Abstract

Aquest article descriu el procés d'implantació d'un sistema de gestió de la documentació administrativa al Departament d'Universitats, Recerca i Societat de la Informació de la Generalitat de Catalunya (actualment Departament d'Innovació, Universitats i Empresa). El projecte, anomenat DursiGED, ha inclòs l'anàlisi, el desenvolupament i la implantació dels productes i serveis necessaris per gestionar la documentació electrònica i en paper de manera normalitzada i d'acord amb uns paràmetres de qualitat. L'article inclou la descripció de la planificació del projecte, el procés d'implantació i seguiment, i els productes i serveis que n'han resultat.Este artículo describe el proceso de implantación de un sistema de gestión de la documentación administrativa en el Departament d'Universitats, Recerca i Societat de la Informació de la Generalitat de Catalunya (actualmente Departament d'Innovació, Universitats i Empresa). El proyecto, denominado DursiGED, ha incluido el análisis, el desarrollo y la implantación de los productos y servicios necesarios para gestionar la documentación electrónica y en soporte papel de manera normalizada y de acuerdo con unos parámetros de calidad. El artículo incluye la descripción de la planificación del proyecto, el proceso de implantación y seguimiento, y los productos y servicios que han resultado de todo ello.This article describes the process of implementing a system for managing administrative records at the Department of Universities, Research and Information Society of the Generalitat de Catalonia (currently the Department of Innovation, Universities and Enterprise). The project –DursiGED—has included the analysis, development and implementation of products and services necessary for managing records in both digital format and in paper, in a standardised fashion and in agreement with quality parameters. The article includes a description of the project planning, the process for implementation and follow-up, and the resulting products and services.

Abstract

Els projectes d’administració electrònica són actualment el principal factor de canvi de les organitzacions públiques. Aquest canvi, que es pot considerar imparable, té implicacions importants pel que fa a la gestió i la conservació dels documents administratius generats per les tramitacions electròniques. Aquest article analitza la problemàtica metodològica, legal i cultural que es deriva de la inclusió del paradigma arxivístic en els projectes d’administració electrònica, i també les conseqüències que pot tenir per als arxius. Finalment, proposa un conjunt de solucions metodològiques per identificar els documents electrònics amb valor evidencial, determinar-ne el cicle de vida, definir una política de preservació i crear un arxiu digital.The projects dealing with e-government are currently the principal factor in instigating the changes taking place within the public administration. This change, which is considered unstoppable, has important implications regarding the management and conservation of administrative documents generated by electronic transactions. This article analyses the methodological, legal and cultural challenges that come about when the archives paradigm is included within e-government projects and also the consequences that this may have on the archives themselves. In conclusion the author proposes a set of methodological solutions for identifying those electronic documents with evidential value, determining their life cycle, defining a preservation policy and creating a digital archive.Los proyectos de administración electrónica son actualmente el principal factor de cambio de las organizaciones públicas. Este cambio, que se puede considerar imparable, tiene implicaciones importantes en lo que respecta a la gestión y a la conservación de los documentos administrativos generados por las tramitaciones electrónicas. Este artículo analiza la problemática metodológica, legal y cultural que se deriva de la inclusión del paradigma archivístico en los proyectos de administración electrónica, y también las consecuencias que puede tener para los archivos. Finalmente, propone un conjunto de soluciones metodológicas para identificar los documentos electrónicos con valor evidencial, determinar su ciclo de vida, definir una política de preservación y crear un archivo digital.

Abstract

Esophagogastric junction (EGJ) barrier function is of fundamental importance in the pathophysiology of gastroesophageal reflux disease. Impaired EGJ barrier function leads to excessive distal esophageal acid exposure or, in severe cases, esophagitis. Hence, proposed high-resolution manometry (HRM) metrics assessing EGJ integrity are clinically important and were a focus of the Chicago Classification (CC) working group for inclusion in CC v4.0. However, the EGJ is a complex sphincter comprised of both crural diaphragm (CD) and lower esophageal sphincter (LES) component, each of which is subject to independent physiological control mechanisms and pathophysiology. No single metric can capture all attributes of EGJ barrier function. The working group considered several potential metrics of EGJ integrity including LES-CD separation, the EGJ contractile integral (EGJ-CI), the respiratory inversion point (RIP), and intragastric pressure. Strong recommendations were made regarding LES-CD separation as indicative of hiatus hernia, although the numerical threshold for defining hiatal hernia was not agreed upon. There was no agreement on the significance of the RIP, only that it could localize either above the LES or between the LES and CD in cases of hiatus hernia. There was agreement on how to measure the EGJ-CI and that it should be referenced to gastric pressure in units of mmHg cm, but the numerical threshold indicative of a hypotensive EGJ varied widely among reports and was not agreed upon. Intragastric pressure was endorsed as an important metric worthy of further study but there was no agreement on a numerical threshold indicative of abdominal obesity.

Abstract

Chicago Classification v4.0 (CCv4.0) is the updated classification scheme for esophageal motility disorders using metrics from high-resolution manometry (HRM). Fifty-two diverse international experts separated into seven working subgroups utilized formal validated methodologies over two-years to develop CCv4.0. Key updates in CCv.4.0 consist of a more rigorous and expansive HRM protocol that incorporates supine and upright test positions as well as provocative testing, a refined definition of esophagogastric junction (EGJ) outflow obstruction (EGJOO), more stringent diagnostic criteria for ineffective esophageal motility and description of baseline EGJ metrics. Further, the CCv4.0 sought to define motility disorder diagnoses as conclusive and inconclusive based on associated symptoms, and findings on provocative testing as well as supportive testing with barium esophagram with tablet and/or functional lumen imaging probe. These changes attempt to minimize ambiguity in prior iterations of Chicago Classification and provide more standardized and rigorous criteria for patterns of disorders of peristalsis and obstruction at the EGJ.

Abstract

The Internet of Things (IoT) will facilitate a wide variety of applications in different domains, such as smart cities, smart grids, industrial automation (Industry 4.0), smart driving, assistance of the elderly, and home automation. Billions of heterogeneous smart devices with different application requirements will be connected to the networks and will generate huge aggregated volumes of data that will be processed in distributed cloud infrastructures. On the other hand, there is also a general trend to deploy functions as software (SW) instances in cloud infrastructures [e.g., network function virtualization (NFV) or mobile edge computing (MEC)]. Thus, the next generation of mobile networks, the fifth-generation (5G), will need not only to develop new radio interfaces or waveforms to cope with the expected traffic growth but also to integrate heterogeneous networks from end to end (E2E) with distributed cloud resources to deliver E2E IoT and mobile services. This article presents the E2E 5G platform that is being developed by the Centre Tecnol\`ogic de Telecomunicacions de Catalunya (CTTC), the first known platform capable of reproducing such an ambitious scenario.

Abstract

Background: STW5 is an herbal medicinal product that, in previous studies, reduced abdominal pain in irritable bowel syndrome (IBS). The effect of STW5 on gas‐related abdominal symptoms is unknown. Aim: To determine the effects of STW5, compared to placebo, on the responses to colonic gas in IBS. Methods: Using a cross‐over design, two gas challenge tests were performed in 10 patients with IBS and bloating after 2‐weeks treatment with (a) STW5 and (b) placebo. The challenge test consisted in continuous infusion of gas into the colon (24 mL/min for 60 min), followed by a 30‐min free evacuation period. Gas evacuation, symptom perception, and abdominal distension were continuously registered. Results: Colonic gas filling was associated to a significant rise in abdominal symptom perception, that was significantly greater when patients were on‐placebo (score increment 4.0 ± 0.3) compared with on‐STW5 (score increment 3.2 ± 0.4; p = 0.035). Gas filling was associated to a progressive abdominal distension that was similar with both treatments. Opening of the rectal cannula produced a massive gas evacuation, similar after both treatments, associated to a return of abdominal perception and distension to basal levels when patients were on‐STW5 (score increment −0.1 ± 0.4; distension 0.3 ± 0.2 cm; p = 0.399, and p = 0.112 vs. basal), whereas both remained increased on‐placebo (score increment 0.5 ± 0.3; distension 0.8 ± 0.3 cm; p = 0.048, and p = 0.016 vs. infusion start). Conclusions: STW5 improves colonic gas tolerance in IBS patients with bloating without a significant effect on gas retention and evacuation. 36. Design of a solar-powered desalination plant for a remote Kenyan Community

Abstract

With this project, a solar-powered desalination plant has been successfully designed to supply potable water to the Molo Community. This community is a rural Kenyan tribe composed of approximately 900 individuals, residing on the shores of Lake Turkana, one of the world's largest saltwater lakes. Recently, their potable water supply system has become unusable due to the rise in the lake's water level, resulting in a critical situation. Through a comprehensive analysis of global water scarcity and direct engagement with local stakeholders, the project identified the urgent need for a solution to address water scarcity in the community. Following extensive research, reverse osmosis was selected as the most suitable desalination method for the plant's design, capable of producing 117.6 cubic meters of potable water daily. The design process has encompassed various stages, including pretreatment, main treatment, and post-treatment processes, each carefully tailored to meet the community's specific requirements, considering the water conditions and parameters to be treated. Additionally, the project has considered the economic feasibility of the plant, estimating a water price of 0.91€/m3, making it accessible to all community members. Furthermore, environmental sustainability has been a priority throughout the project, considered a key variable in its design. On one hand, the use of renewable energy has enabled the elimination of highly polluting fossil fuels. On the other hand, the proposal to utilize the brine produced by the plant for irrigating halophyte plants offers both waste management solutions and potential income generation opportunities for the community. This approach promotes circular economy principles by maximizing the utilization of available resources, even turning waste such as brine into an asset. L'estudi per implementar una nova planta pilot a la fàbrica de Henkel a Montornès ha estat un procés orientat a l'optimització dels processos de fabricació de determinades substàncies ja existents. Aquesta iniciativa tenia com a objectiu fonamental desenvolupar un procediment estandarditzat i més eficaç, que proporcionés millores tangibles en la producció i control de qualitat dels productes. En aquesta incansable recerca de millora i innovació, s'han incorporat noves eines i tecnologies a l'operativa de la fàbrica. Destaquen especialment dues d'aquestes innovacions: la implementació del procés en continu i la introducció d'un mesclador dinàmic. Aquestes incorporacions han significat un gran avanç, ja que no només milloren la qualitat dels processos, sinó que també incrementen la seva eficiència de manera substancial. Per assegurar l'efectivitat i fiabilitat d'aquests nous elements, s'han dut a terme proves exhaustives a tots els nivells, des de l'àmbit local de Montornès fins a escala internacional. Les proves de viabilitat, en concret, es van realitzar durant un viatge a la seu principal de l'empresa, situada a Alemanya. Aquest enfocament global demostra el compromís de l'equip local amb altres plantes industrials, així com l'interès i la dedicació que implica aquesta tasca. El desenvolupament del procés en continu ha estat un dels eixos centrals d'aquest projecte. Aquest sistema permet una producció contínua i meticulosament controlada de les substàncies involucrades, reduint els temps de parada i arrancada i assegurant una major precisió en la dosificació de les matèries primeres. A més a més, ofereix un control en temps real dels productes fabricats, detectant i corregint possibles desviacions de qualitat de manera immediata. Abstract

This conference analyzes the current state of records management in relation to the needs faced today. This state is analyzed from four factors: society, organizations, technology, and the profession, and within the last factor five variables are considered: the crisis of traditional records management, new opportunities in the framework of information governance, new developments in regulatory context, service models for records management, and the evolution of the professional profile.

Abstract

This presentation are the materials of a course. This course presented a process reengineering methodology focused on documents, data and other kind of evidences generated by a business process. Using the records series concept, this methodology allows user to improve document/data flow quality, convert it to only electronic records and automate the flow using ECM technology, and finally convert it to data-based evidence flow and automate it using an ERP or simply a database.

Abstract

Introduction: Stemmed total knee arthroplasty (STKA) periprosthetic fractures (PPFs) are an emerging problem affecting frail patients. Their surgical fixation is challenging, due to intramedullary involvement and poor bone stock. Polyaxial locking plating has yielded good results in implant-related femur fractures. We hypothesized that this treatment would provide similar results for STKA PPFs. Methods: Retrospective analysis of consecutive patients with a femoral PPF or inter-implant fracture around a knee revision stem who had undergone open reduction and periprosthetic-specific polyaxial plate fixation. Results: We found 14 cases of mean age 85.4 years. Cerclages were used in 80% of cases. Fixation of a mean 8.6 cortices around the revision stem was achieved, with an overall screw density of 1:2 or 1:3. Four patients lost their ability to walk, while four experienced postoperative local complications. Bone healing was achieved in all except one who died during hospitalization. The 13 remaining survived the first year of follow-up. Conclusion: STKA PPFs are an emerging and challenging problem affecting frail patients. Treatment using polyaxial locking plates provides stable fixation allowing early mobilization despite high complication rates.
