Background: Fast dose calculation is critical for online and real‐time adaptive therapy workflows. While modern physics‐based dose algorithms must compromise accuracy to achieve low computation times, deep learning models can potentially perform dose prediction tasks with both high fidelity and speed. Purpose: We present a deep learning algorithm that, exploiting synergies between transformer and convolutional layers, accurately predicts broad photon beam dose distributions in few milliseconds. Methods: The proposed improved Dose Transformer Algorithm (iDoTA) maps arbitrary patient geometries and beam information (in the form of a 3D projected shape resulting from a simple ray tracing calculation) to their corresponding 3D dose distribution. Treating the 3D CT input and dose output volumes as a sequence of 2D slices along the direction of the photon beam, iDoTA solves the dose prediction task as sequence modeling. The proposed model combines a Transformer backbone routing long‐range information between all elements in the sequence, with a series of 3D convolutions extracting local features of the data. We train iDoTA on a dataset of 1700 beam dose distributions, using 11 clinical volumetric modulated arc therapy (VMAT) plans (from prostate, lung, and head and neck cancer patients with 194–354 beams per plan) to assess its accuracy and speed. Results: iDoTA predicts individual photon beams in ≈50 ms with a high gamma pass rate of 97.72±1.93%$97.72\pm 1.93\%$ (2 mm, 2%). Furthermore, estimating full VMAT dose distributions in 6–12 s, iDoTA achieves state‐of‐the‐art performance with a 99.51±0.66%$99.51\pm 0.66\%$ (2 mm, 2%) pass rate and an average relative dose error of 0.75 ± 0.36%. Conclusions: Offering the millisecond speed prediction per beam angle needed in online and real‐time adaptive treatments, iDoTA represents a new state of the art in data‐driven photon dose calculation. Background: Ampicillin-sulbactam is a piperacillin-tazobactam-sparing alternative antibiotic administered as surgical prophylaxis during orthotopic liver transplant (OLT), but limited data are available describing its pharmacokinetics and impact of blood product resuscitation. The purpose of this study was to determine the intra-operative pharmacokinetics of ampicillin-sulbactam in patients during OLT and evaluate the effects of blood resuscitation on exposure. Patients and Methods: This was a pharmacokinetic study in 10 OLT patients receiving ampicillin-sulbactam surgical prophylaxis. A 5,000-patient Monte Carlo simulation was conducted to identify optimal ampicillin-sulbactam regimens. Linear regression assessed association between blood product administration and ampicillin exposures. Results: Ampicillin and sulbactam concentrations best fitted two-compartment models. Mean ampicillin pharmacokinetic parameters were central compartment volume (Vc): 6.9 ± 2.0 L, clearance (CL): 26.6 ± 18.4 L/h, and inter-compartmental rate constants (k12 and k21): 4.8 ± 2.6 and 2.3 ± 1.4 h-1. Sulbactam pharmacokinetic parameters were Vc: 8.1 ± 2.7 L, CL: 26.1 ± 7.4 L/h, k12 and k21: 4.9 ± 1.0 and 2.8 ± 1.1 h-1. Participants received between 500 and 23,642 mL of total blood product. No statistical relations were observed between blood product administration and exposures (R2 0.00-0.26). Ampicillin-sulbactam 2/1 g every two hours and 2/1 g bolus followed by 6/3 g continuous infusion provided acceptable probability of target attainment up to minimum inhibitory concentrations (MICs) of 16 and 32 mcg/mL, respectively. Conclusions: High and frequent ampicillin-sulbactam doses are required to maintain 100% fT > MIC at relevant MICs during OLT and no impact of blood product resuscitation was observed on ampicillin exposure. The superior death‐censored graft survival of the pancreas allograft in simultaneous pancreas kidney transplants (SPK) over pancreas alone transplants (PTA) has long been recognized. Using data from the Scientific Registry of Transplant Recipients (SRTR) and a high‐volume pancreas transplant program, we investigated the possible protective role of the kidney allograft in SPK transplants. We analyzed 19 043 primary pancreas transplants between 2000 and 2020, including 735 transplants performed at the University of Minnesota. SPK transplants demonstrated a superior death‐censored graft survival over pancreas after kidney (PAK) and simultaneous pancreas and living donor kidney (SPLK) transplants, which both demonstrated better survival than PTA transplants. This effect was not affected by mode or duration of renal replacement therapy prior to transplant. Furthermore, we found that HLA match at the B‐locus between the prior kidney and current pancreas allografts demonstrated a protective effect (HR.54; 95% confidence interval.29–1.00), with a 2‐antigen match demonstrating superior death‐censored graft survival to a 1‐ or 0‐antigen match. We propose that a homologous kidney allograft in SPK transplants affords protection to the pancreas allograft–likely through a combination of better surveillance for rejection and direct immunoprotection offered by the same‐donor kidney. Seagrass ecosystems rank amongst the most efficient natural carbon sinks on earth, sequestering CO2 through photosynthesis and storing organic carbon (Corg) underneath their soils for millennia and thereby, mitigating climate change. However, estimates of Corg stocks and accumulation rates in seagrass meadows (blue carbon) are restricted to few regions, and further information on spatial variability is required to derive robust global estimates. Here we studied soil Corg stocks and accumulation rates in seagrass meadows across the Colombian Caribbean. We estimated that Thalassia testudinum meadows store 241 ± 118 Mg Corg ha−1 (mean ± SD) in the top 1 m-thick soils, accumulated at rates of 122 ± 62 and 15 ± 7 g Corg m−2 year−1 over the last ~ 70 years and up to 2000 years, respectively. The tropical climate of the Caribbean Sea and associated sediment run-off, together with the relatively high primary production of T. testudinum, influencing biotic and abiotic drivers of Corg storage linked to seagrass and soil respiration rates, explains their relatively high Corg stocks and accumulation rates when compared to other meadows globally. Differences in soil Corg storage among Colombian Caribbean regions are largely linked to differences in the relative contribution of Corg sources to the soil Corg pool (seagrass, algae Halimeda tuna, mangrove and seston) and the content of soil particles < 0.016 mm binding Corg and enhancing its preservation. Despite the moderate areal extent of T. testudinum in the Colombian Caribbean (661 km2), it sequesters around 0.3 Tg CO2 year−1, which is equivalent to ~ 0.4% of CO2 emissions from fossil fuels in Colombia. This study adds data from a new region to a growing dataset on seagrass blue carbon and further explores differences in meadow Corg storage based on biotic and abiotic environmental factors, while providing the basis for the implementation of seagrass blue carbon strategies in Colombia. The clinical course and outcomes of immunocompromised patients, such as transplant recipients, with COVID‐19 remain unclear. It has been postulated that a substantial portion of the disease burden seems to be mediated by the host immune activation to the severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2). Herein, we present a simultaneous heart‐kidney transplant (SHKT) recipient who was hospitalized for the management of respiratory failure from volume overload complicated by failure to thrive, multiple opportunistic infections, and open non‐healing wounds in the setting of worsening renal dysfunction weeks prior to the first case of SARS‐CoV‐2 being detected in the state of Connecticut. After his third endotracheal intubation, routine nucleic acid testing (NAT) for SARS‐CoV‐2, in anticipation of a planned tracheostomy, was positive. His hemodynamics, respiratory status, and ventilator requirements remained stable without any worsening for 4 weeks until he had a negative NAT test. It is possible that the immunocompromised status of our patient may have prevented significant immune activation leading up to clinically significant cytokine storm that could have resulted in acute respiratory distress syndrome and multisystem organ failure. Background: Medical weight loss options are rarely successful long term in young patients. Bariatric surgery has been shown to be safe and effective in this population.Methods: Patients ≤21 years old who had bariatric surgery at our institution between January 2009 and December 2013 were evaluated to determine the safety and efficacy of bariatric surgery in this population. The primary end point was excess weight loss (EWL). Secondary end points included surgical morbidity, improvement in obesity-related metabolic parameters, and subjective obesity-related symptoms at 1 year.Results: Fifty-four patients were identified who had a laparoscopic Roux-en-Y gastric bypass (LGBP) or laparoscopic sleeve gastrectomy (LSG). Fourteen patients were male (25.9 %), and 40 patients were female (74.1 %). Thirty-seven patients (68.5 %) underwent LGBP, and 17 patients (31.5 %) underwent LSG. Median follow-up was 13.3 months. The baseline BMI was 51.7 kg/m2 for the LGBP group and 51.0 kg/m2 for the LSG group. EWL was 35.2, 47.6, 62.4, 58.1, and 61.8 % for the LGBP group; 29.7, 44.7, 57.4, 60.3, and 59.0 % for the LSG group at 3, 6, 12, 24, and 36 months, respectively. Our complications included 1 anastomotic bleed, 1 postoperative stricture, and 1 patient who developed vitamin deficiency that manifested as a peripheral neuropathy in the LGBP group. LGBP was more successful than LSG in improving lipid panel parameters and HbA1c at 1 year, and it also seemed to offer better subjective improvement in obesity-related symptoms.Conclusions: LGBP and LSG seem to confer comparable weight loss benefit in patients ≤21 years old with acceptable surgical morbidity. The constant increase in the production of scientific literature is making it very difficult for experts to keep up to date with the state-of-the-art knowledge in their fields. The use of Natural Language Processing (NLP) is becoming a necessary aid to tackle this challenge. In the NLP field, the task of measuring semantic similarity between two sentences plays a vital role. It is a cornerstone for tasks like Q&A, Information Retrieval, Automatic Summarization, etc., and it is a crucial element in the ultimate goal of computers being able to decode what is conveyed in human language expression. Measuring Semantic Similarity (SS) in short texts has specific challenges. Because there are fewer words to be compared, the meaning contribution of each word is more relevant, and it is important to take into account the syntax's contribution to the composed meaning. In addition, the highly specific and specialized vocabulary — Microbial Transcriptional-Regulation—implies the lack of massive training resources. Our approach has been to use an ensemble of similarity metrics including string, distributional, and knowledge-based metric and to combine the results of such analyses. We have trained and tested these methods in a similarity corpus developed in-house. The task has proved very challenging, and the ensemble strategy has proved to be a good approach. Even though there is still much room for improvement in the precision of our methods concerning the human evaluation, we have managed to improve them reaching a strong correlation (ρ = 0.700). Seagrass meadows form valuable ecosystems, but are considered to have low cultural value due to limited research efforts in this field. We provide evidence that seagrass deposits play a hitherto unrealized central role in preserving valuable submerged archaeological and historical heritage across the world, while also providing an historical archive of human cultural development over time. We highlight three case studies showing the significance of seagrass in protecting underwater cultural heritage in Denmark, the Mediterranean and Australia. Moreover, we present an overview of additional evidence compiled from the literature. We emphasize that this important role of seagrasses is linked to their capacity to form thick sedimentary deposits, accumulating over time, thereby covering and sealing submerged archaeological heritage. Seagrass conservation and restoration are key to protecting this buried heritage while also supporting the role of seagrass deposits as carbon sinks as well as the many other important ecosystem functions of seagrasses. The following article tries to specify some methodological, conceptual and thematic aspects that delineated the development of the investigation and the historical science from the second half of the XXth century, epoch in which it became aware of the need to understand from the social sciences all the areas of human activity and to overcome the barriers traced by traditional political history. This movement of renewal materialized in the historiographical current known as the social history, a focus that used the new forms, objects of study, social theories and sources of information, rethinking the way in which the past is investigated, considering the study of other social actors and the different phenomena that involve life in community. In the final part of the article, three investigations are outlined that illustrate the interest of historians for the»other«topics, which, like the disease or female stereotypes, called attention to social history. Abstract There is considerable variability in the ability of seagrass ecosystems to sequester organic carbon (C org) in their sediments, which act as natural carbon sinks contributing to climate change mitigation. In this work, we studied the chemistry of two Posidonia seagrass species aiming to elucidate whether differences in chemical composition might explain differences in their C org sequestration capacity. Pyrolysis-gas chromatography-mass spectrometry (Py-GC-MS) and Thermally assisted Hydrolysis and Methylation (THM) GC-MS data showed a remarkable difference in phenolic compound patterns between P. oceanica and P. australis bulk plants and individual organs (leaves, sheaths, roots and rhizomes). The lignin of P. australis generates a series of p -hydroxyphenyl (H), guaiacyl (G) and syringyl (S) products that are typical of herbaceous plants, whereas P. oceanica is particularly rich in p -hydroxybenzoic acid (p BA) derivatives. The structural characteristics of the lignins were further investigated by two-dimensional Nuclear Magnetic Resonance (2D-NMR) spectroscopy and Derivatization Followed by Reductive Cleavage (DFRC), focusing on sheath tissues. The analyses confirmed important differences in the lignin content (19.8% in P. australis and 29.5% in P. oceanica) and composition between the two species; intriguingly, the cell-walls of P. oceanica sheaths were highly enriched in p BA, a component that was completely absent in P. australis. 2D-NMR and DFRC data further revealed that p BA was esterified to the lignin, acylating the γ-OH of the lignin side-chain. Interestingly, P. oceanica lignin presented an extremely high degree of p -hydroxybenzoylation in both guaiacyl (73%) and syringyl (61%) lignin units; the highest p -hydroxybenzoylation degree reported in plant lignins to date. It is tempting to conclude that the higher C org storage capacity of P. oceanica ecosystems might be related to the higher abundance of p BA-rich lignin and its recalcitrant nature. Abstract: With increasing organ demand, living kidney donation from older donors (>60‐years‐old) has become more common. Between 1975 and 2014, 3752 donor nephrectomies (DN) were performed at University of Minnesota; 167 (4.5%) were >60‐years‐old Short‐ and long‐term outcomes were compared between contemporaneous >60‐years‐old and <60‐years‐old donors. On univariate analysis, >60‐years‐old were more likely to have had prior abdominal surgery and hypertension; and less likely to smoke. Baseline estimated glomerular filtration rate (eGFR) was lower in >60‐years‐old (80 ± 16 vs 101 ± 26 mL/min/1.73 m2; P < .001). Intraoperative and postoperative complications were similar, except a higher prevalence of <30 day ileus (3% vs 7%; P = .021) and longer postoperative length of stay (LOS) (4.2 vs 4.6 days; P = .005). On multivariate analysis, <30 day ileus and LOS continued to be significantly greater for >60‐years‐old After >20 years post‐DN, systolic blood pressure was significantly higher among >60‐years‐old (142 vs 125 mm Hg; P < .001) and HTN was diagnosed earlier (9 vs 14 years). After donation, eGFR was significantly lower for >60‐years‐old but slope of eGFR and rates of end‐stage renal disease (ESRD) were not significantly different >20 years post‐DN. Background: Bariatric surgery has been established as the most effective long-term treatment for morbid obesity.Methods: We performed a retrospective review of SSO patients treated at our institute between 2008 and 2013 who underwent a laparoscopic gastric bypass (LGBP) or sleeve gastrectomy (LSG). The primary end point for this study was excess weight loss (EWL) at 1, 3, 6, and 12 months. Secondary end points included procedure length (PL), length of stay (LOS), diabetes management and postoperative complications.Results: We identified 135 SSO patients who underwent bariatric surgery (93 LGBP, 42 LSG) at our institute from 2008 to 2013 with a median follow-up of 49 months. The incidence of EWL > 30 % for patients in the LGBP group was 3.9, 29.0, 72.2 and 94.6 % at 1, 3, 6 and 12 months, respectively, while the incidence of EWL > 30 % in patients in the LSG group was 4.2, 25.0, 59.1 and 100 % at 1, 3, 6 and 12 months, respectively. PL was 124 ± 49 min for the LGBP group and 98 + 51 min for the LSG group (p < 0.005). LOS was on average 3.0 days (range 1-21) for the LGBP group and 3.4 days (range 1-13) for the LSG group (p = 0.41). Patients experienced a decrease in their hemoglobin A1C level by 10 % for the LGBP group and 9 % for the LSG group at 1 year (p = 0.89). Postoperative complications were seen in 15.1 % of LGBP patients and 4.8 % of LSG patients.Conclusions: Bariatric surgery is feasible in the SSO patients with comparable EWL outcomes and postoperative complications to historical non-SSO patients. Abstract: The Organ Procurement Transplant Network (OPTN) listing criteria for simultaneous liver‐kidney transplant (SLK) are not well defined. Concerns remain about rising numbers of SLKs, which divert quality kidneys from candidates awaiting kidney transplants (KT). We performed a retrospective review of liver transplants (LTs) at our center from 2004 to 2014; 127 recipients (liver transplant alone; 102 LTA, 25 SLK) were identified with short‐term preoperative kidney dysfunction (creatinine >4 mg/dL or preoperative hemodialysis [HD] for <6 weeks). Both cohorts had comparable baseline demographic characteristics with the exception of higher model for end‐stage liver disease (MELD) score in the LTA group (41.4 vs 32.9, P < .0001) and higher incidence of pre‐LT diabetes in the SLK cohort (52% vs 26.5%, P = .0176). Duration of pre‐LT HD was higher in SLK recipients, but the difference was not statistically significant (P = .39). Renal nonrecovery (RNR) rate in LTA cohort was low (<5%). No significant difference was noted in 1‐year mortality, liver graft rejection/failure, or length of stay (LOS) between the cohorts. Thus, it appears that liver recipients with short‐term (<6 weeks) HD or AKI without HD have comparable outcomes between LTA and SLK. Background: Post-transplant malignancy (PTM) remains a concern among pediatric kidney transplant (PKT) recipients.Study Design: Between 1963 and 2015, 884 pediatric (age 0 to 17 years old) patients received 1,055 PKTs at our institution. Cox proportional hazards models were constructed to identify risk factors for PTM after PKT with time-to-first-PTM as a primary outcome. Secondly, the hazard of death or graft loss was calculated in patients who developed PTM.Results: Median patient survival was 33 years (interquartile range [IQR] 18.7 to 47 years); 260 patients died during the study period and 47 had been diagnosed with PTM. There were 235 PTMs that occurred in 136 (15.4%) recipients at a median age of 29 years (IQR 17.8 to 37 years). The percentages of patients with PTM were 13% at 20 years post-PKT and 26% at 30 years post-PKT. Of PTM patients who died, 63.8% died of PTM. Among those who developed PTM, there was a higher hazard of death or graft loss (hazard ratio [HR] 1.62; 95% CI 1.11 to 2.38). In multivariable proportional hazards models, factors associated with PTM were increasing age at PKT (adjusted HR [AHR] 3.14; 95% CI 1.80 to 5.48 for 14 to 17 year-olds compared with children less than 3 years), having a living unrelated donor (LURD; AHR 3.25; 95% CI 1.27 to 8.35 compared with a living related donor), or implanting an Epstein-Barr virus (EBV)-positive allograft in an EBV-negative recipient (AHR 5.66; 95% CI 1.11 to 29.0). Compared with the general population, the cancer rate for PKT recipients was 6 times higher (126 vs 21 per 100,000 person-years).Conclusions: Pediatric kidney transplant recipients are at increased risk of PTM, which adversely affects survival. Coastal vegetated ecosystems play an important role in carbon cycling and bacterial communities inhabiting coastal sediments are responsible for the remineralization and processing of organic carbon (OC). We collected 1 m-long sediment cores in Posidonia seagrass meadows from coastal and estuarine sites in Australia that differed in their sedimentary organic and inorganic carbon, nitrogen and mud contents. The metabolic diversity of sediment heterotrophic bacterial communities was characterized at different sediment depths, based on the utilization pattern of 31 individual carbon substrates using Biolog EcoPlates™. High metabolic diversity was recorded at both sites, but the carbon substrate utilization rates and the use of carbohydrates were higher at the coastal site compared to the estuarine site. The heterotrophic bacterial community in the coastal sediment appeared to metabolize a more diverse OC pool compared to the estuarine site, which might partly explain the differences in OC storage among the seagrass habitats studied. The Biolog EcoPlates™ provided a useful tool for characte rising the sediment heterotrophic bacterial communities in the meadows and sediment characteristics and biochemical composition of the organic matter played an important role in shaping heterotrophic bacterial communities and their carbon utilization rates, potentially affecting carbon accumulation and preservation within seagrass sediments. Posidonia oceanica forms extensive peat-like deposits (mats) in Mediterranean coastal waters, which have a potential as carbon sinks and archives of environmental change. Nonetheless, the organic chemistry of both P. oceanica plant materials, as well as the environmental and diagenetic effects on the composition of its detritus, is poorly understood. We analyzed plant organs of P. oceanica and the coarse organic matter from a mat core spanning 750 yrs using pyrolysis techniques (PY-GC–MS and THM-GC–MS) to improve our understanding of their molecular properties and their preservation upon mat development. It appeared that leaf sheaths, roots and the outer parts of rhizomes were composed predominantly of phenolic constituents based on p -hydroxybenzoic acid ( p -HBA), which is atypical for vascular plants, in addition to carbohydrates and lignin. The inner rhizome and leaf blades had a different composition, with predominance of carbohydrates. The seagrass detritus in the mat was composed mainly of p -HBA phenolic material and carbohydrates, confirming earlier studies showing that the coarse detritus in the mat originates largely from Posidonia sheath, root and rhizome debris. The intermolecular arrangement of the p -HBA remains unclear, as they seem to correspond to ester-bound phenols yet their persistence in the mat attributes them a refractory nature. Variations in molecular composition within the mat are probably associated with diagenetic alteration of P. oceanica detritus, such as the decomposition of minor amounts of fatty acids, chlorophyll and syringyl lignin, and selective preservation of p -HBA relative to carbohydrates. Seagrasses are a polyphyletic group of angiosperms that colonized marine environments more than 30 million years ago and currently inhabit coastal soft and rocky substrates in all continents except Antarctica. Due to their evolution from terrestrial plants, seagrasses have belowground organs that interact with the substrate, transforming it through chemo-physical processes analogous to terrestrial soil formation. Although seagrass substrates provide valuable ecosystem services including carbon and coastal stabilization, they have been largely regarded as sediments by marine scientists and neglected in soil science research. However, over the last decades, the increasing interest in carbon accumulation by seagrasses has generated multiple data on seagrass soil physical and chemical characteristics. Here, we review clay and silt content (<0.063 mm particle size), redox potential, pH, carbonate content, organic carbon or organic matter contents, dry bulk density, porosity and color of seagrass soils worldwide, summarizing data typically used for soil description, and looking for generalities in soil characteristics across seagrass habitats. The data gathered was biased towards temperate species and high-income countries, while data about color, porosity, redox potential and pH was scarce. Soil characteristics did not show significant differences among seagrass bioregions. Most seagrass substrates showed sandy textures, whereas one of the most sampled genera, Posidonia , was not present in muddy substrates. The soil C org content was significantly higher in meadows formed by persistent species (mean ± SD; 1.76 ± 2.17 %) than in meadows formed by species with opportunistic and colonizing life-strategies (1.52 ± 2.24 and 0.76 ± 0.95 %, respectively), while mud content was significantly higher in meadows formed by opportunistic and colonizing species (27.87 ± 29.58 and 21.23 ± 21.77 %, respectively) than in those formed by persistent species (11.83 ± 14.45 %). Redox potential was significantly lower in intertidal than in subtidal meadows, although caution is needed when interpreting these differences due to methodological limitations. This review provides an overview of current knowledge on seagrass soil characteristics, while identifying knowledge gaps in seagrass soil science, including geographical, species diversity and soil physico-chemical traits that limit our capacity to characterize and classify seagrass soils worldwide. A perforated viscus in the postpancreaticoduodenectomy setting is a rare phenomenon and a devastating complication. In this situation, adherence to damage-control principles demands minimizing the operative intervention while addressing the intestinal perforation as a way to mitigate the injurious effects on a complex gastrointestinal reconstruction. Herein, we describe our intraoperative decision-making with an unconventional approach in the management of a perforated viscus in the postpancreaticoduodenectomy setting using a draining T-tube jejunostomy. Our patient recovered remarkably well from this and was discharged from the hospital in six days with a controlled draining T-tube jejunostomy, which was subsequently removed on postoperative day 35. Our case illustrates an important option when dealing with a perforated viscus in the complex gastrointestinal surgery patient that has minimal morbidity, adequate source control, and the potential for an excellent clinical outcome. As surgical care continues to be delivered in a specialty-driven manner, a draining T-tube jejunostomy presents the ideal technique to get out of trouble for the general surgeon practicing in the community who may not be as experienced with complex gastrointestinal surgery. The study of a Posidonia australis sediment archive has provided a record of ecosystem dynamics and processes over the last 600 years in Oyster Harbour ( SW Australia). Ecosystem shifts are a widespread phenomenon in coastal areas, and this study identifies baseline conditions and the time-course of ecological change (cycles, trends, resilience and thresholds of ecosystem change) under environmental stress in seagrass-dominated ecosystem. The shifts in the concentrations of chemical elements, carbonates, sediments <0.125 mm and stable carbon isotope signatures ( δ13C) of the organic matter were detected between 1850s and 1920s, whereas the shift detected in P concentration occurred several decades later (1960s). The first degradation phase (1850s-1950s) follows the onset of European settlement in Australia and was characterized by a strong increase in sediment accumulation rates and fine-grained particles, driven primarily by enhanced run-off due to land clearance and agriculture in the catchment. About 80% of total seagrass area at Oyster Harbour was lost during the second phase of environmental degradation (1960s until present). The sharp increase in P concentration and the increasing contribution of algae and terrestrial inputs into the sedimentary organic matter pool around 1960s provides compelling evidence of the documented eutrophication of the estuary and the subsequent loss of seagrass meadows. The results presented demonstrate the power of seagrass sedimentary archives to reconstruct the trajectories of anthropogenic pressures on estuarine ecosystem and the associated regime shifts, which can be used to improve the capacity of scientists and environmental managers to understand, predict and better manage ecological change in these ecosystems. Background: Bariatric surgery has been established as the most effective long-term treatment for morbid obesity and obesity-related comorbidities. Despite its success, there is a paucity of data on the outcomes of bariatric surgery on Hispanic patients.Study Design: We performed a retrospective review of obese patients treated at our institute between 2008 and 2014. We identified self-reported Hispanic patients who underwent a laparoscopic gastric bypass (LGBP), sleeve gastrectomy (LSG), or gastric band (LGB) procedure. The primary end point was excess weight loss (EWL) at 6, 12, 24, and 36 months. Secondary end points included improvement of obesity-related metabolic parameters at 1 year. We performed a repeated measures analysis of variance to calculate statistical significance throughout our study time period.Results: We identified 2002 Hispanic patients who underwent bariatric surgery (1,235 LGBP, 600 LSG, 167 LGB) at our institute from 2008 to 2014. Follow-ups at 6, 12, 24, and 36 months were 62.2%, 54.5%, 36.2%, and 19.8%, respectively. Mean preoperative BMIs were 47.0 ± 7.2 kg/m2, 46.1 ± 7.8 kg/m2, and 44.9 ± 6.0 kg/m2 for the LGBP, LSG, and LGB cohorts, respectively. Excess weight loss was significantly more pronounced in the LGBP and the LSG groups than in the LGB group; this difference was accentuated over time (p < 0.0001). Obesity-related metabolic parameters and the need for comorbidity medical therapy decreased in all 3 surgical groups.Conclusions: Bariatric surgery is highly successful in Hispanic obese patients. In the largest series to date, LGBP and LSG seem to yield more effective EWL and reduction of cardiometabolic parameters than LGB among Hispanics; however, outcomes are still markedly reduced when compared with those in non-Hispanic populations. The study of a Posidonia australis sedimentary archive has provided a record of changes in element concentrations (Al, Fe, Mn, Pb, Zn, Cr, Cd, Co, As, Cu, Ni and S) over the last 3000 years in the Australian marine environment. Human-derived contamination in Oyster Harbor (SW Australia) started ~ 100 years ago (AD ~ 1900) and exponentially increased until present. This appears to be related to European colonization of Australia and the subsequent impact of human activities, namely mining, coal and metal production, and extensive agriculture. Two contamination periods of different magnitude have been identified: Expansion period (EXP, AD ~ 1900–1970) and Establishment period (EST, AD ~ 1970 to present). Enrichments of chemical elements with respect to baseline concentrations (in samples older than ~ 115 cal years BP) were found for all elements studied in both periods, except for Ni, As and S. The highest enrichment factors were obtained for the EST period (ranging from 1.3-fold increase in Cu to 7.2-fold in Zn concentrations) compared to the EXP period (1.1-fold increase for Cu and Cr to 2.4-fold increase for Pb). Zinc, Pb, Mn and Co concentrations during both periods were 2- to 7-fold higher than baseline levels. This study demonstrates the value of Posidonia mats as long-term archives of element concentrations and trends in coastal ecosystems. International experiences on peaceful and negotiated conflict transformation demonstrate that the legal issues related to criminal responsibility and prosecution of crimes committed during wartime, represent the central aspects of the negotiation agendas and issues that complicate the construction of final agreements. For this reason, and as part of a justice system that is increasingly globalized extending the jurisdiction of international tribunals to prosecute crimes against humanity, we turn to models of transitional justice and the agreement of alternative sanctions when peace is negotiated and demobilization of armed actors, models constructed from three fundamental criteria: maximum of truth, justice lows and mechanisms for victims reparation. Consequently, public opinion, critics and observers perceive that in these negotiation processes the justice is sacrificed and the path is opened to the "impunity" as the more flexible means for the transition or the end of armed conflict. Thus, this article is intended to compare the peace processes carried out by the Colombian government with the M-19 and FARC-EP guerrilla groups, focusing reflection in the categories of justice and impunity emerged for such negotiation experiences. Arbuscular mycorrhizal fungi (AMF), symbionts with most terrestrial plants, produce glomalin-related soil protein (GRSP), which plays a major role in soil structure and quality. Both fungi hyphae and protein production in soils are affected by perturbations related to land-use changes, implying that GRSP is a sensitive indicator of soil quality. Unfortunately, GRSP degrades within years to decades in oxic environments, preventing its use as palaeoecological proxy. However, GRSP is transported to marine, near-shore anoxic sediments, where it accumulates and remains non-degraded, enabling the assessment of its potential as a palaeoecological proxy for soil ecosystem's health. Exploiting this fact, we have obtained for the first time a long-term record (c. 1250 years) of GRSP content using a Posidonia oceanica seagrass mat sediment core from the Western Mediterranean (Portlligat Bay, Spain). The trends in GRSP content matched well with land-use changes related to agrarian activities reconstructed by pollen analysis. In periods of cultivation, GRSP accumulation in the mat decreased. Given the role played by GRSP, the results suggest that agrarian intensification may have resulted in perturbations to soil quality. Thus, GRSP in seagrass mat sediments can be used to assess long-term trends in continental soil quality induced by human activities. These findings open new possibilities in long-term ecology research, as other anoxic environments could be potentially valid too. Testing them would open the possibility to identify long-term patterns in soil quality and other environmental stressors that could also affect AMF and GRSP production in soils. Because 5-year survival rates for patients with metastatic melanoma remain below 25%, there is continued need for new therapeutic approaches. For some tumors, pharmacologic ascorbate treatment may have a beneficial antitumor effect and may work synergistically with standard chemotherapeutics. To investigate this possibility in melanoma, we examined the effect of pharmacologic ascorbate on B16-F10 cells. Murine models were employed to compare tumor size following treatment with ascorbate, and the chemotherapeutic agents dacarbazine or valproic acid, alone or in combination with ascorbate. Results indicated that nearly all melanoma cell lines were susceptible to ascorbate-mediated cytotoxicity. Compared to saline controls, pharmacologic ascorbate decreased tumor size in both C57BL/6 ( P <0.0001) and NOD- scid tumor bearing mice ( P <0.0001). Pharmacologic ascorbate was superior or equivalent to dacarbazine as an antitumor agent. Synergy was not apparent when ascorbate was combined with either dacarbazine or valproic acid; the latter combination may have additional toxicities. Pharmacologic ascorbate induced DNA damage in melanoma cells, as evidenced by increased phosphorylation of the histone variant, H2A.X. Differences were not evident in tumor samples from C57BL/6 mice treated with pharmacologic ascorbate compared to tumors from saline-treated controls. Together, these results suggest that pharmacologic ascorbate has a cytotoxic effect against melanoma that is largely independent of lymphocytic immune functions and that continued investigation of pharmacologic ascorbate in cancer treatment is warranted. Primary splenic angiosarcoma is an extremely rare and aggressive neoplasm of the vasculature. Uniformly, primary splenic angiosarcoma is a fatal disease despite early diagnosis and treatment. The first successful kidney transplant occurred in 1954. Since then, long-term graft survival has been an elusive idealistic goal of transplantation. Yet 62 years later, we know of only 6 kidney transplant recipients who have achieved ≥ 50-year graft survival while being on no immunosuppression or a substantially reduced regimen. Herein, we report graft survival ≥ 50 years in 2 living donor recipients who have been maintained on standard-of-care immunosuppression the entire time. For our 2 recipients, their living donor's altruism altered the course, length, and quality of their life, which by all accounts can be deemed normal: They attended college, held jobs, had successful pregnancies, raised families, and were productive members of society. Both donors are still alive and well, more than 50 years post-donation; both have an acceptable GFR and normal blood pressure, with hyperlipidemia as their only medical problem. These 2 intertwined stories illustrate the tremendous potential of a successful kidney transplant: long-term survival with a normal lifestyle and excellent quality of life, even after more than 5 decades on full-dose immunosuppression. One of the main problems with biomedical signals is the limited amount of patient-specific data and the significant amount of time needed to record the sufficient number of samples needed for diagnostic and treatment purposes. In this study, we present a framework to simultaneously generate and classify biomedical time series based on a modified Adversarial Autoencoder (AAE) algorithm and one-dimensional convolutions. Our work is based on breathing time series, with specific motivation to capture breathing motion during radiotherapy lung cancer treatments. First, we explore the potential in using the Variational Autoencoder (VAE) and AAE algorithms to model breathing signals from individual patients. We then extend the AAE algorithm to allow joint semi-supervised classification and generation of different types of signals within a single framework. To simplify the modeling task, we introduce a pre-processing and post-processing compressing algorithm that transforms the multi-dimensional time series into vectors containing time and position values, which are transformed back into time series through an additional neural network. The resulting models are able to generate realistic and varied samples of breathing. By incorporating 4% and 12% of the labeled samples during training, our model outperforms other purely discriminative networks in classifying breathing baseline shift irregularities from a dataset completely different from the training set, achieving an average macro F1-score of 94.91 % and 96.54 % , respectively. To our knowledge, the presented framework is the first approach that unifies generation and classification within a single model for this type of biomedical data, enabling both computer aided diagnosis and augmentation of labeled samples within a single framework. Trophic ecology has benefitted from the use of stable isotopes for the last three decades. However, during the last 10 years, there has been a growing awareness of the isotopic biases associated with some pre-analytical procedures that can seriously hamper the interpretation of food webs. We have assessed the extent of such biases by: (1) reviewing the literature on the topic, and (2) compiling C and N isotopic values of marine invertebrates reported in the literature with the associated sample preparation protocols. The factors considered were: acid-washing, distilled water rinsing (DWR), sample type (whole individuals or pieces of soft tissues), lipid content, and gut contents. Two-level ANOVA revealed overall large and highly significant effects of acidification for both δ13C values (up to 0.9‰ decrease) and δ15 N values (up to 2.1‰ decrease in whole individual samples, and up to 1.1‰ increase in tissue samples). DWR showed a weak overall effect with δ13C increments of 0.6‰ (for the entire data set) or decrements of 0.7‰ in δ15 N values (for tissue samples). Gut contents showed no overall significant effect, whereas lipid extraction resulted in the greatest biases in both isotopic signatures (δ13C, up to −2.0‰ in whole individuals; δ15N, up to +4.3‰ in tissue samples). The study analyzed separately the effects of the various factors in different taxonomic groups and revealed a very high diversity in the extent and direction of the effects. Maxillopoda, Gastropoda, and Polychaeta were the classes that showed the largest isotopic shifts associated with sample preparation. Guidelines for the standardization of sample preparation protocols for isotopic analysis are proposed both for large and small marine invertebrates. Abstract: Inorganic carbon removal through acidification is a common practice prior to isotopic analysis of macroinvertebrate samples. We have experimentally tested the effect of acidification on the elemental and isotopic composition of a range of beach arthropod species. Acidification resulted in a significant depletion of 7.7% and 1.2% on average for carbon and nitrogen, respectively, suggesting that acid washing affects body carbon compounds other than carbonates. With a few exceptions, δ13C and δ15N showed no changes following 1N HCl attack. Based on those exceptions, our results show that only those samples with a high CaCO3 content result in impoverished 13C as a consequence of acidification. Those suspected to be carbonate-free are not significantly affected. Concerning δ15N values, only high carbonate species were affected when treated with HCl. As a standard protocol, it is recommended to acidify only carbonate-rich samples prior to δ13C analyses. When possible, muscle tissue samples should be used instead of the entire organism. The study of a Posidonia sinuosa sedimentary archive has delivered a millenary record of environmental change in Cockburn Sound (Western Australia). Ecosystem change is a major environmental problem challenging sustainable coastal development worldwide, and this study shows baseline trends and shifts in ecological processes in coastal ecosystems under environmental stress. The concentrations and fluxes of biogeochemical elements over the last 3,500 years indicate that important changes in ecosystem dynamics occurred over the last 1,000 years, in particular after ~1900's, probably related to establishment of seagrass meadows in the area and to local and regional human activities (industry and coastal development), respectively. The establishment of seagrasses ~1,000 years ago in the area of study is supported by the appearance of Posidonia fibres from ~40 cm soil depth until the core top, higher δ13C values indicating a larger contribution of seagrass-matter to the soil organic carbon pool, and increased concentration of fine sediments driven by the effect of seagrass canopy in enhancing sedimentation. The comparison of organic carbon, nutrients and metal concentrations and fluxes between pre- and post-establishment of seagrasses shows that seagrass establishment resulted in up to 9-fold increase in the soil biogeochemical sink. In ~1900's, shifts in the concentrations of metals, carbonates, organic carbon, sediment grain size, and δ13C and δ15N values of the organic matter were detected, demonstrating an alteration in seagrass ecosystem functioning following the onset of European settlement. Anthropogenic activities, and in particular the construction of a causeway in 1970's, enhanced seagrass soil organic carbon and metal accumulation rates by 36- and 39-fold, respectively, showing that human-made structures can enhance the biogeochemical sink capacity of seagrasses. Abstract: Stable carbon and oxygen isotope ratios (δ 13C and δ 18O) of the epibenthic foraminiferan Elphidium macellum var. aculeatum were measured and compared both in raw aliquots and after various pre-analytical treatments. We tested the effects of chemical cleaning procedures (oxygen plasma ashing, NaClO and H2O2) and physical (ultrasounds) aimed at removing allochthonous particles. We also assessed the effects of using preservatives (formaldehyde and ethanol) and stains (Bengal Rose and Sudan Black B) prior to δ 13C and δ 18O analyses. Although the effects were generally small, three of the pre-treatments had a significant effect on δ 13C or δ 18O values. The use of Sudan Black B for vital identification caused a significant enrichment in the 13C of the samples (0.24‰). In addition, sample oxidation with hydrogen peroxide and ultrasonic cleaning each led to a slight decrease in δ 18O values (–0.18‰, in both cases). Our results demonstrate that Sudan Black B should be avoided prior to isotopic analysis. Bengal Rose showed no significant effect and should be used instead for staining. Because palaeoclimatic studies are based in small isotopic shifts, we discourage the pre-analysis use of unnecessary harsh cleaning treatments of foraminifera shell samples that may lead to isotopic fractionation and hence confound ecological interpretation. The high organic carbon (OC) stores in seagrass meadows have led to their recognition as significant Blue Carbon sinks, though the diagenetic conditions that enable OC retention in seagrass soils remain poorly understood. In this study, seagrass soils were sampled from a Posidonia australis meadow in Oyster Harbour (Albany; south-western Australia) to investigate the preservation of sedimentary OC. We analysed soil characteristics (colour, grain size and redox potential), radiocarbon age, and characterised the soil organic matter (OM) using solid state CP/MAS 13C NMR spectroscopy to examine the preservation of OM down the soil profile. There was minimal change in organic composition over 1,700 years of accumulation, indicating long-term OM preservation. Primarily, this preservation appears to be driven by the recalcitrance of seagrass detritus buried in anoxic soils. The majority (70–83%) of total sedimentary OM comprised components directly attributable to seagrass origins (lignin, carbohydrate and black carbon-like matter), while the remainder consisted mostly of protein, some of which may have been present in seagrass biomass, along with likely contributions from algae and microbes. Although black carbon originates from organic matter combustion, here we provide evidence that the 13C NMR signal identified as black carbon-like matter in our soils is possibly associated with seagrass-derived organic matter consisting of degraded lignin products or other non-pyrogenic aromatics. The increase in the relative abundance of this black carbon-like matter with aging suggests its selective preservation. The relative abundances of carbohydrates significantly decreased with age down core (i.e. they appeared to be selectively decomposed), while lignin and protein did not show any quantitative changes in relative abundance (non-selective preservation). These findings demonstrate the exceptional preservation of P. australis derived OC, which contributes to our understanding of the higher OC storage capacity of Posidonia compared to other seagrass species. Seagrass ecosystems are recognised for their role in climate change mitigation, due to their capacity to form organic-rich sediments. The chemical recalcitrance of seagrass organs is one characteristic driving carbon storage, but the molecular background of this feature is poorly understood. We assessed molecular composition changes of Posidonia australis sheaths (SH) and roots plus rhizomes (RR) along a sediment core, encompassing 3200 cal. yr BP, by means of nuclear magnetic resonance spectroscopy (13C NMR), conventional analytical pyrolysis (Py-GC–MS) and thermally assisted hydrolysis and methylation (THM-GC–MS). Significant trends with depth (age) in the composition of both SH and RR remains of P. australis were observed from all methods. In general terms, polysaccharides become depleted (degraded) and lignin enriched (selectively preserved) as age increases, and the minor constituents cutin, suberin and condensed tannin are also preferentially depleted during ageing in both fractions. Molecular changes with ageing were smaller in SH, especially regarding polysaccharides, indicative of a superior stability compared to RR. The molecular changes observed are most pronounced within the first 75 cm of the record, which reflects the recalcitrance of P. australis detritus once it is buried below that depth (corresponding to approximately 700 cal. yr BP). The capacity of P. australis to act as a long-term carbon sink seems to be mainly related to the resistance of buried lignocellulose materials to decomposition. The results on diagenetic effects on the molecular fingerprint of seagrass detritus contribute to our understanding of carbon sequestration in Blue Carbon ecosystems. A childhood malignancy can rarely progress to ESRD requiring a KT. To date, few reports describe long‐term outcomes of pediatric KT recipients with a pretransplant malignancy. Between 1963 and 2015, 884 pediatric (age: 0‐17 years old) recipients received 1055 KTs at our institution. KT outcomes were analyzed in children with a pretransplant malignancy. We identified 14 patients who had a pretransplant malignancy prior to KT; the majority were <10 years old at the time of KT. Ten (71%) patients received their grafts from living donors, the majority of which were related to the recipient. Wilms' tumor was the dominant type of pretransplant malignancy, seen in 50% of patients. The other pretransplant malignancy types were EBV‐positive lymphoproliferative disorders, non‐EBV‐positive lymphoma, leukemia, neuroblastoma, soft‐tissue sarcoma, and ovarian cancer. Ten of the 14 patients received chemotherapy as part of their pretransplant malignancy treatment. Graft survival at 1, 3, and 5 years was 93%, 83%, and 72%, respectively. Patient survival at 1, 5, and 10 years was 100%, 91%, and 83%, respectively. Six (40%) patients suffered AR following KT; half of them had their first episode of AR within 1 month of KT. Our single‐center experience demonstrates that pediatric KT recipients with a previously treated pretransplant malignancy did not exhibit worse outcomes than other pediatric KT patients. The widespread transmission of severe acute respiratory syndrome coronavirus 2 (SARS‐CoV‐2) continues to propagate the coronavirus disease 2019 (COVID‐19) pandemic with solid organ transplant (SOT) recipients being an exceptionally vulnerable population for poor outcomes. Treatments for COVID‐19 are limited; however, monoclonal antibodies are emerging as a potential therapeutic option to change the trajectory of high‐risk patients. This retrospective single center cohort study evaluated the outcomes of SOT recipients with mild to moderate COVID‐19 who received bamlanivimab monotherapy. Eighteen SOT recipients (15 kidney, 2 liver, and 1 heart) received the medication between November 9, 2020 and February 10, 2021 with no reported infusion reactions. One patient experienced headache and fatigue following the infusion that resolved within 3 days. Fourteen patients continued their recovery as an outpatient with no further escalation in care. Three patients required hospitalization: two for suspected bacterial pneumonia 9 and 32 days postinfusion, respectively, and one for acute kidney injury 7 days postinfusion. One patient had an emergency room visit for gastrointestinal symptoms 24 days postinfusion. In this small cohort of SOT recipients, bamlanivimab monotherapy appeared to be a well‐tolerated option for treatment of mild to moderate COVID‐19, but it was not completely effective in preventing hospitalization. One month following the end of this cohort, COVID‐19 treatment guidance changed due to the rising prevalence of resistant variants. For this reason, bamlanivimab is now recommended to be used only in combination with etesevimab. Further studies are needed to fully elucidate the role of this therapy in SOT recipients. Background: The ability to express the same meaning in different ways is a well-known property of natural language. This amazing property is the source of major difficulties in natural language processing. Given the constant increase in published literature, its curation and information extraction would strongly benefit from efficient automatic processes, for which corpora of sentences evaluated by experts are a valuable resource. Results: Given our interest in applying such approaches to the benefit of curation of the biomedical literature, specifically that about gene regulation in microbial organisms, we decided to build a corpus with graded textual similarity evaluated by curators and that was designed specifically oriented to our purposes. Based on the predefined statistical power of future analyses, we defined features of the design, including sampling, selection criteria, balance, and size, among others. A non-fully crossed study design was applied. Each pair of sentences was evaluated by 3 annotators from a total of 7; the scale used in the semantic similarity assessment task within the Semantic Evaluation workshop (SEMEVAL) was adapted to our goals in four successive iterative sessions with clear improvements in the agreed guidelines and interrater reliability results. Alternatives for such a corpus evaluation have been widely discussed. Conclusions: To the best of our knowledge, this is the first similarity corpus—a dataset of pairs of sentences for which human experts rate the semantic similarity of each pair—in this domain of knowledge. Kidney transplantation entails well‐coordinated complex care delivery. Patient‐provider cultural and linguistic discordance can lead to healthcare disparities. We analyzed kidney transplantation outcomes among our institution's Hmong recipients using a retrospective cohort study. From 1995 to 2015, 2164 adult (age ≥18) recipients underwent kidney transplantation at our institution; 78 self‐identified as Hmong. Survival rates were analyzed and compared to Caucasian recipients (n = 2086). Fifty (64.1%) Hmong recipients consistently requested interpreters. Mean follow‐up was 9.8 years for both groups. Hmong recipients (N = 78) were on average younger at transplant (45.7 vs 49.7 years; P = 0.02), more likely to be female (56% vs 38%; P = 0.001), and had higher gravidity (5.0 vs 1.9 births; P < 0.001). There were 13 (16.7%) Hmong living donor recipients, who were younger (32.8 vs 42.9 years; P = 0.006) at transplant compared to Caucasians (1429, 68.5%). Hmong 1‐ and 5‐year patient survival was 100%; Caucasians, 97.1% and 88% (P < 0.001). Hmong 1‐ and 5‐year graft survival was 98.7% and 84.9%; Caucasians 94.8% and 80.9% (P = 0.013). One‐ and 5‐year rejection‐free survival showed no difference (88.9% vs 82.4%; 86.7% vs 83.4%, P = 0.996). Despite cultural and linguistic differences between Hmong recipients and providers, we found no evidence of inferiority in KT outcomes in the Hmong population. [ABSTRACT FROM AUTHOR]

Background: KT recipients have increased the risk of CVD. The incidence of post‐transplant CVEs among pediatric recipients has not been well‐characterized. Patients and Methods: Between 1963 and 2015, 884 pediatric (age: 0‐17 years old) recipients received 1058 KTs at our institution. The cumulative incidence of CVEs was analyzed. Statistical models were used to estimate risk factors for developing post‐transplant CVEs. Results: Overall median patient survival was 33 years (IQR: 18.7‐47). A total of 362 CVEs occurred in 161 (18.3%) patients at a median age of 20.5 years. Arrhythmias (18%) were most common. Cumulative risk of post‐transplant CVEs was 9% at 10 years, 17% at 20 years, 25% at 30 years, and 36% at 40 years. Development of post‐transplant CVEs was associated with increased mortality (HR 2.25 [95% CI 1.61‐3.14]); of those who developed a CVE and died, 22/51 (43.1%) died of CVD. Multivariable risk factors for post‐transplant CVEs included a history of pretransplant CVD (aHR 1.92 [1.18‐3.13] and graft failure (4.57 [3.13‐6.67]). Discussion: A pretransplant history of CVD and a failed graft are significant risk factors for the development of post‐transplant CVE. CVD increases the risk of post‐transplant death or graft loss.
