Abstract

Bu çalışma,18-22 Mayıs 2022 tarihlerinde Antalya’da düzenlenen 43. Türkiye Endokrinoloji ve Metabolizma Hastalıkları Kongresi‘nde bildiri olarak sunulmuştur. Tiroid stimüle edici hormon (TSH) salgılayan hipofiz adenomu (TSHoma) tirotoksikozun nadir görülen sebeplerinden biridir. Tanı genellikle uygunsuz TSH yüksekliğinin araştırılması ya da hipofizer insidentalomanın tetkiki sırasında konulmaktadır. Olgumuz bilinen poliskistik over sendromu olan 26 yaşında kadın hasta olup altı aydır olan titreme, terleme ve baş ağrısı yakınmalarıyla kliniğimize başvurdu. Laboratuvar analizlerinde serum serbest T3 ve T4 düzeyi yüksek iken TSH düzeyi normal sınırlarda saptandı. Seks hormonu bağlayıcı globülin (SHBG) düzeyi normal sınırlardaydı Manyetik rezonans görüntüleme sonucunda hipofiz bezinde makradenom izlendi. TRH uyarı testine TSH yanıtı saptanmadı. TSHoma tanısı alan hastada preoperatif oktreotid ve metimazol tedavisi ile ötiroidizm sağlandıktan sonra hipofiz cerrahisi uygulandı. Cerrahi materyalin histopatolojik incelemesinde fibrotik değişikliklerle birlikte prolaktin ile pozitif, TSH ile negatif boyanma izlendi. Bu olgu sunumunda SHBG düzeyi normal ve TSH boyaması negatif olmasına rağmen uygunsuz TSH salınımı nedeniyle TSHoma tanısı alan bir hastayı sunmayı amaçladık. Thyroid-stimulating hormone (TSH) secreting pituitary adenoma (TSHoma) is one of the rare causes of thyrotoxicosis. The diagnosis is usually made during investigating inappropriate TSH elevation or pituitary incidentaloma. Our patient, a 26-year-old female patient, known to have polycystic ovary syndrome, was admitted to our clinic with complaints of tremor, sweating, and headache for six months. Laboratory analysis showed serum free T3 and T4 levels were elevated, while TSH levels were within normal limits. Sex hormone binding globulin (SHBG) level was within normal limits. Macroadenoma was observed in the pituitary gland at magnetic resonance imaging. TSH secretion did not increase much in response to the thyrotropin-releasing hormone. The patient was diagnosed with TSHoma, and pituitary surgery was performed after euthyroidism was achieved with preoperative octreotide and methimazole treatment. Positive staining with prolactin and negative staining with TSH were observed along with fibrotic changes in the histopathological examination of the surgical material. We aimed to present a patient diagnosed with TSHoma due to inappropriate TSH secretion, although her SHBG level was normal and TSH staining was negative in the current case report.

34. Two cases of subacute thyroiditis after different types of SARS-CoV-2 vaccination

Abstract

SUMMARY Although the development of subacute thyroiditis (SAT) following viral infections is well-documented, the actual mechanism has not been clearly elucidated. The occurrence of SAT after vaccination has been reported in several case series and possible mechanisms such as molecular mimicry due to the exposure to viral proteins and/or abnormal reactogenicity by adjuvants have been implicated. We describe two cases who developed SAT three days after the messenger RNA vaccine against COVID-19 (Pfizer-BioNTech®) and six days after the inactivated COVID-19 vaccine (CoronaVac®). SAT diagnosis of these patients was delayed for more than two weeks. When the current cases were evaluated together with 1 Pfizer-BioNTech® and 3 CoronaVac® related cases reported previously, the patients were female aged between 30-42, except for the male patient we presented, and the complaints of the patients initiated within the first 2-7 days. While two Pfizer-BioNTech® vaccine-related cases were severely symptomatic and thyrotoxic at presentation, there were cases with mild to moderate clinical manifestations in CoronaVac® vaccine-related group. Physicians should be aware of SAT that may occur within a few days following the COVID-19 vaccination.

Abstract

Objective: Acromegaly is a chronic disease that causes high concentrations of growth hormone (GH) and insulin-like growth factor-1 (IGF-1). GH and IGF-1 levels have important effects on bone homeostasis, bone turnover, and bone remodeling. The results of studies investigating the relationship between acromegaly and bone mineral density (BMD) are controversial. The aim of the current study was to evaluate BMD in newly diagnosed acromegaly patients and to determine the relationship between GH and IGF-1 levels with BMD. Materials and Methods: This cross-sectional study was conducted on 36 newly diagnosed acromegaly patients without a history of hypogonadism, whose BMD values can be obtained from the records of University of Health Sciences Turkey, Dışkapı Yıldırım Beyazıt Training and Research Hospital. The relationship between GH and IGF-1 levels and BMD measured from the femur and lumbar regions was examined separately. Results: The mean age of the patients was 46.2±12.5 years. Median IGF-1 and BH levels were 551 ng/mL and 8.2 ng/mL, respectively. While 58.3% of the patients had osteopenia, no osteoporosis was found in any patient. A positive correlation was found between the BMD value measured from the femoral neck and IGF-1 (r=0.484, p=0.036) and GH (r=0.595, p=0.007) levels. There was no significant correlation between BMD measurements evaluated from lumbar vertebrae and GH and IGF-1 levels. Conclusion: It was found that the increase in GH and IGF-1 levels in newly diagnosed eugonadal acromegaly patients increased BMD measured from the femur. Abstract

Amaç: Feokromositomalar kromaffin hücrelerinden kaynaklanan nöroendokrin tümörlerdir. Bu çalışmanın amacı feokromasitoma hastalarının klinik, laboratuvar ve görüntüleme bulgularını gözden geçirmektir.Materyal ve Metot: Araştırmamızda 2009–2019 yılları arasında feokromasitoma tanısı almış 19 olguya ait klinik, laboratuvar ve radyolojik veriler geriye dönük olarak değerlendirilmiştir.Bulgular: Feokromasitoma tanısı konan 19 hastadan 13’ü kadın, 6’sı erkek olup ortalama yaş 44,1±11,3 yıl olarak saptandı. Hastaların 8 tanesi yeni tanı olmak üzere 18’inde hipertansiyonmevcuttu. Feokromositoma hastalarına eşlik eden hastalıklar incelendiğinde, hipertansiyondan sonra en sık tiroid patolojilerinin olduğu bulundu. Tansiyon yüksekliğinden sonra en sık semptomterleme ve baş ağrısı olarak saptandı. Tüm hastalarda vitamin D eksikliği ya da yetersizliği mevcuttu. Hastaların %94,1’inde 24 saatlik idrarda bakılan normetanefrin düzeyi yüksek olarak saptandı. Yerleşim yerleri açısından on birinde sağda, beşinde solda ve üçünde tümör bilateral adrenal yerleşimliydi. Lezyon boyutu %36,8’inde 4 cm’den küçük, %42,1’inde 4–6 cm arasında saptanırken,%21,1’inde 6 cm’den büyük saptandı. Sonuç: Bu çalışma feokromasitomanın nonspesifik ve değişken bir kliniğinin olduğunu gösterdi. Feokromasitoma şüphesi olan hastalarda özellikle ataklar sırasında tarama testi olarak 24 saatlikidrarda normetanefrin ve metanefrin düzeyleri istenmeli ve görüntüleme yöntemleri ile kitle lokalize edildikten sonra cerrahi tedavi uygulanmalıdır.
