, DenseNet, and EfficientNet, for CT images. The limitations of manual identification procedures highlight the urgent need for a more effective automated approach, making this research necessary. Notably, the painstakingly improved DenseNet model achieves a peak accuracy of 0.86, demonstrating its potential superiority. These results convincingly demonstrate the revolutionary power of deep learning, which is poised to revolutionise the detection of renal stones. This fast, trustworthy, and non-invasive method has the potential to advance clinical procedures and significantly improve patient care.

OBJECTIVES: In the context of acute hypoxemic respiratory failure (AHRF), ensuring effective preoxygenation and apneic oxygenation emerges as the pivotal approach ensuring for averting hypoxemic adverse events during endotracheal intubation. To investigate this, we conducted an open-label randomized controlled trial, aiming to assess the comparative effectiveness of nasopharyngeal high-flow oxygenation in conjunction with Bag-Valve-Mask (BVM) versus standard BVM preoxygenation in patients experiencing AHRF within the emergency department (ED). METHODS: This prospective single-center, open-labeled, randomized controlled trial enrolled patients aged 18 years and above requiring rapid sequence intubation due to AHRF in the ED. Participants were randomly assigned in a 1:1 ratio to either the intervention arm (involving nasopharyngeal high-flow oxygenation and BVM preoxygenation) or the control arm (involving BVM preoxygenation alone). RESULTS: A total of 76 participants were enrolled in the study, evenly distributed with 38 individuals in each arm. Median (interquartile range [IQR]) SpO2 at 0 min postintubation was 95.5 (80%–99%) versus 89 (76%–98%); z-score: 1.081, P = 0.279 in the intervention and control arm, respectively. The most common postintubation complications included hypoxia (intervention arm: 56.7% vs. control arm: 66.7%) and circulatory/hypoxic arrest (intervention arm: 39.5% vs. control arm: 44.7%). There were no adverse complications in 36.7% (n = 11) of patients in the intervention arm. Despite the best possible medical management, almost half (52.6%) of patients in the intervention arm and 47.4% of patients in the control arm succumbed to their illnesses in the ED. CONCLUSION: The primary outcome revealed no statistically significant difference between the two arms. However, patients in the intervention arm exhibited fewer intubation-related adverse effects.

Despite significant progress in tobacco control measures and stringent smoking policies, cigarette smoking remains one of the largest preventable causes of death and disability worldwide. The World Health Organization estimates that over 8 million global deaths are attributed to smoking yearly, and in Singapore, more than 2,000 Singaporeans die prematurely due to smoking-related diseases each year.1,2

Background and objectiveMyDispense is one of the virtual simulations that has already been established as a suitable alternative for live experiential education in the pharmacy curriculum. However, there are no structured validated questionnaires available to assess the students' perception while integrating MyDispense with pharmacy practice experiential education. Therefore, the present study aimed to validate a structured questionnaire and use the questionnaire to assess the student perception of various pharmacy practice experiential education.MethodsContent and construct validity procedure was used to validate the questionnaire. Two hundred students consented to participate in validating the questionnaire. The validated questionnaire assessed the students' perception of integrating MyDispense with Introductory Pharmacy Practice Experience 2 (IPPE2) and Advanced Pharmacy Practice Experience (APPE) courses. The questionnaire was structured with four domains which were: exercise, instructor, technical, and communication. Each domain carried five items; therefore, the whole questionnaire had 20 items that succeeded in content validity. In the survey, 121 fourth-year and 117 fifth-year Pharm.D. students volunteered to convey their perception of integrating MyDispense with IPPE 2 and APPE, respectively. The survey was conducted before and after the MyDispense exam in both the courses.ResultsThe Cronbach's α and McDonald's ω coefficients were > 0.8 in all four domains, indicating that the items related to the four domains have good internal consistency. In Exploratory Factor Analysis (EFA), two items were found to cross-load in the exercise domain and removed. Therefore, the EFA proposes 18 items for the confirmatory factor analysis (CFA). In CFA, five fit indices were found to be satisfactory, and this indicates construct was good enough to assess the student perception. In IPPE 2, the pre-test response, the students had significantly higher satisfaction (p < 0.05) with all five items related to the technical domain. In APPE, the students had a significantly (p < 0.05) higher perception of all the items related to the exercise and technical domain in the pre-test compared to the post-test. Therefore, the student's pre-test feedback allowed the instructor to identify and make the necessary corrections in the exercises to improve the quality exercises.ConclusionThis study provides a validated 18-item questionnaire to assess the student perception of integrating MyDispense in experiential education. The integration of MyDispense in experiential education needs to be done carefully by assessing student perception.

Conclusion: This study evaluated the performance of ChatGPT 3.5 and ChatGPT 4 in answering questions related to endodontics and operative dentistry. The results showed no statistically significant differences between the two versions, indicating comparable response accuracy. The consistency between reviewers further validated the reliability of the assessment process.

Objective:To compare and analyze the precision, accuracy, and reliability of commonly used cone-beam computed tomography (CBCT) software in predicting the mesiodistal diameter of impacted canines.Methods:This study was conducted on 11 patients (six males and five females, mean age: 17.5±5.5 years) with either unilateral or bilateral impacted canines in the maxilla or mandible. DICOM data sets of the patients obtained from CBCT scans were then loaded and visualized with four selected CBCT software to measure the widest mesiodistal diameter of the impacted teeth. Lymphangitis carcinomatosis (LC) is the infiltration and inflammation of lymphatic vessel secondary to the spread of malignancy from a primary site. Most cases result from dissemination of adenocarcinomas. Its presentation may be unilateral or bilateral, it is asymmetrical and is limited to one lobe of the lung especially lower lobes, focal unilateral presentation is observed in 50% of patients, while only 6%–8% of lung metastases present as diffusely infiltrating pattern. In this case report, we present a case of adenocarcinoma with LC presenting as diffuse parenchymal lung disease which is a rare presentation. A 45-year-old female matchbox industry worker by occupation with no comorbid illness presented with 8-month history of cough with scanty mucoid expectoration and Grade II dyspnea according to the modified Medical Research Council along with history of loss of appetite and weight. CT chest shows parenchymal nodule with spiculated margin in the anterior segment of the upper lobe, nodular thickening of the Broncho vascular interstitium and pleura, diffuse interstitial septal thickening and randomly distributed nodules in all lobes. PET scan showed low grade metabolically active nodule in the anterior segment of the left upper lobe, metabolically active mediastinal, supraclavicular and retrocrural lymph nodes, mild pleural effusion and Low-grade diffuse metabolism was noted in the uterine endometrium. Low-grade diffuse metabolism was noted in the uterine endometrium. Abdominal ultrasound revealed an endometrial thickness of 8 mm. Visual inspection with acetic acid (VIA), visual inspection with lugol's iodine (VILI) and Papanicolaou smear was done which was suggestive of inflammatory smear, and Papanicolaou smear was done and was suggestive of inflammatory smear. Fiber-optic bronchoscopy was done, it was normal, and transbronchial lung biopsy revealed adenocarcinoma lung. The patient was referred to medical oncology, and the first cycle of chemotherapy was started with cisplatin and pemetrexed. Physicians should be aware of pulmonary lymphangitic carcinomatosis from lung adenocarcinoma and consider it in patients with pulmonary symptoms who are unresponsive to antibiotics. Despite establishment of diagnosis with cellular etiology even primary source, the condition is fatal. This rare condition needs to be considered in appropriate clinical settings.

Analysis of nanofluid flow phenomena through a stretching sheet in a non-Darcy porous medium has great importance in enhancing transport processes in energy systems like hybrid fuel cell technology, glass blowing, etc. The current study looks at two-dimensional flow over a convectively heated linear stretching sheet in presence of a magnetic field. The boundary layer flow caused by a sheet that is linearly stretched has been investigated numerically. Employing the appropriate similarity transformation, the governing partial differential equations, which define the flow regime, are converted into a set of ordinary differential equations. The mathematical calculations are carried out using a finite difference algorithm with the aid of Newton's linearization method, which allows us to handle non-linear terms very smoothly. The outcomes of this work have been demonstrated are based on eight parameters, such as Prandtl number [Formula: see text], Lewis number [Formula: see text], Brownian motion parameter [Formula: see text], thermophoresis parameter [Formula: see text], magnetic parameter [Formula: see text], porosity parameter [Formula: see text], Frochemier number [Formula: see text], and convection Biot number [Formula: see text]. The impacts of aforementioned parameters on thermal and concentration boundary layers are depicted graphically. It has been found that the heat transfer in the boundary layer rises as [Formula: see text] enhances, and deposits aggravating particle away from the fluid region and increases the volume percentage of nanoparticles. Moreover, it has been found that as the [Formula: see text] number rises, the concentration profiles become steeper and the species border layer becomes thinner. The study highlights the importance of integrating organic resources, such as vermicompost and biofertilizers with inorganic fertilizers to sustain coconut yields and manage costs. The experimental trial was conducted from 2016 to 2020 in a 47-year-old East Coast Tall coconut garden at Coconut Research Station in South India. The research evaluated four nutrient management treatments viz., T1 (75% Recommended Dose of Fertilizer (RDF) + 25% N organic), T2 (50% RDF + 50% N organic), T3 (100% N organic) and a control (100% RDF). Intercrops included black pepper (Piper nigrum), banana (Musa acuminata) and cocoa (Theobroma cacao). Organic manure significantly improved soil physical properties, water retention and overall soil health. The T2 treatment achieved the highest yields for coconut, cocoa, banana and black pepper. The added soil health parameters supported to these findings, with T2 showing the highest fungal (15.27 × 103 cfu/g of soil) and bacterial populations (17.25 × 105 cfu/g of soil), along with a significant earthworm population (26/m2), indicating enhanced soil ecosystem health. Additionally, soil moisture content was highest under T3 (100% organic) across various depths, followed by T2, highlighting the critical role of organic matter in improving soil moisture conservation. The economic feasibility analysis, including a net present value (NPV), benefit/cost ratio (B/C ratio) and an internal rate of return (IRR), revealed T2 to be the most economically viable nutrient management strategy. This study highlights the economic benefits of intercropping coconuts using an integrated nutrient management (INM) approach, demonstrating its superiority over traditional monocropping practices.

Introduction: Family planning (FP) is important in reducing maternal morbidity and mortality as well as foetal and neonatal complications. This study aimed to determine the intention to practise FP among antenatal women at risk of gestational diabetes mellitus (GDM) in the Klang Health District and its associated factors. Methods: A cross-sectional study was conducted at four government health clinics in the Klang Health District. A total of 431 antenatal women at risk of GDM were recruited using systematic random sampling. A validated self-administered questionnaire was used to assess knowledge, attitude, previous practice and intention to use FP after delivery. Multiple logistic regression (MLR) was used to determine the factors associated with the intention to practise FP. Results: Approximately 64.7% (n=279) of the respondents intended to practise FP. MLR showed that the factors associated with the intention to practise FP were Malay ethnicity (odds ratio [OR]=3.319, 95% confidence interval [CI]=1.431–7.697), low income (OR=2.174, 95% CI=1.317–3.588), good knowledge (OR=2.591, 95% CI=1.008–6.174) and good previous practice (OR=3.956, 95% CI=1.428–9.052). Conclusion: The prevalence of the intention to practise FP among antenatal women at risk of GDM was 64.7%. Malay antenatal women from low-income households with good knowledge and previous practice were more likely to have the intention to practise FP after delivery. Thus, interventions targeted at non-Malay women and measures to improve their knowledge might help improve the intention and uptake of FP among these women.

Introduction : In elderly individuals, morbidity has a substantial impact on the physical and psychological well-being. Quality of life in elderly may be affected by the morbidities that they suffer. Material and Methods: A community-based cross-sectional study was conducted from September to November 2021 in the rural areas attached to the Rural Health Training Centre of a teaching hospital. Elderly individuals were selected by multistage sampling. The sample size was estimated as 118. Participants’ data was collected using a pretested semi-structured questionnaire regarding their socio-demographic profile and morbidity status. Quality of life (QOL) was assessed by using the WHOQOL-BREF scale. The difference between mean scores in the domains was tested by using an independent sample t-test. Statistical significance was set at 5% level of significance (p < 0.05). Results: Majority (57.6%) of the study population were males, 57.6% were illiterate and only 9.3% lived in joint families. Major morbidities reported were hypertension (55.9%), impaired vision (52.5%) and joint pain or stiffness (50.8%). The mean perceived overall quality of life scores in the study population were 61.9±17.5. Male participants had higher mean social and environmental domain scores. Literate individuals had higher social domain mean scores which was statistically significant. Conclusion: Appropriate rehabilitation measures along with health education with the assistance of a geriatric psychologist in order to identify gaps in QOL domains and approaches to solve them would help to improve the quality of life of the elderly.
