Abstract Retinitis pigmentosa (RP) is a genetic blinding disease with over 80 causative genes. Disease progression varies between patients with similar genetic backgrounds. We assessed the association between environment, gut microbiota, and retinal degeneration in the RP rat model Royal College of Surgeons (RCS). The rats were born and raised for two generations under specific pathogen-free (SPF, n = 69) or non-SPF conditions (n = 48). At the age of four weeks, SPF rats had significantly shorter dark-adapted a-wave and dark and light-adapted b-wave implicit times by electroretinogram (p = 0.014, p = 9.5*10−6, p = 0.009, respectively). The search aimed to identify the speaking and listening skills of agricultural extension workers in general in Salah Al-Din Governorate. The research community comprised agricultural extension professionals from Salah Al-Din Governorate, represented by the divisions Tikrit, Samarra, Al-Dur, Al-Alam, the Extension Training Centre, and the Directorate of Agriculture's headquarters. There were a total of 93 agricultural employees with an agricultural scientific qualification. The questionnaire pre-test was conducted on an experimental sample of 20 agricultural extension workers. A random sample of 54% was chosen after the agricultural employees who appeared in the study were excluded. As a result, the sample of agricultural employees consists of 50 agricultural employees. The results showed that the majority of workers in agricultural extension in Salah Al-Din governorate have medium speaking and listening skills that tend to be low. This result may be ascribed to the fact that most respondents' terms of reference are not indicative, and they also conduct numerous extra tasks in addition to their indicative job. Hence, the result came at this level. The study's findings revealed a significant relationship between agricultural extension workers' speaking and listening skills and some independent factors (the level of education, the duration of job service, specialization, the duration of job service, and the attitude towards agricultural extension).

Abstract Introduction The emergence of the COVID-19 pandemic has served as a call for enhanced global cooperation and a more robust pandemic preparedness and response framework. As a result of this pressing demand, dialogues were initiated to establish a pandemic treaty designed to foster a synchronized global strategy for addressing forthcoming health emergencies. In this review, we discussed the main obstacles to this treaty. Results Among several challenges facing the pandemic treaty, we highlighted (1) global cooperation and political will, (2) equity in access to resources and treatments, (3) sustainable financing, (4) compliance and enforcement mechanisms, (5) sovereignty concerns, and (6) data sharing and transparency. Conclusion Navigating the hurdles facing the development of the pandemic treaty requires concerted efforts, diplomatic finesse, and a shared commitment to global solidarity. Addressing challenges in global cooperation, equitable access, transparency, compliance, financing, and sovereignty is essential for forging a comprehensive and effective framework for pandemic preparedness and response on the global stage.

Diabetes mellitus (DM) is a chronic metabolic disorder characterized by high blood sugar levels (hyperglycemia). Poorly managed diabetes can lead to complications affecting multiple organ systems. Antioxidants play a crucial role in reducing oxidative stress caused by reactive oxygen species (ROS), primarily triggered by uncontrolled high blood sugar levels in diabetes. Antioxidants like vitamin C, E, selenium, and alpha-lipoic acid, when used as supplements, have shown promise in reducing oxidative stress markers and improving antioxidant status in laboratory and animal studies and diabetic patients. Antioxidant supplementation may help reduce the risk of diabetic complications such as neuropathy, nephropathy, retinopathy, and cardiovascular disease. Additionally, antioxidants also have anti-inflammatory properties, which could be beneficial in reducing inflammation associated with diabetes. Antioxidant supplementation has been shown to enhance endothelial function, insulin sensitivity, and glucose metabolism, thereby aiding in glycemic control and overall diabetic management. Combining antioxidants with certain medications may have therapeutic benefits, such as effectively neutralizing free radicals and enhancing the regulation of antioxidant defense systems. This review presents an update on diabetes, the sources of free radical generation, the body's natural defense mechanisms, the clinical evidence regarding using antioxidants in managing diabetic complications, and the potential new therapeutic approaches. Overall, antioxidant supplementation may offer some benefits in managing diabetic complications. However, further studies are needed to understand the mechanisms of action, determine the optimal supplementation, explore potential interactions with other medications, and conduct long-term studies to establish the possible use of antioxidants for optimal benefits in diabetes care.

To survive, animals must convert sensory information into appropriate behaviours1,2. Vision is a common sense for locating ethologically relevant stimuli and guiding motor responses3-5. How circuitry converts object location in retinal coordinates to movement direction in body coordinates remains largely unknown. Here we show through behaviour, physiology, anatomy and connectomics in Drosophila that visuomotor transformation occurs by conversion of topographic maps formed by the dendrites of feature-detecting visual projection neurons (VPNs)6,7 into synaptic weight gradients of VPN outputs onto central brain neurons. We demonstrate how this gradient motif transforms the anteroposterior location of a visual looming stimulus into the fly's directional escape. Specifically, we discover that two neurons postsynaptic to a looming-responsive VPN type promote opposite takeoff directions. Opposite synaptic weight gradients onto these neurons from looming VPNs in different visual field regions convert localized looming threats into correctly oriented escapes. For a second looming-responsive VPN type, we demonstrate graded responses along the dorsoventral axis. We show that this synaptic gradient motif generalizes across all 20 primary VPN cell types and most often arises without VPN axon topography. Synaptic gradients may thus be a general mechanism for conveying spatial features of sensory information into directed motor outputs.

The quantification of physical properties of biological matter gives rise to novel ways of understanding functional mechanisms. One of the basic biophysical properties is the mass density (MD). It affects the dynamics in sub-cellular compartments and plays a major role in defining the opto-acoustical properties of cells and tissues. As such, the MD can be connected to the refractive index (RI) via the well known Lorentz-Lorenz relation, which takes into account the polarizability of matter. However, computing the MD based on RI measurements poses a challenge, as it requires detailed knowledge of the biochemical composition of the sample. Here we propose a methodology on how to account for assumptions about the biochemical composition of the sample and respective RI measurements. To this aim, we employ the Biot mixing rule of RIs alongside the assumption of volume additivity to find an approximate relation of MD and RI. We use Monte-Carlo simulations and Gaussian propagation of uncertainty to obtain approximate analytical solutions for the respective uncertainties of MD and RI. We validate this approach by applying it to a set of well-characterized complex mixtures given by bovine milk and intralipid emulsion and employ it to estimate the MD of living zebrafish (Danio rerio) larvae trunk tissue. Our results illustrate the importance of implementing this methodology not only for MD estimations but for many other related biophysical problems, such as mechanical measurements using Brillouin microscopy and transient optical coherence elastography.

The purpose of this study was to evaluate an academic support program that aims to improve persistence and graduation for lower-division students who are low income, first generation, and/or underrepresented. Students were organized in 10 academies that serve as a "school within a school" and have three main elements: a pathway of two linked general education courses that students follow, cohort-style, over four semesters; wraparound student services integrated into the classroom; and a 45-hour faculty development process. Program participants (n = 2,281) were compared to a matched comparison group (n = 2,276). Multimodal logistic regression analyses showed that program participants statistically significantly outperformed the comparison group on every outcome measure: GPA, completion of developmental coursework, persistence toward graduation, and graduation rates.

Abstract Background Regardless of a proliferation of interest in reducing unsafe practices in healthcare, threats to patient safety (PS) remain high. Moreover, little attention has been paid towards the role of interprofessional education (IPE) in enhancing PS. This qualitative study was conducted to unfold the insights of the senior medical, dental and health sciences students at the University of Sharjah (UoS) in the United Arab Emirates (UAE) about PS in an online IPE-based workshop. Methods This inductive thematic analysis study was conducted on senior medical and health students at the Colleges of Medicine, Dental Medicine, Health Sciences, and Pharmacy of UoS. During an online workshop, students discussed plausible solutions for four real practice-based clinical scenarios with elements of unsafe healthcare practices. During the breakout rooms, the students exhibited high level of articulation and proactively participated in discussions. The data from the online workshop were transcribed and then coding, categorizing, and labelling of recurrent themes were carried out. Multiple individual deliberations, consolidation, incorporation of the identified preliminary themes, and merging and reorganizing sub-themes led to a final thematic framework. Results This work delved into the perspectives of 248 students regarding teamwork, communication, problem-solving, and other aspects concerning PS in interprofessional settings in an online workshop. The iterative process of data transcription, curating and qualitative analysis surfaced 32 codes. Later, the inductive themaric analysis yielded five themes with distinct yet interconnected nested subthemes in the context of PS in IPE settings. These themes of information sharing and grounding (problem-solving, social skills), maintaining communication (clinical reasoning, shared mental model), executing interprofessional activities (collaborative practice, collaboration scripts), professional cognitive abilities (cognitive maturity, metacognition), and negotiating professional identities (systematic change, socio-economic scaffolding) emerged as fundamental pillars for enhancing PS in healthcare. Conclusion Our study demonstrated the outcome of an innovative and team-based workshop which embedded PS within a scaffold of IPE environment. This research calls for incorporation of the emerging areas of clinical reasoning, problem solving, collaborative practice, and shared mental model into medical curricula for structured IPE in improving PS domains in medical education. These findings underscore the need for multifaceted dimensions of IPE imperatives for cultivating collaborative competence.

BackgroundArtificial intelligence (AI) has emerged as a transformative force across the health sector and has garnered significant attention within sexual and reproductive health and rights (SRHR) due to polarizing views on its opportunities to advance care and the heightened risks and implications it brings to people’s well-being and bodily autonomy. As the fields of AI and SRHR evolve, clarity is needed to bridge our understanding of how AI is being used within this historically politicized health area and raise visibility on the critical issues that can facilitate its responsible and meaningful use. ObjectiveThis paper presents the protocol for a scoping review to synthesize empirical studies that focus on the intersection of AI and SRHR. The review aims to identify the characteristics of AI systems and tools applied within SRHR, regarding health domains, intended purpose, target users, AI data life cycle, and evidence on benefits and harms. MethodsThe scoping review follows the standard methodology developed by Arksey and O’Malley. We will search the following electronic databases: MEDLINE (PubMed), Scopus, Web of Science, and CINAHL. Inclusion criteria comprise the use of AI systems and tools in sexual and reproductive health and clear methodology describing either quantitative or qualitative approaches, including program descriptions. Studies will be excluded if they focus entirely on digital interventions that do not explicitly use AI systems and tools, are about robotics or nonhuman subjects, or are commentaries. We will not exclude articles based on geographic location, language, or publication date. The study will present the uses of AI across sexual and reproductive health domains, the intended purpose of the AI system and tools, and maturity within the AI life cycle. Outcome measures will be reported on the effect, accuracy, acceptability, resource use, and feasibility of studies that have deployed and evaluated AI systems and tools. Ethical and legal considerations, as well as findings from qualitative studies, will be synthesized through a narrative thematic analysis. We will use the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews) format for the publication of the findings. ResultsThe database searches resulted in 12,793 records when the searches were conducted in October 2023. Screening is underway, and the analysis is expected to be completed by July 2024. ConclusionsThe findings will provide key insights on usage patterns and evidence on the use of AI in SRHR, as well as convey key ethical, safety, and legal considerations. The outcomes of this scoping review are contributing to a technical brief developed by the World Health Organization and will guide future research and practice in this highly charged area of work. Elle exige au moins deux sièges permanents avec droit de veto, en plus de cinq non permanents et se réserve le choix des éventuels représentants. De fait, plusieurs nations ont déjà manifesté l’ambition d’assumer une telle responsabilité pour le continent. Cependant, dans ce groupe de pays en développement qui affichent encore des difficultés à assurer la paix et la sécurité à l’intérieur de leurs frontières, seule une mutualisation des ressources et des compétences pourrait avoir une influence sur la marche du monde. De surcroît, l’efficacité d’un siège permanent au Conseil reste conditionnée par un minimum de capacité à garantir la paix et la sécurité dans le monde. Au regard de son incapacité stratégique et diplomatique, il serait plus réaliste d’organiser sa représentation permanente non pas par des États, mais bien par l’autorité de Union Africaine à travers son Conseil de Paix et de Sécurité, qui siègera pour une place permanente dotée d’un véto double au Conseil de Sécurité.

Abstract Extracellular matrix (ECM) deposition after central nervous system (CNS) injury leads to inhibitory scarring in humans and other mammals, whereas it facilitates axon regeneration in the zebrafish. However, the molecular basis of these different fates is not understood. Here, we identify small leucine-rich proteoglycans (SLRPs) as a contributing factor to regeneration failure in mammals. We demonstrate that the SLRPs chondroadherin, fibromodulin, lumican, and prolargin are enriched in rodent and human but not zebrafish CNS lesions. Targeting SLRPs to the zebrafish injury ECM inhibits axon regeneration and functional recovery. Mechanistically, we find that SLRPs confer mechano-structural properties to the lesion environment that are adverse to axon growth. Our study reveals SLRPs as inhibitory ECM factors that impair axon regeneration by modifying tissue mechanics and structure, and identifies their enrichment as a feature of human brain and spinal cord lesions. These findings imply that SLRPs may be targets for therapeutic strategies to promote CNS regeneration.

Abstract Hydroxyapatite (HA) can be used in odontology and orthopedic grafts to restore damaged bone due to its stable chemical characteristics, composition, and crystal structural affinity for human bone. A three-step hydrothermal method was used for the extraction of biogenic calcined HA from the buffalo waste bones at 700 °C (HA-700) and 1000 °C (HA-1000). Extracts were analyzed by thermogravimetric analysis, differential scanning calorimetry, X-ray fluorescence, X-ray diffraction, Fourier transform infrared spectroscopy, scanning electron microscopy, and in vivo examination of HA xenografts for femoral lesions in experimental rats. Crystallinity, purity, and morphology patterns showed that the HA main phase purity was 84.68% for HA-700 and 88.99% for HA-1000. Spherical HA nanoparticles were present for calcined HA-700 samples in the range 57–423 nm. Abstract Introduction As low-income countries (LICs) shoulder a disproportionate share of the world’s burden of critical illnesses, they must continue to build critical care capacity outside conventional intensive care units (ICUs) to address mortality and morbidity, including on general medical wards. A lack of data on the ability to treat critical illness, especially in non-ICU settings in LICs, hinders efforts to improve outcomes. Methods This was a secondary analysis of the cross-sectional Malawi Emergency and Critical Care (MECC) survey, administered from January to February 2020, to a random sample of nine public sector district hospitals and all four central hospitals in Malawi. This analysis describes inputs, systems, and barriers to care in district hospitals compared to central hospital medical wards, including if any medical wards fit the World Federation of Intensive and Critical Care Medicine (WFSICCM) definition of a level 1 ICU. We grouped items into essential care bundles for service readiness compared using Fisher’s exact test. Results From the 13 hospitals, we analysed data from 39 medical ward staff members through staffing, infrastructure, equipment, and systems domains. No medical wards met the WFSICCM definition of level 1 ICU. The most common barriers in district hospital medical wards compared to central hospital wards were stock-outs (29%, Cl: 21% to 44% vs 6%, Cl: 0% to 13%) and personnel shortages (40%, Cl: 24% to 67% vs 29%, Cl: 16% to 52%) but central hospital wards reported a higher proportion of training barriers (68%, Cl: 52% to 73% vs 45%, Cl: 29% to 60%). No differences were statistically significant. Conclusion Despite current gaps in resources to consistently care for critically ill patients in medical wards, this study shows that with modest inputs, the provision of simple life-saving critical care is within reach. Required inputs for care provision can be informed from this study.

IntroductionThe decision to discharge a patient from the hospital with confirmed or suspected COVID-19 is fraught with challenges. Patients who are discharged home must be both medically stable and able to safely isolate to prevent disease spread. Socioeconomically disadvantaged patient populations in particular may lack resources to safely quarantine and are at high risk for COVID-19 morbidity. MethodsWe developed a telehealth follow-up program for emergency department (ED) patients who received testing for COVID-19 from April 24 to June 29, 2020 and were discharged home. Patients who were discharged with a pending COVID-19 test received follow up calls on Days 1, 4 and 8. The objective of our program was to screen and provide referrals for health-related social needs (HRSNs); conduct clinical screening for worsening symptoms; and deliver risk-reduction strategies for vulnerable individuals. Retrospective chart review was conducted on all patients in this cohort to collect demographic information, testing results, and outcomes of clinical symptom and HRSN screening. Our primary outcome measurement was the need for clinical reassessment and referral for an unmet HRSN. ResultsFrom April 24th to June 29th, 2020, we made calls to 1,468 patients tested for COVID-19 and discharged home. On Day 4, we reached 67.0% of the 1,468 patients called. Of these, 15.9% were referred to a PA out of concern for clinical worsening and 12.4% were referred to an ED Patient Navigator for HRSNs. On Day 8, we reached 81.8% of the 122 patients called. Of these, 19.7% were referred to a PA for clinical reassessment and 14.0% of patients were referred to an ED Patient Navigator for HRSNs. Our intervention reached 1,069 patients, of which 12.6% required referral for HRSNs and 1.3% (n=14) were referred to the ED or Respiratory Illness Clinic due to concern for worsening clinical symptoms. ConclusionIn this patient population, the demand for interventions to address social needs was as high as the need for clinical reassessment. Similar ED-based programs should be considered to help support patients’ interdependent social and health needs, beyond those related to COVID-19.

The optimal use of antibiotics represents a cornerstone in controlling antibiotic resistance. Strategies such as antibiotic stewardship programs (ASPs) have been developed to influence the rational use of antibiotics. This study evaluated the appropriateness of the empirical use of antibiotics based on antibiotic susceptibility results with the aim of participating effectively in improving local ASPs. In a cross-sectional study conducted at a tertiary hospital in Saudi Arabia, 500 inpatients received empirical antibiotics, and their culture and sensitivity results were included. The appropriateness of the empirical use of antibiotics was determined based on their alignment with the culture and sensitivity results. More than half of the participants (56.4%) were men, and nearly half (43%) were over 61 years old. The empirical uses of antibiotics were appropriately prescribed in 58% of the patients. Ciprofloxacin and ceftriaxone were the most prescribed antibiotics, while vancomycin, piperacillin–tazobactam, and tigecycline were the most appropriately prescribed antibiotics. E. coli was the main microorganism isolated in the susceptibility results and was appropriately prescribed in 59% of the patients. The highest microbial sensitivity was observed for linezolid, vancomycin, and tigecycline. Antibiotics were appropriately prescribed empirically in more than half of the participants. Activating interventional ASP is crucial to fill the gap in prescribing antimicrobials. Considering the expected type of organisms and the local susceptibility pattern is likely to yield a more appropriate empirical use of antibiotics.

Gd/Fe-SnO2 nanopowders as novel photocatalysts for the active removal of Rose Bengal dye and methyl parathion pesticide were synthesized with a low-cost coprecipitation route. The X-ray diffraction analysis of SnO2, Sn0.96Gd0.02Fe0.02O2 and Sn0.94Gd0.02Fe0.04O2 nanopowders proved the formation of a tetragonal phase of tin oxide with average crystallite sizes in the range of 13–18 nm. The Fourier transform infrared (FTIR) spectra of all samples displayed the characteristic absorption bands of SnO2. The nanopowder of the pure SnO2 sample, as seen in its transmission electron microscope (TEM) image, contains spherical-like particles of variable sizes. The TEM images of the Sn0.96Gd0.02Fe0.02O2 and Sn0.94Gd0.02Fe0.04O2 powders revealed the synthesis of fine spherical nanoparticles. Based on the TEM images, the average particle size of the pure, (Gd, 2 wt% Fe) and (Gd, 4 wt% Fe) codoped SnO2 nanopowders was estimated to be 14, 10 and 12 nm, respectively. After the addition of (Gd, 2 wt% Fe) and (Gd, 4 wt% Fe) to the SnO2 structure, the band gap energy of SnO2 was reduced from 3.4 eV to 2.88 and 2.82 eV, respectively. Significantly, the Sn0.96Gd0.02Fe0.02O2 nanocatalyst exhibited a high removal efficiency of 98 and 96% for Rose Bengal dye and methyl parathion pesticide after activation by sunlight for 35 and 48 min, respectively. Furthermore, this catalyst has shown perfect mineralization as well as high stability properties for the treatment of Rose Bengal dye and methyl parathion pesticide. These results suggest the suitability of the Sn0.96Gd0.02Fe0.02O2 nanocatalyst for the treatment of agriculture and industrial effluent under sunlight light energy.

Photocatalysis is considered as simple, green, and the best strategy for elimination of hazardous organic contaminants from wastewater. Herein, new broad spectrum photocatalysts based on pure and Sm-doped CuO/ZnO/CuMn2O4 ternary composites were simply prepared by co-precipitation approach. The X-ray diffraction results proved the formation of a composite structure. The transmission electron microscope (TEM) images displayed that most particles have a spherical shape with average mean sizes within 26–29 nm. The optical properties of both samples signified that the addition of Sm ions significantly improves the harvesting of the visible light spectrum of CuO/ZnO/CuMn2O4 ternary composites. The photocatalytic study confirmed that 97% of norfloxacin and 96% of methyl green pollutants were photo-degraded in the presence of the Sm-doped CuO/ZnO/CuMn2O4 catalyst after 50 and 40 min, respectively. The total organic carbon analysis revealed the high mineralization efficiency of the Sm-doped CuO/ZnO/CuMn2O4 catalyst to convert the norfloxacin and methyl green to carbon dioxide and water molecules. During three cycles, this catalyst presented a high removal efficiency for norfloxacin and methyl green contaminants. As a dielectric energy storage material, the Sm-doped CuO/ZnO/CuMn2O4 ternary composite has large dielectric constant values, mainly at low frequencies, with low dielectric loss compared to a pure CuO/ZnO/CuMn2O4 composite.

Coronavirus disease 2019 (COVID-19), originating in China, has rapidly spread worldwide. Physicians must examine infected patients and make timely decisions to isolate them. However, completing these processes is difficult due to limited time and availability of expert radiologists, as well as limitations of the reverse-transcription polymerase chain reaction (RT-PCR) method. Deep learning, a sophisticated machine learning technique, leverages radiological imaging modalities for disease diagnosis and image classification tasks. Previous research on COVID-19 classification has encountered several limitations, including binary classification methods, single-feature modalities, small public datasets, and reliance on CT diagnostic processes. Additionally, studies have often utilized a flat structure, disregarding the hierarchical structure of pneumonia classification. This study aims to overcome these limitations by identifying pneumonia caused by COVID-19, distinguishing it from other types of pneumonia and healthy lungs using chest X-ray (CXR) images and related tabular medical data, and demonstrate the value of incorporating tabular medical data in achieving more accurate diagnoses. Resnet-based and VGG-based pre-trained convolutional neural network (CNN) models were employed to extract features, which were then combined using early fusion for the classification of eight distinct classes. We leveraged the hierarchal structure of pneumonia classification within our approach to achieve improved classification outcomes. Since an imbalanced dataset is common in this field, a variety of versions of generative adversarial networks (GANs) were used to generate synthetic data. The proposed approach tested in our private datasets of 4523 patients achieved a macro-avg F1-score of 95.9% and an F1-score of 87.5% for COVID-19 identification using a Resnet-based structure. In conclusion, in this study, we were able to create an accurate deep learning multi-modal to diagnose COVID-19 and differentiate it from other kinds of pneumonia and normal lungs, which will enhance the radiological diagnostic process.

The high capacity electrode material design with rapid charging/discharging and long life capability has become a critical issue and main concern in recent years. Nickel oxide (NiO) has received much attention in the field of energy storage as a cathode electrode material owing to its layered structure with large spacing, crystal structure, and high specific surface area. In this study, the chitosan derived carbon–nickel oxide (CDC@NiO) nanocomposite was synthesized from a NiO nanoparticle precursor using a precipitation approach, and chitosan (a renewable biopolymer) was used as a carbon resource. The formation mechanism, structural behavior, and morphological properties were examined using various types of microscopic and spectroscopic characterization techniques. The design material was tested further as electrode material in an electrochemical half-cell and full cell symmetric assembly. In a three electrode system, the CDC@NiO nanocomposite exhibited satisfactory electrochemical performance with a high specific capacitance of around 1011.10 Fg−1 and better cyclic stability of 94.20% after 3000 cycles. In a two electrode symmetric supercapacitive system, the fabricated CDC@NiO delivered maximum Cs 88.30 Fg−1, stability with a higher power density of 7.84 Whkg−1 at an energy density of 133.05 Wkg−1. The fabricated CDC@NiO electrode material retains the cyclic stability of around 90% after 3500 consecutive charge/discharge cycles.
