Abstract

Altres ajuts: ALCOTRA Program (Alpes Latines COopération TRAnsfrontalière). By definition, neuromuscular diseases are rare and fluctuating in terms of symptoms; patients are often lately diagnosed, do not have enough information to understand their condition and be proactive in their management. Usually, insufficient resources or services are available, leading to patients' social burden. From a medical perspective, the rarity of such diseases leads to the unfamiliarity of the medical staff and caregiver and an absence of consensus in disease assessment, treatment, and management. Innovations have to be developed in response to patients' and physicians' unmet needs. It is vital to improve several aspects of patients' quality of life with a better comprehension of their disease, simplify their management and follow-up, help their caregiver, and reduce the social and economic burden for living with a rare debilitating disease. Database construction regrouping patients' data and symptoms according to specific country registration on data privacy will be critical in establishing a clear consensus on neuromuscular disease treatment. Clinicians also need technological innovations to help them recognize neuromuscular diseases, find the best therapeutic approach based on medical consensus, and tools to follow patients' states regularly. Diagnosis also has to be improved by implementing automated systems to analyze a considerable amount of data, representing a significant step forward to accelerate the diagnosis and the patients' follow up. Further, the development of new tools able to precisely measure specific outcomes reliably is of the matter of importance in clinical trials to assess the efficacy of a newly developed compound. In this context, creation of an expert community is essential to communicate and share ideas. To this end, 97 clinicians, healthcare professionals, researchers, and representatives of private companies from 9 different countries met to discuss the new perspective and challenges to develop and implement innovative tools in the field of neuromuscular diseases.

Abstract

Little is known about the characteristics and treatments of patients with severe asthma across Europe, but both are likely to vary. This is the first study in the European Respiratory Society Severe Heterogeneous Asthma Research collaboration, Patient-centred (SHARP) Clinical Research Collaboration and it is designed to explore these variations. Therefore, we aimed to compare characteristics of patients in European severe asthma registries and treatments before starting biologicals.This was a cross-sectional retrospective analysis of aggregated data from 11 national severe asthma registries that joined SHARP with established patient databases.Analysis of data from 3236 patients showed many differences in characteristics and lifestyle factors. Current smokers ranged from 0% (Poland and Sweden) to 9.5% (Belgium), mean body mass index ranged from 26.2 (Italy) to 30.6 kg·m-2 (the UK) and the largest difference in mean pre-bronchodilator forced expiratory volume in 1 s % predicted was 20.9% (the Netherlands versus Hungary). Before starting biologicals patients were treated differently between countries: mean inhaled corticosteroid dose ranged from 700 to 1335 µg·day-1 between those from Slovenia versus Poland when starting anti-interleukin (IL)-5 antibody and from 772 to 1344 µg·day-1 in those starting anti-IgE (Slovenia versus Spain). Maintenance oral corticosteroid use ranged from 21.0% (Belgium) to 63.0% (Sweden) and from 9.1% (Denmark) to 56.1% (the UK) in patients starting anti-IL-5 and anti-IgE, respectively.The severe asthmatic population in Europe is heterogeneous and differs in both clinical characteristics and treatment, often appearing not to comply with the current European Respiratory Society/American Thoracic Society guidelines definition of severe asthma. Treatment regimens before starting biologicals were different from inclusion criteria in clinical trials and varied between countries.

Abstract

Little is known about the characteristics and treatments of patients with severe asthma across Europe, but both are likely to vary. This is the first study in the European Respiratory Society Severe Heterogeneous Asthma Research collaboration, Patient-centred (SHARP) Clinical Research Collaboration and it is designed to explore these variations. Therefore, we aimed to compare characteristics of patients in European severe asthma registries and treatments before starting biologicals.This was a cross-sectional retrospective analysis of aggregated data from 11 national severe asthma registries that joined SHARP with established patient databases.Analysis of data from 3236 patients showed many differences in characteristics and lifestyle factors. Current smokers ranged from 0% (Poland and Sweden) to 9.5% (Belgium), mean body mass index ranged from 26.2 (Italy) to 30.6 kg·m−2 (the UK) and the largest difference in mean pre-bronchodilator forced expiratory volume in 1 s % predicted was 20.9% (the Netherlands versus Hungary). Before starting biologicals patients were treated differently between countries: mean inhaled corticosteroid dose ranged from 700 to 1335 µg·day−1 between those from Slovenia versus Poland when starting anti-interleukin (IL)-5 antibody and from 772 to 1344 µg·day−1 in those starting anti-IgE (Slovenia versus Spain). Maintenance oral corticosteroid use ranged from 21.0% (Belgium) to 63.0% (Sweden) and from 9.1% (Denmark) to 56.1% (the UK) in patients starting anti-IL-5 and anti-IgE, respectively.The severe asthmatic population in Europe is heterogeneous and differs in both clinical characteristics and treatment, often appearing not to comply with the current European Respiratory Society/American Thoracic Society guidelines definition of severe asthma. Treatment regimens before starting biologicals were different from inclusion criteria in clinical trials and varied between countries.

19. Changes in cerebral blood flow after acetazolamide: an experimental study comparing near-infrared spectroscopy and SPECT

Abstract

Udgivelsesdato: 2009-Apr, BACKGROUND AND PURPOSE: It is important to find a reliable and bedside method, which can estimate the cerebral blood flow (CBF) of patients in clinical settings. Estimation of CBF by calculating a blood flow index (BFI) using continuous wave near-infrared spectroscopy (CW-NIRS) and indocyanine green (ICG) as an i.v. tracer has been proposed to be a feasible and promising method. To validate if the BFI method can detect relative changes in CBF we compared data with the established method (133)Xenon single photon emission computer tomography ((133)Xe-SPECT). METHODS: Ten healthy subjects were investigated before and after a bolus of acetazolamide. NIRS data were obtained using a multi source detector separation configuration in order to assess a corrected BFI (BFI(corr)) value, which attempts to eliminate contamination of skin blood flow. RESULTS: Data obtained showed no significant correlation between CBF changes measured by (133)Xe-SPECT and BFI(corr) (0.133, P = 0.732). After acetazolamide, a 49% increase in CBF was detected using the (133)Xe-SPECT method, whereas no changes in any ICG variables were observed after acetazolamide. CONCLUSION: The study shows that it is not possible to obtain reliable BFI data, which reflect changes in CBF after acetazolamide infusion, using the CW-NIRS and ICG method.

Abstract

Water-soluble polymers containing small amounts of hydrophobic groups have been synthesized in an aqueous medium by free radical copolymerization of a hydrophilic monomer (acrylamide) with a micelle-forming cationic polymerizable surfactant. Such hydrophobically modified water-soluble polymers exhibit particulr aqueous solution properties due to attractive hydrophobic interactions and repulsive electrostatic interactions. The competition between these two effects, as well as the balance between intra- and intermolecular interactions, give rise to various rheological behaviors depending on polymer concentration, ionic strength and shear time. Abstract

Water-soluble polymers containing a small amount of hydrophobic groups have been prepared in aqueous medium by radical copolymerization of acrylamide and ethylphenylacrylamide as the hydrophobic comonomer. Two methods of synthesis h ave been used: a "micellar" process in which the use of a surfactant ensures the solubilization of the water insoluble monomer, and a "heterogeneous" process wherein the insoluble monomer is simply dispersed in water. The copolymer compositions have been determined by UV spectrophotometry and the molecular weights by light scaattering. The effect of sample preparation on the rheological behavior is discussed. The more pronounced thickening effect observed for copolymers compared to homopolyacrylamide results from intermolecular hydrophobic associations. Under shear, associations are disrupted and a strong but reversible shear-thinning effect is observed. This dissociation process is relatively slow, as indicated by the thixotropic behavior at high-shear rates. On the contrary, a rheopexic behavior can be observed at low-shear rates. 40. Fluorocarbon−Hydrocarbon Nonionic Surfactants Mixtures:  A Study of Their Miscibility

Abstract

Binary mixtures of fluorocarbon and hydrocarbon nonionic surfactants derived from the tris(hydroxymethyl)acrylamidomethane (THAM) have been examined by 19F NMR spectroscopy and UV−visible spectroscopy in the presence of pinacyanol chloride as a probe. Aqueous solutions have been studied as a function of total surfactant concentration over a range of fluorocarbon/hydrocarbon ratios. For molar fractions in fluorinated surfactant lower than 0.7, the variations of both the 19F chemical shift and of the absorbance show two inflection points as a function of surfactant concentration. This was assigned to two critical micelle concentrations (cmc's). Above the second cmc, two kinds of micelles (fluorocarbon-rich and hydrocarbon-rich) should coexist as a result of the incompatibility between the two types of surfactants. The experimental results cannot be adequately described by current models on demixing micellar systems. An explanation of this behavior is suggested, by taking into account the interactions between the polyhydroxylated headgroups of the surfactants.

Abstract

The phase behavior and the rheological properties of aqueous solutions containing mixtures of multiblock hydrophobically modified polyacrylamides (HMPAM) have been investigated. Mixtures of a HMPAM with a homopolyacrylamide (PAM) phase separate above a given overall polymer concentration and a given PAM content. These results are in qualitative agreement with the predictions of a model based on the entropy changes due to the associations between the hydrophobic units [Annable, T.; Ettelaie, R. Macromolecules 1994, 27, 5616]. The viscoelastic behavior of semidilute solutions of various series of copolymer mixtures with variable molecular weights (Mw ≈ 105 to 2 × 106), hydrophobe contents ([H] = 0.5−2 mol %), and hydrophobic block lengths (NH ≈ 2−7 units per block) has been studied using steady-flow and oscillatory experiments. The linear viscoelasticity can be described by a slow relaxation process and other faster complex relaxation processes. When both HMPAM are mutually entangled, the slow relaxation is in agreement with the expectation from a hindered reptation model with in particular a plateau modulus that only depends on polymer concentration. When the two HMPAM are unentangled, the behavior is more complex, the properties being strongly dependent on the hydrophobe characteristics of the two copolymers. In particular, when there is a strong mismatch between the hydrophobic parameters, the viscosity of the system is depressed likely due to a local segregation between the two copolymers.

44. Effect of Surfactant on the Viscoelastic Behavior of Semidilute Solutions of Multisticker Associating Polyacrylamides

Abstract

The interactions between well-defined hydrophobically modified polyacrylamides (HMPAM) and the anionic surfactant SDS (sodium dodecyl sulfate) or the cationic surfactant DTAB (dodecyltrimethylammonium bromide) were studied in aqueous solution using steady-flow and oscillatory rheological experiments. The structure of the HMPAM used consists of randomly distributed blocks of hydrophobic monomers (N,N-dihexylacrylamide) in the polyacrylamide backbone. A thorough investigation of the rheological behavior of HMPAM/surfactant mixtures as a function of polymer concentration and of different molecular parameters (molecular weight, hydrophobe content, and hydrophobic block length) provides some novel insight into the surfactant binding properties of multisticker chains. The experiments were performed on series of HMPAM located both in unentangled and entangled semidilute regimes. In both regimes, the rheological behavior strongly depends on the level of surfactant addition, with first an increase and then a decrease in the values of the zero-shear viscosity η0 and of the terminal relaxation time TR without significant change in the plateau modulus G0. The latter observation that differs from most of the other reported conclusions suggests that the viscosity enhancement is not due to the formation of additional mixed plurifunctional aggregates, but to an increase in the lifetime of the preexisting cross-links resulting from surfactant binding. At high surfactant concentrations (≳0.05 M), the solubilization of the individual hydrophobic blocks by the surfactant micelles leads to a decrease of G0, η0, and TR and the HMPAM lose their associative properties. The results also suggest that the maximum of viscosity corresponds to an optimum surfactant binding which is about the same, whatever the concentration of hydrophobic units in the HMPAM solution. SDS has a much larger effect on the η0 variation than DTAB, likely due to a different lifetime of the mixed aggregates.

In the semidilute range, two different regimes can be clearly distinguished in the zero-shear viscosity η0 = f(C) curves:  a first unentangled regime where the viscosity increase rate strongly depends on NH and [H]; a second entangled regime where the viscosity follows a scaling behavior of the polymer concentration with an exponent close to 4, whatever [H] or NH. The linear viscoelasticity can be described by (i) a slow relaxation process with a plateau modulus that only depends on polymer concentration and (ii) other faster complex relaxation processes. In the latter regime, the results can be quite well accounted for by a hindered reptation model.

46. Multicompartment Polymeric Micelles Based on Hydrocarbon and Fluorocarbon Polymerizable Surfactants

Abstract

The synthesis of multicompartment polymeric micelles (MCPMs) has been achieved by aqueous radical terpolymerization of a water-soluble monomer (acrylamide) with both hydrocarbon (H) and fluorocarbon (F) surfactants (surfmers) in the micellar state. The selected H- and F-surfmers are CH2&dbd;CHCON(C2H5)CH2CH2N(CH3)2CH2COOC16H33/Br/Cl (HS6) and CH2&dbd;CHCONHCH2CH2N(CH3)2CH2COOCH2CH2C8F17/Br (FS3), respectively. Their mutual incompatibility in aqueous solution has been checked by conductivity and surface tension experiments. Two cmc values are found, in favor of the coexistence of two distinct types of micelles at surfactant concentrations above 1 mmol/L (second cmc) over a broad composition range. The solubilization properties of the pure and mixed surfactant systems have been studied for different hydrophobic probes. Significant differences in the solubilization capacity occur due to the nature of the dye, of the surfactant, and of the micelle shape and composition. A kinetic study on the incorporation behavior of the H- and F-surfmers in the polyacrylamide backbone during a batch polymerization shows a compositional drift as a function of conversion which is attributed to micellar effects. A semicontinuous process has been designed which allows the correction for this compositional drift. The presence of well-segregated H- and F-microdomains in terpolymers could be inferred from viscosity and fluorescence experiments. Furthermore, the solubilization of decalin is enhanced in terpolymer solutions with respect to that of copolymer solutions. This was attributed to the formation of larger hydrocarbon cores in the former case due to repulsive H/F interactions in the polymeric chains.

49. Influence of the Hydrophobe Structure on Composition, Microstructure, and Rheology in Associating Polyacrylamides Prepared by Micellar Copolymerization

Abstract

Acrylamide polymers modified with low amounts of alkyl- or alkylarylacrylamides (1−5 mol %) have been prepared by an aqueous micellar copolymerization technique. This method is known to lead to multiblock copolymers in which the number and length of the hydrophobic blocks vary with the initial number of hydrophobes per micelle. The incorporation behavior of different types of hydrophobes and their effects on the rheological copolymer properties have been investigated. Interestingly, the use of disubstituted acrylamides leads to an average copolymer composition independent of the degree of conversion, in contrast to what is observed with monosubstituted acrylamides. Solubility measurements of both types of hydrophobes indicate that the micellar dynamics is not responsible for this behavior, but rather the difference in polarity between the bulk phase and the micellar phase. This microenvironment effect modifies the reactivity ratios of those hydrophobes capable of forming hydrogen bonds, whereas the reactivity of the other hydrophobes remains unaffected. The rheological properties of the samples are discussed in terms of copolymer microstructure and type of hydrophobe used (bulkiness, degree of branching, and alkyl chain length). For example, at similar hydrophobe levels, double-chain hydrophobes considerably enhance the thickening efficiency with respect to single-chain hydrophobes.
