Abstract

Circadian and other physiological rhythms play a key role in both normal homeostasis and disease processes. Such is the case of circadian and infradian seizure patterns observed in epilepsy. In this paper we explore a new implantable stimulator that implements chronotherapy as a feedforward input to supplement both open-loop and closed-loop methods. This integrated algorithm allows for stimulation to be adjusted to the ultradian, circadian and infradian patterns observed in patients through slowly-varying temporal adjustments of stimulation and algorithm sub-components, while also enabling adaption of stimulation based on immediate physiological needs such as a breakthrough seizure or change of posture. Embedded physiological sensors in the stimulator can be used to refine the baseline stimulation circadian pattern as a "digital zeitgeber". This approach is tested on a canine with severe drug-resistant idiopathic generalized epilepsy exhibiting a diurnal pattern correlated with sleep-wake cycles. Prior to implantation, the canine's cluster seizures evolved to status epilepticus (SE) and required emergency pharmacological intervention. The cranially-mounted system was fully-implanted bilaterally into the centromedian nucleus of the thalamus. Using time-based modulation, thalamocortical rhythm-specific tuning of frequency parameters as well as fast-adaptive modes based on activity, the canine experienced no further SE events post-implant as of the time of writing (seven months). Importantly, no significant cluster seizures have been observed either, allowing the reduction of rescue medication. The use of chronotherapy as a feedforward signal to augment adaptive neurostimulators could prove a useful method in conditions where sensitivity to temporal patterns are characteristics of the disease state, providing a novel mechanism for tailoring a more patient-specific therapy approach., Comment: 6 text pages, 4 main figures, Fig 1 has 4 panels, Fig 2 has 3 panels, Fig 4 has 2 panels. 2 text pages of supplementary material, 1 supplementary figure

Abstract

To support the global restart of elective surgery, data from an international prospective cohort study of 8492 patients (69 countries) was analysed using artificial intelligence (machine learning techniques) to develop a predictive score for mortality in surgical patients with SARS-CoV-2. We found that patient rather than operation factors were the best predictors and used these to create the COVIDsurg Mortality Score (https://covidsurgrisk.app). Our data demonstrates that it is safe to restart a wide range of surgical services for selected patients.

26. Quality Parenting Initiative: Engaging Diverse Stakeholders as Champions for Child Welfare Systems Reform

Abstract

Objective:Quality Parenting Initiative (QPI) is one approach to advance foster care reform efforts by engaging key stakeholders in the implementation process. Using a participatory action approach, QPI has been implemented in more than 80 sites in 10 states. However, no formal evaluation has been conducted. We sought to identify the strategies used to engage stakeholders in QPI implementation. Method:This qualitative study included QPI stakeholders from a larger process evaluation study that examined perceptions of QPI satisfaction, usefulness, and impact. We conducted three focus groups with birth and foster parents, agency providers, and community leaders (N=37) involved in QPI implementation. We purposively selected foster care agencies representing various stages of QPI implementation. Data were analyzed through content analysis that included codebook development. Results:We identified three major themes: (a) increasing inclusion in decision-making and other processes, (b) strengthening relationships and building partnerships, and (c) sharing success stories. Recommendations for sustained engagement were also captured. Conclusions:Findings suggest that using a participatory approach, wherein diverse stakeholders collaborate around a shared vision, can increase engagement in implementation activities. Moreover, keeping stakeholders engaged while using an iterative process that is customized around local issues may contribute to significant systems change.

Abstract

This paper brings together the practice-based creative research of artists Charlotte Gould and Paul Sermon, culminating in a collaborative interactive installation that investigates new forms of social and political narrative in multi-user virtual environments. The authors' artistic projects deal with the ironies and stereotypes that are found within Second Life in particular. Paul Sermon’s current creative practice looks specifically at the concepts of presence and performance within Second Life and 'first life', and attempts to bridge these two spaces through mixed reality techniques and interfaces. Charlotte Gould’s Ludic Second Life Narrative radically questions the way that users embody themselves in on-line virtual environments and identifies a counter-aesthetic that challenges the conventions of digital realism and consumerism.These research activities and outcomes come together within a collaborative site-specific public installation entitled Urban Intersections for ISEA09, focusing on contested virtual spaces that mirror the social and political history of Belfast. The authors' current collaborative practice critically investigates social, cultural and creative interactions in Second Life. Through these practicebased experiments the authors' argue that an enhanced social and cultural discourse within multi-user virtual environments will inevitably lead to growth, cohesion and public empowerment, and like all social networking platforms, contribute to greater social and political change in first life.

Abstract

Objectives: ANP32A is a key protector of cartilage health, via preventing oxidative stress and Wnt hyper-activation. We aimed to unravel how ANP32A is regulated in cartilage.Methods: A bioinformatics pipeline was applied to identify regulators of ANP32A. Pathways of interest were targeted to study their impact on ANP32A in in vitro cultures of the human chondrocyte C28/I2 cell-line and primary human articular chondrocytes (hACs) from up to five different donors, using Wnt-activator CHIR99021, hypoxia-mimetic IOX2 and a hypoxia chamber. ANP32A was evaluated using real-time quantitative polymerase chain reaction (RT-qPCR) and Western blot. In vivo, the effect of hypoxia was examined by immunohistochemistry in mice injected intra-articularly with IOX2 after destabilization of the medial meniscus. Effects of Wnt hyper-activation were investigated using Frzb-knockout mice and wild-type mice treated intra-articularly with CHIR99021. Wnt inhibition effects were assessed upon intra-articular injection of XAV939.Results: The hypoxia and Wnt signaling pathways were identified as networks controlling ANP32A expression. In vitro and in vivo experiments demonstrated increases in ANP32A upon hypoxic conditions (1.3-fold in hypoxia in C28/I2 cells with 95% confidence interval (CI) [1.11-1.54] and 1.90-fold in hACs [95% CI: 1.56-2] and 1.67-fold in ANP32A protein levels after DMM surgery with IOX2 injections [95% CI: 1.33-2.08]). Wnt hyper-activation decreased ANP32A in chondrocytes in vitro (1.23-fold decrease [95% CI: 1.02-1.49]) and in mice (1.45-fold decrease after CHIR99021 injection [95% CI: 1.22-1.72] and 1.41-fold decrease in Frzb-knockout mice [95% CI: 1.00-1.96]). Hypoxia and Wnt modulated ataxia-telangiectasia mutated serine/threonine kinase (ATM), an ANP32A target gene, in hACs (1.89-fold increase [95% CI: 1.38-2.60] and 1.41-fold decrease [95% CI: 1.02-1.96]).Conclusions: Maintaining hypoxia and limiting Wnt activation sustain ANP32A and protect against osteoarthritis. Abstract

Universities have increased the number of female students of color. However, inconsistency remains in terms of the number of women of color in positions of power and privilege. This discussion attempts to forward the dialogue about the challenges as well as the resilience of female students of color based on the experiences of four female African-American students in a graduate social work program at a predominantly white institution (PWI). Participants shared their daily experience of isolation and exclusion in their graduate programs compared to their experience in Historically Black College or Universities (HBCUs). Participants also discussed the divide that exists in institutions, as well as the sustaining forces and implications for education and research.

43. Sub-harmonic entrainment of cortical gamma oscillations to deep brain stimulation in Parkinson's disease: Model based predictions and validation in three human subjects.

Abstract

The exact mechanisms of deep brain stimulation (DBS) are still an active area of investigation, in spite of its clinical successes. This is due in part to the lack of understanding of the effects of stimulation on neuronal rhythms. Entrainment of brain oscillations has been hypothesised as a potential mechanism of neuromodulation. A better understanding of entrainment might further inform existing methods of continuous DBS, and help refine algorithms for adaptive methods. The purpose of this study is to develop and test a theoretical framework to predict entrainment of cortical rhythms to DBS across a wide range of stimulation parameters. We fit a model of interacting neural populations to selected features characterising PD patients' off-stimulation finely-tuned gamma rhythm recorded through electrocorticography. Using the fitted models, we predict basal ganglia DBS parameters that would result in 1:2 entrainment, a special case of sub-harmonic entrainment observed in patients and predicted by theory. We show that the neural circuit models fitted to patient data exhibit 1:2 entrainment when stimulation is provided across a range of stimulation parameters. Furthermore, we verify key features of the region of 1:2 entrainment in the stimulation frequency/amplitude space with follow-up recordings from the same patients, such as the loss of 1:2 entrainment above certain stimulation amplitudes. Our results reveal that continuous, constant frequency DBS in patients may lead to nonlinear patterns of neuronal entrainment across stimulation parameters, and that these responses can be predicted by modelling. Should entrainment prove to be an important mechanism of therapeutic stimulation, our modelling framework may reduce the parameter space that clinicians must consider when programming devices for optimal benefit. • Entrainment of brain rhythms at half stimulation frequency is predicted by theory. • Models fitted to off-stimulation patient data predict 1:2 synchronisation regions. • Observations of 1:2 entrainment in patients are in line with model predictions. • 1:2 entrainment is biased to low stimulation frequencies and lost at high amplitudes. 45. Broadband Photo/Gas Dual Sensors Enabled by ZnO Nanorod/Graphene Hybrid Structures

Abstract

In this study, 2-D graphene was integrated with zinc oxide (ZnO) nanorods (NRs) to form broadband photo/gas dual sensors. Since ZnO-based photodetectors (PDs) perform excellent in ultraviolet (UV) light detection, their inability to sense long-wavelength visible and infrared (IR) light limits the PDs’ applications. By combining a 2-D graphene layer on ZnO NRs, the 2-D graphene extended light detection range of device from UV to IR light. In terms of gas sensing, this ZnO NR/graphene-based sensor was capable of sensing hydrogen (H2) carbon monoxide (CO) gases. This device exposed to H2 and CO gases delivered comparable sensitivity of 0.25 under an operated temperature of 300 °C. Multiple material characterizations confirm that a 2-D graphene layer has successfully deposited on ZnO NRs to form a distinct hybrid ZnO NR/graphene nanostructure. Through a series of sensing analyses, results have been demonstrated that the ZnO NR/graphene-based sensor outperformed ZnO-based sensor in response to light illumination. However, ZnO NR/graphene-based sensor exhibited inferior gas sensitivity than the ZnO-based sensor. The coverage of graphene on the surface of ZnO NRs could passivate the surface defects of ZnO NRs. Although the surface passivation was beneficial for reducing the charge recombination and improving the photocurrent, the surface passivation of graphene reduced the adsorption sites of target gas. As a result, the gas sensitivity of ZnO NR/graphene-based sensor was reduced. Although the ZnO NR/graphene-based sensor exhibited a reduced gas sensitivity, the hybrid ZnO/2-D graphene nanocomposites pave the way for broadband PDs and gas sensors with promising applications in various fields.

This investigation sought information about principals and their relationship with computer technology. Several questions were fundamental to the inquiry. Are principals prepared to facilitate the attainment of technology's promise through the integration of computer technology into the teaching and learning process? Are principals prepared to use computer technology to accomplish administrative and managerial tasks? What can be stated about principals' current expertise to use computer technology? Principals' responses to items on the Computer Technology Survey enabled an examination of their (a) role in facilitating and participating in the integration of computer technology into teaching and learning, (b) perceptions about computer technology for managerial and administrative tasks, (c) expertise acquired to use computer technology, and (d) professional development needs to enhance computer technology skills.
