ZusammenfassungDie Behandlung von Patienten mit chronischen Wunden ist weiterhin eine interdisziplinär und interprofessionell relevante Herausforderung. Die Grundlage einer erfolgreichen Therapie der Betroffenen basiert auf der Behandlung der zugrunde liegenden, pathophysiologisch relevanten Erkrankungen. Zusätzlich sollte aber auch immer eine lokale Wundtherapie erfolgen, um die Wundheilung zu unterstützen und Komplikationen zu vermeiden. Um die Vielzahl von Wundprodukten besser strukturieren zu können, wurde durch eine multidisziplinäre Expertengruppe von WundDACH, dem Dachverband deutschsprachiger Fachgesellschaften, das M.O.I.S.T.-Konzept entwickelt. Hier beschreibt M das Exsudatmanagement (Moisture balance), O die Sauerstoffzufuhr (Oxygenation), I die Infektionskontrolle (Infection control), S die Unterstützung des Heilungsprozesses (Support) und T das Gewebemanagement (Tissue management).Das M.O.I.S.T.-Konzept soll Therapeuten eine Hilfestellung bei der systematischen Planung und auch für die Edukation im Hinblick auf die Lokaltherapie von Patienten mit chronischen Wunden bieten. Hier wird nun das Update 2022 dieses Konzeptes erstmalig vorgestellt.

Die vorliegende aktualisierte und auf S2k-Niveau angehobene Leitlinie befasst sich mit der Diagnostik und der Therapie der Rosazea, bei welcher es sich um eine häufige, chronisch-entzündliche Hauterkrankung handelt, die meist das Gesicht betrifft. Der Verlauf der Rosazea ist initial durch rezidivierende Erytheme, Teleangiektasien sowie Flushing gekennzeichnet. Später überwiegt die entzündliche Komponente, wobei es zu persistierenden Erythemen mit follikulären Papeln, Papulopusteln und Pusteln kommt. Die Bildung von Phymen, die meist an den Akren auftreten, stellt dabei die schwerste Ausprägung der Erkrankung dar. Zur Behandlung empfiehlt die interdisziplinäre Leitlinienkommission, die aus Vertretern der Deutschen Dermatologischen Gesellschaft (DDG), des Berufsverbandes der Deutschen Dermatologen (BVDD), der Deutschen Ophthalmologischen Gesellschaft (DOG), der Gesellschaft für Dermopharmazie (GD), der Schweizerischen Gesellschaft für Dermatologie und Venerologie (SGDV) und der Deutschen Rosazea Hilfe e. V. besteht, neben der Meidung von Triggerfaktoren, die topische Anwendung der Wirkstoffe Metronidazol, Azelainsäure oder Ivermectin. Zur symptomatischen Behandlung persistierender zentrofazialer Erytheme können zudem die Vasokonstriktoren Brimonidin oder Oxymetazolin topisch angewandt werden. Bei therapieresistenten sowie bei schweren Formen der Rosacea papulopustulosa wird eine systemische Therapie empfohlen. Hierfür ist niedrigdosiertes Doxycyclin das Präparat der 1. Wahl. Alternativ kann niedrigdosiertes Isotretinoin empfohlen werden. Für die okulären Rosazea kann neben einer Lidrandhygiene die topische Behandlung mit Ciclosporin-haltigen Augentropfen, Azithromycin, Ivermectin oder Metronidazol empfohlen werden.

Edema of the lower extremities is always associated with a pathological condition that should be treated, especially in patients with chronic wounds. Because the underlying causes of edema can vary greatly and sometimes be complex, clinical and, if necessary, various diagnostic tests should also be performed. Often, a suspected clinical diagnosis can already be made after clinical inspection with testing of Stemmer's and Godet's signs. Sonographic examination should then be performed as the next diagnostic test. Although measurement techniques such as water plethysmography are currently considered gold standard for volume measurements, they are very complex and prone to error, so that they are rarely used in clinical routine today. In summary, it is recommended to perform a clinical examination, if possible in combination with sonography, for edema diagnosis. Especially at the beginning of the decongestion phase, regular circumferential measurements should be performed and documented. This documentation is of high relevance for evaluation of therapeutic success.Ödeme der unteren Extremitäten entsprechen immer einem pathologischen Zustand, der insbesondere bei Betroffenen mit chronischen Wunden einer Therapie bedarf. Weil die Ursachen dieser Ödeme sehr unterschiedlich und teilweise auch komplex sein können, sollte zuerst eine klinische und ggf. apparative Diagnostik erfolgen. Oft kann nach einer klinischen Untersuchung mit Testung des Stemmer- und Godet-Zeichens bereits eine klinische Verdachtsdiagnose gestellt werden. Als weiterführende apparative Diagnostik kann eine sonographische Untersuchung erfolgen. Messtechniken wie beispielsweise die Wasserplethysmographie gelten derzeit zwar als Goldstandard für Volumenmessungen, sind aber sehr aufwendig und fehleranfällig, sodass sie in der klinischen Routine heute kaum angewendet werden. Zusammenfassend wird empfohlen, für die Ödemdiagnostik eine klinische Untersuchung möglichst in Kombination mit einer Sonographie durchzuführen. Insbesondere zu Beginn der Entstauungsphase sollten regelmäßig Umfangsmessungen durchgeführt und dokumentiert werden. Diese Dokumentation ist für die Bewertung des therapeutischen Erfolgs von hoher Aussagekraft.

Wund-D.A.CH., as the umbrella organization of German-speaking wound treatment societies, has currently developed a best practice recommendation for skin damage caused by body fluids, which is known as moisture-associated skin damage (MASD) in English-speaking countries. In this expert consensus, the diseases incontinence-associated dermatitis (IAD), intertriginous dermatitis, including intertrigo, gram-negative bacterial toe web infection and toxic contact dermatitis, including periwound and peristomal dermatitis are presented in a differentiated manner. A common feature of these clinical diseases is a deterioration of skin integrity due to prolonged exposure to body fluids such as urine, stool, sweat or wound exudate with associated physical-irritative and/or chemical irritation. In addition, other comorbidities and cofactors play an important role. The diagnosis of these interdisciplinary and interprofessionally relevant MASD is difficult in everyday clinical practice because there are currently no uniform definitions and many relevant differential diagnoses. Effective strategies for the prevention and therapy of these skin diseases are, for example, continence management, use of efficient, absorbent aids with good retention as well as consistent skin protection and adequate skin care. Another important aspect is the education of patients and relatives about the origin, treatment and prevention of MASD.

This updated and upgraded S2k guideline deals with the diagnosis and treatment of rosacea, which is a common, chronic inflammatory skin disease mostly affecting the face. Initially, rosacea is characterized by recurrent erythema, telangiectasia and flushing. Later, the inflammatory component predominates, with persistent erythema with follicular papules, papulopustules and pustules. The development of phyma, which usually occurs on the acral localizations, is the most severe manifestation. For the treatment of rosacea, the interdisciplinary guideline committee, with representatives of the German Dermatological Society (DDG), the Professional Association of German Dermatologists (BVDD), the German Opthalmological Society (DOG), the Society for Dermopharmacy (GD), the Swiss Society for Dermatology and Venereology (SGDV) and the German Rosacea Aid e. V., recommends the avoidance of trigger factors and topical applications of metronidazole, azelaic acid or ivermectin. For symptomatic treatment of persistent centrofacial erythema, the topical vasoconstrictors brimonidine or oxymetazoline can also be used. Systemic therapy is recommended for therapy-resistant and severe forms of rosacea papulopustulosa. The drug of choice is low-dose doxycycline. Alternatively, low-dose isotretinoin can be recommended. Ocular rosacea should be treated with lid margin hygiene. For topical treatment, ciclosporin eye drops, azithromycin, ivermectin or metronidazole are suggested.

As the skin is an accessible organ and many dermatological diagnostics still rely on the visual examination and palpation of the lesions, dermatology could be dramatically impacted by augmented and virtual reality technologies. If the emergence of such tools raised enormous interest in the dermatological community, we must admit that augmented and virtual reality have not experienced the same breakthrough in dermatology as they have in surgery. In this article, we investigate the status of such technologies in dermatology and review their current use in education, diagnostics, and dermatologic surgery; additionally, we try to predict how it might evolve in the near future.

Efficient vaccination can be achieved by injections of in vitro transcribed mRNA (ivt mRNA) coding for antigens. This vaccine format is particularly versatile and allows the production of individualised vaccines conferring, T-cell immunity against specific cancer mutations. The CDR3 hypervariable regions of immune receptors (T-cell receptor, TCR or B-cell receptor, BCR) in the context of T- or B-cell leukaemia or lymphoma are targetable and specific sequences, similar to cancer mutations. We evaluated the functionality of an mRNA-based vaccine designed to trigger immunity against TCR CDR3 regions in an EL4 T-lymphoma cell line-derived murine in vivo model. Vaccination against the hypervariable TCR regions proved to be a feasible approach and allowed for protection against T-lymphoma, even though immune escape in terms of TCR downregulation paralleled the therapeutic effect. However, analysis of human cutaneous T-cell lymphoma samples indicated that, as is the case in B-lymphomas, the clonotypic receptor may be a driver mutation and is not downregulated upon treatment. Thus, vaccination against TCR CDR3 regions using customised ivt mRNA is a promising immunotherapy method to be explored for the treatment of patients with T-cell lymphomas.

Hidradenitis suppurativa (HS) ist eine schmerzhafte, belastende entzündliche Hauterkrankung mit chronisch-intermittierendem Verlauf. Das zentrale pathogenetische Ereignis scheint die Okklusion von Haarfollikeln zu sein. Die 1-Jahres-Prävalenz der HS beträgt rund 1%. Die Erkrankung tritt typischerweise nach der Pubertät auf und ist durch schmerzende, tief sitzende, entzündete Läsionen in Körperregionen mit apokrinen Drüsen gekennzeichnet, vor allem in der Achsel-, Inguinal- und Anogenitalregion. HS geht mit erheblicher Beeinträchtigung der Lebensqualität der Patienten schon bei geringer Krankheitslast einher und die Diagnosestellung erfolgt oft erst mit erheblicher Verzögerung. Die vorliegende Kurzfassung für die Praxis enthält diagnostische und therapeutische Empfehlungen auf der Grundlage einer systematischen Literaturanalyse sowie eines informellen Expertenkonsenses von Schweizer Dermatologen und dermatologischen Chirurgen.

Protamine is a natural cationic peptide mixture used as a drug for the neutralization of heparin and in formulations of slow-release insulin. In addition, Protamine can be used for the stabilization and delivery of nucleic acids (antisense, small interfering RNA (siRNA), immunostimulatory nucleic acids, plasmid DNA, or messenger RNA) and is therefore included in several compositions that are in clinical development. Notably, when mixed with RNA, protamine spontaneously generates particles in the size range of 20–1000 nm depending on the formulation conditions (concentration of the reagents, ratio, and presence of salts). These particles are being used for vaccination and immuno-stimulation. Several grades of protamine are available, and we compared them in the context of complex formation with messenger RNA (mRNA). We found that the different available protamine preparations largely vary in their composition and capacity to transfect mRNA. Our data point to the source of protamine as an important parameter for the production of therapeutic protamine-based complexes.

Patients with chronic wounds should receive wound treatment in addition to causative therapy. In this context, the lack of adequate evidence for wound healing products has been repeatedly discussed. Using the example of TLC-sucrose octasulfate (TLC: technology lipido-colloid), the present review shows that there is significant data with good evidence and comparability in this area. One therapeutic approach to promote wound healing is the inhibition of matrix-metalloproteinases, for example by sucrose octasulfate. For wound products containing TLC-sucrose octasulfate, several sequential clinical studies have been conducted in recent years. The WHAT study was an open randomized controlled trial (RCT) with 117 patients with venous leg ulcers (VLU). The CHALLENGE study was a double-blind RCT with 187 patients with VLU. The SPID study was a pilot study with 33 patients with diabetic foot ulcers (DFU). The two prospective, multicenter clinical pilot studies NEREIDES and CASSIOPEE examined a total of 88 patients with VLU in different phases of healing. In the REALITY study, a pooled data analysis was performed on eight observational studies with 10,220 patients with chronic wounds of different genesis. In the double-blind, two-armed EXPLORER RCT, 240 patients with neuro-ischemic DFU were followed from first presentation until complete healing. In all studies, a significant promotion of wound healing could be shown by the use of wound healing products with TLC-sucrose octasulfate.

Zusammenfassung Bei Patienten mit chronischen Wunden sollte neben einer ursächlichen Therapie auch immer eine Wundbehandlung erfolgen. Wiederholt wurde in diesem Kontext über die unzureichende Evidenz von Wundheilungsprodukten diskutiert. An dem Beispiel von TLC(„technology lipido-colloid“)-Sucrose Octasulfat wird in der vorliegenden Übersichtsarbeit gezeigt, dass auch in diesem Bereich eine aussagekräftige Datenlage mit guter Evidenz und Vergleichbarkeit vorliegt. Ein therapeutischer Ansatzpunkt, die Wundheilung zu fördern, ist die Hemmung von Matrixmetalloproteinasen beispielsweise durch Sucrose Octasulfat. Für Wundprodukte mit TLC-Sucrose Octasulfat wurden in den letzten Jahren mehrere, aufeinander aufbauende klinische Studien durchgeführt. Die WHAT-Studie war eine offene RCT (randomisierte kontrollierte Studie) mit 117 Patienten mit Ulcus cruris venosum. Bei der CHALLENGE-Studie handelte es ich um eine doppelblind durchgeführte RCT mit 187 Patienten mit Ulcus cruris venosum. Die SPID-Studie war eine Pilotstudie mit 33 Patienten mit diabetischem Fußulkus (DFU). In 2 prospektiven, multizentrischen klinischen Pilotstudien NEREIDES und CASSIOPEE wurden insgesamt 88 Patienten mit Ulcera crurum in verschiedenen Heilungsphasen untersucht. In der REALITY-Studie erfolgte eine gepoolte Datenanalyse über 8 Anwenderbeobachtungen mit 10.220 Patienten mit chronischen Wunden unterschiedlicher Genese. In der doppelblinden, zweiarmigen EXPLORER-RCT wurden 240 Patienten mit neuroischämischem DFU erstmalig bis zur vollständigen Abheilung untersucht. In allen Studien konnte eine signifikante Förderung der Wundheilung durch den Einsatz von Wundheilungsprodukten mit TLC-Sucrose Octasulfat gezeigt werden.

Objective: Matrix metalloproteinases (MMPs) substantially contribute to the development of chronicity in wounds. Thus, MMP-inhibiting dressings may support healing. A systematic review was performed to determine the existing evidence base for the treatment of hard-to-heal wounds with these dressings. Methods: A systematic literature search in databases and clinical trial registers was conducted to identify randomised controlled trials (RCTs) investigating the efficacy of MMP-inhibiting dressings. Studies were analysed regarding their quality and clinical evidence. Results: Of 721 hits, 16 relevant studies were assessed. There were 13 studies performed with collagen and three with technology lipido-colloid nano oligosaccharide factor (TLC-NOSF) dressings. Indications included diabetic foot ulcers, venous leg ulcers, pressure ulcers or wounds of mixed origin. Patient-relevant endpoints comprised wound size reduction, complete wound closure, healing time and rate. Considerable differences in the quality and subsequent clinical evidence exist between the studies identified. Substantial evidence for significant improvement in healing was identified only for some dressings. Conclusion: Evidence for the superiority of some MMP-inhibiting wound dressings exists regarding wound closure, wound size reduction, healing time and healing rate. More research is required to substantiate the existing evidence for different types of hard-to-heal wounds and to generate evidence for some of the different types of MMP-inhibiting wound dressings.

A large body of animal and human laboratory research has linked social interaction and support to pain perception, with a possible role for the neuropeptide oxytocin as a neuroendocrine mediator. However so far, it has been unclear whether these effects translate to ecologically valid everyday life behavior and pain perception. In a randomized placebo-controlled study, a standard suction blister skin wound was induced to N = 80 romantic couples (N = 160 individuals). Couples then received intranasal oxytocin or placebo twice daily and were either instructed to perform a positive social interaction (partner appraisal task, PAT) once in the laboratory and two times during the following five days, or not. During these days, all participants reported their subjective pain levels multiple times a day using ecologically momentary assessment. Results from hierarchical linear modeling suggest that pain levels within the couples were inter-related. In men, but not in women, oxytocin reduced pain levels. Women reported lower pain levels in the group of positive social interaction, while this effect did not show in men. These results suggest that intranasal oxytocin might have sex-specific effects with pain reducing effects in men but the opposite effects in women. In contrast, especially women benefit from positive interaction in terms of dampened pain levels after positive interaction. The results add to the evidence for health-beneficial effects of positive couple interaction and point to underlying neuroendocrine mechanisms in everyday life pain specifically. The sex-specific effects, in particular, may have implications for psychopharmacological treatment of pain in men and women.

Background: Oxygen is mandatory for almost all wound healing processes, in particular the processes of cell regeneration and angiogenesis. As it is lacking in chronic and even acute wounds, oxygen substitution seems to be an obvious adjuvant tool. In vitro and in vivo tests showed improvement of microenvironment essential for the onset of appropriate healing processes.

Zusammenfassung. Vier Pathologien bilden zusammen die makrovaskulären Ursachen des Ulcus cruris: Venöse Ulzera (50 %), gemischte venös-arterielle Ulzera (20 %), arterielle Ulzera (5 %) und das Ulcus hypertonicum Martorell (5 %). Die übrigen 20 % verteilen sich auf sehr viele weitere Ursachen. Jedes Ulcus cruris erfordert eine vaskuläre (arterielle und venöse) Abklärung, gegebenenfalls ergänzt durch Biopsie, Mikrobiologie, und eine vertiefte internistische Diagnostik. Venöse Ulzera werden zunächst durch Kompression therapiert. Insuffiziente Stammvenen und deren Seitenäste werden saniert, sofern das tiefe Venensystem durchgängig ist. Okkludierte Beckenvenen werden nach Möglichkeit rekanalisiert und gestentet. Refraktäre venöse Ulzera werden je nach Fläche mit Spalthaut oder Punch Grafts gedeckt. Je nach Ausdehnung der Dermatolipofasziosklerose kann zuvor die Fibrose mit der «Shave-Therapie» oder Fasziektomie abgetragen werden. Mit der Unterdruck-Wundtherapie können tiefere oder kritisch kolonisierte Wunden konditioniert werden. Hautersatzverfahren sind eine Alternative zur Behandlung oberflächlicher venöser Ulzera ohne Epithelisierungstendenz. Für alle weiteren Indikationen muss ihr Stellenwert noch näher untersucht werden. In der chirurgischen Behandlung komplexer chronischer Wunden führt der Aufbau einer Neo-Dermis vor einer Hautverpflanzung zu einer stabileren Narbe. Gemischte venös-arterielle Ulzera (Ulcus cruris mixtum) heilen langsamer und rezidivieren häufiger. Die Kompressionstherapie muss reduziert werden. Bei ausbleibendem Therapieerfolg wird eine arterielle Revaskularisation angestrebt, womit das gemischte in ein venöses Ulcus cruris umgewandelt wird. Arterielle Ulzera (Ulcus cruris arteriosum) benötigen in aller Regel eine arterielle Revaskularisation und danach eine Spalthautverpflanzung. Das Ulcus hypertonicum Martorell ist noch vielerorts unbekannt und wird oft mit dem Pyoderma gangraenosum verwechselt, was die Therapie in eine falsche Richtung lenkt. Wundchirurgie in kleinerer oder grösserer Form, Antibiotika-Therapie bei klinisch relevanter Superinfektion, und Hautverpflanzungen, oft mehrfach wiederholt, führen in aller Regel zur Abheilung dieser äusserst schmerzhaften und bei falscher Behandlung potenziell lebensgefährlichen Wunden.

The pressure exerted by a compression device on a part of the body corresponds to the dosage of the compression therapy. Therefore, the pressure course under compression materials should be investigated in different clinical situations.Pressure measurements were carried out under different compression materials in lying, standing and walking positions within the framework of training, self-experimentation and in patients with venous leg ulcers.The results showed that the pressure varied considerably depending on the material used, the firmness of application, the local configuration (body position) and the time interval between applications. A loss of pressure occurred under each compression therapy, especially under inelastic short-stretch material, mainly due to movement and edema reduction. This pressure loss is decisive for the timing of dressing changes and a reason for the good tolerance of high-pressure levels in mobile patients.Low pressures are particularly suitable for edema reduction. Hemodynamic effects require higher pressures (60-80 mmHg). For this purpose, inelastic materials are preferred which enable lower pressures when lying down (40-60 mmHg). As compression bandages are too loosely applied by many users, pressure indicators on bandages or adaptive bandages with templates are helpful to apply the material with the correct pressure. As a consequence of these findings it is postulated that, at least in studies comparing different compression media, pressure measurements should be carried out in the future, whereby the measuring point and body position should be documented.

Leg ulcers (ulcus cruris): The frequent macrovascular causes

Die Herausforderungen der modernen Wundversorgung wie die Behandlung chronischer Wunden oder eine phasengerechte Versorgung sind anspruchsvoll und erfordern optimal angepasste Therapiemasnahmen. Die Prinzipien der feuchten Wundbehandlung sowie ein adaquates Debridement stehen dabei im Vordergrund. Um diese erforderlichen Masnahmen zu unterstutzen, stehen mehrere Optionen zu Verfugung, u. a. eine neue Produktklasse mit phasenubergreifender Wirkung. Es erfolgt die Vorstellung eines Expertenkonsenses zu einer neuen Behandlungsoption in der modernen Wundversorgung. Auf Basis klinischer Erfahrungen und den Grundlagen publizierter Evidenz wurden aktuelle und neue Prinzipien der Wundbehandlung in einer Expertenrunde diskutiert und als Konsens formuliert. Enzym-Alginogele bestehen aus einer Wirkstoffkombination mit autolytischen, absorbierenden sowie antimikrobiellen Eigenschaften, die 3 Wundmanagementkomponenten des TIME-Konzepts abdecken. Dadurch unterscheiden sie sich von anderen Praparaten und konnen gemas Expertenkonsens als eigene Produktklasse klassifiziert werden. Evidenz fur die Wirksamkeit der Enzym-Alginogele belegen klinische Studien sowie Erfahrungen aus der klinischen Praxis. Das Potenzial der Enzym-Alginogele, angewendet unter Berucksichtigung der Prinzipien der feuchten Wundbehandlung, liegt gemas Expertenmeinung v. a. in der dreifachen Wirkung (kontinuierliches und deutlich vereinfachtes Debridement, Aufrechterhaltung eines feuchten Wundmilieus, antimikrobielle Wirkung ohne Zytotoxizitat) sowie der einfachen und flexiblen Anwendung. Zudem birgt die Flexibilitat dieser Produktklasse hinsichtlich Anwendungshaufigkeit, Behandlungsdauer und der Kombination mit Sekundarverbandstoffen medizinokonomisches Potenzial.

Zusammenfassung: Pruritus ani ist eines der häufigsten Symptome, die den Patienten in die proktologische Sprechstunde führen. Als Ursache können zahlreiche Krankheiten zugrunde liegen. Bei einem Großteil aller Patienten mit Pruritus ani finden sich dermatologische Ursachen. Fast jede Dermatose kann sich auch perianal manifestieren und zu Pruritus führen. Eine Blickdiagnose ist selten möglich, da die klinische Präsentation häufig atypisch ist. So sind für die Diagnosestellung neben ausführlicher Anamnese und körperlicher Untersuchung mit Ganzkörperhautstatus mikrobiologische und allergologische Abklärungen sowie eine Hautbiopsie wichtige diagnostische Maßnahmen. Begleitend zur Behandlung der Grundkrankheit empfiehlt sich als Basistherapie eine milde Reinigung mit seifenfreiem Wasser, anschließendem vorsichtigen Trocknen sowie die Anwendung von rückfettenden oder zinkhaltigen Externa

Zusammenfassung: Hintergrund: Das Ulcus cruris ist ein häufiges Symptom einer heterogenen Krankheitsgruppe, das stark belastend ist und aufgrund der langen Abheilungsdauer und aufwendigen Wundversorgung erhebliche Kosten verursacht. Es gibt wenig Information zur Abheilungsdauer und der Notwendigkeit einer Spitalbehandlung bei Ulzera verschiedener Ätiologien. Material und Methoden: In einer retrospektiven Studie wurden die Abheilungsdauer und die Häufigkeit der Hospitalisation von 355Patienten mit Ulcus cruris in der Wundsprechstunde einer universitären Klinik untersucht. Ergebnisse: Bei einer durchschnittlichen Behandlungsdauer von 6,1Monaten für alle Ulzera waren nach 3Monaten 32,0 % und nach 6Monaten 54,2 % abgeheilt. Für die venösen Ulzera waren die Abheilungsraten mit 45,5 und 63,0 % nach 3 respektive 6Monaten höher, während nach 6Monaten lediglich 30,0 % der Patienten mit gemischt arteriell-venösen Ulzera und 35,0 % mit Ulcus hypertonicum Martorell eine Abheilung zeigten. Zudem wurden 71 % der Patienten der letzteren Gruppen mindestens 1-mal hospitalisiert im Vergleich zu 47 % der Patienten mit venösem Ulkus, und die durchschnittliche Hospitalisationsdauer war mit 30 vs. 23Tagen länger. Schlussfolgerungen: Diese Daten zeigen, dass sich die Abheilungsdauer von Ulzera je nach Ätiologie erheblich unterscheidet und insbesondere Ulzera, die durch eine Arteriosklerose mitverursacht sind, eine längere Behandlungsdauer benötigen. Da Letztere auch häufiger und länger hospitalisiert werden, ergeben sich erhebliche soziale und ökonomische Konsequenzen

Hidradenitis suppurativa (HS) is a painful, inflammatory, debilitating skin disease with a chronic intermittent course. The central pathogenetic event seems to be the occlusion of the hair follicle. HS has a 1-year prevalence of about 1%. It typically presents after puberty with painful, deep-seated, inflamed lesions in the apocrine gland-bearing areas of the body: most commonly the axillae, inguinal, and anogenital regions. HS has a high negative impact on patients' quality of life even in patients with only limited disease burden, and the diagnosis of HS is often made with a long diagnostic delay. In this practical short version we present diagnostic and therapeutic recommendations which are based on a systematic literature search as well as an informal expert consensus of Swiss dermatologists and dermatosurgeons.

Rosacea (in German sometimes called 'Kupferfinne', in French 'Couperose' and in Italian 'Copparosa') is a chronic and frequently relapsing inflammatory skin disease primarily affecting the central areas of the face. Its geographic prevalence varies from 1% to 22%. The differential diagnosis is wide, and the treatment is sometimes difficult and varies by stage of rosacea. For erythematous lesions and telangiectasia, intense pulsed light (IPL) therapy and lasers are popular treatment option. In addition, a vasoconstrictor agent, brimonidine, has recently been developed. For papulopustular rosacea, topical antibiotics, topical and systemic retinoids, as well as systemic antibiotics are used. A topical acaricidal agent, ivermectin, has undergone clinical development and is now on the market. In the later stages, hyperplasia of the sebaceous glands develops, resulting in phymatous growths such as the frequently observed bulbous nose or rhinophyma. Ablative laser treatments have largely replaced classical abrasive tools. Here, we reviewed the current evidence on the treatment of rosacea, provide a guideline (S1 level) and discuss the differential diagnosis of rosacea.

Background: To evaluate efficacy and cost-effectiveness of a plant-derived wound dressing (ONE), a mixture of hypericum oil (Hypericum perforatum L.) and neem oil (Azadirachta indica A. Juss.), in scalp wounds with exposed bone. Patients and Methods: All patients with scalp wounds with exposed bone, following the excision of skin tumors, and treated with ONE in 2011 were included. Time of healing, wound size, area of exposed bone, ease of handling, pain, and complications were evaluated. Costs of dressings and nursing time were compared with those cited in literature for other treatment modalities. Results: Fifteen consecutive patients with a mean age of 76.87 ± 10.3 years (59-90 years) were analyzed. The mean wound size was 10.9 ± 6.84 cm2 (0.4-22.6 cm2) with 4.8 ± 5.9 cm2 (0.3-20.7 cm2) of exposed bone. The time of complete healing by secondary intention was 8.1 (4-20) weeks. Rapid formation of granulation tissue was observed which after 4 weeks covered the entire exposed bone surface in 11 of 15 cases (73%). Dressing change was simple with no pain reported; no infections or other complications occurred. Using ONE for a mean healing time of 56.7 days resulted in mean costs of EUR 423.73, which is substantially lower than those published for fascia lata, negative pressure therapy, or collagen matrix followed by skin grafting (EUR 1,612.82, EUR 4,411.80 and EUR 1,503.72, respectively). Conclusion: This retrospective, non-controlled analysis supports ONE as a simple-to-use and safe treatment option for scalp wounds with exposed bone. Treatment costs compare favorably to those published for other treatment modalities.

Introduction Martorell hypertensive ischemic leg ulcer (HYTILU) and pyoderma gangrenosum share a violaceous, rapidly progressive necrotic wound border and excruciating pain as essential clinical features. Clinicians unaware of Martorell HYTILU readily misdiagnose pyoderma gangrenosum or necrotic vasculitis. This can be detrimental since disease management is totally different. Material and methods This review includes two patient series with Martorell hypertensive ischemic leg ulcer (HYTILU), 31 patients from the University Hospital of Zurich and 64 patients from a randomized controlled trial in France. The analysis of confounding Martorell HYTILU and histological study is based on the Zurich data and the analysis of therapeutic issues is based on the French and Zurich data. Results Fifty-two percent (16/31 patients) were referred with an erroneous diagnosis of pyoderma gangrenosum, and another 19% (6/31 patients) with an erroneous diagnosis of necrotizing vasculitis. The hallmark of Martorell HYTILU are the location at the laterodorsal leg or Achilles tendon (in the present series 100%) in a patient with usually long-standing and controlled hypertension (95–100%), often accompanied by diabetes (40–60%). Histology of a several centimeter long, 4–6 mm narrow, and deep (to fascia) performed spindle-shape skin and ulcer biopsy shows a skin infarction with sclerotic subcutaneous arterioles. Arteriolosclerosis is defined by thick vessel walls at the cost of a narrow lumen, and in approximately 70% of histologic sections part of the arterioles show a striking medial calcification. The narrow lumen may be occluded by acute or organized thrombosis. In the French multicenter serie, at 8 weeks only 10% (3/31 patients) healed by conservative means, and this could not be improved with the use of topical application of platelet-derived growth factor-BB (becaplermin) (18% healing, 5/28 patients, no significant difference). In the Zurich series, 84% of patients (26/31) had wound surgery (necrosectomy, split skin graft) to heal the ulcer and 10% (3/31) died from wound infection and sepsis, amongst two under immunosuppression received for misdiagnosed pyoderma gangrenosum. Conclusions Martorell HYTILU can easily be confounded with pyoderma gangrenosum or necrotizing vasculitis. Physicians involved in wound treatment should be sensitized to the typical clinical setting: a progressive and extremely painful skin infarction at the laterodorsal leg in a hypertensive and often diabetic subject. Diagnosis is confirmed histologically on a long-enough, narrow, but deep spindle-shape skin and ulcer biopsy, showing a highly characteristic form of stenotic and occlusive subcutaneous arteriolosclerosis. Management is essentially based on wound surgery. Conservative means are almost uneffective. Immunosuppression must be avoided, and antibiotic treatment introduced as required.

Malgre sa prevalence elevee pouvant atteindre 1%, cette maladie est souvent interpretee comme une furonculose recidivante et aucun traitement efficace n’est mis en place. Cet article resume les aspects principaux de cette pathologie facile a reconnaitre et propose quelques diagnostics differentiels a envisager en cas d’abces dans les grands plis cutanes.

Wund-D.A.CH. ist der Dachverband deutschsprachiger Fachgesellschaften, die sich mit den Thematiken der Wundbehandlung beschaftigen. Experten verschiedener Fachgesellschaften aus Deutschland, Osterreich und der Schweiz haben nun einen aktuellen Konsens der Kompressionstherapie fur Patienten mit Ulcus cruris venosum erstellt. In Europa ist das Ulcus cruris venosum eine der haufigsten Ursachen fur chronische Wunden. Neben der konservativen und interventionellen Wund- und Venentherapie, ist die Kompressionstherapie die Basis der Behandlungsstrategien. Die Kompressionstherapie kann heute mit sehr unterschiedlichen Materialien und Systemen durchgefuhrt werden. Wahrend in der Entstauungsphase insbesondere Verbande mit Kurzzugbinden oder Mehrkomponentensysteme zur Anwendung kommen, sind es anschliesend uberwiegend Ulkus-Strumpfsysteme. Eine weitere, bislang wenig verbreitete Alternative sind adaptive Kompressionsbandagen. Insbesondere fur die Rezidivprophylaxe werden medizinische Kompressionsstrumpfe empfohlen. Durch die Vielzahl der heute zur Verfugung stehenden Behandlungsoptionen, kann fur nahezu alle Patienten ein Konzept entwickelt werden, dass sich an den individuellen Bedurfnissen und Fahigkeiten orientiert und daher auch akzeptiert und durchgefuhrt wird. Die Kompressionstherapie ist fur die Behandlung von Patienten mit Ulcus cruris venosum essentiell. In den letzten Jahren sind viele verschiedene Therapieoptionen verfugbar, die in den deutschsprachigen Landern unterschiedlich angewendet oder durchgefuhrt werden. Daher soll dieser Expertenkonsens dazu beitragen, konkrete Empfehlungen fur die praktische Durchfuhrung der Kompressionstherapie von Patienten mit Ulcus cruris venosum darzustellen.

Wund-D.A.CH. is the umbrella organization of the various wound care societies in German-speaking countries. The present consensus paper on practical aspects pertinent to compression therapy in patients with venous leg ulcers was developed by experts from Germany, Austria, and Switzerland. In Europe, venous leg ulcers rank among the most common causes of chronic wounds. Apart from conservative and interventional wound and vein treatment, compression therapy represents the basis of all other therapeutic strategies. To that end, there are currently a wide variety of materials and systems available. While especially short-stretch bandages or multicomponent systems should be used in the initial decongestion phase, ulcer stocking systems are recommended for the subsequent maintenance phase. Another - to date, far less common - alternative are adaptive Velcro bandage systems. Medical compression stockings have proven particularly beneficial in the prevention of ulcer recurrence. The large number of treatment options currently available enables therapists to develop therapeutic concepts geared towards their patients' individual needs and abilities, thus resulting in good acceptance and adherence. Compression therapy plays a crucial role in the treatment of patients with venous leg ulcers. In recent years, a number of different treatment options have become available, their use and application differing among German-speaking countries. The present expert consensus is therefore meant to outline concrete recommendations for routine implementation of compression therapy in patients with venous leg ulcers.

Polyhexanidhaltige Wundbehandlungs-Produkte werden häufig für die Behandlung von akuten und chronischen Wunden eingesetzt. Obwohl ausreichende Informationen zur Verwendung von Polyhexanid in der Literatur zu finden sind, ist die angemessene Anwendung dieser Produkte in der klinischen Praxis nicht immer klar. Das Ziel dieser Konsensus-Empfehlung ist es, Ärztinnen/Ärzten und Pflegefachleuten, die Patienten mit akuten und chronischen Wunden behandeln, klinisch relevante Empfehlungen zur Verfügung zu stellen. Dieser Bericht beschreibt die klinisch relevanten Eigenschaften von Polyhexanid und gibt Empfehlungen für die Prophylaxe und Behandlung von Wundinfektionen.

Rund ein Viertel der Patienten mit einem Ulcus cruris haben ein "nicht-vaskuläres" Ulkus, d. h. kein venöses, kein gemischt venös-arterielles, und kein arterielles Ulkus. Die Differenzialdiagose umfasst etwa 70 Entitäten. Im Alltag relevant sind das hypertensiv-ischämische Ulkus (Ulcus hypertonicum Martorell) und seine beiden "Imitatoren", das Pyoderma gangraenosum und die nekrotisierenden Vaskulitiden der Haut, Hautnekrosen in Folge von Infekten (Ecthymata u. a.), durch physikalische Traumen verursachte chronische Hautwunden (z. B. Kontusionen [tiefes dissezierendes Hämatom]), oder nach Radiotherapie oder Kryotherapie wegen Hautkrebs am Unterschenkel), im Rahmen kongenitaler Krankheiten auftretende Ulzera (beispielsweise bei Klinefelter-Syndrom oder Sichelzellanämie), Hautnekrosen als Folge von Medikamenten-Toxizität (z. B. unter Hydroxyurea, Anagrelid oder Methotrexat) und last not least die ulzerierten Hauttumore (primär und sekundär bei maligner Entartung auf chronischer Entzündung). Die klinische Präsentation sowie ein pragmatischer Algorhythmus zur Abklärung und Therapie dieser Krankheiten werden vorgestellt.
