Abstract

Objective: A substantial number of children and adolescents with ADHD show severe affective and behavioral dysregulation. A dysregulation profile (DP) distinguishes those participants at a higher risk of comorbidity, dysfunction, and a poorer response to treatment. It was identified as high scoring in certain subscales of the Strengths and Difficulties Questionnaire (SDQ-DP). We developed a cross-sectional study to assess the clinical utility of the SDQ-DP in ADHD. Method: Two hundred fifty clinic-referred children and adolescents with ADHD were assessed. Univariate and linear regression analyses were performed to compare those participants with high levels of SDQ-DP with those with low levels, as well as to examine the association between the SDQ-DP and clinical variables. Results: In all, 28% of ADHD participants had high levels of SDQ-DP. These participants showed higher functional impairment and clinical severity. Conclusion: The use of the SDQ-DP will enable identification of those ADHD patients at greater risk. [ABSTRACT FROM AUTHOR]

Monográfico con el título: “Trastorno por Déficit de Atención e Hiperactividad (TDAH) y escuela : inquietudes compartidas” Resumen basado en el de la publicación Título, resumen y palabras clave también en inglés Se explica en qué consiste el Trastorno por Déficit de Atención e Hiperactividad (TDAH) y la necesidad de tratamiento médico en algunos pacientes por gravedad o falta de mejoría con otras intervenciones. Se argumenta que debe basarse en una adecuada valoración del menor y la familia, que debe formar parte de un abordaje integral y que el seguimiento por el especialista es imprescindible. Como medida de tratamiento, los psicoestimulantes y la atomoxetina son los fármacos indicados. En este sentido, se deben monitorizar los beneficios clínicos y los efectos adversos con información de la familia y la escuela. Finalmente, hay que facilitar información del tratamiento farmacológico al paciente y los familiares y, si estos dan su consentimiento, a los profesionales de la enseñanza. ESP

Abstract

Available treatment methods have shown little effect on the burden associated with mental health disorders. We review promising universal, selective, and indicated preventive mental health strategies that might reduce the incidence of mental health disorders, or shift expected trajectories to less debilitating outcomes. Some of these interventions also seem to be cost-effective. In the transition to mental illness, the cumulative lifetime effect of multiple small effect size risk factors progressively increases vulnerability to mental health disorders. This process might inform different levels and stages of tailored interventions to lessen risk, or increase protective factors and resilience, especially during sensitive developmental periods. Gaps between knowledge, policy, and practice need to be bridged. Future steps should emphasise mental health promotion, and improvement of early detection and interventions in clinical settings, schools, and the community, with essential support from society and policy makers.
