Abstract

LLMs demonstrate an uncanny ability to process unstructured data, and as such, have the potential to go beyond search and run complex, semantic analyses at scale. We describe the design of an unstructured analytics system, Aryn, and the tenets and use cases that motivate its design. With Aryn, users specify queries in natural language and the system automatically determines a semantic plan and executes it to compute an answer from a large collection of unstructured documents. At the core of Aryn is Sycamore, a declarative document processing engine, that provides a reliable distributed abstraction called DocSets. Sycamore allows users to analyze, enrich, and transform complex documents at scale. Aryn includes Luna, a query planner that translates natural language queries to Sycamore scripts, and DocParse, which takes raw PDFs and document images, and converts them to DocSets for downstream processing. We show how these pieces come together to achieve better accuracy than RAG on analytics queries over real world reports from the National Transportation Safety Board (NTSB). Abstract

Machine learning models perform well on several healthcare tasks and can help reduce the burden on the healthcare system. However, the lack of explainability is a major roadblock to their adoption in hospitals. \textit{How can the decision of an ML model be explained to a physician?} The explanations considered in this paper are counterfactuals (CFs), hypothetical scenarios that would have resulted in the opposite outcome. Specifically, time-series CFs are investigated, inspired by the way physicians converse and reason out decisions `I would have given the patient a vasopressor if their blood pressure was lower and falling'. Key properties of CFs that are particularly meaningful in clinical settings are outlined: physiological plausibility, relevance to the task and sparse perturbations. Past work on CF generation does not satisfy these properties, specifically plausibility in that realistic time-series CFs are not generated. A variational autoencoder (VAE)-based approach is proposed that captures these desired properties. The method produces CFs that improve on prior approaches quantitatively (more plausible CFs as evaluated by their likelihood w.r.t original data distribution, and 100$\times$ faster at generating CFs) and qualitatively (2$\times$ more plausible and relevant) as evaluated by three physicians.

20. High clinical burden of classical homocystinuria in the United States: a retrospective analysis.

Abstract

Background: Classical homocystinuria (HCU) is a rare genetic metabolic disorder resulting in elevated homocysteine and methionine levels. The clinical characteristics and associated complications of HCU are well documented. However, there is limited published research on the clinical burden of patients with HCU, especially stratified by total homocysteine (tHcy) levels. This study aimed to describe the overall clinical burden of patients with HCU in the United States and key clinical events by tHcy levels using administrative claims data. Methods: This non-interventional retrospective cohort analysis from January 01, 2016, through September 30, 2021, used Optum's de-identified Market Clarity Data. Patients who had 1 or more International Classification of Diseases, Tenth Revision code for homocystinuria (E72.11) or the signs, disease, and symptoms term homocystinuria in the natural language processing dataset were included. To obtain a study population most likely to have HCU, stratifications by tHcy levels, clinical characteristics, and phenotypic expressions were applied to refine the cohort. Included patients were then stratified by highest tHcy level. Clinical burden was measured by category of HCU-related events. Descriptive statistics were reported. Results: Six hundred thirty-three patients met the inclusion criteria, and 601 patients had a tHcy level: < 50 µM (n = 278), 50 to < 100 µM (n = 212), and ≥ 100 µM (n = 111). Among the 601 patients with a tHcy level, almost one-half (n = 297, 49.4%) had at least one thrombotic/thromboembolic, skeletal, ocular, or neurological event and 14.1% (n = 85) had multiple events. Thrombotic/thromboembolic events (n = 186, 30.9%) were the most common type of events, followed by skeletal (n = 100, 16.6%), ocular (n = 63, 10.5%), and neurological events (n = 50, 8.3%). During follow-up, 5.7% (n = 34) of the patients died. All events assessed were more prevalent in the 50 to < 100 µM group and ≥ 100 µM group compared with those in the < 50 µM group. Conclusions: As has been believed, patients with tHcy ≥ 100 µM carried a substantial clinical burden, but the burden is also very high in those whose levels were ≥ 50 µM. Thrombotic/thromboembolic events were more common than skeletal, ocular, or neurological events. Meaningfully lowered tHcy levels may help to reduce significant clinical events. Abstract

Objective: In office methods for immediate relief of dentinal hypersensitivity (DH) has long been an area of research. This study compared the efficacy of 660 nm diode laser, 980 nm diode laser, and amorphous calcium phosphate–casein phosphopeptide (ACP–CPP) agent in the treatment of DH. Materials and Methods: A total of 39 patients with minimum three hypersensitive teeth in at least one quadrant were selected and randomly divided into three groups; Group A, B, and C patients were treated by 660 nm diode laser, 980 nm diode laser, and ACP–CPP agent, respectively. All the hypersensitive teeth were stimulated with tactile; thermal and air stimuli, and the pretreatment and posttreatment hypersensitivity scores were evaluated with the numeric rating scale (NRS) at baseline, 30 min, 1 week, 1 month, 3 months, and 6 months. The Shapiro–Wilk test was used to determine the uniformity of the data. The Chi‐square (χ2) test of significance was used to compare proportions. For intergroup comparison, the Kruskal–Wallis test and the Mann–Whitney U test were utilized. Results: At 30 min and 1 week, the 980 nm laser was more effective than the 660 nm laser, but there was no statistically significant difference between the two groups at 1, 3, and 6 months. Throughout the trial period, the 660 nm and 980 nm diode lasers were more effective than the ACP–CPP agent in lowering DH. Conclusions: 660 nm diode laser, 980 nm diode laser, and ACP–CPP agent showed positive prospective as effective desensitizers when utilized as a clinical procedure. [ABSTRACT FROM AUTHOR]

Abstract

Energy is a growing component of the operational cost for many "big data" deployments, and hence has become increasingly important for practitioners of large-scale data analysis who require scale-out clusters or parallel DBMS appliances. Although a number of recent studies have investigated the energy efficiency of DBMSs, none of these studies have looked at the architectural design space of energy-efficient parallel DBMS clusters. There are many challenges to increasing the energy efficiency of a DBMS cluster, including dealing with the inherent scaling inefficiency of parallel data processing, and choosing the appropriate energy-efficient hardware. In this paper, we experimentally examine and analyze a number of key parameters related to these challenges for designing energy-efficient database clusters. We explore the cluster design space using empirical results and propose a model that considers the key bottlenecks to energy efficiency in a parallel DBMS. Abstract

Data management systems have traditionally been designed to support either long-running analytics queries or short-lived transactions, but an increasing number of applications need both. For example, online games, socio-mobile apps, and e-commerce sites need to not only maintain operational state, but also analyze that data quickly to make predictions and recommendations that improve user experience. In this paper, we present Minuet, a distributed, main-memory B-tree that supports both transactions and copy-on-write snapshots for in-situ analytics. Minuet uses main-memory storage to enable low-latency transactional operations as well as analytics queries without compromising transaction performance. In addition to supporting read-only analytics queries on snapshots, Minuet supports writable clones, so that users can create branching versions of the data. This feature can be quite useful, e.g. to support complex "what-if" analysis or to facilitate wide-area replication. Our experiments show that Minuet outperforms a commercial main-memory database in many ways. Abstract

Energy costs are quickly rising in large-scale data centers and are soon projected to overtake the cost of hardware. As a result, data center operators have recently started turning into using more energy-friendly hardware. Despite the growing body of research in power management techniques, there has been little work to date on energy efficiency from a data management software perspective. In this paper, we argue that hardware-only approaches are only part of the solution, and that data management software will be key in optimizing for energy efficiency. We discuss the problems arising from growing energy use in data centers and the trends that point to an increasing set of opportunities for software-level optimizations. Using two simple experiments, we illustrate the potential of such optimizations, and, motivated by these examples, we discuss general approaches for reducing energy waste. Lastly, we point out existing places within database systems that are promising for energy-efficiency optimizations and urge the data management systems community to shift focus from performance-oriented research to energy-efficient computing., Comment: CIDR 2009

Abstract

Many emerging Web services, such as email, photo sharing, and web site archives, need to preserve large amounts of quickly-accessible data indefinitely into the future. In this paper, we make the case that these applications' demands on large scale storage systems over long time horizons require us to re-evaluate traditional storage system designs. We examine threats to long-lived data from an end-to-end perspective, taking into account not just hardware and software faults but also faults due to humans and organizations. We present a simple model of long-term storage failures that helps us reason about the various strategies for addressing these threats in a cost-effective manner. Using this model we show that the most important strategies for increasing the reliability of long-term storage are detecting latent faults quickly, automating fault repair to make it faster and cheaper, and increasing the independence of data replicas.
