19. Care and occupational therapy: what kind of care definition do we have?/Cuidado e Terapia Ocupacional: que tipo de definição de cuidado nós temos?

Abstract

Care can mean different things to different people. Often, it is understood as referring to professional actions associated with an individualistic approach. In the context of healthcare, it is usually understood as a professional action, referring to service provision. In occupational therapy, the definition of care is not well explored. Drawing on sociological and anthropological literature, we performed a non-systematic literature review to problematize the definition of care and its application in occupational therapy. Defining care as an expression of social support, a fundamental attribute for maintaining life and the basis for the development of essential components of social life, we discuss four of its dimensions: the biomedical, the person-centred, the tacit, and the collective. We argue that occupational therapy, historically, has been aligned with biomedical care and has made moves towards the person-centred dimension. We argue that occupational therapists should explore ways to articulate all four care dimensions of care to inform their practice.

Introducción. La producción de conocimiento en el área de terapia ocupacional ha avanzado en todo el mundo.Objetivo. Comprender cómo se ha desarrollado el campo de la terapia ocupacional como disciplina académica, en particular en lo que se refiere a sus áreas de investigación en las universidades.Materiales y métodos. Se mapearon los programas de postgrado en terapia ocupacional (maestrías y doctorados, académicos y profesionales) que fueron registrados en la Federación Mundial de Terapeutas Ocupacionales (WFOT) hasta 2015. También se consultaron los sitios web de cada programa para completar las informaciones.Resultados. Se recogieron y analizaron datos de 266 instituciones que ofrecían programas de postgrado en terapia ocupacional. Hasta 2015, había 348 programas en 11 países: 225 maestrías profesionales, 69 maestrías académicas, 30 doctorados profesionales (OTD) y 24 doctorados académicos (PhD). Este tipo de especializaciones existen desde 1918 y han tenido dos grandes períodos de crecimiento: uno en 1940 y otro 1990.Conclusiones. El número de programas de posgrado en terapia ocupacional, especialmente a nivel doctoral, es bastante limitado. Soflo el 16% de los países que ofrecen cursos de pregrado también incluyen programas de maestría o doctorado., Introduction: The production of knowledge in Occupational Therapy has been making progress worldwide.Objective: To understand how Occupational Therapy has developed as an academic discipline, particularly with respect to its areas of research at different universities.Methods: The Occupational Therapy postgraduate programs around the world (such as master’s degrees and doctoral programs), registered at World Federation of Occupational Therapists (WFOT) until 2015, were mapped and the websites of each program were visited to complete the information.Results: Data from 266 institutions that offered postgraduate programs were collected and analyzed. As of 2015, there were 348 programs in 11 countries; 225 professional master’s degrees, 69 academic master’s degrees, 30 post-professional doctorates in Occupational Therapy (PPOTD), and 24 academic doctorates (PhD). Such programs have existed since 1918 and had two major growth periods in 1940 and 1990.Conclusions: The number of postgraduate programs in occupational therapy is fairly limited. Only 16% of the countries that offer undergraduate courses also have master’s or doctoral programs. There are few postgraduate programs, especially at the doctoral level, which has limited the academic scope of this field.

24. The development of occupational science outside the anglophone sphere: enacting global collaboration

Abstract

[Abstract] The emergence of occupational science in non-English speaking countries is frequently hampered by diverse barriers to global collaboration, knowledge dissemination, and inclusion in international dialogue. Epistemological, cultural, and institutional resources may explain these barriers, yet these have not been explored within the discipline. This paper discusses three main issues and three priorities for action put forward by participants during sessions held at two separate, international occupational science conferences. The sessions aimed to engage the audience in critical reflexivity and dialogue around the challenges present when non-English speaking countries attempt to develop occupational science scholarship and possible ways to support global collaboration. To stimulate discussion, we used a participatory methodology, ‘Metaplan’. The sessions included a statements exercise, reflections presented by the authors, individual reflexivity, and small group debate. The findings are structured as a reflexive dialogue where participants’ voices, theory, and the authors’ reflections are interwoven to enrich discussion of the issues participants identified and priorities for action. This paper contributes to decolonizing the development of occupational science and promoting an international dialogue that is open to diverse worldviews, by drawing attention to the visible and invisible barriers that limit collaboration and inclusion of the diverse ways in which occupation is understood and enacted worldwide

[Resumen] El desarrollo de la ciencia ocupacional en países no anglófonos enfrenta constantes barreras que impactan su participación en instancias de colaboración global, en la difusión del conocimiento y en el diálogo a nivel internacional. Algunos de estos obstáculos se deben a motivos epistemológicos, culturales e incluso institucionales y aún no han sido estudiados en el interior de la propia disciplina. Este artículo discute tres problemas principales y tres líneas de acción prioritarias que fueron planteadas por los participantes de dos sesiones llevadas a cabo en dos conferencias internacionales sobre la ciencia ocupacional. Las sesiones tenían por objetivo involucrar a la audiencia en el diálogo y la reflexión crítica en torno a los desafíos que se presentan cuando países no anglófonos intentan desarrollar estudios en ciencia ocupacional y posibles maneras de apoyar la colaboración global. Para estimular el debate, usamos una metodología participativa denominada ‘Metaplan’. La sesión incluyó un ejercicio de declaraciones, el planteamiento de reflexiones por parte de las autoras, reflexiones individuales y un breve debate grupal. Los resultados se estructuran en forma de diálogo reflexivo que vincula las opiniones de los participantes con la teoría y las reflexiones de las autoras para enriquecer la discusión de los temas y las líneas de acción prioritarias identificadas por los participantes. En ese contexto, este artículo se plantea como una contribución a la descolonización del desarrollo de la ciencia ocupacional y la promoción de un diálogo internacional abierto a distintas formas de concebir el mundo, y llama la atención sobre los obstáculos visibles e invisibles que limitan la colaboración y la incorporación de las diversas formas de comprender y representar la ocupación a nivel global.

28. Política brasileña para la juventud: la propuesta de los Centros de Juventud

Tendo como base a Política Nacional da Juventude no Brasil (PNJ), buscou-se compreender a proposta dos Centros da Juventude (CJ's), espaços específicos de cultura e lazer para a juventude. Como método, foram entrevistados gestores de CJ's em quatro cidades brasileiras, o gestor estadual e o nacional. Os resultados demonstraram diferenças entre os discursos em cada nível governamental, evidenciando um distanciamento nos seus posicionamentos. Nos níveis federal e estadual destacaram a garantia ao acesso de direitos. Por outro lado, os gestores municipais apontaram sua agenda política na assistência social, direcionada aos jovens pobres, na focalização da atenção ao invés da universalidade dos direitos. Portanto, os CJ's pouco fazem interface com a PNJ, demonstrando que as ações governamentais brasileiras precisam avançar na percepção do jovem e da jovem como sujeitos de direitos. This study draws on the National Youth Policy of Brazil with the goal of understanding the role of Youth Centers (YCs) that are specific places where young people can engage in culture and leisure activities. The method used in this study was in-depth interviews with YC managers in four Brazilian cities, as well as the State Manager and with National Manager of the program. The results showed differences in the discourse at each government level, evidencing a gap in their positions. The discourse from the federal and state levels highlighted the guarantee for young people to access their rights. In contrast, managers from the municipal level demonstrated that their political agenda is based on the provision of social assistance that targets poor young people while focusing their attention at the same time on the universality of rights. The authors conclude that the YCs have a limited connection with the National Youth Policy, demonstrating that the Brazilian government's actions need to improve the perception of young people as subjects of rights. Con base en la política nacional de la juventud de Brasil, en esta investigación se buscó comprender la propuesta de los Centros para Juventud (CJ's), que son espacios de promoción de la cultura y del ocio para la juventud. Como método de investigación se entrevistó a los gerentes de los CJ's en cuatro ciudades brasileñas, así como a los gerentes a nivel estatal y nacional. Los resultados presentaron diferencias entre los discursos en cada nivel del gobierno. En los niveles federales y estatales se destacó la garantía de acceder a los derechos de los jóvenes. Sin embargo, los gestores municipales mostraron su agenda política guiada por la asistencia social dirigida a los jóvenes pobres, centrando la atención en una problemática con el fin de garantizar la universalidad de los derechos. Por lo tanto, hay pocas interfaces entre CJ's con nuevas políticas para los jóvenes. La investigación muestra que las acciones del gobierno brasileño deben avanzar en la percepción de los jóvenes como sujetos de derechos.

Abstract

Background: Calls for embracing the potential and responsibility of occupational therapy to address socio-political conditions that perpetuate occupational injustices have materialized in the literature. However, to reach beyond traditional frameworks informing practices, this social agenda requires the incorporation of diverse epistemological and methodological approaches to support action commensurate with social transformative goals. Aim: Our intent is to present a methodological approach that can help extend the ways of thinking or frameworks used in occupational therapy and science to support the ongoing development of practices with and for individuals and collectives affected by marginalizing conditions. Method: We describe the epistemological and theoretical underpinnings of a methodological approach drawing on Freire and Bakhtin's work. Results: Integrating our shared experience taking part in an example study, we discuss the unique advantages of co-generating data using two methods aligned with this approach; dialogical interviews and critical reflexivity. Discussion: Key considerations when employing this approach are presented, based on its proposed epistemological and theoretical stance and our shared experiences engaging in it. Significance: A critical dialogical approach offers one way forward in expanding occupational therapy and science scholarship by promoting collaborative knowledge generation and examination of taken-for-granted understandings that shape individuals assumptions and actions. Na última década, o investimento público brasileiro para a juventude constituiu um discurso sobre política pública no cenário nacional. Destaca-se a criação da Secretaria Nacional de Juventude, em 2005. Com o intuito de mapear o que vem ocorrendo, traçou-se um panorama histórico das ações políticas nos últimos 12 anos, no nível federal, com base nos documentos oficiais e nas leis disponibilizadas. Segundo os registros oficiais, conclui-se que houve um avanço na agenda pública para esta população, sendo que há um enfoque predominante em um recorte de classe social, priorizando políticas para a juventude pobre. En la última década, la inversión pública brasileña para la juventud ha producido un discurso de las políticas públicas en el ámbito nacional, destacando la creación de la Secretaría Nacional de la Juventud en 2005. Esto con el objetivo de consignar lo que está ocurriendo y elaborar una visión histórica de las acciones políticas en los últimos 12 años a nivel federal basado en documentos y leyes oficiales disponibles. De acuerdo con los registros oficiales, es posible concluir que no ha habido un gran avance en la agenda pública para esta población, aun habiendo un interés predominante en la clase social, con la priorización de políticas para la juventud pobre. In the last decade, the Brazilian public investment for youth has produced a speech about public policy at the national level. It is important to mention the creation of the National Youth Secretariat in 2005. In order to map what is happening, it was developed a historical vision of political actions in the last 12 years at the federal level, based on official documents and official laws available. According to official records, it is concluded that there has not been a progress on the public agenda for this population, despite having a predominant focus in social class, prioritizing social policies for poor youth.
