4. Exploration of user involvement and collaboration in a co-design project : a case study

Abstract

Introduction: Collaborative design (co-design) is an approach that includes ‘users’ in the design process. This approach is growing in popularity amongst companies seeking to develop a competitive advantage in the field of outdoor clothing and equipment. Health research is steadily developing a substantial evidence base of the effectiveness of public involvement in research, however, in the co-design research field, there is a less developed evidence base concerning user involvement aspects. This qualitative case study presents an exploration of the factors that influence the experience of user involvement in a study utilising a co-design approach. The case concerned is the New Dynamics of Ageing-funded study ‘Design for Ageing Well’, which presented an opportunity to explore both the co-design mechanisms utilised along with the experiences of the individuals involved in the study. They included research team members, project partners (e.g., outdoor clothing manufacturers), User Reference Group members (members of the public) and User Advisory Group members (also members of the public). Methods: A single case study approach was utilised. Sampling was purposeful, taken from the above four groups of stakeholders. Data collection methods included one-to-one semi-structured interviews (35 people) and non-participant observations (44 total hours). Findings: This study demonstrated that strong project management, including leadership and change management, is crucial for a successful co-design experience. A clear vision, plus the aim and purpose of the co-design process, need to be communicated to all stakeholders. The three main factors that impact public involvement in co-design are: • The facilitator’s knowledge and skills regarding co-design methods and moderation; • Multidisciplinary collaboration; • The management of the co-design setting. Conclusion: Effective public involvement in co-design research requires suitable leadership, project management, and a clear communication strategy, and attention should be paid to the facilitation and management of the co-design setting. These objectives are best achieved when user needs are heard and embedded in all aspects of the co-design approach.

Abstract

Background Antidepressant use has been associated with increased fall risk. Antidepressant-related adverse drug reactions (e.g. orthostatic hypotension) depend partly on genetic variation. We hypothesized that candidate genetic polymorphisms are associated with fall risk in older antidepressant users. Methods The association between antidepressant use and falls was cross-sectionally investigated in a cohort of Dutch older adults by logistic regression analyses. In case of significant interaction product term of antidepressant use and candidate polymorphism, the association between the variant genotype and fall risk was assessed within antidepressant users and the association between antidepressant use and fall risk was investigated stratified per genotype. Secondly, a look-up of the candidate genes was performed in an existing genome-wide association study on drug-related falls in antidepressant users within the UK Biobank. In antidepressant users, genetic associations for our candidate polymorphisms for fall history were investigated. Results In antidepressant users(n = 566), for rs28371725 (CYP2D6*41) fall risk was decreased in TC/variant allele carriers compared to CC/non-variant allele carriers (OR = 0.45, 95% CI 0.26–0.80). Concerning rs1057910 (CYP2C9*3), fall risk was increased in CA/variant allele carriers compared to AA/non-variant allele carriers (OR = 1.95, 95% CI 1.17–3.27). Regarding, rs1045642 (ABCB1), fall risk was increased in AG/variant allele carriers compared to GG/non-variant allele carriers (OR = 1.69, 95% CI 1.07–2.69). Concerning the ABCB1-haplotype (rs1045642/rs1128503), fall risk was increased in AA-AA/variant allele carriers compared to GG-GG/non-variant allele carriers (OR = 1.86, 95% CI 1.05–3.29). In the UK Biobank, in antidepressant users(n = 34,000) T/variant-allele of rs28371725 (CYP2D*41) was associated with increased fall risk (OR = 1.06, 95% CI 1.01–1.12). Abstract

For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion.

Abstract

Background: Health care professionals are often reluctant to deprescribe fall-risk-increasing drugs (FRIDs). Lack of knowledge and skills form a significant barrier. To support clinicians in the management of FRIDs and to facilitate the deprescribing process, a deprescribing tool was developed by a European expert group for STOPPFall (Screening Tool of Older Persons Prescriptions in older adults with high fall risk) items. Methods: STOPPFall was created using an expert Delphi consensus process in 2019 and in 2020, 24 panellists from EuGMS SIG on Pharmacology and Task and Finish on FRIDs completed deprescribing tool questionnaire. To develop the questionnaire, a Medline literature search was performed. The panellists were asked to indicate for every medication class a possible need for stepwise withdrawal and strategy for withdrawal. They were asked in which situations withdrawal should be performed. Furthermore, panellists were requested to indicate those symptoms patients should be monitored for after deprescribing and a possible need for follow-ups. Results: Practical deprescribing guidance was developed for STOPPFall medication classes. For each medication class, a decision tree algorithm was developed including steps from medication review to symptom monitoring after medication withdrawal. Conclusion: STOPPFall was combined with a practical deprescribing tool designed to optimize medication review. This practical guide can help overcome current reluctance towards deprescribing in clinical practice by providing an up-to-date and straightforward source of expert knowledge.

In the present statement, the European Geriatric Medicine Society (EuGMS) Task and Finish group on fall-risk-increasing drugs (FRIDs), in collaboration with the EuGMS Special Interest group on Pharmacology and the European Union of Medical Specialists (UEMS) Geriatric Medicine Section, outlines its position regarding knowledge dissemination on medication-related falls in older people across Europe. The EuGMS Task and Finish group is developing educational materials to facilitate knowledge dissemination for healthcare professionals and older people. In addition, steps in primary prevention through judicious prescribing, deprescribing of FRIDs (withdrawal and dose reduction), and gaps in current research are outlined in this position paper.

Abstract

Apathy, a common and disabling behavioural syndrome in older persons, has been associated with impaired physical performance and executive dysfunction. Both are fall risk factors and they share pathophysiological pathway. We cross-sectionally examined the association between apathy and recurrent falling (≥2 falls in the past 12 months) and number of falls in the past 12 months in 243 outpatients aged ≥65 years with ≥3 fall risk-factors visiting a fall-clinic after a fall. We calculated Odds Ratio's (ORs), Incidence Rate Ratio's (IRRs) and their 95% Confidential Intervals (CI95) using multivariable regression and negative binomial regression analyses. We adjusted for cognitive functioning, depression, the use of fall risk increasing drugs, visual impairment, urine incontinence, comorbidity, smoking, use of alcohol, body mass index (BMI), and the number of months between assessment of fall risk and of apathy. We assessed effect modification by age and gender. In our study, apathy was independently associated with recurrent falling in patients aged 65–75 years: OR 2.8 (CI95 1.0–7.7). Overall, patients with apathy experienced 1.46 times as many falls in the past 12 months compared to patients without apathy (IRR 1.46 (CI95 1.0–2.1). To conclude, in high fall-risk older outpatients, apathy was cross-sectionally associated with recurrent falling in patients aged 65–75 years and the number of falls. Apathy appeared to be especially relevant in relation to falling in this age group. Whether apathy predicts recurrent falling is yet to be determined.

Abstract

The purpose of the Listening Event was to enable a wide range of people, including professionals working in statutory, voluntary and other organisations and members of the public, to ‘have a say’ about health and social care and what we as a University can do for and with these partners and the public. We particularly wanted to hear about key concerns of the University such as:\ud • Strengthening community engagement and partnerships\ud • Health and social care training we should be providing, for whom, and how this is delivered\ud • Ideas relating to the University themes including media, use of space and buildings, human rights, social justice and security \ud • Research topics we should be addressing\ud However the main strength of the Listening Event approach is that topics for discussion are mostly led by participants who attend. On this occasion, the\ud discussion topics were very much focused on the concerns of participants and lots of information and ideas were generated. The task now is for the event planning team to review the discussion notes and identify what can be\ud addressed and how, in the short, medium and long term. This planning will be taking place over the Autumn in 2011, and any participants or readers of this report are more than welcome to get in touch to work with us or add their views.\ud The purpose of this report is to record all discussion summaries for sharing amongst participants and others. It is important that participants especially get to read what others had said at the event. The report will lead to changes in University practices such as the content of some of our courses and new business ideas and relationships will also be explored. The event itself provided a useful means of public engagement that others may wish to adopt.

Abstract

Significant progress has been made toward solving the century-old problem of chromatic aberrations in diffractive optics. Our approach exploits modern materials and microfabrication technology and is very different from the 'purely diffractive strategy', which is commonly employed and which results in multiple diffractive elements separated by a finite distance. We have developed a Fresnel zone plate lens comprised of a serial stack of patterned minus-filters which allows broadband radiation to be focussed (or imaged) without longitudinal or transverse chromatic aberrations. Diffractive optics, such as the Fresnel zone plate shown in Figure 1, are commonly used for imaging and focussing radiation at wavelengths for which refraction is not a viable option.

Abstract

The motional Stark effect (MSE) diagnostic on DIII-D has been expanded to take advantage of a change in the neutral beam geometry, adding 24 new MSE channels viewing a beam injected counter to the plasma current. When data from these channels are used with those from two older MSE arrays viewing a different beam, the overall radial resolution improves near the magnetic axis at least a factor of 2, and the uncertainty in calculations of vertical magnetic field and radial electric field decreases in the edge at least a factor of 4. The new design uses two optical systems mounted on the same vacuum port with a common shutter and shielding. [ABSTRACT FROM AUTHOR]

Abstract

(Abridged) We describe here the most ambitious survey currently planned in the optical, the Large Synoptic Survey Telescope (LSST). A vast array of science will be enabled by a single wide-deep-fast sky survey, and LSST will have unique survey capability in the faint time domain. The LSST design is driven by four main science themes: probing dark energy and dark matter, taking an inventory of the Solar System, exploring the transient optical sky, and mapping the Milky Way. LSST will be a wide-field ground-based system designed to obtain multiple images covering the sky visible from Cerro Pach\'{o}n in northern Chile. The telescope will have an 8.4 m (6.5 m effective) primary mirror, a 9.6 deg$^2$ field of view, and a 3.2 Gigapixel camera. This system can image about 10,000 square degrees of sky in three clear nights using pairs of 15-second exposures twice per night, with typical 5$\sigma$ depth for point sources of $r\sim24.5$ (AB). Abstract

This Resource Book reviews the physics opportunities of a next-generation e+e- linear collider and discusses options for the experimental program. Part 4 discusses options for the linear collider program, at a number of levels. First, it presents a broad review of physics beyond the Standard Model, indicating how the linear collider is relevant to each possible pathway. Next, it surveys options for the accelerator and experimental plan, including the questions of the running scenario, the issue of one or two interaction regions, and the options for positron polarization, photon-photon collisions, and e-e- collisions. Finally, it reviews the detector design issues for the linear collider and presents three possible detector designs.
