*Çayır, Serkan ( Aksaray, Yazar ), Objective: This study aimed to determine how prevalent allergic rhinitis (AR) is in Turkey and to compare the current prevalence with the figures obtained 10 years earlier. Methods: This study included 9,017 participants. The minimum number of participants required from each center was determined via a stratified sampling technique according to regional demographic characteristics as ascertained from the last census. For each region, both men and women were administered the score for allergic rhinitis (SFAR) questionnaire and a score for each participant was calculated based on the responses supplied. Results: A total of 9,017 individuals (55.3% men and 44.7% women) took part in this study. Of these, 94.4% were urban residents and 5.6% lived in a rural setting. Of the men, 38.5% self-reported as suffering from AR. The corresponding figure in women was 40.5%. The overall prevalence of AR, as deduced on the basis of the SFAR, was found to be 36.7%. Comparing the prevalence in different regions, we found that AR was the least prevalent in the Black Sea region with a frequency of 35.8%. The highest prevalence was in the Mediterranean region, where the prevalence was 37.7%. There was no statistical significance in the apparent differences in prevalence between different geographical regions. Despite this, however, there was a clear increase in the frequency of AR over the preceding decade. This increase was most pronounced in the South-Eastern Anatolian region, where the frequency rose from 21.0% to 36.9%. Conclusion: Our results indicate that there has been a marked increase in the prevalence of AR in every region in Turkey over the last 10 years. This could be related to living conditions in urban environments. Alterations in lifestyle, urban living, air pollution causing impairments in immune defense mechanisms, and other aspects of modern lifestyles may account for the increase in AR in Turkey.

Amaç: Tıkayıcı uyku apnesi sendromu olan hastalarda başarılı cerrahi için obstrüksiyon alanlarının ve obstrüksiyon boyutunun değerlendirilmesi ve tesbiti için en uygun tanısal yöntemi saptamaktır.Gereç ve Yöntem: Çalışmamıza horlama şikayeti ile başvuran septoplasti operasyonu sonrası polisomnografisi de yapılan 40 hasta dahil edildi. Hastaların yaş, cinsiyet, Epworth uyku skalası, vücut kitle endeksi, modifiye mallampati skoru, Müller manevrası ve uyku endoskopisi değerlendirildi. Hastaların Müller manevrasında ve uyku endoskopisinde tıkanıklığı %50 üzerinde olanlar ciddi obstrüksiyon olarak değerlendirildi. Bulgular: Laringeal bölgede uyku endoskopisi ile modifiye mallampati skorları arasında anlamlı farklılık tesbit edilmedi. Uyku endoskopisi ve Müller manevrası değerlendirildiğinde iki yöntem arasında uvulopalatal bölgede, lateral faringeal band ve dil kökünde anlamlı farklılık izlendi. Sonuç: Uyku sırasında önemli değişiklikleri gözlemlenmesinden dolayı uyku endoskopisi daha sık olarak tercih edilmektedir. Çalışmamızda üst seviyelerde uyku endoskopisinin avantajlarını destekleyen ve öğreten veriler elde edilmiştir. Uyku endoskopisi üst hava yollarının değerlendirilmesinde daha faydalı bilgiler verir., Purpose: The aim of this study was to determine the most appropriate diagnostic method for the detection of the site and configuration of obstructed regions for the successful management of patients with obstructive sleep apnea. Materials and Methods: The study included 40 patients with snoring complaints who underwent septoplasty surgery followed by polysomnography. Evaluation was made in respect of age, gender, Epworth Sleepiness Scale, bodymass index (BMI),modified Mallampati scoring, Müller’s maneuver, and sleep endoscopy data. Patients with collapse of >50% in Müller’s maneuver and sleep endoscopy were considered as serious obstructions.Results: No significant difference was observed between the assessments of Mallampati scoring and sleep endoscopy in the laryngeal region. In the comparison of sleep endoscopy and Müller’s maneuver, significant differences were observed in the lateral band, uvulapalatal region and tongue base.Conclusion: Sleep endoscopy is increasingly preferred since it enables the observation of important changes during sleep. The data of the current study support the view that sleep endoscopy is a more advantageous method at the upper levels as useful information is provided for upper airway evaluation.

GİRİŞ ve AMAÇ: Biz bu çalışmada apne veya hipopne belirgin obstrüktif uyku apne sendrom (OSAS)’lu hastalarda, uyku yapısı başta olmak üzere, vücut kütle indeksi (BMI), epworth uykululuk skalası (ESS), eşlik eden hastalıklar ve üst hava yolu morfolojisindeki farklılıkları değerlendirmeyi amaçladık.YÖNTEM ve GEREÇLER: Hipopne oranı %50' den fazla olan hastalar hipopne grubuna alınırken apne oranı %50' den fazla olan hastalar apne grubuna alındı. Hastaların BMI, ESS ve polisomnografi (PSG) parametreleri kaydedildi. Bu yazıda, literatürde nadir izlenen ve submandibüler kitle nedeniyle polikliniğimize başvuran ektopik tiroid dokusu olgusu sunuldu., Ectopic tyhroid tissue is a rarely seen benign congenital anomaly. It can cause very serious problems acording to localization. Ectopic tyhroid tissue commonly locates between foramen cecum and medial line of mediasten. In this article we present ectopic tyhroid tissue case which admitted to our outpatient clinic with rarely seen in literature submandibular mass.

Dermoid kistler boyun orta hattında herhangi bir bölgede bulunabilmekle birlikte, ağız tabanında nadir görülür. Cerrahi öncesi kistin boyun uzanımı doğru belirlenip intraoral veya transservikal yaklaşımla tam olarak çıkarılmaları yeterli tedaviyi sağlayacaktır. Ancak mylohiyoid kası aşıp boyuna uzanan kitlelerde intraoral yolla yetersiz kitle eksizyonu nüksle sonuçlanacaktır. Biz yazımızda intraoral yolla yetersiz tedavi edilen ve bu nedenle nükseden, transservikal yaklaşımla tedavi ettiğimiz bir olgu ile sublingual dermoid kistleri hatırlatmak istedik., Dermoid cysts of the floor of the mouth are rare, although they may involve any area in the midline of the neck. By determining the correct extension of the cyst through the neck prior to surgery, total excision by intraoral or transcervical approach will provide adequate management. However, inadequate intraoral excision of the mass that exceeds mylohyoid muscle and extends through neck will result in recurrence. In this article, we wanted to remind the sublingual dermoid cysts with a case having previous intraoral insufficient excision and, therefore, recurrent mass excised via transcervical approach.

Amaç: Birincil ve düzeltme ameliyatı uygulanan hastalarda kronik otitis media, düzeltme ameliyatı tipleri, kolesteatomun en sık gözlendiği bölgeler, işitme sonuçları ve greft zar durumları değerlendirildi. Hastalar ve Yöntemler: Mayıs 2003 - Mart 2009 tarihleri arasında kliniğimizde kronik otitis media tanısıyla ameliyat edilen 495 hastadan düzeltme ameliyatı yapılan 43 hasta 21 erkek, 22 kadın; ort yaş. 38.4±15.2 yıl; dağılım 15-76 yıl geriye dönük olarak değerlendirildi. Düzeltme ameliyatı endikasyonu 32 hastada %74.4 hastalığın nüksetmesi, 11 hastada %25.6 işitmenin rekonstrüksiyonu idi. Düzeltme ameliyatı 40 hastaya %93.0 bir kez uygulanırken, üç %7.0 hastaya iki kez uygulandı. Birincil ve düzeltme ameliyatı sonrası ortalama 3.2 dağılım 1-6 yıllık takiplerde hastaların düzeltme ameliyatı tipleri, kolesteatomun en sık gözlendiği bölgeler, işitme sonuçları ve greft zar durumları değerlendirildi. Bulgular: Düzeltme ameliyatında kolesteatom nüksü 28 hastada %65.1 gözlendi. Yirmi yedi hastanın, 23’ünde %85.1 greft zar intakt, dördünde %14.8 ise perfore olarak izlendi. Sonuç: Düzeltme ameliyatında ilk hedef hastalığı ortadan kaldırmaktır. Kolesteatomlu kronik otitis medianın tedavisi ameliyattır. Nüks oranlarının yüksekliği nedeniyle ameliyat sonrası dönemde uzun süre takip gereklidir., Objectives: In patients who underwent primary and revision surgery for chronic otitis media, the types of revision surgery, most frequently observed regions of cholesteatoma, hearing results and the status of graft membrane were evaluated. Patients and Methods: Forty-three of 495 patients 21 males, 22 females; mean age 38.4±15.2 years; range 15 to 76 years with chronic otitis media who underwent revision surgery in our clinic between May 2003 and March 2009 were evaluated retrospectively. Indications for revision surgery were recurrence of the disease in 32 patients 74.4% and reconstruction of hearing in 11 patients 25.6% . Forty patients 93.0% underwent revision surgery once and three patients 7.0% underwent revision twice. Over an average of 3.2 range 1 to 6 years follow-up after primary and revision surgery, the types of revision surgery, the most frequently observed regions of cholesteatoma, hearing results and the status of graft membrane were assessed. Results: Recurrence of cholesteatoma in revision surgery was seen in 28 patients 65.1% . Out of 27 patients, 23 patients 85.1% showed intact graft membranes and four patients 14.8% showed perforated graft membranes. Conclusion: In revision surgery, the first goal is to eliminate the disease. The management of chronic otitis media with cholesteatoma is surgery. Because of high postoperative recurrence rates, long-term followup is necessary.

Amaç: Larengeal tümörlü hastalarda, endolarengeal lezyon tayininde bilgisayarlı tomografi BT sanallarengoskopinin etkinliği araştırıldı.Hastalar ve Yöntemler: Larengeal kanserli 21 hastada kontrastlı aksiyel BT kesitlerinden sanal larengoskopi görüntüleri elde edildi. Hastaların rijit teleskopik videolarengoskopik muayene bulguları ve sanal larengoskopi bulguları ameliyat kayıtlarıyla karşılaştırıldı.Bulgular: Dil kökü, priform sinüs, ariepiglottik bantve aritenoid seviyelerindeki lezyonlar her iki yöntemle de güvenilir bir şekilde gösterilebildi. Ventriküler bant, ventriküler kavite ve anterior komissürseviyelerindeki lezyonlar için her iki yöntem de güvenilir değildi. Subglottik bölge ve vokal kordlardasanal larengoskopinin lezyon tayini daha güvenilirbulundu.Sonuç: Sanal larengoskopi endolarengeal yüzeylerin görüntülenmesinde ve tümör yayılımının saptanmasında invaziv olmayan ve güvenilir bir yöntemdir.Larenks kanserli hastalarda cerrahi öncesi evrelemeve uygun cerrahi tekniğin seçiminde yararlı olabilir, Objectives: We evaluated the utility of computed tomography Virtual laryngoscopy CTVL in identifying endolaryngeal lesions in laryngeal tumors.Patients and Methods: Virtual laryngoscopic images were obtained from axial CT seans of 21 patients with known laryngeal carcinoma. Findings from rigid telescopic videolaryngoscopy RTV and CTVL images were evaluated and compared with reference to operative records.Results: Lesions localized in the base of the tongue, pyriform sinüs, aryepiglottic folds, and arytenoids were well visualized by both RTV and CTVL. The two techniques were not found effective in identifying lesions of the ventricular bands, ventricular cavities, and the anterior commissure. Virtual laryngoscopy was superior to RTV in the visualization of the sub- glottic area and vocal cords.Conclusion: Virtual laryngoscopy is a noninvasive and reliable technique that provides visualization of endolaryngeal surfaces and tumor extension. İt may be beneficial in staging larynx carcinoma and plan- ning the most appropriate surgical procedure.

Amaç: Rekürren sekretuar otitis mediası SOM olanerişkin hastalarda, göreceli yeni bir yöntem olan Phadiatop®ile atopi varlığı araştırıldı; yöntemin duyarlılığı,özgüllüğü ve tarama testi olarak kullanılabilirliği incelendi.Hastalar ve Yöntemler: Çalışmaya rekürren SOM tanısı alan 84 hasta alındı. Hastalarda öykü, laboratuvarbulguları, deri testi ve total IgE düzeyi incelendi. Enaz bir deri testi pozitif ve/veya total IgE sonucu yüksek, öyküsü pozitif olan hastalar atopik olarak kabuledildi.Bulgular: Belirlenen kriterlere göre rekürren SOM’uolan 84 hastanın 31’inde atopi saptandı. Atopik olmayan 53 hastanın hepsinde ve bir atopik hastada Phadiatop®değerleri negatif bulundu. Otuz bir atopik hastanın 30’unda Phadiatop®değerleri pozitif bulundu Özgünlük:1.00, duyarlılık: 0.97, pozitif tahmin değeri: 1.00,negatif tahmin değeri: 0.98 .Sonuç: Bu bulgular, rekürren SOM’u olan hastalardaatopinin saptanması için Phadiatop®’un etkin bir taramatesti olduğunu göstermektedir, Objectives: This is a preliminary report of a relatively new method: Phadiatop®, in screening atopic adult patients with recurrent secretory otitis media SOM .The sensitivity and specificity, and its use in atopic adult patients with recurrent SOM are described.Patients and Methods: Eighty four patients 54 females, 30 males; mean age 36.7; range 19 to 62 years who were diagnosed as recurrent SOM were included in the study.The patients were evaluated with detailed history, labora- tory tests, skin-prick test and total IgE. Patients with at least one positive skin-prick test and/or high total IgE with a positive history were considered to be in atopic status.Results: According to certain criteria, atopy was detected in 31 of the 84 patients. Negative Phadiatop® values were found on ali of 53 non-atopic patients as well as in one atopic patient. Among 31 patients having atopy, 30 of them shovved positive Phadiatop® values Specificity: 1.00, sensitivity: 0.97, predictive value, positive: 1.00, predictive value, negative: 0.98 .Conclusion: These results indicate that Phadiatop® is an effective test to detect atopy in patients with recurrent SOM.

Ekstranodal lezyonlar Rosai-Dorfman hastalığının RDH tek bulgusu olabilir. Baş-boyun bölgesi RDH’de en sık tutulan bölgelerden biri olmasına karşın, hasta- lığın larengeal tutulumu çok nadirdir. Kırk dört yaşında bir kadın hasta ilerleyici nefes darlığı yakınmasıyla başvurdu. Öyküsünden, astım için üç yıldır tedavi gör- düğü ve kendisini 15 yıldır etkileyen bir burun kitlesi nedeniyle ameliyat geçirdiği, bu lezyona da rinosklero- ma tanısı konduğu öğrenildi. Fizik muayenede, sol alt gözkapağında, sağ epikantal bölgede ve sol önkolda üç adet subkutan lezyon görüldü. Lenfadenopatiye rastlanmadı. Larengoskopik incelemede, subglottik bölgede, 1 cm çapında üç adet solid polipoid kitle gözlendi. Larenksteki kitleler ve subkutan lezyonlar iki aşamalı bir ameliyatla çıkarıldı. Histolojik incelemede örneklerin hepsinde histiosit proliferasyonu görüldü. İmmünhistokimyasal boyamada tipik S-100 proteini için pozitiflik gösteren histiositler ve emperipolez göz- lendi. Larenks kitleleri ve subkutan lezyonların tanısı RDH şeklinde kondu. Hastanın daha önce geçirmiş olduğu ameliyatta çıkarılan burun kitlesine ait kesit- lerin yeniden incelenmesi aynı morfolojik özellikleri gösterdi. Ameliyattan dokuz ay sonra hasta sağlıklıydı ve nüks yoktu, Extranodal lesions may be the sole manifestation of Rosai-Dorfman disease RDD . Although the head and neck region is one of the most common extranodal sites, laryngeal involvement is very rare. A 44-yearold woman presented with a complaint of progressive dyspnea. She had a three-year history of treatment for asthma and a history of operation for a nasal mass that afflicted her for 15 years and was diagnosed as rhinoscleroma. On physical examination, she had three subcutaneous lesions, in the left lower eyelid, right epicanthal area, and left forearm, respectively. No lymphadenopathy was present. Laryngoscopic examination revealed three solid, polypoid masses in the subglottic region, 1 cm in diameter. With a two-staged operation, the laryngeal masses were excised totally together with the subcutaneous lesions. Histological examination of all the specimens showed proliferation of histiocytes. Immunohistochemical staining revealed typical S-100 protein-positive histiocytes and emperipolesis. Both laryngeal and subcutaneous lesions were diagnosed as RDD. A re-evaluation of sections from the previous operation specimen of the nasal mass showed the same morphological features. The patient was healthy without recurrence, nine months following surgery.

Hastaların CPAP tedavisi öncesi ve sonrası altıncı ayda tam kan sayımları ölçüldü ve ortalama trombosit hacim OTH değerleri karşılaştırıldı.Bulgular: Şiddetli TUAS’lı hastaların CPAP öncesi OTH değerleri ortalama 8.9±1.3 p, Objectives: This study aims to show the effects of nasal continuous positive airway pressure CPAP treatment on platelet activation in patients with severe obstructive sleep apnea syndrome OSAS . Amaç: Akut epiglottit nedeniyle tedavi edilen erişkinolgular değerlendirildi.Hastalar ve Yöntemler: Çalışmaya sekiz aylık birdönem içinde kliniğimizde akut epiglottit saptananbeş erişkin hasta sunuldu. Hastaların tümü erkektive 77 yaşında bir hasta dışında, yaşları 38-54 arasında değişmekteydi. Hastaların başvuru anındakisolunum sıkıntısı ve semptomların derecesi Friedman sınıflamasına göre değerlendirildi. Hastalaraparenteral steroid ve antibiyotik tedavisi uygulandı.Ortalama takip süresi bir yıl dağılım 11-17 ay idi.Bulgular: Tüm hastalarda asıl başvuru yakınmasısolunum sıkıntısıydı. Solunum sıkıntısının derecesibir hastada Friedman evre 4, iki hastada 3, iki hastada 2 idi. Tanı, larenks muayenesi ve boyun yan filmi ile kondu. Boyun radyografisinde tüm hastalardakalınlaşmış epiglot, “başparmak belirtisi” izlendi. Boyun ve larenks bilgisayarlı tomografisinde bir olgudaepiglot apsesi görüldü. Bir hastada trakeotomi gerekti. Parenteral steroid ve antibiyotik tedavisi sonucunda tüm olgularda solunum sıkıntısı geçti ve yutma güçlüğü düzeldi. Hastanede kalış süresi ortalama 5-6 gün dağılım 3-8 gün idi. Takip sırasındahiçbir hastada nüks görülmedi.Sonuç: Akut epiglottitli erişkin hastalarda doğru tanızamanında konmalı, zaman kaybetmeden uygun tedaviye başlanmalı ve gerektiğinde trakeotomidenkaçınılmamalıdır, Objectives: We evaluated adult patients who were treated for acute epiglottitis.Patients and Methods: The study included five adult patients who were diagnosed as having acute epiglot titis during an eight-month period. AH the patients were males with an age range of 38 to 54 years, except for a 77-year-old patient. The Friedman staging system was used to evaluate the severity of dyspnea and symptoms. Treatment was comprised of parenteral steroids and antibiotic therapy. The mean follow-up was one year range 11 to 17 months .Results: The main presenting symptom was moderate to severe dyspnea in ali the patients, which was rated as stage 4 in one patient, stage 3 in two patients, and stage 2 in two patients. Diagnoses were made by laryn- goscopic examination and lateral radiograms of the neck, which showed the thumb sign in ali the patients.Computed tomography of the neck revealed an epiglot- tic abscess in one patient. One patient required tra- cheotomy. AH the patients benefited from parenteral steroid and antibiotic treatment, with relief of dyspnea and difficulty in swallowing. The mean hospitalization was 5-6 days range 3 to 8 days . No recurrences were observed during the follow-up period.Conclusion: Acute epiglottitis in adult patients requires a timely and correct diagnosis and prompt appropriate treatment. A tracheotomy should not be avoided when necessary.
37. The role of upper airway morphology in apnea versus hypopnea predominant obstructive sleep apnea patients: an exploratory study.

The objective of this study was to investigate the association between upper airway morphology and apnea vs hypopnea predominance in patients with obstructive sleep apnea (OSA) syndrome. A retrospective study on consecutively CT scans obtained from 54 OSA patients and 53 non-snoring controls. CT scans were measured to evaluate upper airway and surrounding structures' morphologic characteristics. OSA patients (matched for age, gender and body mass index) compared as two groups; apneic group: apnea ratio >50% and hypopneic group: hypopnea ratio >50%. Morphologic characteristics were analyzed using Kruskal–Wallis test and Pearson's correlation test. Apneic group (17 patients) showed 76.56% apnea rate and hypopneic group (37 patients) showed 78.46% hypopnea rate. Minimal lateral and anteroposterior dimensions of velopharynx in the apneic group (0.86 ± 0.73 and 0.21 ± 0.13 cm, respectively) was statistically lesser from that of the hypopneic group (1.2 ± 0.42 and 0.54 ± 0.22 cm, respectively). Minimum cross-sectional area of the velopharynx was also lesser in apneic group (0.21 ± 0.16 cm
2 ) than that in hypopneic group (0.65 ± 0.38 cm2 ). Almost all upper airway parameters in both apneic and hypopneic groups were tended to be smaller than in controls. Decrease in airway volume does not signify the type of respiratory event, but significant narrowing of velopharynx in both dimensions; thus having the narrowest value below a certain level causes more apnea. We did not find a similar study when we did a literature search, showing the relationship of apnea vs hypopnea predominance and upper airway parameters in CT in patients with OSA. 38. Ginkgo Biloba and Lycopene are Effective on Cisplatin Induced Ototoxicity?

OBJECTIVE: The purpose of this study was to examine the anti-ototoxic impact of Ginkgo biloba extract and lycopene on the model of cisplatin-induced ototoxicity in rats. MATERIALS and METHODS: Thirty-two Wistar albino rats were examined with the distortion product otoacoustic emission (DPOAE) test (MADSEN Capella2 ; GN Otometrics, ICS Medical, Chicago USA), and they were randomly divided into four groups. Group 1 (n=8) was defined as the healthy control group. Cisplatin was given intraperitoneally as single dose of 12 mg/kg to group 2 (n=8), group 3 (n=8), and group 4 (n=8). Group 2 was determined as ototoxic control group. G. biloba extract (100 mg/kg) was given to group 3, and 20 mg/kg lycopene was given to group 4 with orogastric feeding tube daily for 10 days. DPOAE test was repeated on day 10 on all the groups. Finally, histopathological examination was performed. The study has been lead in agreement with the principles by the Institutional Animal Care and Use Committee Review Board at Kocaeli University Medical Center (KOU HADYEK- 1/9-14). The animals were treated in accordance with protocols approved by this committee. RESULTS: When DPOAE tests were compared, there was no significant difference in the four groups before the application (p>0.05). At the end of day 10, in groups 2 to 4, statistically significant changes were observed (p<0.05). According to the cisplatin group, a significant increase in the DPgrams on G. biloba and lycopene groups was observed (p<00.5). Corti organ and spiral ganglion neurons of groups 1, 3, and 4 were observed to have weak expression. Strong reactions were determined in organum spirale and some spiral ganglions of the cisplatin group. The striae vascularis damage on group 2 was found to be more significant more compared with groups 3 and 4. CONCLUSION: There is a protective effect of G. biloba and lycopene on cisplatin-dependent ototoxic rat model. [ABSTRACT FROM AUTHOR]

39. THE EFFECT OF PATIENTS' HABITS OF INTERNET USE FOR HEALTH CONSULTATIONS ON THE PHYSICIAN-PATIENT RELATIONSHIP.

- Abstract
Objective: The cim of this study was to determine the patients' habits of Internet use for health consultations and by means of this data to foresee the effect of the Internet on the physician-patient, relationship. Material and Method: A questionnaire was given to Group 1 (n=1330, using the Internet for health consultations) and Group 2 (n=1280, not using the Internet for health consultations) patients. Patients were analyzed based on their responses to questions, age, gender and education levels. Results: Patients using the Internet for health consultations are younger (p<0.001), more educated (p<0.001), and mainly male in gender (p=0.028). Patients stated their reason for using the Internet mostly just to get a second opinion (41.1%). 5.1% of patients stated that they rely on the information obtained from the Internet more than their physician. Conclusion: The Internet seems to be an important source of health-related information for patients, but it is not the primary source. The rate of patients who seems to displayed negative behaviors that were likely to adversely affect the physician-patient relationship was small. [ABSTRACT FROM AUTHOR]

- Abstract
The complications of bone anchored hearing aids are very rare. The most seen postoperative complication is soft tissue reaction locally occuring around the titanium implant. Chronic headache is a rarely seen complication related to bone anchored hearing aids application. We presented a patient having complaints of chronic headache and burning sensation after bone anchored hearing aids application. Follow up for one year after the operation, despite the local and systemic therapies, there was no improvement of symptoms and the device was compulsorily removed. This rare complication of bone anchored hearing aids application is reviewied in the literature and probable reasons of failure are discussed.

Complete blood counts were analyzed and mean platelet volume (MPV) values were compared in patients before and after CPAP treatment at six months., Results: The mean MPV values before CPAP were 8.9±1.3 (p<0.001), while the mean values were 7.7±1.1 (p<0.001) at six months after CPAP treatment in patients with severe OSAS. There was a statistically significant decrease in the MPV values after CPAP treatment. Patients' number of platelets which was measured six months after the CPAP treatment increased at a significant level compared to baseline (p<0.001)., Conclusion: Continuous positive airway pressure treatment caused a significant decrease in the MPV values in patients with severe OSAS. As a negative acute phase reactant, MPV can be used as a marker to exert the cardioprotective effect of CPAP.

Objectives: In patients who underwent primary and revision surgery for chronic otitis media, the types of revision surgery, most frequently observed regions of cholesteatoma, hearing results and the status of graft membrane were evaluated., Patients and Methods: Forty-three of 495 patients (21 males, 22 females; mean age 38.4+/-15.2 years; range 15 to 76 years) with chronic otitis media who underwent revision surgery in our clinic between May 2003 and March 2009 were evaluated retrospectively. Indications for revision surgery were recurrence of the disease in 32 patients (74.4%) and reconstruction of hearing in 11 patients (25.6%). Forty patients (93.0%) underwent revision surgery once and three patients (7.0%) underwent revision twice. Over an average of 3.2 (range 1 to 6) years follow-up after primary and revision surgery, the types of revision surgery, the most frequently observed regions of cholesteatoma, hearing results and the status of graft membrane were assessed., Results: Recurrence of cholesteatoma in revision surgery was seen in 28 patients (65.1%). Out of 27 patients, 23 patients (85.1%) showed intact graft membranes and four patients (14.8%) showed perforated graft membranes., Conclusion: In revision surgery, the first goal is to eliminate the disease. The management of chronic otitis media with cholesteatoma is surgery. Because of high postoperative recurrence rates, long-term follow-up is necessary.

- Abstract
Extranodal lesions may be the sole manifestation of Rosai-Dorfman disease (RDD). Although the head and neck region is one of the most common extranodal sites, laryngeal involvement is very rare. A 44-year-old woman presented with a complaint of progressive dyspnea. She had a three-year history of treatment for asthma and a history of operation for a nasal mass that afflicted her for 15 years and was diagnosed as rhinoscleroma. On physical examination, she had three subcutaneous lesions, in the left lower eyelid, right epicanthal area, and left forearm, respectively. No lymphadenopathy was present. Laryngoscopic examination revealed three solid, polypoid masses in the subglottic region, 1 cm in diameter. With a two-staged operation, the laryngeal masses were excised totally together with the subcutaneous lesions. Histological examination of all the specimens showed proliferation of histiocytes. Immunohistochemical staining revealed typical S-100 protein-positive histiocytes and emperipolesis. Both laryngeal and subcutaneous lesions were diagnosed as RDD. A re-evaluation of sections from the previous operation specimen of the nasal mass showed the same morphological features. The patient was healthy without recurrence, nine months following surgery.

- Abstract
Objectives: We evaluated adult patients who were treated for acute epiglottitis., Patients and Methods: The study included five adult patients who were diagnosed as having acute epiglottitis during an eight-month period. All the patients were males with an age range of 38 to 54 years, except for a 77-year-old patient. The Friedman staging system was used to evaluate the severity of dyspnea and symptoms. Treatment was comprised of parenteral steroids and antibiotic therapy. The mean follow-up was one year (range 11 to 17 months)., Results: The main presenting symptom was moderate to severe dyspnea in all the patients, which was rated as stage 4 in one patient, stage 3 in two patients, and stage 2 in two patients. Diagnoses were made by laryngoscopic examination and lateral radiograms of the neck, which showed the thumb sign in all the patients. Computed tomography of the neck revealed an epiglottic abscess in one patient. One patient required tracheotomy. All the patients benefited from parenteral steroid and antibiotic treatment, with relief of dyspnea and difficulty in swallowing. The mean hospitalization was 5.6 days (range 3 to 8 days). No recurrences were observed during the follow-up period., Conclusion: Acute epiglottitis in adult patients requires a timely and correct diagnosis and prompt appropriate treatment. A tracheotomy should not be avoided when necessary.

Objectives: We evaluated the utility of computed tomography virtual laryngoscopy (CTVL) in identifying endolaryngeal lesions in laryngeal tumors., Patients and Methods: Virtual laryngoscopic images were obtained from axial CT scans of 21 patients with known laryngeal carcinoma. Findings from rigid telescopic videolaryngoscopy (RTV) and CTVL images were evaluated and compared with reference to operative records., Results: Lesions localized in the base of the tongue, pyriform sinus, aryepiglottic folds, and arytenoids were well visualized by both RTV and CTVL. The two techniques were not found effective in identifying lesions of the ventricular bands, ventricular cavities, and the anterior commissure. Virtual laryngoscopy was superior to RTV in the visualization of the subglottic area and vocal cords., Conclusion: Virtual laryngoscopy is a noninvasive and reliable technique that provides visualization of endolaryngeal surfaces and tumor extension. It may be beneficial in staging larynx carcinoma and planning the most appropriate surgical procedure.

- Abstract
Objectives: This is a preliminary report of a relatively new method, Phadiatop, in screening atopic adult patients with recurrent secretory otitis media (SOM). The sensitivity and specificity, and its use in atopic adult patients with recurrent SOM were evaluated., Patients and Methods: Eighty-four patients (54 females, 30 males; mean age 36.7; range 19 to 62 years) who were diagnosed as recurrent SOM were included in the study. The patients were evaluated with respect to history, laboratory tests, skin-prick test, and total IgE. Patients with at least one positive skin-prick test result and/or a high total IgE level with a positive history were considered to be in atopic status., Results: According to certain criteria, atopy was detected in 31 of 84 patients. Negative Phadiatop values were found in all non-atopic patients as well as in one atopic patient. Among 31 patients having atopy, 30 patients showed positive Phadiatop values (specificity 1.00, sensitivity 0.97, positive predictive value 1.00, negative predictive value 0.98)., Conclusion: These results indicate that Phadiatop is an effective test to detect atopy in patients with recurrent SOM.
