This review attempts to summarize the current understanding of the SCD concept along with its evolution and presents data from previous studies conducted. Suggestions for further research are also delineated. As listed in DSM-5, the criteria for this novel diagnosis are vague, display elevated comorbidity with other neurodevelopmental disorders and other childhood psychopathologies, and show partial overlap with autistic spectrum disorders both in terms of genetics and family histories. Data on cross-cultural presentations and temporal stability are also limited. The social communication model proposed by Catani and Bambini may help integrate the neurobiological findings pertaining to SCD. Valid and reliable assessment methods need to be developed for SCD. This may involve either development of novel instruments capturing the DSM-5 criteria or application of statistical methods such as item response theory to existing instruments. The relationships between broad autism phenotype, pragmatic language impairment, nonverbal learning disorder, learning disorders, autistic spectrum disorders, and SCD should be evaluated with further studies. To date, several studies have focused on possible psychosocial risk factors for delinquency among youths and on the implications of childhood mental illness on child criminality. However, the literature on prevalence of psychopathology and predictors of crime severity among delinquent youths in Turkey is sparse. Therefore, the aim of this study was to show the associations between crime severity and psychosocial factors such as gender, age, criminal history, concomitant attention deficit hyperactivity disorder (ADHD) and other comorbid psychiatric conditions, along with behavioral problem domains of Child Behavior Checklist (CBCL). Participants and methods: This analytical cross-sectional study sample consisted of 52 individuals (30 females and 22 males) who were sent to a pilot detention facility in Istanbul, Turkey. The participants’ age ranged from 8 to 18 years (M =13.4; SD =2.9). Self-rating scales were administered in an interview format, and the crime severity information was provided by participants’ admission documents. Results: No differences were found in terms of gender, age, children’s past history of crime and substance abuse. However, family crime history was significantly higher in the high severity crime group (P=0.026). Having one or more comorbid psychiatric disorder was associated with high crime severity (P=0.018). The most common psychiatric disorders were found to be ADHD, oppositional defiant disorder, conduct disorder (CD) and anxiety disorder. Conclusion: Findings suggest that a family history of crime comes across as a very strong predictor of severity of crime. Among psychiatric factors, ADHD and CD were associated with commitment of more severe crimes in delinquent youths in our sample. Anxious/depressed traits as depicted by CBCL are found to be associated with less severe crimes. Abstract

Objective: The aim of this study is to compare the outcomes of patients who underwent simultaneous transurethral resection of the prostate (TURP) and transurethral resection of bladder tumor (TURBT) with those who underwent only TURBT. Design: Historical-cohort Setting: Department of Urology, Ege University School of Medicine, Turkey Subjects: One hundred and eighty-six patients who underwent TUR were included in the study. Intervention: Tumor characteristics, complication, recurrence and progression rates, recurrence localizations, and elapsed time to recurrence were compared between the groups. Main outcome measures: Outcomes of simultaneous resection Results: The bladder tumor recurrence rate was 36.6% for group A and 29.8% for group B (P=.068), and the progression rates were 8.2% and 6.4%, respectively (P=.084). There was no significant difference between the elapsed time to recurrence (8.28±1.17 vs. 6.54±1.28 months) and the rate of prostatic urethral recurrence (1.8% vs. 1.7%, P=.712 and P=.395, respectively). Complication rates and progression distribution of tumors were also similar in both groups. Conclusion: It is safe to resect an incidentally detected bladder tumor along with prostate resection, with similar rates of progression, recurrence, and complications. © 2020, Kuwait Medical Association. 5. The effect of percutaneous nephrostomy implementation on the outcome of ureteroscopic stone treatment

EgeUn###, Purpose: We aimed to investigate the effect of percutaneous nephrostomy (PCN) implementation on the second ureteroscopy (URS) outcomes after a failed URS. Materials and Methods: The data of four hundred forty-eight patients with an unsuccessful URS history were evaluated. Patients were divided into two groups; patients who underwent PCN before second URS (Group A) and patients who did not (Group B). We compared the stone access rate in the second URS between the two groups according to patient and stone characteristics and operative data. Then, group A was subdivided into two groups according to stone access as; access succeeded (Group A1) and access failed (Group A2). We also compared stone access rates between these two groups in terms of gender, age, body mass index, stone size, side, location, grade of hydronephrosis and PCN duration. All data were available immediately after surgery and obtained from patient files and the outcome assessment was performed during the study period. Results: Stone access rate was higher in group A than group B (143/196 vs 41/252, P = .0018). Mean nephrostomy duration and mean hydronephrosis grade were significantly higher and mean stone size was significantly lower in group A1 than group A2 (18.74 vs 9.62 days, P < .001; grade 3.25 vs 1.21, P = .038; and 7.286 vs 12.631 mm P < .001, respectively). Conclusion: PCN is a favourable intervention after a failed URS and increases the success rate of the second operation with ease of implementation and minimal morbidity. © 2019, Urology and Nephrology Research Centre.

Background Attention-deficit/hyperactivity disorder (ADHD) is among the most common psychiatric disorders of childhood that often persists into adulthood and old age. Yet ADHD is currently underdiagnosed and undertreated in many European countries, leading to chronicity of symptoms and impairment, due to lack of, or ineffective treatment, and higher costs of illness. Methods The European Network Adult ADHD and the Section for Neurodevelopmental Disorders Across the Lifespan (NDAL) of the European Psychiatric Association (EPA), aim to increase awareness and knowledge of adult ADHD in and outside Europe. This Updated European Consensus Statement aims to support clinicians with research evidence and clinical experience from 63 experts of European and other countries in which ADHD in adults is recognized and treated. Results Besides reviewing the latest research on prevalence, persistence, genetics and neurobiology of ADHD, three major questions are addressed: (1) What is the clinical picture of ADHD in adults? (2) How should ADHD be properly diagnosed in adults? (3) How should adult ADHDbe effectively treated? Conclusions ADHD often presents as a lifelong impairing condition. The stigma surrounding ADHD, mainly due to lack of knowledge, increases the suffering of patients. Education on the lifespan perspective, diagnostic assessment, and treatment of ADHD must increase for students of general and mental health, and for psychiatry professionals. Instruments for screening and diagnosis of ADHD in adults are available, as are effective evidence-based treatments for ADHD and its negative outcomes. The aim of this study was to provide data about the presentation of adult patients with ADHD and evaluate the treatment strategies of Turkish adult psychiatrists based on their personal clinical experience in different hospital settings.Methods: A cross-sectional online survey to be filled out by Turkish adult psychiatrists was designed and administered in May 2014. The survey focused on the treatment environment, patterns of patient applications and transition, treatment strategies, and medication management for adults with ADHD.Results: Significant differences were observed in the number of adult patients with ADHD in follow up, and a significant positive correlation was found between number of adult patients with ADHD in follow up and the clinician’s opinion about their level of self-competence to treat adult ADHD. A significant portion of adult psychiatrists have not received any information about their adult ADHD patients’ treatment during childhood. The most preferred medical treatment was stimulants and the majority of the participants always preferred psychoeducation in addition to medication treatment. A majority of participants did not define themselves competent enough to treat and follow up adult patients with ADHD.Conclusion: The findings of this study indicate the need to increase the knowledge, skills, and awareness of adult psychiatrists about adult ADHD. In addition, a more collaborative working relationship between child and adolescent psychiatrists and adult psychiatrists with a definite transition policy is required in order to help patients with ADHD more effectively. Abstract

Aim: We aimed to investigate the relationship between unexplained male infertility, and the -677C/T (rs1801133) and -1298A/C (rs1801131) polymorphisms of the MTHFR gene in a group of Turkish infertile men with non-obstructive azoospermia and severe oligozoospermia in this study. Materials and Methods: Study group includes 50 non-obstructive azoospermic patients, 50 severe oligospermic patients and 50 healthy controls with normal sperm parameters who had had more than one child. Genotyping was performed by generated amplicons from melting curve analysis after real time PCR. Results: The distribution of the 677CC genotype was significantly higher in the control group than the infertile group (p= 0.046). There was a significant frequency of the polymorphic T allele in infertile patients higher than the control group (p=0.015). Neither the frequency, nor the allelic distribution of A1298C genotype was different between infertile groups compared with the control. Conclusions: The MTHFR 677TT genotype is a genetic risk factor for unexplained male infertility, especially in the group with oligospermia and non-obstructive azoospermia.

Abstract

Purpose: Juvenile delinquency is a serious and common problem. To date, several studies have focused on possible psychosocial risk factors for delinquency among youths and on the implications of childhood mental illness on child criminality. However, the literature on prevalence of psychopathology and predictors of crime severity among delinquent youths in Turkey is sparse. Therefore, the aim of this study was to show the associations between crimeseverity and psychosocial factors such as gender, age, criminal history, concomitant attention deficit hyperactivity disorder (ADHD) and other comorbid psychiatric conditions, along with behavioral problem domains of Child Behavior Checklist (CBCL). Participants and methods: This analytical cross-sectional study sample consisted of 52 individuals (30 females and 22 males) who were sent to a pilot detention facility in Istanbul, Turkey. The participants' age ranged from 8 to 18 years (M = 13.4; SD = 2.9). Self-rating scales were administered in an interview format, and the crime severity information was provided by participants' admission documents. Results: No differences were found in terms of gender, age, children's past history of crime and substance abuse. However, family crime history was significantly higher in the high severity crime group (P=0.026). Having one or more comorbid psychiatric disorder was associated with high crime severity(P=0.018). The most common psychiatric disorders were found to be ADHD, oppositional defiant disorder, conduct disorder (CD) and anxiety disorder. Conclusion: Findings suggest that a family history of crime comes across as a very strong predictor of severity of crime. Among psychiatric factors, ADHD and CD were associated with commitment of more severe crimes in delinquent youths in our sample. Abstract

Objective: Factors contribute to commercial sexual exploitation of youth (CSEY) and a variety of physical and psychological consequences of CSEY were previously discussed in the literature. The aim of the current study was to determine the long term prognosis of a sample of CSEY in Turkey. Methods: A sample of 108 CSEY (all females) with a mean age of 16.7 +/- 2.7 (range: 9-22) years living in a specialized center for protection against perpetrators were examined in terms of their sociodemographic variables, previous abuse history, family history and current psychopathology. Identified cases were referred for psychiatric evaluation. We obtained follow-up data 11 years after initial contact from this cohort (mean age 25.9 +/- 2.6 at time of follow-up) with the subjects and received information regarding their current functioning. Results: Follow up data revealed that recommendations during psychiatric follow up could not be acted upon. 71.4% of the initial sample (n= 76) could be reached via direct or indirect contacts. We found that good/superior functioning was very rare and that one-third of the sample that could be reached still was being sexually exploited. Abstract

Objective: The aim of this study was to determine the frequencies of Y chromosome microdeletions in infertile azoospermic and oligozoospermic Turkish men and in healthy control subjects. Material and methods: Sixty-four azoospermic and 51 oligozoospermic patients with infertility and 70 healthy men who had a child naturally without assisted reproductive technology were included in this study. DNA was extracted from peripheral blood samples collected from the patients. Following multiplex PCR performed with 15 different primer sequences, Y chromosome AZFa, AZFb, AZFc and AZFd region microdeletions were determined by agarose gel electrophoresis. Results: Y chromosome microdeletions were detected in 8 (12.5%) patients in the azoospermia group and 3 (5.9%) patients in the oligozoospermia group. The overall prevalence of Y chromosome microdeletions in infertile men was 9.6%. Y chromosome microdeletions were not found in the healthy control group. Among the infertile cases, there were 4 (3.48%) AZFa, 2 (1.74%) AZFb, 3 (2.61%) AZFc and 7 (6.09%) AZFd region microdeletions. Conclusion: The presence of Y chromosome microdeletions among azoospermic and oligozoospermic infertile males suggests that routine genetic testing and genetic counseling prior to the use of assisted reproduction techniques are necessary. © 2013 by Turkish Association of Urology.

Abstract

Objective: To examine the mental health states of a sample of Yazidi refugee children and adolescents who migrated from war-torn Iraq’s Sinjar region and to determine the risk and protective factors for psychiatric disorders among the refugee children and adolescents. Method: The participants of this research were children and adolescents between ages 6 and 17 who live in a refugee camp in Turkey. Their parents/guardians provided written informed consents for the research. The research was approved by the Ethics Council of Sakarya University. Participants with intellectual disabilities and autism spectrum disorders were excluded from the study. Two child and adolescent psychiatrists speaking their native language interviewed and evaluated each of the participants. Collected data included socio-demographic information about previous and current living situations, Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime–Turkish Version (K-SADS-PL). Results: One hundred and thirty-six children and adolescents (76 boys, 63 girls; mean age = 11.05 ± 3.11 (SD)). At the time of the assessment, 43.4% had posttraumatic stress disorder (PTSD) (n = 59), 27.9% depression (n = 38), 10.3% nocturnal enuresis (n = 14), 9.6% behavioural problems (n = 7), and 5.1% anxiety disorders (n = 13). Conclusions: Many of the refugee children and adolescents had developed psychiatric disorders, or are at risk for PTSD and depression. Also, the ongoing ambiguity regarding their living conditions, interruption of their education, a lack of hope for the future and anxiety regarding the ones they left behind are considered to be risk factors for the development of psychiatric and social problems in the future. Abstract

Objective: In children referred for forensic reports by juvenile courts, particularly as psycho-sexual maturation starts in the initial phases of adolescence, actions defined as criminal by society may be expected to have been committed. These actions, which are frequently the results of unconscious impulses and sometimes affectations, are crimes in the general sense for this reason; great care is required when determining the presence of the ability of children to perceive the legal meaning and consequences of crimes that they commit and their ability to channel their behaviors. The aim of this present study is to examine the mental problems in children driven to committing crimes and the preparation of forensic reports in a Turkish forensic sample. Methods: Children referred by courts to Van Regional Training and Research Hospital Outpatient Clinics of Pediatric Psychiatry between 2013 and 2014 for forensic expert reports evaluating their ability to perceive the legal meaning and consequences of criminal behavior, and their ability to control their behaviors. Seventy-four children were evaluated retrospectively, with interviews of approximately 45 minutes on The Diagnostic and Statistical Manual of Mental Disorders in order to determine psychiatric diagnosis. Results: After evaluations, 26% of children and adolescents were found to possess the ability to perceive the legal meaning and consequences of the crime they had committed and have the ability to control their behaviors, while 27% could perceive the legal meaning and consequences but could not control their behavior, and 47% did not have the ability to perceive the legal meaning and consequences of the crime they had committed and to control their behaviors. Conclusions: The ability to perceive the meaning and consequences of crime and to direct one’s behaviors are affected by pathologies related with the family and the child.

Abstract

Purpose: Juvenile delinquency is a serious and common problem. To date, several studies have focused on possible psychosocial risk factors for delinquency among youths and on the implications of childhood mental illness on child criminality. However, the literature on prevalence of psychopathology and predictors of crime severity among delinquent youths in Turkey is sparse. Therefore, the aim of this study was to show the associations between crimeseverity and psychosocial factors such as gender, age, criminal history, concomitant attention deficit hyperactivity disorder (ADHD) and other comorbid psychiatric conditions, along with behavioral problem domains of Child Behavior Checklist (CBCL). Participants and methods: This analytical cross-sectional study sample consisted of 52 individuals (30 females and 22 males) who were sent to a pilot detention facility in Istanbul, Turkey. The participants' age ranged from 8 to 18 years (M = 13.4; SD = 2.9). Self-rating scales were administered in an interview format, and the crime severity information was provided by participants' admission documents. Results: No differences were found in terms of gender, age, children's past history of crime and substance abuse. However, family crime history was significantly higher in the high severity crime group (P=0.026). Having one or more comorbid psychiatric disorder was associated with high crime severity(P=0.018). The most common psychiatric disorders were found to be ADHD, oppositional defiant disorder, conduct disorder (CD) and anxiety disorder. Conclusion: Findings suggest that a family history of crime comes across as a very strong predictor of severity of crime. Among psychiatric factors, ADHD and CD were associated with commitment of more severe crimes in delinquent youths in our sample. Abstract

Objective: The aim of this study is to analyze socio demographic characteristics and mental disorder diagnoses for girls between 16 and 17 years old who had been referred for a report stating whether any obstacles existed to their marriage. Methods: Eighty cases between January 1st 2010 and January 1st 2016 seeking a juridical report identifying any condition that might be obstacle to the marriage of girls aged 16 and 17 have been included within the scope of this Sakarya University Child and Adolescent Psychiatry clinic study. The adolescents were examined by 5 different children and adolescent child psychiatrists working in the relevant hospitals for 6 years. The cases were diagnosed in DSM-IV criteria. Results: In 53.8% (n=43) of the cases no mental disorder was identified. Among the rest, the following was detected 18.8% (n=15) attention deficiency hyperactivity disorder (ADHD), 5% (n=4) conduct disorder, 3.8% (n=3) anxiety disorder, 2.5% (n=2) depressive disorder, 1.3% (n=1) adjustment disorder, 6.2% (n=6) mental retardation and 19.8% (n=16) borderline intelligence. Discussion: It should be well considered and kept in mind that the girls who have not married also yet to show significant mental disorders. In an attempt to prevent early marriages for girls, close follow-up of children’s education and social and mental health must be conducted.

Abstract

Objective: Factors contribute to commercial sexual exploitation of youth (CSEY) and a variety of physical and psychological consequences of CSEY were previously discussed in the literature. The aim of the current study was to determine the long term prognosis of a sample of CSEY in Turkey. Methods: A sample of 108 CSEY (all females) with a mean age of 16.7 +/- 2.7 (range: 9-22) years living in a specialized center for protection against perpetrators were examined in terms of their sociodemographic variables, previous abuse history, family history and current psychopathology. Identified cases were referred for psychiatric evaluation. We obtained follow-up data 11 years after initial contact from this cohort (mean age 25.9 +/- 2.6 at time of follow-up) with the subjects and received information regarding their current functioning. Results: Follow up data revealed that recommendations during psychiatric follow up could not be acted upon. 71.4% of the initial sample (n= 76) could be reached via direct or indirect contacts. We found that good/superior functioning was very rare and that one-third of the sample that could be reached still was being sexually exploited. 43. A systematic review and meta-analysis comparing the severity of core symptoms of attention-deficit hyperactivity disorder in females and males.

Abstract

In the past decade, there have been substantial changes in diagnostic nomenclature. This study investigated sex differences in attention-deficit/hyperactivity disorder (ADHD) symptom severity based on Diagnostic and Statistical Manual of Mental Disorders (DSM)-IV, DSM-IV(TR), and DSM-5 criteria, separating rating scale and clinical interview data in children and adults with ADHD. PubMed, PsycINFO, and Scopus were searched for published studies (1996-2021) reporting severity of attention, and hyperactivity/impulsivity in males and females. We compared data: (1) across the entire lifespan aggregating rating scale and clinical interview data (51 studies), (2) drawing solely on rating scale data (18 studies), and (3) drawing solely on clinical interview data (33 studies). Fifty-two studies met inclusion criteria comparing data for females ( n = 8423) and males ( n = 9985) with ADHD across childhood and/or adulthood. In total, 15 meta-analyses were conducted. Pooled data across the lifespan aggregating both rating scale and clinical diagnostic interview data, showed males had significantly more severe hyperactivity/impulsivity symptoms than females. Rating scale data were similar; boys had significantly more severe hyperactivity/impulsivity than girls. In adulthood, men were rated to have significantly more severe inattention than women with no difference in the hyperactivity/impulsivity dimension. All significant differences were of small effect size. No significant sex differences in the severity of symptoms emerged for clinical interview data for children or adults, in contrast. Possible reasons for the discrepancy in findings between rating scales and clinical diagnostic interviews are discussed.

Abstract

Two major earthquakes hit Turkey at the Kahramanmaraş region on February 6th 2023. The earthquakes affected almost 15 million individuals, resulting in more than forty thousand deaths, thousands of wounded and the destruction of ancient cities of humankind. Immediately after the earthquakes, the Psychiatric Association of Turkey organized an educational event to address the needs for a guidance on how to approach a trauma of such a big scale. The experts in this educational event summarized their presentations and prepared this review to guide the mental health professionals serving victims of this disaster. The review summarizes the early symptoms of trauma, and puts a framework on the principles of psychological first aid, the approach at the initial stages of the disaster, principles of planning, triage, and psychosocial support systems and the proper use of medications. The text covers the evaluation of the impact of trauma, aligning psychiatric practice with psychosocial interventions, the improvement of counseling skills and methods to better understand the mind during the acute post trauma phase. A set of presentations highlight the challenges in child psychiatry, brings a systematic overview to the earthquake and discuss the symptomatology, first aid and intervention principles in children and adolescents. Last, the forensic psychiatric perspective is presented, followed by a piece on the essentials of delivering bad news and the review is concluded with the emphasis on burnout, a syndrome to avoid particularly for field professionals, and possible preventive measures. 45. Linguistic Equivalence, Validity and Reliability Study of the Mind Excessively Wandering Scale.

Abstract

Introduction: The main objective was to study the linguistic equivalence, validity and reliability of the transliterated Turkish version of Mind Excessively Wandering Scale (MEWS) developed by Prof. Philip Asherson in England (2016). Mind excessively wandering defines uncontrolled mental phenomena, which is proposed as the psychological counterpart of Default Mode Network in literature., Method: Mind Excessively Wandering Scale, Adult Attention Deficit Hyperactivity Disorder Self-Report Scale, Barratt Impulsiveness Scale, Difficulties in Emotion Regulation Scale have been used. The sample group consists of 64 patients previously diagnosed as adult Attention Deficit Hyperactivity Disorder for validity, 60 students for transliteral equivalence, and 80 healthy controls for test re-test reliability., Results: Transliteral equivalence study demonstrates that Turkish version of MEWS is highly correlated with the English version and is statistically significant. The sixth item in the scale was removed in order to ensure the consistency model established by LISREL in the validity study according to the confirmatory factor analysis. When the sixth item was excluded, it was concluded that the structure of the scale was compatible. In the reliability study of the MEWS, the Cronbach's alpha value (α) of the scale was found to be 0.826. It is seen that the item with the highest distinctiveness feature is Item-10 (0.618) and the item with the lowest distinctiveness feature is Item-5 (0.318). In the linguistic equivalence study, no statistical difference was found between the English form and the Turkish versions of the scale., Conclusion: As a result of the practical and statistical evaluations, our study demonstrated that the Turkish version of the MEWS is a valid and reliable measurement tool., Competing Interests: Conflict of Interest: Authors declared that there is no conflict of interest., (Copyright: © 2022 Turkish Neuropsychiatric Society.)

46. Sociodemographic and Clinical Factors Affecting Treatment Adherence in Adults with Attention Deficit and Hyperactivity Disorder.

Abstract

Background: This study aims to explore sociodemographic and clinical factors affecting medication adherence in adults with attention deficit and hyperactivity disorder and elicit dysfunctional domains and comorbidities with a focus on gender differences., Methods: Patients were recruited from 2 specialty clinics using chart records in a natural treatment design. Adult attention deficit and hyperactivity disorder self-report scale, Diagnostic Interview for attention deficit and hyperactivity disorder in adults, was applied. Adherence is defined if the patient declared ≥80% adherence to medication throughout the last 8-12 weeks., Results: From 205 attention deficit and hyperactivity disorder patients (male = 112 female = 93 (age (median) min-max = 29 (18-56)), 29% were non-adherent to attention deficit and hyperactivity disorder medication. In the multivariate analysis, having 2 or more comorbid disorders ( P = .009), dysfunctions in academic/work ( P  = .049), and dysfunctions in family and other relationships ( P = .047) increased the likelihood of adherence. Adherence rates did not significantly differ between methylphenidate and atomoxetine ( P = .405). Women were more likely to have 2 or more comorbid psychiatric disorders ( P = .004) and dysfunctions in social relationships ( P  = .001), free time activities, hobbies ( P < .001), self-confidence, and self-image ( P < .001)., Results: Nearly one-third of adult patients with attention deficit and hyperactivity disorder did not adhere to medication treatment. Comorbid psychiatric disorders and dysfunctions in life domains appear to increase the likelihood of adherence to attention deficit and hyperactivity disorder medications, possibly through increasing motivation for treatment. The effect of cognitive-behavioral therapy on compliance with attention deficit and hyperactivity disorder medication should further be explored with prospective controlled studies., Conclusion: We suggest that future longitudinal studies use objective measures of adherence and confirm the role of dysfunctional life domains and comorbid psychiatric disorders as correlates of medication adherence., (2022 authors.)

Abstract

Introduction: We aimed to evaluate the impact of the resection technique (tumor enucleation (TE) or standard partial nephrectomy (SPN)) on trifecta outcomes in patients having undergone partial nephrectomy (PN)., Materials and Methods: We retrospectively analyzed the clinical and pathologic parameters in patients with localized renal cell carcinoma (pT1-2N0M0) who had undergone PN between January 2001-December 2018 at one of 15 different tertiary referral centers. Multivariable logistic regression analysis was applied to investigate independent predictors of trifecta failure, decreased postoperative renal functions (decreased estimated glomerular filtration rate (eGFR) > 10%), perioperative complications (Clavien-Dindo > 1), and positive surgical margins., Results: A total of 1070 patients with a mean age 56.11 ± 11.88 years were included in our study. PN was performed with TE in 848 (79.25%) and SPN in 222 (20.75%) patients. Trifecta failure rate was 56.2% for TE and 64.4% for SPN ( p  = 0.028). On multivariable analysis, TE was associated with less trifecta failure ( p  = 0.025) and eGFR decrease >10% rates ( p  = 0.024). On the other hand, there was no statistically significant difference between TE and SPN according to positive surgical margins ( p  = 0.450) and complication > Clavien-Dindo grade 1 ( p  = 0.888) rates. The only independent predictive factor for complications > Clavien-Dindo 1 was the Charlson comorbidity index (CCI) ( p  = 0.001)., Conclusion: TE is associated with less trifecta failure than SPN. This result is mainly due to better preservation of renal function with TE.

Abstract

Aims: Children of parents with mood disorders have an elevated risk for various psychopathologies. In this study rate of psychopathologies among adolescent offspring of parents with major depressive (MDDoff) and bipolar disorder (BDoff), including disruptive mood dysregulation disorder (DMDD) along with the offspring ability to resolve cognitive and emotional conflicts were evaluated., Method: 12-16 years old children of parents with MDD ( n  = 31, children= 36), BP ( n  = 20, children = 26) and controls ( n  = 25, children = 28) were enrolled. Children and parents were evaluated by using the Kiddie Schedule for Affective Disorders and Schizophrenia (K-SADS) and the Structured Clinical Interview for DSM-IV Axis I Disorders (SCID); respectively. The parents completed the Child Behavior Checklist (CBCL)-dysregulation profile. The Stroop test-TBAG form and emotional Stroop test were given out to evaluate conflict resolution ability., Results: The most common diagnoses among the whole sample were attention deficit and hyperactivity, separation anxiety and oppositional defiant disorders. Five cases (5.5%) of lifetime DMDD were found (three from MDDoff, the rest from BDoff). Completion times for the Stroop test-TBAG form were ranked as: BDoff > MDDoff > Hoff. In the emotional Stroop test, the BDoff responded significantly later and had significantly reduced correct responses., Conclusion: Rates of lifetime DMDD were similar in the MDDoff and BDoff groups. BDoff may experience greater difficulties in resolving cognitive and emotional conflicts.

Abstract

Binary (AC, AG), ternary (ACG, ACP, AGP), quaternary (ACGP) composite beads of alginate (A), carrageenan (C), gelatin (G), and poly (lactic-co-glycolic acid) (P) were prepared. The dried beads had a 700 μm average diameter. The microspheres with and without P were characterized by FT-IR, TGA/DTA, SEM, and PZC analysis. The results proved that the features of the composites were completely different from their bare components. Density Functional Theory (DFT) calculations were performed at the B3LYP/6-311++G** level to enlighten the elementary physical and chemical properties of A, C, P, and G compounds. The vibrational modes obtained by calculations were compared with those observed in the FT-IR spectra. The Frontier Molecular Orbital (FMO) analyses showed that the component G was the softer and had smaller energy gap than the other components and vice versa for component P. NBO (Natural Bond Orbital) analyses implied that the n → П* (resonance) interactions for components A, G, and P contributed to the lowering of the molecular stabilization, whereas that the n → σ* (anomeric) interactions were responsible for decreasing of the stabilization of the component. Abstract

Purpose: We aimed to investigate the effect of percutaneous nephrostomy (PCN) implementation on the second ureteroscopy (URS) outcomes after a failed URS., Materials and Methods: The data of four hundred forty-eight patients with an unsuccessful URS history were evaluated. Patients were divided into two groups; patients who underwent PCN before second URS (Group A) and patients who did not (Group B). We compared the stone access rate in the second URS between the two groups according to patient and stone characteristics and operative data. Then, group A was subdivided into two groups according to stone access as; access succeeded (Group A1) and access failed (Group A2). We also compared stone access rates between these two groups in terms of gender, age, body mass index, stone size, side, location, grade of hydronephrosis and PCN duration.  All data were available immediately after surgery and obtained from patient files and the outcome assessment was performed during the study period., Results: Stone access rate was higher in group A than group B (143/196 vs 41/252, P = .0018). Mean nephrostomy duration and mean hydronephrosis grade were significantly higher and mean stone size was significantly lower in group A1 than group A2 (18.74 vs 9.62 days, P < .001; grade 3.25 vs 1.21, P = .038; and 7.286 vs 12.631 mm P < .001, respectively)., Conclusion: PCN is a favourable intervention after a failed URS and increases the success rate of the second operation with ease of implementation and minimal morbidity.
