Abstract

Juvenile diabetic patients were studied 60-72 hours after insulin withdrawal when moderate ketoacidosis had developed. Somatostatin infusion for 4 hours in five patients resulted in almost complete suppression of plasma pancreatic glucagon and growth hormone, a fall in plasma-cyclic-adenosine-monophosphate (A.M.P.) concentrations, and a large fall in plasma-glucose concentration. After infusion plasma concentrations of these substances rose again. Blood-ketone-bodies, plasma-free-fatty-acids (F.F.A.), and plasma glycerol concentrations, however, did not decrease appreciably with somatostatin administration. In three patients 2 to 3 h somatostatin infusions were twice superimposed upon a continuous 9·5 h insulin infusion (1 unit/h). An insulin effect was noticeable within 30 minutes, with pronounced falls in the concentrations of plasma glucose, pancreatic glucagon, F.F.A., and bloodketone-bodies. There was no significant change in these patterns when somatostatin was administered or withdrawn. These results do not indicate that somatostatin infusion would be useful in the treatment of manifest diabetic ketoacidosis.

Abstract

The biomechanical properties of skin and aorta were studied in rats with experimental (streptozotocin) diabetes.After 30 days of diabetes the collagen content of the skin was diminished by 30%.By biomechanical testing collagen from diabetic rats was found to exhibit increased stiffness and strength: maximal stiffness was increased by 20% and the strain at maximum stress was decreased by 10%.Insulin treatment prevented all changes.No differences were found between aortic specimens from diabetic and normal rats.

17. Renal Hypertrophy in Streptozotocin-Diabetic Rats

Abstract

1. Kidney weight and content of protein, RNA and DNA were measured in rats with streptozotocin diabetes of varying duration. 2. Diabetic rats had larger kidneys than control rats: after 3 days of diabetes the weight increase was 15% and after 42 days of diabetes it was 90%. The protein content rose in parallel to the weight. 3. RNA content was already increased after 36 h of glycosuria, whereas DNA content was unchanged for the first 3 days of diabetes, and increased thereafter. The protein/DNA ratio increased rapidly during the first 3 days but remained constant thereafter. 4. Insulin treatment decreased the renal weight gain by about 67% during the first 8 days of diabetes, but did not prevent the increase in DNA. When insulin was started after 25 days of diabetes there was only a slight regression of kidney growth.

Abstract

Injection of porcine growth hormone (200 microgram) or saline 5 days a week for 16 to 20 weeks in streptozotocin-diabetic rats showed that compared to saline growth hormone produced a 2 1/2-fold larger increase in glomerular capillary basement membrane thickness (2p = 0.027). The possible significance of this effect of an elevated level of growth hormone for diabetic microangiopathy is discussed.

Abstract

Introduction: We describe epidemiological, prophylactic, and clinical aspects of imported malaria in the county of Funen, 1987-1999., Material and Methods: The medical records of 136 patients were reviewed for age, gender, nationality, place of exposure, chemoprophylaxis, time lag from departure to diagnosis, Plasmodium species, treatment, and complications. Data on prescribed chemoprophylaxis dispensed from the pharmacies in the county of Funen were recorded., Results: Seventy-two per cent of the patients were Danish, 28% foreigners. Sixty per cent of the cases were caused by P. falciparum, of more than 90% which was acquired in sub-Saharan Africa. Cases of benign malaria were most often acquired in SE Asia. In the 49 patients with falciparum malaria, who had taken chemoprophylaxis, only 31 (63%) were fully compliant. Compliance was 76% in patients taking chloroquine phosphate + proguanil and 36% in patients taking only chloroquine phosphate. Six patients had complications, but all recovered., Discussion: Contributory causes in a large number of the reported cases of imported malaria in this study were no chemoprophylaxis or poor compliance. With respect to falciparum malaria, prescription of non-recommended chemoprophylaxis also contributed. Chemoprophylaxis dispensed from the pharmacies on Funen over recent years indicates that general practitioners are aware of altered recommendations.

Abstract

One hundred and ten consecutive diabetic patients with acute myocardial infarction (AMI) were compared to a matched group of non-diabetics with AMI, and the pain experienced during the acute episode by each patient was estimated by counting the number of morphine injections given. No significant difference was found regarding the frequency of painless infarctions, the distribution of injections given during the first three days in the coronary unit, or the need for injections after that time. We conclude that contrary to the common supposition, painless AMI in hospitalised patients is almost as frequent among nondiabetics as among diabetics.

Abstract

The kidney changes seen in rats shortly after the induction of experimental diabetes were reviewed: Kidney weight increases by 15 to 20% during the first 4 to 5 days of diabetes and by 70 to 90% after some weeks. GFR increases after a lag-time of some days in parallel to kidney weight. Morphometric analysis shows glomerular hypertrophy to be prominent during the first few days of diabetes. The glomerular filtration surface increases by 40% after 4 days. Diabetic renal hypertrophy is related to the severity of the diabetes, and kidney growth rate relates linearly to blood glucose. Moderate diabetes mellitus and unilateral nephrectomy have approximately the same effect on the kidney (weight increase 20%/4 days). When diabetes mellitus and nephrectomy are combined, renal growth rate almost doubles.

Abstract

Based on the literature and on experience and on experience with 23 cases the clinical diagnosis of insulinomas is reviewed. In clinical praxis the diagnostic steps can usually be simplified to a demonstration of fasting, symptomatic hypoglycaemia. Hence, we have used the 72-hour fast as the single diagnostic test for insulinomas during the last 5 years. Pro-insulin measurements may be a valuable supplementary tool. Other diagnostic tests are probably of little importance.

Abstract

Serum growth hormone was significantly higher in rats fed on a non-protein than on a 25% casein diet. In both dietary groups, hypophysectomy provoked a decrease in the RNA/DNA ratio of the liver, and, especially in the protein-deprived rats, a decrease in the RNA synthesis per liver cell, measured as the product of the RNA/DNA ratio, and the ratio of specific RNA activity to specific nucleotide activity, two hours after administration of [3H]orotic acid. In the protein-deprived rats, full reconstitution of these figures was obtained with a combination of growth hormone, hydrocortisone and triiodothyronine, but not quite with growth hormone alone. The possible role of growth hormone in protein deprivation is discussed.

Abstract

A 22-year-old man developed typical insulin-dependent diabetes mellitus. At the time of diagnosis no signs of diabetic retinopathy were found by an experienced ophthalmologist, but florid proliferative diabetic retinopathy was observed only seven months later. Despite treatment with laser and vitrectomy the eye lesions progressed to total blindness within 30 months after the diagnosis of diabetes.

Abstract

A considerable enlargement of the renal glomeruli is present very early after the acute onset of juvenile diabetes mellitus. In the hypertrophied glomeruli the surface area of the capillary walls, the filtration surface, is increased, and thereby also the amount of basement membrane in the capillary wall. The rate of development of this type of diabetic basement membrane accumulation has been studied in streptozotocin diabetic rats with 4- and 47-day duration of diabetes. During the first 4 days a 40% increase in the total amount of peripheral basement membrane material occurs, whereas the following period up to 47 days shows no further change. The structural changes of the glomerular capillaries seen in conjunction with the basement membrane accumulation comprise an enlargement of the filtration surface and capillary luminal volume. In another experiment, 4-week diabetic animals were transplanted with pancreatic islets. The renal structures were studied quantitatively after 4 weeks of normalization. It was found that the increased amounts of basement membrane material were still present.
