3. Unfixing place : a study of Istanbul through topographical practices

Abstract

This thesis explores how topographical theories and practices can unfix place with specific reference to selected contemporary maps, documentary films and guidebooks of Istanbul. The project questions the traditional surveying methods that architecture and geography employ to represent a place and suggests instead mapping as a subjective practice that performs place as hybrid and multiple, Each chapter of this thesis discusses the shift from surveying to mapping, in relation to different topographical practices in twentieth century Istanbul, in terms of the graphic (chapter 3), filmic (chapter 4) and the textual (chapter 5). In chapter 1, I identify how the term topography has been used to date in architectural theory and geography, and define instead a newly emerging form of topography, which unfixes rather than fixes place. Drawing on contemporary feminist and poststructuralist theories of travel and representation. I critique traditional surveying methods as inadequate since they assume stable and single positions of knowledge in relation to place. I argue instead that the critical topographical practices starting in the 1990s suggest an understanding of topography that is more performative than representational. In chapter 2. 1 explore these ideas in relation to some critical topographical practices evident in certain contemporary artworks. This thesis intends to make an original contribution to the knowledge and understanding of topographical theory and practice in relation to contemporary Istanbul in four ways. F irst. I use feminist and poststructuralist theories to explore how contemporary maps, guidebooks and documentary films perform rather than represent Istanbul as place as unfixed. multiple and hybrid. Second. I rethink these often generic theories in relation to the particular topographic practices that map urban space, specifically Istanbul. Third, I critique the conventional understanding of topography as site in architectural theory and recast topography as both site and more importantly practice, fourth, I propose a critical topographical practice in the form of a DVD, which explores and critiques the maps, guidebooks, and documentary films which are also explored in the text.

Abstract

Aim: To evaluate the tendency, knowledge, awareness and behavior patterns of urology residents training at different institutions in Turkey about the use of fluoroscopy in operations. Methods: The 13-questioned survey prepared using “Google Forms©” as of 01.03.2021 was shared for four weeks in the “WhatsApp®” application group, which includes 279 urology residents studying with university hospitals and training and research hospitals in Turkey. One hundred and thirteen participants, who completed the questionnaire were included in the study. Results: Of the 113 urology residents included in the study, 56 (49.6%) were studying in university hospitals and 57 (50.4%) were in training and research hospitals. 67.3% of the residents stated that they never hesitated to participate in the operations which fluoroscopy was used. Abstract

The European Federation for Pharmaceutical Sciences (EUFEPS) was founded 25 years ago by more than 20 national pharmaceutical societies and faculty members. As a pan-European organization, it brings together pharmaceutical societies as well as academic, industrial and regulatory scientists engaged in drug research and development, drug regulation and education of professionals working in these fields. EUFEPS represents pharmaceutical sciences in Europe and is recognized as such by both the European Commission and the European Medicines Agency. EUFEPS cooperates with the European Federation of Pharmaceutical Industries and other European organizations and maintains global connections with agencies such as the US Food and Drug Administration and the American Association of Pharmaceutical Scientists. EUFEPS has established specified networks forming the basis of its activities. The creation of a Network on Veterinary Medicines is prompted by the manifold problems resulting from the use of veterinary drugs and its inherent interconnections with human medicine, environmental and public health. A long-term goal of this initiative was to expand the spectrum of available therapeutics for use in animals, including the development of innovative delivery systems.

Abstract

Objective. To assess the performance of the new 2012 provisional European League Against Rheumatism (EULAR)/ American College of Rheumatology (ACR) polymyalgia rheumatica (PMR) clinical classification criteria in discriminating PMR from other mimicking conditions compared with the previous 5 diagnostic criteria in a multicenter prospective study. Methods. Patients older than 50 years, presenting with new-onset bilateral shoulder pain with elevated acute-phase reactants (APR), were assessed for the fulfillment of the new and old classification/diagnostic criteria sets for PMR. At the end of the 1-year followup, 133 patients were diagnosed with PMR (expert opinion) and 142 with non-PMR conditions [69 rheumatoid arthritis (RA)]. Discriminating capacity, sensitivity, and specificity of the criteria sets were estimated. Results. Discriminating capacity of the new clinical criteria for PMR from non-PMR conditions and RA as estimated by area under the curve (AUC) were good with AUC of 0.736 and 0.781, respectively. The new criteria had a sensitivity of 89.5% and a specificity of 57.7% when tested against all non-PMR cases. When tested against all RA, seropositive RA, seronegative RA, and non-RA control patients, specificity changed to 66.7%, 100%, 20.7%, and 49.3%, respectively. Except for the Bird criteria, the 4 previous criteria had lower sensitivity and higher specificity (ranging from 83%-93%) compared with the new clinical criteria in discriminating PMR from all other controls. Conclusion. The new 2012 EULAR/ ACR clinical classification criteria for PMR is highly sensitive; however, its ability to discriminate PMR from other inflammatory/noninflammatory shoulder conditions, especially from seronegative RA, is not adequate. Imaging and other modifications such as cutoff values for APR might increase the specificity of the criteria.

Abstract

Although gout is potentially curable, the management of this disease is often suboptimal. In this study, we investigated the treatment of gout in Turkey and also compared the management approaches to gout in different clinical specialties. Three hundred and nineteen consecutive patients (mean age 58.60 +/- 12.8 years; 44 females, 275 males) were included in this multicenter study. A standardized form was generated to collect data about the patient's first admission to health care, the specialty of the doctor first diagnosed the gout, the treatment options for gout including attack management, patient referral, chronic treatment including medical treatment, and life style modifications. Forty patients were referred to another center without any treatment (12.8 %), and referral rate is most common among the primary care physicians (28.8 %). Colchicine was more commonly used for attack prophylaxis than allopurinol. Ninety-two patients had never been treated with allopurinol (28.8 %). Allopurinol prescription was less common among the primary care physicians and orthopedists, and highest among the rheumatologists. Recommendation of diet and life style modifications was less common among the primary care physicians and orthopedists, and highest among the rheumatologists. The rates of life style modification recommendation and long-term allopurinol prescription were 83.7 and 77.6 %, respectively, among the rheumatologists. Both acute and chronic management of gout is suboptimal in Turkey especially among the primary care physicians and orthopedists. Moreover, chronic treatment is even suboptimal among rheumatologists.

WOS: 000319224700017, PubMed ID: 23377817, BACKGROUND: The major renal involvement in Familial Mediterranean Fever (FMF) is the occurrence of amyloidosis that primarily affects the kidneys manifested by proteinuria and ending in death from renal failure. AIM: This study aims to investigate whether serum cystatin (cys-C) levels could be used as an early marker of renal involvement in FMF patients. PATIENTS AND METHODS: Forty-six patients with FMF during the attack period (AP), and 41 patients with FMF during attack-free periods (AFP), and 11 patients with FMF associated amyloidosis, and 38 healthy controls were enrolled in the study. We determined cys-C levels in the serum of FMF patients and healthy controls. RESULTS: Serum cys-C levels were significantly increased in patients with FMF and secondary amyloidosis, and serum cys-C is a more accurate and efficient marker for detecting renal involvement than estimated glomerular filtration rate (e-GFR) in patients with FMF. Abstract

WOS: 000325207600014, PubMed ID: 23640444, BACKGROUND: Familial Mediterranean fever is an auto-inflammatory disorder. Long term complications of the disease include decreased quality of life. The measurement of quality of life in the patients with chronic disease has become an important research topic during the last years. AIM: We aimed to evaluate life quality of the FMF patients by SF-36, and examine its relationship with the disease parameters. PATIENTS AND METHODS: One hundred voluntary patients (69 female, 31 male) admitted to the rheumatology clinic were included in the study. The control group consisted of 100 healthy individuals. All subjects in the study were asked to complete SF-36 questionnaire. Age of onset of FMF, age at diagnosis, age at the beginning of colchicine therapy, number of attacks per month, family history of FMF and dialysis were inquired of patients with FMF. Disease severity was determined using the FMF severity score. RESULTS: The mean age of the patient group was 31 +/- 12 and that of the control group was 29 +/- 9. Sixty-nine patients (69%) were female, and 31 patients were male (31%) in both groups. The mean scores of the physical function, physical role function, emotional role function, mental health, and general health parameters of the patients were statistically significantly lower than those of healthy volunteers (p < 0.05). The difference in social function and vitality between two groups was found to be insignificant (p > 0.05). CONCLUSIONS: We have shown that FMF had a negative impact on SF-36. FMF reduces quality of life both in physical and mental dimensions.
