Amaç: Bu çalışma, Türkiye’de yüz yüze eğitimin sürdürüldüğü iki dönemde uygulanan COVID-19 yönetim algoritmalarını ve sonuçlarını karşılaştırarak, çocukların salgında pozitiflik durumunun belirlenmesi ve algoritmaların değerlendirilmesi amacı ile yapılmıştır. Gereç ve Yöntemler: Çalışma, tanımlayıcı türde yapılmıştır. Yüz yüze eğitimin başladığı dönemler olan 21 Eylül-16 Kasım 2020 tarih aralığı ile 6-20 Eylül 2021 tarih aralığında Kayseri ilinde eğitim görmekte olan 6-18 yaş aralığındaki çocukların PCR test sayıları ve sonuçları değerlendirilmiştir. Her iki dönemde uygulanan COVID-19 yönetim algoritmaları bu sonuçlar üzerinden ele alınarak, COVID-19 pandemisindeki etkileri ve sonuçları karşılaştırılmıştır. Hayatımızın her alanına giren elektronik ve bilgisayar teknolojilerinden diğer tüm sektörlerde olduğu gibi inşaat sektöründe de İş Sağlığı ve Güvenliği (İSG) ’nin sağlanmasına yönelik yararlanma çabaları, son yıllarda başta gelişmiş ülkelerde olmak üzere tüm dünyada artış göstermektedir. İnşaat sektörü, ülkemizde iş kazası sonucu yaralanma ve ölümlerin en sık yaşandığı sektörlerin başında gelmektedir. Bu açıdan sektörde İSG’nin sağlanması için diğer her türlü imkânın yanında teknolojik yeniliklerin de devreye sokulması önemlidir. Bu noktadan hareketle gerçekleştirilen bu çalışmada, çeşitli veri tabanları üzerinden “teknoloji” “inşaat sektörü” ve “iş sağlığı ve güvenliği” anahtar kelimeleri kullanılarak kapsamlı bir tarama yapılmış ve inşaat sektörüne yönelik İSG teknolojileri belirlenmiştir. Belirlenen bu teknolojiler, kullanım alanlarına göre gruplandırılarak literatürdeki uygulamaların yanı sıra, sahada kullanılan ticari uygulamalar da incelenmiştir. Çalışma, inşaat sektöründe İSG’nin sağlanmasına yönelik olarak geniş bir yelpazede birçok teknolojik uygulama bulunduğunu ortaya koymuştur. Çalışmanın bazen çok küçük maliyetlerle ölüm ile hayatta kalma arasındaki farkı yaratan İSG teknolojilerinin Türk inşaat sektörü paydaşlarına tanıtılmasına ve bu sayede bu teknolojilerin kullanımının yaygınlaştırılmasına katkı sağlayacağı değerlendirilmektedir.

Odontogenic cysts are originated from maxillary and mandibular teeth bearings odontogenic tissue and pathological cavities which are surrounded by fibrous connective tissue with epithelial layers. Cystic conditions on the jaw would end up bone damage, reabsorption or loss of the related tooth. Odontogenic cysts may occurdevelopmental, or inflammation-related. They are more common in adults than in pediatrics. Reports shows that matrix metalloproteinase synthesis which is activating by Nitric Oxyde (NO) participates bone resorption and cyst enlargement due to high NO production. eNOS gene variants change gene expression and activity due to increasing or over NO production and this situation causes various pathological conditions. The aim of this study is to investigate eNOS gene VNTR 4b/4a variants and oral cysts possible relationships. Genotyping revealed by the PCR method and 31 patients have I/I, 5 patients I/D, 1 patients D/D genotype with a total of 37 patients in the experiment group. According to obtained results, eNOS variantsI/I alleles and genotype frequency patients have more oral cyst possibility than D/D allele and genotype patients. Current results show that eNOS polymorphism may represent possible risk factors for the oral cyst.

Objectives Botulinum toxin injections are an important issue that is widely used, and many scientific articles have been published in patients with temporomandibular disorders (TMD). The aim of this study was to evaluate the status of activity the use of botulinum toxin in TMD during the period 1978-2023. Materials and methods Articles published until April 1, 2023, were searched in Web of Science. The top 50 articles with the highest citations and suitable for the study topic were selected among the totally 299 articles. Results Even though the articles published about botulinum injection in TMD have increased in recent years, there is no regular increase over the years. The most cited article was published in 2015 and the number of citations in Web of Science was 194. While there was no difference in productivity among the authors, the most productive country was the US. Türkiye was found to be above the average among other countries. Conclusions This bibliometric study of the top 50 most-cited papers in Web of Science the use of botulinum toxin in TMD recognized a quantitative and qualitative analysis of this very favorable research field.

We reported on 65 years old patient who has colon cancer and referred to our palliative care center with pain due to enlarging metastatic mass on the dorsal of the right hand. She had swelling and numbness on her jaw. Computed tomography (CT) scan was performed for mandible imaging and two pathologic fractures were detected on the right corpus and right condyle of the mandible. Clinicians should consider possible metastases for terminal stage cancer patients.

Amaç: Kayseri İlindeki KETEM’lerine 2018 yılında başvurup HPV pozitif çıkan kadınlardaki HPV tiplerinin, histopatolojik tiplendirme dağılımının ve ileri merkez muayene sonuçlarının incelenmesidir.Yöntem: Çalışma için Erciyes Üniversitesi Klinik Olmayan Araştırmalar Etik Kurulu’ndan etik onay alınmıştır (Tarih: 11.12.2019 Sayı:2019/835). Bu çalışma 2018 yılı boyunca KETEM birimlerinde HPV testi alınan 30-65 yaş arası 28148 kişinin verilerinin taranmasıyla yapılmıştır. HSYS veri tabanından 2018 yılına ait HPV sonucu pozitif olan hastaların yaş, HPV tipi, servikal sitoloji sonucu ve kamu hastanelerinden hastalara yapılan muayene, tetkikler ve diğer işlem sonuçları incelenerek SPSS 21.0 programına aktarılmıştır.Bulgular: HPV testi pozitif bulunan kişi sayısı 1171 (%4.16)’dir. En sık HPV tip 16 (%26.8) görülmüştür. HPV testi pozitif olan 1171 kadının 771’inin (%65.8) kamu hastanelerinde muayene olduğu, 478’inden (% 62.0) biyopsi alındığı, 62’sine (% 8.0) smear incelemesi 597’sine (%77.4) ultrasonografi yapıldığı görülmüştür. Muayene sonrası 596’sında(%77.3) patolojik bulgu saptanmamış olup, 78’inde (%10.1) LSIL, 66’sında (%8.6) HSIL saptanmıştır. Üç kişide ise (%0.4) invaziv kanser tespit edilmiştir.Sonuç: Hedef nüfusun her yıl %20’sinin taranması için tarama programlarının daha geniş kitleye ulaştırılması ve bu kapsamda çalışmalar yapılması gerekmektedir., Objective: To examine the distribution of HPV types, histopathological typing and advanced center examination results in women who applied to KETEM in Kayseri in 2018 and were positive for HPV.Methods: Ethical approval was obtained from Erciyes University Nonclinical Research Ethics Committee for the study. This study was carried out by scanning the data of 28148 people between the ages of 30-65 who received HPV tests in KETEM units throughout 2018. The age, HPV type, cervical cytology result of patients with positive HPV results for 2018 from the HSYS database, and the results of examinations, tests and other procedures performed on patients from public hospitals were examined and transferred to the SPSS 21.0 program.Results: The number of people with positive HPV test is 1171 (4.16%). HPV type 16 (26.8%) was the most common. It was observed that 771 (65.8%) of 1171 women with positive HPV test were examined in public hospitals, 478 (620%) had biopsy, 62(8.0%) had smear examination and 597 (77.4%) had ultrasonography. Pathological findings were not detected in 596 (77.3%) after the examination, LSIL was found in 78(10.1%) and HSIL in 66 (8.6%). Invasive cancer was detected in three patients (0.4%)Conclusion: It is clear that with cervical cancer screening, patients can be diagnosed at an early stage or when they have a precancerous lesion and this situation is of vital importance.

Aim: This study aims to investigate the awareness and knowledge level of patients about the use of herbal products in dentistry.Materials and Methods: Present study was carried out on a total of 200 patients who applied to the Faculty of Dentistry Oral and Maxillofacial Surgery Clinic in 2020. Data were collected using questionnaires. Herbal product usage levels, attitudes towards phytotherapy and general opinions of the patients were evaluated.Results: Herbal products used for oral and dental health were found to be at lower levels than herbal products used in the prevention or treatment of other diseases. The patients preferred these products for halitosis and wounds. The most commonly used plants for oral health were mint, clove, thyme and black mulberry.Conclusion: The results of present study showed that patients are willing to use herbal products. This result suggests that it would be beneficial for dentists to integrate phytotherapy into treatment protocols.

İntihar, kişinin hayatını bilerek sonlandırmak istemesidir. Dünyada ilk on ölüm nedeni arasında sayılmakla birlikte önemli bir halk sağlığı sorunudur. İntihar psikolojik faktörlerden etkilenen en önemli ölüm nedenlerinden biridir. Çalışmamız retrospektif tanımlayıcı bir çalışma olarak tasarlanmıştır. Ölüm bildirim sistemi üzerinden 2019 yılında intihar eden kişiler taranarak toplam 61 kişiye ulaşılmıştır. Demografik özellikleri ve intihar ettikleri tarih, sistemde kayıtlı intihar şekilleri, psikiyatrik hastalık tanıları, kullandıkları ilaçlar değerlendirilmiştir. İstatistiksel veri analizlerinde SPSS 21.0 paket programı kullanılmıştır. 2019 yılında ilimizde toplam 61 kişi intihar etmiştir. %75.4’ü erkek, %24.6’sı kadındır. Yaş ortancası (Q1-Q3) 31 (24.5-46) idi. %62.3’ü kendini asarak, %16.4’ü ise ateşli silah, %21.3’ü diğer şekillerde intihar etmişti. %19.7’sinin psikiyatrik bir hastalığı varken %80.3’ünün ruh sağlığı kliniğine başvurusu bulunmamaktaydı. Erkek cinsiyet ve yaş şiddet içeren intihar vakalarını etkileyen faktör iken (p=0.010, p=0.046) medeni hal ve mesleğin şiddet içeren intihar vakalarını etkilemediği saptanmıştır (p=0.54, p=0.761). Ölüm nedenleri arasında üst sıralarda olan, toplumsal bir sorun olan ve multidisipliner ya da disiplinlerarası yönetilmesi gereken bir durumdur. Kişileri, çare arama davranışı yönünden desteklemek gerektiğini düşünmekteyiz. İntiharın azalması için kişilerin bireysel tedavi ve terapisinin sağlanması yanında toplumsal ve yasal değişiklikler de yapmak gerekmektedir.

The prevalence of DC / TMD diagnosis of individuals with internal derangement of TMJ who want to receive TMD treatment in a tertiary clinic in the Turkish population and comparison of the criteria applied in Axis I and Axis II.This study was carried out on 200 individuals older than 18 years of age who have internal disorder of Temporomandibular Joint (TMJ). Diagnostic Criteria for Temporomandibular Disorders (DC / TMD) Axis I and II were applied.The female to male ratio of individuals with internal derangement of TMJ in the Turkish population was 3.5. The Pearson correlation coefficient between the internal derangement of the Right TMJ and the internal derangement of the Left TMJ is 0.804 and has a statistically significant relationship (p0.05).For DC / TMD, a more comprehensive study is needed to compare the results found in the Turkish population with other populations.

Introduction: It has been observed that outbreaks experienced throughout history have caused anxiety by profoundly affecting societies. COVID-19 pandemic soon affected the entire world and caused the deaths of thousands of people. Frequently encountering COVID-19 patients increase the anxiety levels of healthcare workers during the pandemic. This study aimed to determine the anxiety levels of healthcare workers of family health centers in Kayseri during the pandemic. Materials and Methods: A total of 214 physicians and family health workers between the ages of 20 and 65 and actively working in Family Health Centers in Kayseri participated in the study. The Beck Anxiety Inventory (BAI) was used to determine the level of anxiety. Results: Of 214 people who participated in the study, 71.0% were female, 82.7% were married, 52.8% were physicians and 47.2% were family health workers. Their mean age was 38.85. Median employment duration was 14 (min-max: 1-34) years and the median Beck Anxiety Inventory Score was 17 (min-max: 0-63). Of the participants, 53.7% stated that they attended a training program on COVID-19 while 84.1% stated that the outbreak had psychologically affected them. Conclusion: The pandemic has increased the anxiety level of healthcare workers. Mental health of healthcare workers in family health centers affects the quality of the service they provide in the primary care. Therefore, psychosocial support teams should be built up in our country and around the world, and health workers should be supported psychologically as well as patients.

İş kazaları yol açtıkları ölüm, yaralanma ve işgücü kayıpları ile iş hayatının en önemli sorunlarından birisi olmaya devam etmekte, istatistikler ise gelişmiş ülkelerde iş kazalarının diğer ülkelere göre daha az olduğunu göstermektedir. Bu çalışmada Türkiye’nin gelişmişlik seviyesi ile iş kazası istatistikleri arasındaki ilişkinin ortaya konması amaçlanmıştır. Bu amaçla gelişmişlik seviyesi göstergeleri olan kişi başı gayri safi yurt içi hasıla (GSYİH), ortalama eğitim süresi ve İnsani Gelişme Endeksi (İGE) verilerinin ölümcül ve ölümcül olmayan iş kazaları ile ilişkisi basit regresyon analizi yöntemi ile incelenmiştir. Benzer analizler, Türkiye ile benzer gelişmişlik düzeyindeki Rusya, Romanya, Portekiz, Hırvatistan ve Meksika için İGE verisi baz alınarak yapılmıştır. Çalışma sonuçları Türkiye için İGE ile ölüm sayıları arasında ters yönlü yüksek düzeyde anlamlı bir ilişki ortaya koymuş, ancak 2000-2012 yılları arası iş kazası verisi için yine ters yönlü bir ilişki görülürken, 2013-2018 yıllarında bu ilişkinin ne yazık ki pozitife döndüğü görülmüştür. Karşılaştırma yapılan ülkelerin genelinde ise gelişmişlik düzeyi artarken kaza ve ölümlerin azaldığı görülmüştür. Çalışma sonuçlarının paydaşlara Türkiye’de iş güvenliğinin gelişmişlikle ilişkisi ve mevcut durumu hakkında önemli veriler sağlayacağı değerlendirilmektedir.

In this paper, a new multiband frequency mapping function is proposed to design multiband filter. The presented mapping function is a generalized form of the sequential low pass to band pass (LPtoBP) transformation. The multiband filter design is based on the application of the frequency mapping function on a LP prototype. The synthesis of the resulting multiband filter is obtained by lumped element resonators. Objective: The incidence of chronic kidney diseases increases in older individuals, like all other chronic diseases. Renal biopsy provides important guidance in the diagnosis of many kidney diseases. The impact of biopsy on patient management in older individuals is controversial. In this study, we evaluated the patient population of over 65 years of age in our clinic in the last decade, biopsy-related complications, and the effects of biopsy results on clinical evaluation.

INTRODUCTION[|]Infant mortality is closely related tothe welfare and development level of society. Primary care interventions can be effective in preventing these deaths. The present study aims to investigate the prevalence of infant mortality in Bingöl province in 2016-2017, which is one of Turkey's eastern cities, the reasons and factors that maybe related to the prevalence of infant mortality.[¤]METHODS[|]The type of this research is routine surveillance study. The data were collected retrospectively by examining the deaths of 99 infants in Bingöl province in 2016 and the deaths of 69 infants in 2017 through their medical record files in this study.[¤]RESULTS[|]The infant mortality rate, which was 0.18% in 2016, decreased to 0.12% in 2017. In 2016, the most common cause of infant mortality was prematurity 43 (43.4%) and congenital anomalies 35 (35.4%). The most common cause of death in 2017 was a congenital anomaly 28 (41.7%) and prematurity 25 (37.3%).[¤]DISCUSSION AND CONCLUSION[|]In our study, infant mortality rates in Bingöl were higher than the national average in 2016 and 2017. Congenital anomaly and prematurity were the most common causes of infant mortality in both years. Such studies showing the causes of infant mortality and their changes over the years can be a guide for health policymakers.[¤]

This study presents a new compact design technique for dual band stop (DBS) filter. Analytical formulation of DBS frequency transformation/mapping is derived in terms of parametric representation of band characteristic of DBS filter. The analytical derivation of the transformation function is based on sequential application of classical low pass to band stop (LP to BS) and low pass to band pass (LP to BP) transformations. Assigned band characteristic of DBS filter can be achieved by applying the proposed transformation function on an LP prototype network. The explicit network topologies for DBS reactance mapping are presented. Several numerical examples are provided to validate the proposed mapping function. An implantation example of DBS filter for GSM900 and GSM1800/UMTS bands is given. Mixed element realisation of the designed filter and measurement results of the design are presented. Amaç: Bu çalışmada Kayseri ilinde 2017 yıllarında meydana gelmiş bebek ölümlerinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Çalışma 2017 yılında Kayseri ilinde meydana gelen bebek ölümlerinin incelenmesi ile yapılmıştır. Çalışmanın yapılabilmesi için İl Sağlık Müdürlüğünden yazılı izin alınmıştır. Çalışma kesitsel tipte bir araştırmadır. Ölen bebekler ile ilgili bilgiler dosyalardan elde edilerek analiz edilmiştir. Bulgular: Kayseri ilinde 2017 yılında 21456 canlı doğum ve 171 bebek ölümü meydana gelmiştir. Bebek ölüm hızı binde 8 olarak hesaplanmıştır. Bebeklerin 89’u (% 52’si) erken neonatal dönemde, 35’i (%20.5’i) geç neonatal dönemde, 47’si (%27.5’i) post neonatal dönemde ölmüştür. Bebeklerin ilk üç ölüm nedeni prematüre (% 48,0), konjenital anomali (% 21,6) ve ani bebek ölümü sendromudur (% 9,9). Sonuç: Bu çalışmada bebek ölüm hızı Kayseri ilinde binde 8 bulunmuş ve en sık bebek ölüm nedeninin prematürite olduğu görülmüştür. Bebek ölüm hızı Kayseri ilinde ülke ortalamasının altında olup, geçmiş yıllara göre düşüş göstermiştir. Bebek ölümlerinin hızının ve nedenlerinin ortaya konulması bebek ölümlerini önlemeye yönelik faaliyetler açısından yol gösterici olabilir. Devam eden yıllarda bu tür çalışmaların yapılması sağlık hizmetlerinin etkinliğinin gösterilmesi açısından anahtar rol oynayabilir.

Background: Prevalence and characteristics of non-diabetic renal diseases (NDRD) in patients with type 2 diabetes mellitus is different between populations, and seems to be largely dependent on biopsy policies. Aim: To investigate clinical clues for NDRD in patients with type 2 diabetes mellitus and to analyse renal prognosis of patients based on pathological diagnosis. Methods: We retrospectively searched medical records of 115 patients with type 2 diabetes who underwent a renal biopsy between 2004 and 2018. Patients were divided into three groups as diabetic nephropathy (DN), NDRD + DN or NDRD based on histopathological examination. Results: Thirty-six (31.3%) patients had DN, 33 (28.7%) had DN + NDRD and 46 (40%) had NDRD. The absence of diabetic retinopathy, recent onset of diabetes, abnormal disease chronology, and blood haemoglobin was associated with the presence of NDRD in univariate analysis. Abnormal disease chronology which was defined as the presence of acute proteinuria and/or acute kidney injury that are unexpected to be related to evolution of diabetic nepropathy (odds ratio 4.65, 95% confidence interval 1.44-15.00; P = 0.010) and absence of diabetic retinopathy (odds ratio 3.44, 95% confidence interval 1.32-8.98; P = 0.012) were independently associated with the presence of NDRD in multivariate analysis. Focal segmental glomerulosclerosis was the most frequent type of NDRD. Diseases that affect tubulointerstitial area were more prevalent in the DN + NDRD group compared to the NDRD group (P = 0.001). Renal survival, which was defined as evolution to end-stage renal disease, was 59.5 +/- 14.4 months, 93.7 +/- 11.7 months and 87.2 +/- 2.6 months for DN, DN + NDRD and NDRD groups, respectively (P = 0.005). Conclusions: Renal biopsy is essential in certain clinical conditions as diagnosis of NDRD is vital for favourable renal survival. DN may facilitate superimposed tubular injury in the presence of toxic insults.

Background: The aim of the study was to evaluate the management and outcomes of the patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in a secondary hospital. Methods: A total of 699 hospitalized patients who had positive rRT-PCR for SARS-CoV-2 and/or typical findings of COVID-19 at chest computed tomography (CT) were enrolled in this study. Demographics, comorbities, initial laboratory tests on admission, treatment modalities, complications and outcomes were evaluated retrospectively. Results: The mean age was 57.0±15.6 (range:16-94 years), and male:female ratio was 1.24. 58.7% of the patients had at least one underlying comorbidity, the most common was hypertension. 72.8% of the patients had positive RT-PCR. 18.1% of the patients had lymphopenia, 35.7% hyperferritinemia, 58.3% increased lactate dehydrogenase, and 58.5% increased D-dimer. Chest CT revealed moderate and severe stage in 57.9% of the patients, and bilateral lung involvement in 78.7%. Hydroxychloroquine was given to 37.2% and favipiravir 67.1% of the patients. No significant difference was observed between treatment groups in terms of mortality (P=0.487). 5.8% of the patients were transferred to the ICU, of whom 75.6% were needed non-invasive and 36.5% invasive mechanical ventilation. The overall case fatality rate was 0.9. Conclusions: Older age, male sex, low lymphocyte count, CT findings including bilateral involvement and severe stage were significantly associated with poor prognosis and mortality.

Objective: Granulomatous interstitial nephritis is a rare finding, and etiology differs by geography. We aimed to investigate the distribution of causes of granuloma/granulomata in the kidney and renal survival of these patients in a tertiary care hospital in Western Turkey. Material and Method: Medical records of adults who underwent a kidney biopsy procedure in our institution between January 2000 and June 2019 were reviewed. Pathology reports were searched for biopsies where a granuloma was identified. Results: Nineteen of 1121 (1.7%) kidney biopsies included granuloma, 17 in native kidneys, and 2 in transplants. The majority of indications for native kidney biopsy was a rise in serum creatinine. Etiologies of granuloma included the following: pauci-immune vasculitis (n=11, 64.7%), tuberculosis (n=2, 11.8%), drug-induced (n=2, 11.8%), tubulointerstitial nephritis/uveitis (TINU) syndrome (n=1, 5.9%), and systemic-lupus erythematosus (n=1, 5.9%). Despite treatment, 6 of 11 (54.5%) patients with vasculitis developed end-stage kidney disease (ESKD) during the median follow-up of 16 months. Both of the patients with tuberculosis, and the patient with TINU syndrome developed ESKD months after the kidney biopsy, despite appropriate therapies. The only case with drug-induced granuloma and both cases with allograft kidney granuloma responded well to glucocorticoids, achieving a complete renal recovery. Conclusion: The majority of our series had granuloma in the kidney secondary to vasculitis and renal outcomes appear considerably unfavorable despite treatment, probably related to the primary diagnosis. Multicenter studies are needed to better determine the etiology and outcome of each granuloma etiology at different geographic locations.

Iron has been implicated in oxidative tissue injury owing to its ability to generate reactive oxygen species (ROS). We investigated the reno-protective effects of alpha lipoic acid (ALA) by investigating its effects on the kidney isoform of NADPH oxidase (Nox4) and the specific signaling pathways, p38 MAPK and PI3K/Akt, which participate in apoptosis and survival, respectively. We established four groups of seven rats: control, 100 mg/kg ALA, 80 mg/kg iron sucrose (IS) and IS + ALA. IS and ALA were injected intravenously and rats were sacrificied after 6 h. The mRNA expression of the subunits of NADPH oxidase, Nox4 and p22phox; tumor necrosis factor-alpha (TNF-α); and kidney injury molecule-1 (KIM-1) were measured using quantitative real time polymerase chain reaction (qRT-PCR). Active caspase-3 protein expression was evaluated by immunostaining. Also, p38 MAPK and PI3K/Akt signaling pathways were analyzed using western blot. ALA suppressed the mRNA expression of Nox4, p22phox, TNF-α and KIM-1. Active caspase-3 protein expression induced by IS was decreased by ALA. ALA also suppressed p38 MAPK and activated the PI3K/Akt signaling pathway following IS administration. We found that ALA may be an effective strategy for preventing oxidative acute kidney injury caused by IS. © 2020 The Biological Stain Commission.

Nephrotoxicity following colistin administration is common and factors alleviating nephrotoxicity are yet to be determined. We retrospectively evaluated outcomes of subjects who were treated with colistin (n = 133) and with antibiotics other than colistin (control, n = 133) in intensive care units. Acute kidney injury (AKI) occurred in 69.2% and 29.3% of patients in colistin and control groups, respectively (p < 0.001). In the colistin group, glucocorticoid exposure was more common in subjects who did not develop AKI (p < 0.001). This was not the case in the control group. In the colistin cohort, older age (per 10 years, odds ratio [OR] 1.41, 95% CI 1.05-1.91; p = 0.025), PPI use (OR 3.30, 95% CI 1.18-9.23; p = 0.023) and furosemide treatment (OR 2.66, 95% CI 1.01-6.98; p = 0.047) were independently associated with the development of AKI while glucocorticoid treatment (OR 0.23, 95% CI 0.10-0.53; p = 0.001) was independently associated with reduced risk of AKI. Mortality was observed in 74 patients in the colistin cohort (55.6%). A higher APACHE-II score (OR 1.17, 95% CI 1.08-1.26; p < 0.001) was independently associated with mortality while a higher serum albumin level (per 1 g/dL increase, OR 0.20, 95% CI 0.070-0.60; p = 0.004) was associated with a lower risk of mortality. In conclusion, glucocorticoid exposure is associated with a lower risk of AKI caused by colistin therapy in critically-ill patients. Prospective studies are needed to confirm these findings and determine the optimal type, dose and duration of glucocorticoid therapy.

Halk Sağlığı Yönetim Sistemi (HSYS) üzerinden veriler taranmış ve SPSS 21.0’a aktarılmıştır.Beden Kütle İndeks’leri (BKİ) hesaplanarak obezite sınıflandırmaları yapılmıştır. Data were scanned through the Public Health Management System (HSYS) andtransferred to SPSS 21.0. Obesity classifications were made by calculating Body Mass Index (BMI). Written permission from KayseriProvincial Health Directorate and ethical approval from Erciyes University Non-Clinical Researches Ethics Committee were obtainedfor the study.Results: In 2018, 5462 people over the age of 18 applied to dietician outpatient clinics in 5 healthy life centers in Kayseri. It was observedthat 58.4% of the applicants had over 30 BMI. 53.2% of those who come at least once are stage 1 obese. 52.7% of those who come at leasttwice are stage 1 obese. While the median BMI of obese individuals who came for the second time was 34.7 at the first visit, their BMImedian at the second visit was found to be 33.1. When the medians are compared with the Wilcoxon test, the differences are statisticallysignificant.Conclusion: In our study, it was determined that diet polyclinics that serve in health centers are effective in losing weight. Gyms withinhealthy life centers; The creation of physiologists and sports coaches will contribute to the fight against obesity.

Objective: The goal of this study was to identify risk factors among donor characteristics that affect delayed graft function (DGF) in paired kidney transplants.

We aimed to investigate the protective effect of alpha lipoic acid (ALA), a powerful antioxidant, against oxidative kidney damage induced by iron overload in rats. Male Wistar albino rats were separated into groups: control (n = 7), ALA (100 mg/kg (n = 7), iron sucrose (IS) (40 mg/kg) (n = 7), and IS + ALA (40 mg/kg IS administration followed by 100 mg/kg ALA) (n = 7). IS and ALA were injected weekly for 4 weeks via the tail vein. Blood and kidneys were collected at sacrification on day 29. Serum creatinine and iron levels were analyzed. Tubular injury and iron deposits were evaluated histopathologically. Additionally, iron, malondialdehyde (MDA), superoxide dismutase (SOD), catalase, and glutathione (GSH) levels and mRNA expressions of the subunits of NADPH oxidase, known as NOX4 and p22(phox), tumor necrosis factor (TNF)-alpha, kidney injury molecule-1 (KIM-1), and also p38 MAPK signaling in the kidneys, were evaluated biochemically. In the IS group, serum creatinine and iron level, tubular dilation, and loss of brush border in the kidneys were significantly higher than those of the control. Although those changes were reduced by ALA, the differences were not statistically significant. However, ALA reduced significantly MDA level and increased SOD activity in the kidney during IS administration. ALA also significantly reduced mRNA expressions of NOX4 and p22(phox) induced by IS, which was parallel to significant decreases of TNF-alpha and KIM-1 mRNA expressions. Moreover, ALA could suppress the activation of p38 MAPK during IS administration. In conclusion, ALA may be an effective strategy to attenuate in IS-induced oxidative kidney injury.

Now-a-days, sustainability is one of the most important goals of also the construction sector, as it is in most of the other sectors. However, sustainability in the construction sector is dealt mostly with its environmental and economic dimensions and its social dimension remains in the background. This situation causes the Occupational Health and Safety (OHS), which is perhaps the most important issue within the scope of social sustainability, to be addressed on its own, and its relationship with sustainability to be not introduced clearly. In this study, firstly, the relationship between social sustainability and OHS in the construction sector was discussed. Based on the fact that in construction projects the sustainability goal should be revealed forward to a great extent at the design stage, how to contribute to OHS by budgeting OHS activities together with project activities was explained. The study is expected to help construction sector stakeholders to understand the relationship between social sustainability and OHS, and to provide a clear picture of the role of budgeting in this respect.

Urban renewal practices could improve urban social sustainability with well-designed and well-constructed built environments. This study shows the development of a model, which explains the relation between built environment design and social sustainability in urban renewal. To do so, firstly, a comprehensive literature review was conducted, after which a pilot study was administered. This led to the identification of 32 built environment design elements. Their contributions to social sustainability were evaluated utilizing a survey among 323 people who previously worked in urban renewal projects. The design elements were grouped by exploratory factor analysis under five factors, and these factors were named as, accessibility and quality of social life, conservation of resources, quality of the built environment, protection of disadvantaged groups, and commercial and economic opportunities. Then, the analytic hierarchy process (AHP) was carried out with 60 academics and practitioners to identify the weights of the factors. Accessibility and quality of social life factor reached the highest weight with 33 %, while the other four factors shared close weights. The model obtained as a result of this study will increase the awareness of urban renewal stakeholders about social sustainability, by demonstrating clearly which design elements should be considered to improve the social sustainability of urban renewal projects. © 2020 Elsevier Ltd, 1505F505, This study is supported by the Anadolu University Scientific Research Committee (Project No. 1505F505).

BackgroundGlomerular immunoglobulin G deposition in patients with immunoglobulin A nephropathy (IgAN) has been shown to be associated with adverse renal outcomes. Clinical significance of mesangial immunoglobulin M (IgM) deposition in these patients remains to be proven.MethodsOne hundred patients who had a diagnosis of IgAN between 2001 and 2017 were enrolled. Patients were divided into two groups based on mesangial IgM deposition status. Groups were compared for demographic, clinical, and pathologic variables at baseline and in follow-up. Cox regression analysis was performed to evaluate the effect of mesangial IgM positivity on renal survival.ResultsIgM-positive group included 51% of participants. Baseline demographic and clinical parameters were not significantly different between groups. Mesangial IgM deposition was significantly associated with a higher segmental sclerosis score (p=0.008). At last visit, median serum creatinine was higher (p=0.021) and eGFR was lower (p=0.006) in IgM-positive group. Nineteen (19%) of all patients reached the combined primary outcome which includes doubling in serum creatinine or evolution to ESRD. Cumulative renal survival was lower (p=0.001) and resistant disease was more frequent in IgM-positive group (p=0.026). Intracranial hemorrhage due to vitamin K deficiency is a serious disease that can lead to morbidity, mortality, and mental retardation. Our goal in this study is to determine the frequency of VKORC1-1639 G>A polymorphism in patients who have undergone intracranial hemorrhage due to vitamin K deficiency bleeding (VKDB). To study VKORC1-1639 G>A polymorphism, blood was drawn from patients (n = 51, age 8:0 ± 6:5 years) followed at the Pediatric Neurology and Hematology section, Faculty of Medicine, Erciyes University, between 1990 and 2016, diagnosed with VKDB as idiopathic or from patients diagnosed with intracranial hemorrhage due to secondary vitamin K deficiency and also from volunteers (n = 51, age 11 ± 4.5 years). Intensive care and nutrition needs of patients and the laboratory radiological imaging results and treatments that were applied were analyzed through scanning the files of the patients and information received from families. Through detailed physical examination, patients with neurologic sequelae and ongoing epilepsy were determined. The results were compared to clinical and laboratory results with control group. Eight (15.7%) of the patients were normal, 29 (56.9%) heterozygous carrier, and 14 (27.5%) homozygous mutants. In the control group, 19 (37.3%) were normal, 19 (37.3%) heterozygous carriers, and 13 (25.5%) homozygous mutants. The VKOR1-1639>A (SNP:rs9923231) mutant positivity (homozygous plus heterozygous mutant) was significantly higher in the patient group when compared to controls. There were no significant differences between patient and control groups in terms of the prognosis.

Introduction: The aim of this study is to evaluate the satisfaction of the people about family medicine process which adopted 5 years ago in Turkey. Methods: This descriptive study is conducted via a structured face to face interview on 854 people who admitted to outpatient departments of Erciyes University Hospitals in the January-February 2015. Sample size was calculated 864 person with count if dissatisfaction rate was 40%. Satisfaction answers was self-reported with likert type questionnaire. Results: The mean age of the study group is 41.54± 5.33 and the vast majority of them were 25-49 years of age. Of the entire group 11.9% of them stated that they never consult the family physician before. The main reason for admission to family physician is to print prescription (58.4%). The rate of recommend their family physicians to others is 59.0%. The most frequently cited reasons for recommendation are interest of family physicians. The rate of those that previously applied to family physicians for the same complaint is low with 24.6%. 87.8% of people stated that they do not choose their family physicians. Of the research group 71.5% of the participants stated that they are satisfied with the services received from the family physician. When we examined satisfaction of family physicians by gender, there is no statistical relationship. Although there is no statistically significant difference between age groups, satisfaction rate is increasing with age. The highest satisfaction level is over 65 age group with 83.7%. Satisfaction rate of individuals from family physicians is decreasing as education level increases. Satisfaction of family physicians was significantly high in Individuals with chronic disease than those without chronic diseases. The mean number of application to the family physician in the past year is 4.32 ± 3.86. This application number is significantly high in women (4.59±4.11) than in men (4.01±3.51). The mean number of application increases with age, inversely decreases with the level of education. Conclusion: In our study we concluded that the satisfaction rate from the family medicine system has increased over time, but the applicant rate is still not at the desired level. The most reason that people use family medicine practice is repeat receipt which is we think need to intervention for this. In this study, the contributions of built environment design elements to environmental sustainability were evaluated through a questionnaire survey with the participation of 323 people who previously worked in urban renewal projects. Implementing factor analysis, interrelated elements have been gathered under five factors: transportation and accessibility', conservation of natural resources', built environment quality', supporting social life' and high-density usage'. To identify the importance weights of these factors, analytic hierarchy process (AHP) analysis was carried out with 40 academics and practitioners. Based on the results, conservation of natural resources' was found as the most important factor affecting the environmental sustainability of urban renewal projects. Dysregulated expression of matrix metalloproteinases (MMPs) is closely associated with the pathogenesis of renal ischemia/reperfusion injury (I/R). The production of excessive reactive oxygen species (ROS) causes tissue damage. Increased ROS production causes activation of p38 mitogen-activated protein kinase (MAPK) signaling, which participates in gene regulation of MMPs, especially MMP-2 and MMP-9 (gelatinases). Taurine (2-aminoethanesulfonic acid) in mammalian cells functions in bile acid conjugation, maintenance of calcium homeostasis, osmoregulation, membrane stabilization, and antioxidation, antiinflammatory, and antiapoptotic action. We investigated the effects of taurine and the possible role of p38 MAPK signaling on regulation of MMP-2 and MMP-9 in a renal I/R injury model in rats. Rats were divided into three groups: sham, I/R, and I/R + taurine treated. After a right nephrectomy, I/R was induced by clamping the left renal pedicle for 1 h followed by 6 h reperfusion. Taurine was administered 45 min prior to induction of ischemia. Renal function was assessed by serum creatinine and blood urea nitrogen (BUN) levels. Tubule injury and structural changes were evaluated by light microscopy. Malondialdehyde (MDA) levels were analyzed by high performance liquid chromatography (HPLC). Superoxide dismutase (SOD) activity levels were measured using a colorimetric kit. mRNA expression of MMP-2 and MMP-9 was determined by real-time polymerase chain reaction. MMP-2 and MMP-9 activities were measured using a fluorimetric kit. Phosphorylated p38 (p-p38) and total p38 MAPK protein expressions were evaluated by western blot. Taurine pretreatment significantly attenuated renal dysfunction and histologic damage, such as renal tubule dilation and loss of brush borders. The pretreatment also decreased the MDA level and attenuated the reduction of SOD activity in the kidney during I/R. Taurine pretreatment also decreased significantly both MMP-2 and MMP-9 mRNA expression and MMP-9 activity induced by I/R. In addition, the activity of p38 MAPK signaling was down-regulated significantly by taurine administration. Inhibition of MMP-2 and MMP-9 expression and MMP-9 activity caused by taurine may be associated with suppression of p38 MAPK activation during I/R induced renal injury in rats. Therefore, taurine administration may prove to be a strategy for attenuating renal I/R injury. © 2017 The Biological Stain Commission.

OBJECTIVE: The aim of the study was to retrospectively investigate and evaluate clinical findings and influencing factors related with graft and patient survival in kidney transplant recipients at Dokuz Eylul University Hospital.

Objectives: Holmium:Yttrium Aluminum Garnet laser lithotripsy is used in Retrograde Intrarenal Surgery. Fragmentation is made with a certain value of pulse energy (Joule) and frequency (Hertz) in Holmium laser lithotripsy and the multiplication of these values gives us total power (Watt). Devices with maximum power of 20 Watt and 30 Watt are used in clinical practice. We want to compare the efficiency, safety and pain scores of the lithotripsy made below 20 Watt and over 30 Watt with 30 Watt laser device. Materials and methods: 60 patients who had 2-3 cm sized kidney stones and operation planned were prospectively divided into three groups. Groups were random identified. In the first group, fragmentation was performed below 20 Watt power with 20 Watt laser device. In the second group, fragmentation was performed below 20 Watt power with 30 Watt laser device. In the third group, fragmentation was performed over 20 Watt power with 30 Watt laser device. Demographic, stone, intraoperative and postoperative data were recorded. We compared these groups regarding efficiency, safety and pain score.Results: For demographic and stone data, there was a statistically significant difference only for stone number. For intraoperative and postoperative data, there was a statistically significant difference only for ureteral access sheath usage between the groups. Success was lower than the other groups in Group 1.Conclusions: Success was higher in groups using 30 Watt laser device. There was not statistically significantly difference between complications and pain. 30 Watt laser device is safe and efficient in Retrograde Intrarenal Surgery.
