INTRODUCTION: There are several studies about the mortality of the CABG patients with the importance of the inflammation. The aim of our study was to evaluate the relationship between RDW and in-hospital mortality in CABG patients. METHODS: A total of 346 patients undergoing coronary artery bypass grafting operation were analyzed for our study. Pre-operative 2 ml blood sample was put into tubes for evaluation all patients complete blood count. All patients underwent same anesthesia protocol. The type of CABG procedure (off-pump vs on-pump CABG) was determined by the operation team. RESULTS: There was ten patients died in-hospital period (Group 1) and three hundred and thirty-six patients lived during in hospital follow up (Group 2). WBC (10.8 ± 5.2 x 103 vs 7.9 ± 2.2 x 103 p: 0.04), Neutrophil (7.3 ± 3 x 103 vs 5.1 ± 2 x 103 p: 0.03), N/L Ratio (4.8 ± 2.6 % vs 2.7 ± 1.6 % p: 0.01) and RDW (14.8 ± 1.6 % vs 13.9 ± 6.6 %, p: 0.002) values were higher in group 1 than group 2, respectively. A logistic multivariate regression analysis showed that only RDW (OR 1.860, 95% CI 1.105-3.132; p: 0.02) was an independent predictor of the development of mortality in-hospital period in CABG patients. DISCUSSION AND CONCLUSION: To our knowledge, this is the first report of an association between RDW and the development postoperative mortality in CABG patients.

OBJECTIVE Heart failure (HF) has a high prevalence and mortality rate in elderly patients; however, there are few studies that have focused on patients older than 80 years. The aim of this study is to describe and compare the age-specific demographics and clinical features of Turkish elderly patients with HF who were admitted to cardiology clinics. METHODS The Epidemiology of Cardiovascular Disease in Elderly Turkish population (ELDER-TURK) study was conducted in 73 centers in Turkey, and it recruited a total of 5694 patients aged 65 years or older. In this study, the clinical profile of the patients who were aged 80 years or older and those between 65 and 79 years with HF were described and compared based on the ejection fraction (EF)-related classification: HFrEF and HFpEF (is considered as EF: ≥50%). RESULTS A total of 1098 patients (male, 47.5%; mean age, 83.5+-3.1 years) aged ≥80 years and 4596 patients (male, 50.2 %; mean age, 71.1+-4.31 years) aged 65-79 years were enrolled in this study. The prevalence of HF was 39.8% for patients who were ≥80 years and 27.1% for patients 65-79 years old. For patients aged ≥80 years with HF, the prevalence rate was 67% for hypertension (HT), 25.6% for diabetes mellitus (DM), 54.3% for coronary artery disease (CAD), and 42.3% for atrial fibrilation. The study aimed to examine the association between thoracic periaortic fat tissue volume and the long-term incidence of major adverse cardiovascular events.This retrospective cohort study included 433 consecutive patients (372 male and 61 female). Periaortic fat tissue volume was measured via electrocardiogram-gated 64-multidetector computed tomography. The patients were evaluated on an average 3 years of follow-up for major adverse cardiovascular events. The patients were divided into groups according to the presence of major adverse cardiovascular events.Major adverse cardiovascular events were noted in 44 (10.2 %) patients during follow-up. Periaortic fat tissue volume was significantly higher in the major adverse cardiovascular events (+) group (35.4 ± 26.1 cm(3) vs. 24.1 ± 14.9 cm(3), P = 0.001). The logistic regression model showed that periaortic fat tissue volume (hazard ratio: 1.03; 95 % CI: 1.01-1.05; P = 0.001), the glomerular filtration rate (hazard ratio: 0.98; 95 % CI: 0.96-0.99; P = 0.03), and male gender (hazard ratio: 4.76; 95 % CI: 1.08-20.90; P = 0.04) were independent predictors of major adverse cardiovascular events.Thoracic periaortic fat tissue volume may be considered a useful new parameter for predicting major adverse cardiovascular events.

Several methods have been described previously for the treatment of stent dislodgement. We herein report a case of balloon rupture during stenting that was not deployed properly. Incidence of post-infarction left ventricular (LV) thrombus is reduced by modern methods of revascularization and treatment with intensive antithrombotic agents. LV thrombus may be an important cause of morbidity and mortality due to relatively high risk of embolization. Therefore, the diagnosis and appropriate management is crucial. The paper introduces a successful treatment of massive, mobile LV thrombus originating from the apical aneurysm and extending to the left ventricular outflow tract.

Angiographic detection of critical stenosis of left main coronary artery (LMCA) dramatically alters the patient's treatment approach. Therefore it should be examined further. This image of the LMCA may be due to sharp angulation, catheter-induced spasm or the relative stenosis due to ectatic LMCA. Operators can be avoided unnecessary revascularization procedures with further investigation. We report the image of angiographically severe stenosis of the LMCA due to a sharp angulation.

Objective: Heart rate recovery (HHR) after exercise is a function of vagal reactivation. This study aimed to evaluate HHR index in patients with erectile dysfunction. Methods: Men over the age of 18 years who were diagnosed with erectile dysfunction were included in the study. Ninety patients with erectile dysfunction (mean age=56.1±8.3 years) and 50 healthy subjects as controls (mean age=53.1±10.4 years) were compared. The erectile status of patients was evaluated using the sexual health inventory for men questionnaire. Basal electrocardiography, echocardiography, and treadmill exercise testing were performed in all patients and controls. The HHR index was defined as the reduction in heart rate from the rate at peak exercise to the rate at the first minute (HRR1), second minute (HRR2), third minute (HRR3), and fifth minute (HRR5) after terminating exercise stress testing. An independent sample t-test, Pearson correlation coefficient test, linear multivariate regression analysis, and receiver operating characteristic curve analysis were used for statistical assessment. The electrocardiogram showed 1 mm ST segment elevation in leads DII, DIII, aVF derivations and also complete AV block. His general condition and hemodynamic parameters were suddenly deteriorated. His blood pressure and heart rate dropped to 60/40 mmHg and 40/min respectively. Isotonic solution infusion started 1000 cc/hour and Atropin 1 mg initiated intravenously. After that his cardiac rhythm changed to the ventricular fibrillation. Following cardiac defibrillation, immediately coronary angiography performed.

We investigated the relationship between epicardial fat volume (EFV) measured by multislice computed tomography (MDCT) and long-term major adverse cardiac events (MACEs). Consecutive patients (n = 564) were enrolled in this retrospective study. Patients were divided into tertiles according to EFV. Patients were followed up for an average of 18 months. Patients in each tertile were similar in terms of gender and risk factors. Patients with greater EFV in the third group were more likely to be overweight ( P = .001) and older ( P = .001). High-density lipoprotein cholesterol levels were relatively lower in the third tertile (45 ± 9, 45 ± 11, and 43 ± 9 mg/dL, respectively; P = .018). The third group had a significantly higher rate of myocardial infarction (0.6%, 1.1%, and 3.7%, respectively; P = .043). The incidence of MACEs during the follow-up period was highest in the third group 15.9% (4.1%, 7.7%, and 15.9%, respectively; P = .001). Epicardial fat volume measured by MDCT was associated with increased long-term cardiovascular risk.

Endothelial dysfunction plays a major role in erectile dysfunction (ED). Uric acid (UA) is a marker of endothelial dysfunction. We hypothesized that increased UA levels may be associated with ED and aimed to investigate whether there is a relationship between, UA and ED in hypertensive patients.A total of 200 hypertensive patients who have a normal treadmill exercise test were divided into two groups based on the Sexual Health Inventory for Men (SHIM) test (21 defined as ED n = 110, and ≥ 21 defined as normal erectile function n = 90). The differences between the ED and normal erectile function groups were compared and determinants of ED were analyzed.The prevalence of ED was found to be 55.0%. Office blood pressure level was comparable between groups. UA levels were significantly increased in the ED group (6.20 ± 1.56 vs 5.44 ± 1.32, p = 0.01). In a regression model, age [odds ratio (95% confidence interval): 1.08 (1.04-1.14), p = 0.001], smoking [odds ratio: 2.33 (1.04-5.20), p = 0.04] and UA [odds ratio: 1.76 (1.28-2.41), p = 0.04] were independent determinants of ED. An UA level of5.2 mg/dl had 76.2% sensitivity, 43.7% specificity, 62.9% positive and 59.4% negative predictive value for determining ED.UA is an independent determinant of ED irrespective of blood pressure control and questioning erectile function for hypertensive patients with increased UA levels may be recommended.

Objective: The pre-procedural neutrophil to lymphocyte ratio (N/L) is associated with adverse outcomes among patients with coronary artery disease but its prognostic value in ST-segment elevation myocardial infarction (STEMI) has not been fully investigated. This study evaluated the relations between pre-procedural N/L ratio and the in-hospital and long-term outcomes in STEMI patients undergoing primary percutaneous coronary intervention (PCI)., Objective: The pre-procedural neutrophil to lymphocyte ratio (N/L) is associated with adverse outcomes among patients with coronary artery disease but its prognostic value in ST-segment elevation myocardial infarction (STEMI) has not been fully investigated. This study evaluated the relations between pre-procedural N/L ratio and the in-hospital and long-termoutcomes in STEMI patients undergoing primary percutaneous coronary intervention (PCI). Methods: A total of 682 STEMI patients presentedwithin the first 6 h of symptom onsetwere enrolled and stratified according to tertiles of N/L ratio based on the blood samples obtained in the emergency room upon admission. Results: The mean follow-up period was 43.3 months (1–131 months). In-hospital in-stent thrombosis, non-fatal myocardial infarction, and cardiovascular mortality increased as the N/L tertile ratio increased (p b0.001, pb0.001, p=0.003, respectively). Long-term in-stent thrombosis, non-fatalmyocardial infarction and cardiovascular mortality also increased as the N/L ratio increased (p b0.001, pb0.001, p=0.002, respectively). On multivariate analysis, N/L ratio remained an independent predictor for both in-hospital (OR 1.189, 95% CI 1.000 –1.339; pb0.001) and long-term major (OR 1.228, 95% CI 1.136 –1.328; pb0.001) adverse cardiac events. Conclusion: The N/L ratio was an independent predictor of both in-hospital and long-term adverse outcomes among STEMI patients undergoing primary PCI. Our findings suggest that this inexpensive, universally available hematological marker may be incorporated into the current established risk assessment model for STEMI.

We studied 92 patients to determine the relationship between the QRS treadmill score (QTS) and extent of coronary artery disease. The results were compared to those of ST segment deviation in a(+)QRS scorepopulation. A cut off point of5 mm was used for mild to moderate risk patients whereas a QTS of3 was considered high risk for the patients. 43% of the patients had high risk QTS and 57% mild to moderate QTS. A highly significant correlation was observed between the extent of coronary artery disease and the high risk group of patients, whereas nonsignificant changes were observed in the control group (p0.001). Finally, the results indicate that in patients with a (+) QTS score (5 mm), coronary artery disease was significant and accordingly extensive during coronary angiography. On the other hand, a (+) QRS score proved to be superior to ST deviation in predicting critical coronary stenoses in the same population (F ratio; 41 vs 24).
