4. The bimodality of South Asia High and its relationship with Asian rainfall in September.

This study examines daily longitudinal locations of South Asia High (SAH) in September and its connection with Asian September rainfall variation. Results suggest that daily longitudinal locations of September SAH exhibits as bimodality, which is featured by two preferable locations with one high‐frequency location lying over southern Tibetan Plateau and the other over southern Iranian Plateau. Referred to the two preferred locations, the SAH is classified into east and west SAH mode. The two modes may be attributed to a warm column over central eastern Tibetan or eastern Iranian Plateau, which warms the above troposphere and maintains a SAH at upper troposphere over corresponding location. In addition, bimodality of the SAH has a close relationship with Asian September rainfall. The east mode of SAH is associated with an increasing rainfall over western China and decreasing rainfall over northern Indian Peninsula, southwestern Tibetan Plateau, Indochina Peninsula, eastern China and Korea Peninsula, while the west mode of SAH is linked with the reverse pattern. Specifically, the atmospheric circulation at upper troposphere is featured by one large anticyclonic circulation above eastern Tibetan Plateau and two cyclonic circulations over northern Iranian Plateau and eastern Korea. In the lower troposphere, southerly and northerly anomalies form a convergence zone over western China. Meantime, a cyclonic moisture circulation with warm and wet southerly vapour flow results in above‐normal rainfall over western China. An anticyclone over northern Iranian Plateau at lower troposphere results in an anticyclonic moisture divergence and deficient rainfall over southwestern Tibetan Plateau and northern India. Moreover, a cyclonic circulation over eastern Korea Peninsula with marked northwesterly and moisture flux divergence results in below‐normal rainfall over eastern China and Korea Peninsula. A study on the effect of tip pruning of shoots at two pruning levels (10 cm and 20 cm) with and without paclobutrazol application on the yield parameters of mango hybrid Ratna was conducted in the mango orchard attached to the Department of Fruit Science, College of Agriculture, Vellanikkara during 2018- 2019. Seventeen treatments (T1-T8 consisted of pruning at 10 cm and 20 cm length from the tip of the shoot during June, July, August and September, T9-T16 consisted of pruning at 10 cm and 20 cm length from the tip of the shoot along with paclobutrazol soil drench during June, July, August and September months and T17 as the control) with two replications were carried out following completely randomised block design (CRD). Pruning all the shoots of the tree to a length of 20 cm from the shoot tip along with paclobutrazol soil drench during June month was observed to produce maximum number of fruits per tree and maximum fruit yield (kg/tree) in Ratna. The individual fruit weight was maximum in the trees pruned at 20 cm length during September month drenched with paclobutrazol. [ABSTRACT FROM AUTHOR]

Purpose – The purpose of this paper is to examine evolution of the American strategy toward terrorism in the aftermath of the events of September 11, 2001. In other words, this study revolves around a key question: How and why the American security strategy toward terrorism evolved in the aftermath of the September 11 attacks in 2001? Based on the neorealist approach in international relations, this paper attempted to answer that question: first, through defining the concept of terrorism and how the Americans perceive it; second, via pinpointing the characteristic of the American counter-terrorism strategy before September 11 attacks; and third, through examining the effects of those terrorist attacks on that strategy. Design/methodology/approach – The nature of the subject of this study calls for reliance on the analytical descriptive approach to highlight the role and strategy of the USA in the fight against terrorism following the events of September 11, in addition to the use of the system analysis methodology, which can identify the inputs and outputs of the system that had an impact in formulating the US counter-terrorism strategy. Findings – This study has come up with seven findings. The first finding was that the 9/11 attacks served as a turning point of the US counter-terrorism strategy and restructured its agenda. Confronting the communist threat had been its primary objective, until terrorism came to the fore and became its first and foremost priority. The USA vowed that terrorism is its enemy and waged the war on terror to thwart its risk as a global threat. The second finding revolves around the idea of double standards in the American foreign policy. True to its long-standing tradition of favoring its own interests, in complete disregard of the interests of any other party, the USA continued to uphold the double-standards policy. Originality/value – This study adds a new study to the Arab Library in the field of counter-terrorism studies, national security strategies and American foreign policy. In addition, the researcher seeks to complete the scientific effort to study the US strategy against terrorism, with a clear impact on the development of the situation in the region. This study contributes to the study of how one of the great powers in the international system, the USA, deals with the terrorist organizations that have become widespread in the Arab region.

Exam season is a stressful period for university students, and the possibility of having to take resits in September only adds more pressure. In some universities, this resit period has been eliminated, and instead, the creation of an extraordinary resit period without September has been proposed, which allows students to have a second chance in a shorter timeframe. This change has been the subject of debate among education experts and has sparked interest among researchers to analyze the potential effects on students' academic outcomes. In this article, a comparative study is presented between academic results with the change from September resits to the new extraordinary resit periods in February for the first semester and July for the second semester at the University of Almería from the implementation of the agricultural technical engineer degree in the 2010 academic year until the 2021/22 academic year. Additionally, the academic results of the COVID-19 academic year have been analyzed due to the uniqueness of the online teaching and examination model. As a final conclusion of this study, it can be affirmed that the change in the extraordinary resit period, the elimination of September, has not had a negative impact on the pass rates in the extraordinary resits in the new scenario. The results of this study provide valuable information for educators and policymakers as they offer data to determine whether this new modality of extraordinary resit periods is a beneficial option for students and universities. Furthermore, the study could also contribute to the ongoing debate on best practices in learning assessment and fostering students' academic success., La época de exámenes es un periodo estresante para los estudiantes universitarios, y la posibilidad de tener que presentarse a convocatorias extraordinarias en septiembre solo añade más presión. En algunas universidades, se ha eliminado esta convocatoria, en su lugar, se ha propuesto la creación de una convocatoria extraordinaria sin septiembre, que permite a los estudiantes tener una segunda oportunidad en un plazo de tiempo más corto. Este cambio ha sido objeto de debate entre los expertos en educación, y ha despertado el interés de los investigadores para analizar los posibles efectos en los resultados académicos de los estudiantes. En este artículo, se presenta un estudio comparativo entre los resultados académicos con el cambio de las convocatorias extraordinarias de septiembre a las nuevas convocatorias extraordinarias, febrero para las del primer cuatrimestre y julio para las del segundo cuatrimestre, en la universidad de Almería desde la implantación de la titulación de ingeniero técnico agrícola en el curso 2010 hasta el curso 2021/22. Adicionalmente se han analizado los resultados académicos del curso del Covid por la singularidad del modelo de enseñanza y de exámenes on-line. Como conclusión final de este estudio podemos afirmar que el cambio en la convocatoria extraordinaria, la eliminación de septiembre, no ha tenido un impacto negativo en los resultados de aprobados en las convocatorias extraordinarias en el nuevo escenario. Los resultados de este estudio proporcionan una información valiosa para los educadores y formuladores de políticas educativas, ya que ofrecen datos para determinar si esta nueva modalidad de convocatorias extraordinarias es una opción beneficiosa para los estudiantes y las universidades. Además, el estudio también podría contribuir al debate en curso sobre las mejores prácticas en la evaluación del aprendizaje y el fomento del éxito académico de los estudiantes.

Federal Reserve Chairman Jerome Powell is facing political risks as the election approaches, particularly in relation to the September interest-rate cut that the Fed is expected to make. While inflation is finally approaching the central bank's target, there are signs that the economy is softening. The challenge for the Fed is to translate these observations into specific policy decisions, which is complicated by the need to balance predicted future inflation against predicted future economic growth. Powell's previous reliance on faulty economic models and his politicization of monetary policy have created challenges for him in explaining the basis for the Fed's judgment on interest rates. There is political pressure from both sides, with President Donald Trump warning against rate cuts before the election and Democrats expecting a market tumble if the Fed fails to deliver. Summary Our objective is to demonstrate the importance of considering microcalcifications even without evidence of nodules as a potential sign of malignancy. Current guidelines, such as those of the British Thyroid Association, acknowledge the clinical significance of microcalcifications only when found within nodules. In this case, they are considered a suspicious feature, classifying the nodules as U5 (i.e. high risk) where fine-needle aspiration biopsy (FNAB) is warranted, following the high likelihood of cancer in these nodules. In addition, there is a dearth of evidence of ultrasound scan (USS) detection of microcalcifications in the thyroid gland outside of nodules, along with their associated clinical implications. Yet, this clinical manifestation is not so infrequent considering that we do encounter patients in the clinic showing these findings upon ultrasound examination. Three patients who presented to our clinic with thyroid-related symptoms were shown to have areas of microcalcifications without a nodule upon sonographic evaluation of their thyroid gland. These incidentally detected hyperechoic foci were later confirmed to correspond to areas of papillary thyroid carcinoma (PTC) on histopathological examination of resected tissue following thyroidectomy. Four more cases were identified with sonographic evidence of microcalcifications without nodules and given their clinical and other sonographic characteristics were managed with active surveillance instead. Learning points Echogenic foci known as microcalcifications may be visible without apparent association to nodular structures. Microcalcifications without nodules may not be an infrequent finding. Microcalcifications are frequently indicative of malignancy within the thyroid gland even without a clearly delineated nodule. Empirically, the usual guidelines for the management of thyroid nodules can be applied to the management of microcalcifications not confined to a nodule, but such a finding per se should be classified as a ‘high-risk’ sign.

Summary A 33-year-old gentleman of Egyptian heritage presented with a 21 years history of unexplained and recurrent hypercalcaemia, nephrolithiasis, nephrocalcinosis, and myocarditis. A similar history was also found in two first-degree relatives. Further investigation into the vitamin D metabolism pathway identified the biochemical hallmarks of infantile hypercalcaemia type 1 (IIH). A homozygous, likely pathogenic, variant in CYP24A1 was found on molecular genetic analysis confirming the diagnosis. Management now focuses on removing excess vitamin D from the metabolic pathway as well as reducing calcium intake to achieve serum-adjusted calcium to the middle of the reference range. If undiagnosed, IIH can cause serious renal complications and metabolic bone disease. Learning points Infantile hypercalcaemia type 1 (IIH) is an autosomal recessive disorder characterised by homozygous mutations in the CYP24A1 gene that encodes the 24-hydroxylase enzyme used to convert active vitamin D metabolites such as 1,25-(OH)2-vitamin D into their inactive form. IIH should be questioned in individuals presenting with a history of unexplained hypercalcaemia, especially if presenting from childhood and/or where there is an accompanying family history of the same in first and/or second degree relatives, causing complications such as nephrocalcinosis, pericarditis, and calcium-based nephrolithiasis. Associated biochemistry of IIH is persistent mild to moderate hypercalcaemia, normal or raised 25-(OH)-vitamin D and elevated 1,25-(OH)2-vitamin D. An elevated ratio of 25-(OH)-vitamin D to 24,25-(OH)2-vitamin D can be a useful marker of defects in the 24-hydroxylase enzyme, whose measurement can be facilitated through the supra-regional assay service. Management should focus on limiting the amount of vitamin D introduced into the body either via sunlight exposure or supplementation in addition to calcium dietary restriction to try and maintain appropriate calcium homeostasis

Summary A 38-year-old female was initially seen in the intensive care unit after severe postpartum hemorrhage. She was initially diagnosed to have Sheehan’s syndrome and after discharge, she was diagnosed to have a vesicovaginal fistula which initially caused a delay in diagnosing diabetes insipidus in the patient because she was having urinary incontinence. The patient had the vesicovaginal fistula repaired and was on replacement with levothyroxine, prednisone, and desmopressin. Years after her diagnosis, the patient experienced recurrent episodes of hyponatremia in the setting of desmopressin therapy. This case highlights the challenges of diagnosing diabetes insipidus in a patient with Sheehan’s syndrome and a vesicovaginal fistula, as well as the long-term management of central diabetes insipidus in a resource-limited setting. Learning points Sheehan’s syndrome is rarely associated with diabetes insipidus, and in our patient, it was initially missed due to a vesicovaginal fistula which caused urinary incontinence. Water intoxication is more common in young children and older adults but can occur years after initiating treatment with desmopressin in adults and should be kept in mind when treating patients with hyponatremia who have hypopituitarism associated with diabetes insipidus. Water intoxication is much more common in patients with diabetes insipidus being treated with intranasal desmopressin than in those using the oral preparations.

The study analyzes changes in September precipitation in the Mediterranean region and their possible causes. The research period is 1950-2014. The main finding is that the reduction in aerosol pollution over Europe in the late twentieth century has led to an upward shift of air temperatures in the region, which in turn has reduced the meridional temperature gradient, leading to weakening and shift to the north of the Azores High (the north end of Hadley circulation). This northward shift placed the Mediterranean region in an area with decreasing SLP, which results in an increase in the number or intensity of cyclones, increase in cloudiness and precipitation and a decrease in air temperatures. In the period 1995-2014 the region (especially its eastern part) lies within the boundaries of Inter Tropical Convergence Zone in September. [ABSTRACT FROM AUTHOR]

In two Australian coming-of-age feature films, Australian Rules and September, the central young characters hold idyllic notions about friendship and equality that prove to be the keys to transformative on-screen behaviours. Intimate intersubjectivity, deployed in the close relationships between the indigenous and nonindigenous protagonists, generates multiple questions about the value of normalised adult interculturalism. I suggest that the most pointed significance of these films lies in the compromises that the young adults make. As they reach the inevitable moral crisis that awaits them on the cusp of adulthood, despite pressures to abandon their childhood friendships they instead sustain their utopian (golden) visions of the future.

Summary Thyroid storm (TS) is a life-threatening condition that may suffer thyrotoxic patients. Therapeutic plasma exchange (TPE) is a rescue approach for TS with acute hepatic failure, but it should be initiated with careful considerations. We present a 55-year-old male patient with untreated Graves’ disease who developed TS. Severe hyperthyroidism and refractory atrial fibrillation with congestive heart failure aggregated to multiple organ failure. The patient was recovered by intensive multimodal therapy, but we had difficulty in introducing TPE treatment considering the risk of exacerbation of congestive heart failure due to plasma volume overload. In addition, serum total bilirubin level was not elevated in the early phase to the level of indication for TPE. The clinical course of this patient instructed delayed elevation of bilirubin until the level of indication for TPE in some patients and also demonstrated the risk of exacerbation of congestive heart failure by TPE. Learning points: Our patient with thyroid storm could be diagnosed and treated promptly using Japan Thyroid Association guidelines for thyroid storm. Delayed elevation of serum bilirubin levels could make the decision of introducing therapeutic plasma exchange difficult in cases of thyroid storm with acute hepatic failure. The risk of worsening congestive heart failure should be considered carefully when performing therapeutic plasma exchange.

Summary We present a 60-year-old woman who underwent successful surgical resection (partial pancreatectomy) for a low grade non-functioning pancreatic neuroendocrine tumour (pNET), with no biochemical or radiological features of recurrence on follow-up visits for 5 years. Fourteen years after the initial surgery, she developed spontaneous severe hypoglycaemic episodes which required hospitalisation, with subsequent investigations confirming the diagnosis of a metastatic insulin-secreting pNET (insulinoma). Medical management of her severe spontaneous hypoglycaemic episodes remained challenging, despite optimum use of diazoxide and somatostatin analogue therapy. Based on a discussion at the regional neuroendocrine tumour multidisciplinary team meeting, she underwent an elective hepatic trans-arterial embolization which was unfortunately unsuccessful. She ended up requiring an emergency right hemihepatectomy and left retroperitoneal mass resection which finally stabilised her clinical condition. Learning points: Ours is only the seventh case report of a previously benign pNET presenting as a functional insulin secreting metastatic tumour. However, it is the first case report, in which the metastatic functional pNET presented after such a long hiatus (14 years). There is currently no clear consensus regarding the length of follow-up of non-functional pNET which are deemed cured post-surgical resection, with most guidelines advocating a median follow up of 5 years (1). The delayed presentation in our case suggests additional considerations should be made regarding optimal post-operative surveillance duration based on the age of the patient, location of the tumour, lymph node spread and Ki-67 index. Hepatic artery embolization and/or partial hepatectomy remains a treatment option for pNET patients with significant hepatic metastasis.

Summary Medullary thyroid carcinoma (MTC) has a varying clinical course; distant metastases are frequently present even at diagnosis. We present two MTC cases with unusual metastatic sites. Two female patients are presented with slow progressive MTC. The first case developed distant metastases 23 years after diagnosis and underwent locoregional therapies. At the same time a breast mass developed representing MTC metastasis. Treatment with vandetanib led to long-term disease stabilization. The second patient is presented with metastases in the pancreas 13 years after diagnosis. Shortly, a painful mass developed in the mandible and metastasis of MTC was diagnosed. Disease progression was recorded 20 months after the initiation of local and systemic therapy. Such cases have only rarely been reported in the literature and highlight the need for prompt recognition of unexpected MTC metastases. Learning points Unusual sites of metastasis may appear in patients with medullary thyroid carcinoma (MTC) years after the initial diagnosis. Although rare, unexpected MTC metastases highlight the need for prompt recognition and appropriate treatment. Local recurrences accompanied by inappropriately low calcitonin levels should prompt further investigation for possible distant metastatic disease. Systemic treatment with tyrosine kinase inhibitors may be effective even in patients with unusual metastases from MTC.

Summary Ectopic ACTH (adrenocorticotrophic hormone) syndrome (EAS) is rarely associated with small-cell lung cancer (SCLC). Although chemotherapy is initially effective for SCLC, complicated EAS scarcely improves. Recently, immune checkpoint inhibitors have been used to treat SCLC. Atezolizumab plus chemotherapy for SCLC improved progression-free survival compared to conventional chemotherapy. However, little has been reported on the efficacy of the combination therapy for SCLC with EAS. We report a 72-year-old male who presented with 4-week history of leg oedema, proximal myopathy, weight loss, and worsened symptoms of diabetes and hypertension. Laboratory findings revealed hypokalaemia, increased plasma ACTH, and serum cortisol levels. Cortisol levels were not suppressed by the high-dose dexamethasone test. Chest and abdominal CT revealed a right lower lobe tumour with multiple metastases on the hilar lymph nodes, liver, lumbar spine, and bilateral enlarged adrenal glands. The patient was diagnosed with stage 4B SCLC with EAS. Hypercortisolaemia was then treated with metyrapone and atezolizumab plus chemotherapy, which was started for SCLC. After 10 days, the tumour shrank noticeably, and the ACTH level drastically decreased concomitantly with low cortisol levels with symptoms of fever, appetite loss, and general fatigue. Hydrocortisone treatment was initiated, and the symptoms resolved immediately. We describe a case of SCLC with EAS treated with atezolizumab plus chemotherapy, presenting with adrenal insufficiency. Close observation is required for patients with adrenal insufficiency receiving atezolizumab plus chemotherapy because of its stronger effect. Furthermore, advances in cancer therapy and care for endocrine paraneoplastic syndrome needs to be adapted. Learning points The immune checkpoint inhibitor atezolizumab has recently been approved for the treatment of small-cell lung cancer (SCLC). Approximately 1–6% of tumour ectopically produce ACTH and cause ectopic ACTH syndrome (EAS) as an endocrine paraneoplastic syndrome. The use of combined chemotherapy and atezolizumab in the ectopic ACTH syndrome secondary to small-cell lung cancer may cause a precipitous fall in circulating ACTH/cortisol, resulting in symptomatic adrenal insufficiency The advances in cancer therapy and treatment for endocrine paraneoplastic syndrome need to be adapted.

Summary Recombinant human growth hormone therapy (rhGH) has been available since 1985 for a variety of conditions and has expanded the indications for rhGH therapy and the number of patients receiving therapy. The very nature of the therapy exposes individuals to years of injections. There are a number of well-known adverse events, however, a lesser-known and rarely reported adverse event of rhGH therapy is localized lipoatrophy. We report nine cases of localized lipoatrophy during rhGH therapy accounting for 14.5% of patients taking rhGH presenting to a single centre for routine follow-up over just a 2-month period. The development of localized lipoatrophy does not appear to be age, indication or dose-related but rather related to repeated administration of rhGH into a limited number of sites. The most likely putative mechanism is the local lipolytic action of growth hormone (GH) itself, although the possibility of an excipient-based interaction cannot be excluded. Given the high prevalence of this adverse event and the potential to prevent it with adequate site rotation, we can recommend that patients be informed of the possible development of localized lipoatrophy. Doctors and nurses should closely examine injection sites at each visit, and site rotation should be emphasized during injection technique education. Learning points There are a number of well-known adverse events, however, a lesser-known and rarely reported adverse event of rhGH therapy is localized lipoatrophy. Examination of the injection sites at each visit by the treating healthcare practitioner. To advise the parents/caregivers/patients to change their injection site with each injection. To advise the parents/caregivers/patients to change the needles after every use. For parents, caregivers and patients to self-inspect their injection sites and have a high alert for the development of lipoatrophy and to then immediately report it to their doctor.
