Abstract The superior mesenteric artery syndrome (SMAS), also known as Wilkie’s syndrome, is a duodenal obstruction caused by the compression of the third part of the duodenum between the superior mesenteric artery (SMA) and the aorta. This condition often arises due to the disappearance of perivascular fatty tissue, as well as advanced stages of malnutrition and weight loss. Patients typically experience symptoms such as vomiting, fullness, and postprandial epigastric pain. While initial management involves conservative measures, surgery becomes necessary if symptoms do not improve. Here, we present the case of a 25-year-old man diagnosed with SMAS. Despite undergoing conservative management for three months, the symptoms did not regress. Subsequently, a laparoscopic duodeno-jejunostomy was performed, resulting in an uneventful postoperative period, disappearance of vomiting and epigastric pain, and a weight gain of 14 kg after one year.

Introduction: Acute myeloid leukemia (AML) is a hematological malignancy with a poor prognosis. The early consequences of induction chemotherapy are represented by pancytopenia with severe immunosuppression, which favors the emergence of so-called “opportunistic” infections and in particular invasive fungal infections (IFI) such as invasive aspergillosis (IA). This justifies the implementation of early, empirical, pre-emptive or prophylactic antifungal treatment. The objective of this study is the evaluation of a comparative study before and after the use of primary prevention with posaconazole versus fluconazole, in terms of incidence and mortality of invasive fungal infections (IFIs) during AML inductions, in real life in Algeria. Patients and methods: This study was performed in two periods in the 2 centers concerning the same AML patients receiving the same induction chemotherapy (daunorubicin 60 mg/m2 for 3 days and cytarabine 100 mg/m2 for 7 days) but with a different type of hospitalization with common rooms for Tlemcen and protected single rooms for Oran. Period 1: The retrospective study was performed from 2014 to 2016 and involved 188 patients. 70 patients were hospitalized in a common room (Tlemcen) and 118 patients in a protected single room (Oran). All patients received as primary IFI prophylaxis fluconazole 400 mg/day. Period 2: The prospective study was conducted from April 2017 to September 2018 and involved 55 patients. Fourteen patients were hospitalized in Tlemcen and 41 patients in an Oran. These patients received posaconazole prophylaxis at a dose of 200 mgx3/day. The evaluation and comparison of descriptive and incidence data was performed using Chi2 statistical tests using SPSS version 19 software. Overall survival (OS) were calculated using the Kaplan-Meier estimate and were compared using the log-rank test with a significance level α of 0.05. Results: Period 1: The incidence rate of IFI: 34% at the Oran hospital versus 49% at the Tlemcen hospital (p=0.049). Among the 74 patients who presented with IFI, 39 (53%) died from IFI as the main cause of death, among these deaths, 16 (40%) were recorded at Oran and 23 (68%) at Tlemcen (p=0.011). Period 2: The IFI rate with fluconazole was 39% and that observed with posaconazole 22% (p=0.001) and the cumulative incidence was 39% at 21 days versus 10% at 15 days (p=0.046) with a highly significant decrease in IFI-related deaths: 53% versus 25% (p=0.004). Conclusion: This work has demonstrated the interest in terms of IFI incidence (17% versus 33%, p=0.04) and IFI-related mortality of posaconazole prophylaxis (25% versus 53%, p=0.004) used during AML in the context of hospitalization in common rooms. Furthermore, this work highlights that the combination of posaconazole prophylaxis and hospitalization in a protected single room allows an optimization of the management of this type of patients.

Papillary fibroelastomas are benign tumours, usually found on cardiac valves. They are attached to the aortic valve in 90% of cases. Even though benign and mostly asymptomatic, right-sided tumours can cause pulmonary embolism, while left-sided tumours can cause serious complications. We report a 76-year-old woman treated initially for a suspicious infective endocarditis of the aortic valve, in whom papillary fibroelastoma was histologically confirmed, with no evidence of infective endocarditis.

Abstract Introduction - The objective of the study was to identify the risk factors related to occupational radiological exposure in dentistry of the municipality of Oran, during radiodiagnostic and interventional procedures. Methods. This was a descriptive cross-sectional study conducted at the level of dental facilities in the commune of Oran, from November 2017 to February 2018. It involved 198 manipulators working in the private and public sectors. During an actual radiological procedure, dosimetric measurements were recorded. According to a standard protocol, points were targeted for the placement of dosimeters. The dosimeters were placed in the left pocket (thorax) to record the whole-body dose, in the manipulator's hand and in the eye (a biologically highly radiosensitive organ). The dosimeters used were previously calibrated. Results -The recorded doses exceeded the regulatory tolérable limit in 43 out of 198 manipulators. The position of the manipulators in relation to the radiological devices noticed in the sector of activity an OR of 2.76 [1.09-6.99], the sex with an OR of 2.48 [1.18-5.19], the seniority (less than 10 years) with an OR of 1.99 [0.99-4.02] and the age (less than 36 years) with an OR of 0.55 [0.28-1.12] ,were significantly related to the risk of radiological exposure. The use of the angulator, but without leaving the danger zone, exposed the handlers to the risk of radiation exposure. Conclusion - The assessment presented contributed to the identification of the risk factors of radiological exposure observed in dentistry of the commune of Oran. These results encourage us to make dentists aware of the use of radiation protection means in order to reduce the health risks associated with ionizing radiation.

Introduction-Objective-Glass ionomer cements (CVI) are complex polyacrylates or polyalkenoates of glasses composed of basic glasses and an acidic polymer. Considerable changes have been made in the formulations and handling properties of CVIs for different clinical applications, resulting in the invention of several types of CVI. The oral microflora is very heterogeneous in quality and quantity. Some germs are helpful and protect the oral cavity, but there are others that are pathogenic. The aim of this study was to isolate and to identify the bacteria involved in dental caries and compared the effect of two types of glass ionomer cements on the growth of these germs. Material and methods - Microbiological, physiological and biochemical methods were used for the isolation and identification of bacteria isolated from patients suffering from dental caries. These bacteria were used to assess the effect of glass ionomer cement. CVI activity was performed by the semi-solid medium diffusion method. Results -The results obtained have shown that the bacteria involved in dental caries belonged to the species of Streptococcus mutans (28.6%). The species of St. suis (28.6%), St. porcinus (14.3%) with St. bovis (14.3%) and Aerococcus viridans (14.2%) were also present, causing hemolytic activity. The antibiotic sensitivity test reveals that the species of St. mutans are the most sensitive. The two types of CVI (CVIc, CVIMAR) studied produced a bactericidal effect on St. mutans. There is no significant difference in the antibacterial effect between CVIc and CVIMAR on St. mutans. Conclusion - The use of CVI or CVIMAR as dental restorative materials, to prevent or at least limit the spread of residual and recurrent caries, deserves to be considered by practitioners. They should be widely recommended in daily practice, particularly in patients with high caries risk.

- Abstract
Background: Anastomotic leak affects 8 per cent of patients after right colectomy with a 10-fold increased risk of postoperative death. The EAGLE study aimed to develop and test whether an international, standardized quality improvement intervention could reduce anastomotic leaks. Methods: The internationally intended protocol, iteratively co-developed by a multistage Delphi process, comprised an online educational module introducing risk stratification, an intraoperative checklist, and harmonized surgical techniques. Clusters (hospital teams) were randomized to one of three arms with varied sequences of intervention/data collection by a derived stepped-wedge batch design (at least 18 hospital teams per batch). Patients were blinded to the study allocation. Low- and middle-income country enrolment was encouraged. The primary outcome (assessed by intention to treat) was anastomotic leak rate, and subgroup analyses by module completion (at least 80 per cent of surgeons, high engagement; less than 50 per cent, low engagement) were preplanned. Results: A total 355 hospital teams registered, with 332 from 64 countries (39.2 per cent low and middle income) included in the final analysis. The online modules were completed by half of the surgeons (2143 of 4411). The primary analysis included 3039 of the 3268 patients recruited (206 patients had no anastomosis and 23 were lost to follow-up), with anastomotic leaks arising before and after the intervention in 10.1 and 9.6 per cent respectively (adjusted OR 0.87, 95 per cent c.i. 0.59 to 1.30; P = 0.498). The proportion of surgeons completing the educational modules was an influence: the leak rate decreased from 12.2 per cent (61 of 500) before intervention to 5.1 per cent (24 of 473) after intervention in high-engagement centres (adjusted OR 0.36, 0.20 to 0.64; P < 0.001), but this was not observed in low-engagement hospitals (8.3 per cent (59 of 714) and 13.8 per cent (61 of 443) respectively; adjusted OR 2.09, 1.31 to 3.31

Medicine - Abstract
Introduction More than 300 million surgical procedures are performed each year. Acute kidney injury (AKI) is a common complication after major surgery and is associated with adverse short-term and long-term outcomes. However, there is a large variation in the incidence of reported AKI rates. The establishment of an accurate epidemiology of surgery-associated AKI is important for healthcare policy, quality initiatives, clinical trials, as well as for improving guidelines. The objective of the Epidemiology of Surgery-associated Acute Kidney Injury (EPIS-AKI) trial is to prospectively evaluate the epidemiology of AKI after major surgery using the latest Kidney Disease: Improving Global Outcomes (KDIGO) consensus definition of AKI.Methods and analysis EPIS-AKI is an international prospective, observational, multicentre cohort study including 10 000 patients undergoing major surgery who are subsequently admitted to the ICU or a similar high dependency unit. The primary endpoint is the incidence of AKI within 72 hours after surgery according to the KDIGO criteria. Secondary endpoints include use of renal replacement therapy (RRT), mortality during ICU and hospital stay, length of ICU and hospital stay and major adverse kidney events (combined endpoint consisting of persistent renal dysfunction, RRT and mortality) at day 90. Further, we will evaluate preoperative and intraoperative risk factors affecting the incidence of postoperative AKI. In an add-on analysis, we will assess urinary biomarkers for early detection of AKI.Ethics and dissemination EPIS-AKI has been approved by the leading Ethics Committee of the Medical Council North Rhine-Westphalia, of the Westphalian Wilhelms-University Münster and the corresponding Ethics Committee at each participating site. Results will be disseminated widely and published in peer-reviewed journals, presented at conferences and used to design further AKI-related trials.Trial registration number NCT04165369.

The carob tree is an agro-salvo-pastoral species that has an important socio-economic and ecological role. For many years, the horticultural in Algeria has been in regression, mainly because of anthropic factors. This regression has led to the increase in carob tree prices, which makes this speculation gainful for farmers and agro-industrialists. However, the problem of sexual determinism in carob remains an important difficulty for the creation of orchards and the development of genetic improvement programs for the species. So, the object of this study is to identify and distinguish by a morpho-geometric characterization of the leaflets, the carob trees of male, female and hermaphrodite sex, from 21 different regions of the Algerian north with 781 leaflets characterized through the selection of 16 landmarks. Once recorded in digital format (TPS), the morpho-geometric conformations are aligned and superimposed using the Procrustean superposition technique. Analysis of variance (ANOVA) of the data revealed highly significant differences in shape (p=0.0001), and significant centroid size (p=0.0243) between the three populations studied. Descriptive analysis of size showed only one significant difference (p=0.035) between the size of hermaphrodite and male leaflets. Principal component analysis (PCA) showed inertia of 70.45% between the first three components and revealed three different shapes. The analysis of canonical variants (CA) distinguished these different forms, the female leaflet so much towards an elliptical form with a base contracted in node downwards and with apex characterized by a closed angle. While the shape of the male leaflet is generally oval with a scattered base and with apex characterized by an open angle. The leaflets of hermaphrodites resemble the leaflets of males (oval) but with a flattened base. These results can be used to make a preliminary selection of young carob plants. [ABSTRACT FROM AUTHOR]

When examining the research literature relating to single slope solar stills it is apparent that much of the work has been conducted in an ad-hoc manner. This has led to several recommendations relating to the geometry of stills having developed that do not appear to have a sufficient evidentiary basis. To address this issue, this study used computational fluid dynamics simulations to examine the natural convection in single slope solar stills with cover angles between 0° and 60° and aspect ratios ranging from 1 to 8, with the results validated experimentally. Treating cover angle and aspect ratio as independent design variables showed that there are some features of the natural convection flow that account for the variation in yields reported in the literature (transitions between uni- and multi-cellular flow). More specifically, it was shown that the geometric effects could be correlated in the form of a generalized relationship and that this was able to predict the yield from several independent solar stills reported in the literature. As such, the use of the relationship will allow single slope solar stills designers to predict their yield far more accurately than is currently possible.

Hemorrhagic manifestations during COVID-19 infections are increasingly described in the literature. We report the first case of spontaneous subcapsular hematoma of the liver revealing a COVID-19 infection in a 44-year-old woman with no underlying health condition history, a computerized tomography evaluation showed an aspect of lung ground-glass opacities, with moderate impairment estimated about 20%. Reverse transcription-polymerase chain reaction confirmed the diagnosis of COVID-19 infection. During the covid-19 pandemic, non-traumatic bleeding such as spontaneous hematomas in patients with no coagulation disorder could be a manifestation of COVID-19 infection.

- Abstract
The deformation of metallic materials during the machining operation requires a significant amount of energy. During the chip formation process and due to the plastic deformation of the metal and the friction along the tool-part interface, the thermal loads generated are strongly impacted by the cutting factors. Thus, the choice of optimized cutting conditions is essential to control the quality of the work required. The aim of the present experimental study is to optimize the cutting parameters using temperature measurements. The average temperature of the cutting tool is studied using a FLIR A325sc type infrared camera. Optimal cutting parameters for each performance metric were obtained using the Taguchi techniques.

The study is aimed at evaluating, under laboratory conditions, the side effects of a commercial formulation of diflubenzuron (Dimilin® 25 WP), which is an insecticide considered a chitin synthesis inhibitor, on some non-targeted aquatic species. The effect of two lethal doses of Dimilin LC10 (3.9 ng/L) and LC50 (16 ng/L) previously determined on fourth instar larvae of Culex pipiens L. 1758 were tested on some non-target aquatic species, abundant in the Lake of Birds (Northeast Algeria), a site classified under Ramsar Convention. The tested species belonged to two classes of invertebrates (Insects and Crustaceans). The insects included four species of Heteroptera: Corixa punctata Illiger, 1807, Notonecta glauca Latreille, 1802, Anisops sardea Latreille, 1802 and Plea minutissima Leach, 1817, and one species of Coleoptera Berosus signaticollis Charpentier,1825. For benthic crustaceans Daphnia magna Straus, 1820 (Cladocera) was retained. The results showed significant mortality recorded in C. punctata and medium mortality for B. signaticollis, A. sardea, N. glauca, P. minutissima and D. magna. Three-way ANOVA indicated highly significant effects of species, dose, and time. According to the sensitivity to Dimilin, the pairwise comparison of Tukey's test indicates that the most sensitive species was C. punctata followed by B. signaticollis and the least sensitive was N. glauca, followed by D. magna; then A. sardea and finally P. minutissima.

- Abstract
Tetraclinis articulata is an endemic, medicinal and aromatic species of the cupressaceae family with an economic importance. The objective of our study is based on the morpho-metric characterization of a 210 individual sample from seven populations distributed in five regions in western Algeria: Tlemcen (Ain fezza (Yfri forest), Ain Ghraba (Ahfire forest), Beni Snous (Zarifate forest)), Ain Temouchent ( Madagh 2 forest) , Oran (the Virgin forest) , Relizane ( Zemmora forest) and finely Sidi Bel Abbes (Tenira forest) using eight morphological traits :Circumference of the tree (CRF),The tree height (TH),The tree crown (TCR), The length of primary branch (LgB1),The length of secondary branch (Lg B2),The length of the needles(LgNd) which were statistically studied using the R software. Phenotypic diversity was determined by the Shannon-Weaver diversity index (H') at different levels. The estimated H' showed a large phenotypic variability for different traits with a mean of 0.90. The results of the multiple correspondence analysis (MCA) and hierarchical clustering (HCA) showed a clear distinction between the populations. The extraction of essential oils was done for all populations; the yield of essential oils from the trees of the Beni Snouss population was higher than the other populations which is very important for future economic perspectives and we all so found that there is a negative correlation between the yield of essential oils and The length of the needles (LgNd) which can be used to identify the subjects with the best performance.

The Holocene representations are among last testimonies of the presence of wild and domestic mammals of the Atlas massifs (Tellian and Saharan) and of the central Sahara. On the rock walls of the same places or on the ancient mosaics of the first Afro-Roman cities, carnivores can be found nearby or associated with herds of wild or domestic animals, cheep, cattle, dogs and figures. On these territories, we owe to the recurrent Holocene movements, the intense presence of herds of wild and domestic animals, associated with the numerous com-munities of hunters/ agro-pastoralists, and later to the conquerors who built modern cities.

- Abstract
In the dependability context, the diagnosis is fundamental for identifying and classifying failures to reduce system malfunctions and accidents that can cause serious damage to property and people. Currently, systems are generally complex and the distribution approach of the diagnostic function makes it possible to better managing the systems to be diagnosed. This article proposes a methodology for the distributed diagnosis of Discrete Event Systems, which are dynamic systems widely spread. The proposed approach, based on a generic model, is formed of two sequential steps. The first one consists in the system modeling giving a mean to understand the real behavior of the studied system, through a distributed recovery of signals. The second step is to standardize and process the obtained data for the classification of failures. To achieve this challenge, two learning classification methods are adopted: the Learning Algorithm for Multivariate Data Analysis (LAMDA) coupled with the K-Nearest Neighbor algorithm (K-NN). To validate the approach, the generic model is applied on a particular discrete event system, a railway transport network following a real case of railway line and the obtained results showed the improvement of the classification by recognition. The proposed framework of distributed diagnosis, based on the two learning methods, seems an efficacy and robust methodology for classify any failures of systems., Advances in Systems Science and Applications, Выпуск 1 2023, Pages 115-128
