Amaç: Bu çalışmada dil ve konuşma terapisi öğrencilerinin yatak başı yutma değerlendirmesi bilgilerinin incelenmesi amaçlanmıştır. Yöntem: Araştırma, genel tarama modeli ve 151 katılımcı ile gerçekleştirilmiştir. Bu kapsamda araştırmacılar üç bölümden oluşan bir anket geliştirmişlerdir. Bulgular: Anketin birinci bölümünde çalışmaya katılan öğrencilerin %42.4’ü (n=64) yutma terapisi eğitimlerinin yeterli olduğunu ve ders kapsamında %88.7’si (n=134) yatak başı yutma değerlendirmesine yönelik eğitim aldığını ifade etmişlerdir. Katılımcıların %45’i (n=68) bu eğitimin yeterli olduğunu ve %88.08’si (n=133) ders kapsamında yatak başı yutma değerlendirmesi ile ilgili formlara yönelik bilgi sahibi olduğunu bildirmişlerdir. Anketin ikinci bölümünde ise lisans eğitimlerinde öğrendikleri yatak başı yutma değerlendirmesi testlerini seçmeleri istenmiştir. Bu formlar incelendiğinde, %70.9 (n=107) 3 Oz Yutma Testi, %62.9’u (n=95) Standardize Yutma Değerlendirme Ölçeği ve %62.25’i (n=94) Gugging Swallowing Screen testi olduğu görülmüştür. Anketin üçüncü bölümünde ise katılımcıların %50’sinden fazlasının sorulara doğru yanıt verdiği görülmüştür. Yalnızca yatak başı yutma değerlendirmesinin işlevine yönelik düzenlenen 3. sorunun katılımcıların %39.7’si (n=60) tarafından doğru yanıt verdiği görülmüştür. Sonuç: Sonuç olarak yatak başı yutma değerlendirmesi ile ilgili bilgilerin lisans dönemimde sadece teorik olarak değil, pratik olarak sunulmasının önemini yansıttığı düşünülmektedir.

Amaç: Yatak başı yutma değerlendirmesi (YBYD) yutma terapisinin doğru gerçekleştirilebilmesi için önem taşımaktadır. Bu çalışmada dil ve konuşma terapistlerinin (DKT) YBYD’ne yönelik uygulamaları ve bilgilerinin incelenmesi amaçlanmaktadır. Örneklem ve Yöntem: Genel tarama modeli ile yürütülen bu çalışmada, araştırmacılar tarafından geliştirilen “Dil ve Konuşma Terapistlerinin Yatak Başı Yutma Değerlendirilmesine Yönelik Bilgi Düzeyini Belirleme Anketi” kullanılmıştır. Araştırmaya 86 DKT katılmıştır. Anket üç bölümden oluşmaktadır. İlk bölüm DKT’lerin demografik bilgilerini, ikinci bölümde aktif olarak yutma terapisi gerçekleştiren terapistlerin YBYD uygulamalarını ve son bölümde ise 10 soruluk bilgi anketi içermektedir. Bulgular: Çalışmaya katılan DKT’lerin %90.7’si (n=78) eğitim süreçlerinde YBYD’ye yönelik eğitim aldığı ancak %61.6’sı (n=53) bu eğitimin yetersiz olduğunu düşünmektedirler. Katılımcıların sadece %12.8’nin (n=11) yutma terapisi yaptığı görülmüştür. Yutma terapisi yapan terapistlerin YBYD uygulamalarında farklılıklar olduğu kadar benzerlikler olduğu da görülmüştür. Genel gözlem, oral-motor muayene ve bilişsel farkındalık değerlendirmesinin tüm terapistler tarafından YBYD sırasında uygulanmaktadır. Bilgi anketini içeren bölümde, besin alımı kararı bilgisini içeren 5.soruya (%16.3 n=14) ve YBYD’ni uygulayan meslek grubu bilgisini içeren 8.soruya (%48.8 n=42) doğru yanıtların az olduğu görülmüştür. Sonuç: Ülkemizde DKT mesleğinin yakın geçmişe dayanmasının yutma terapisti ile ilgilenen terapist sayısını etkilediği düşünülmektedir. İkinci bölümü dolduran terapistlerin YBYD uygulamalarının, alanyazın ile uyumluluk gösterdiği ve gelen hastaya göre uygulamalarını modifiye edebildikleri görülmüştür. Yutma terapisi gerçekleştiren DKT sayısının az olmasına karşın katılımcıların üçüncü bölüme verdikleri yanıtların çoğunlukla doğru olduğu görülmüştür. Terapistlerin yetersiz hissetmelerindeki sebebin aktif uygulama yapmamaktan kaynaklandığı düşünülmektedir. Araştırma kapsamında elde edilen tüm bulgular alanyazın ile tartışılmış olup lisans ve yüksek lisans eğitim süreçlerinde YBYD ile ilişkili eğitim veren eğitimcilere bir yol göstereceği düşünülmektedir.

Dysphagia is one of the most common and important complications of stroke. It is an independent marker of poor outcome after acute stroke and may become chronic after the acute period and continues to affect all aspects of the patient's life. Patients with stroke may encounter any of the medical branches in the emergency room or outpatient clinic, and as in our country, there may not be specialists specific for dysphagia, such as speech-language pathologists (SLP), in every hospital. This study aimed to raise awareness and create a common opinion of medical specialists for stroke patients with dysphagia. This recommendation paper has been written by a multidisciplinary team and offers 45 recommendations for stroke patients with dysphagia. It was created using the eight-step Delphi round via e-mail. This study is mostly specific to Turkey. However, since it contains detailed recommendations from the perspective of various disciplines associated with stroke, this consensus-based recommendation paper is not only a useful guide to address clinical questions in practice for the clinical management of dysphagia in terms of management, diagnosis, and follow-up, but also includes detailed comments for these topics.

Currently, there is no comprehensive and multidisciplinary recommendation study covering all aspects of pediatric dysphagia (PD). This study aimed to generate PD management recommendations with methods that can be used in clinical practice to fill this gap in our country and in the world, from the perspective of experienced multidisciplinary experts.This recommendation paper was generated by a multidisciplinary team, using the seven-step process and a three-round modified Delphi survey via e-mail. First, ten open-ended questions were created, and then detailed recommendations including management, diagnosis, treatment, and follow-up were created with the answers from these questions. Each recommendation item was voted on by the experts as overall consensus (strong recommendation), approaching consensus (weak recommendation) and divergent consensus (not recommended).In the 1st Delphi round, a questionnaire of 414 items was prepared based on the experts' responses to ten open-ended questions. In the 2nd Delphi round, 59.2% of these items were accepted as pre-recommendation. In the 3rd Delphi round, 62.6% of 246 items were accepted for inclusion in the proposals. The final version recommendations consisted of 154 items.This study includes comprehensive and detailed answers for every problem that could be posed in clinical practice for the management of PD, and recommendations are for all pediatric patients with both oropharyngeal and esophageal dysphagia.

Objective. To treat androphonic voice quality after failed cricothyroid approximation (CTA) surgery. Study Design. Prospective case series. Methods. The voices of three male-to-female transsexuals, two constitutional androphonic females, and one patient with ambiguous genitalia assigned to the female phenotype were deemed to be unimproved after CTA. The patients underwent laser reduction glottoplasty (LRG) surgery tailored to each case between 2001 and 2005. Vocal outcomes were evaluated according to fundamental frequency (FF), satisfaction status, voice-related quality of life (V-RQOL) measures, and ratings of recorded voices by 10 blinded listeners after 12 months postoperatively. Results. The LRG provided an additional mean gain of 45.17 +/- 8.47 Hz (mean +/- standard deviation) in the FF, equal to 4.36 +/- 0.84 semitones. The mean FF increased significantly (P < 0.05) from 158.33 +/- 12.14 Hz preoperatively to 203.50 +/- 13.34 Hz postoperatively. All patients reported satisfactory female quality in their voices and greetings with female salutations in nonvisual communications. Their voice-related quality of life (V-RQOL) measures were high and significantly elevated (P < 0.05) after LRG. The masculine voice-quality ratings significantly shifted to feminine in the listeners' assessment. No major complications were observed during follow-up, with the exception of mild vocal fold edema that disappeared by 8 weeks postoperatively. Conclusions. LRG can be used in androphonic cases that fail to obtain a high-pitched feminine voice after CTA.

We investigated the relationship between muscle tension dysphonia (MTD) type 1 and soft phonation index (SPI) and evaluated the effect of voice therapy on MTD type 1.The study included 20 patients (all females; mean age 27+/-6 years) whose diagnosis was made as MTD type 1 on videolaryngostroboscopic examination. Acoustic analysis was performed before and after voice therapy using the Multi-Dimensional Voice Program. Acoustic parameters were compared with those of a control group comprising 20 females (mean age 28+/-4 years) without any voice problem.In the patient group, 19 patients exhibited a significantly increased SPI compared to two subjects in the control group (p0.05). The most significant improvements were seen in SPI, jitter, and PPQ following voice therapy, which was found to be effective in 19 patients.Our findings showed a close relationship between MTD type 1 and SPI. Voice therapy improves parameters of amplitude and frequency perturbations, in particular SPI.

Objective: To present the success rate of a less invasive modification of Isshiki type III anterior commissure relaxation laryngoplasty technique in patients with high-pitched voice disorders. Design: Prospective case series. Setting: KBB Major Private Clinic of Istanbul Surgery Hospital and the University of Yeditepe Hospital. Patients: Twenty-one adult patients who believed that their high-pitched voices conflicted with their body image and/or gender identity. Intervention: Type III thyroplasty for pitch alteration. Main Outcome Measures: Comparison of preoperative and postoperative (>6 months) fundamental frequency levels, diplophonia, perception of body image and pitch, and subjective ratings of comfort during vocalization. Results: The patients were mostly male (mean age, 30.5 years). The most frequent cause of high-pitched voice was sulcus vocalis (n=14), followed by constitutional causes (n=5), mutational falsetto (n=1), and severe glottic scarring secondary to childhood diphtheria (n=1). After surgery, the fundamental frequency dropped significantly from a mean of 213.81 Hz to 149.86 Hz (P
