Las evaluaciones médicas ocupacionales (EMO) se definen como exámenes que determinan las condiciones de salud física y mental de un trabajador antes de su contratación, durante su permanencia en la organización o una vez finalizado el vínculo laboral. Esta investigación tiene como propósito reconocer la importancia y validez de las EMO en la identificación de alteraciones en la función orgánica o mental que puedan afectar el desempeño laboral. Los EMO son parte central de los programas de vigilancia en salud para los trabajadores en el marco de los sistemas de gestión de seguridad y salud en el trabajo. Adicionalmente, se exploró la estandarización de estas evaluaciones y se evaluó su importancia tanto para los trabajadores como para las empresas en términos de seguimiento y estrategias de promoción y prevención de la salud. Teniendo en cuenta el mencionado objetivo, la justificación de esta investigación se basa en la escasez de estudios que destaquen la importancia en salud laboral asociada a estos exámenes. Para abordar esta problemática, se llevó a cabo un análisis bibliométrico y una revisión rápida de la literatura en la cual se encuentra que no se reconoce la importante de las EMO en el desempeño laboral.

This paper verifies whether and how the country's cultural dimensions moderate the relationship between the quality of the audit firm and the level of earnings management. The sample is formed by 88,428 observations from 18,842 companies in 23 countries, covering the period from 2011 to 2018 in an unbalanced panel. The dimensions of national culture were based on the GLOBE project, and the level of earnings management was estimated by Dechow et al. (1995). The results show that national culture influences the monitoring of the largest auditing firms and benefits companies in preparing financial statements. Such results suggest a relationship of audit bias due to cultural aspects, which may compromise the quality of accounting information. [ABSTRACT FROM AUTHOR]

This article emphasizes the importance of graduate follow-up as a means of evaluating and providing feedback to higher education institutions. In conducting these studies, valuable knowledge about alumni experiences and outcomes is gained. this follow-up act has a dual purpose within an institution as to provide valuable self-awareness that can inform the development of improvement policies or institutional direction and also facilitate resource allocation to initiatives that improve academic quality and support projects that involve graduates, thus raising the training of future professionals. That is why the objective of this paper is to know how the follow-up to graduates is constituted in this link with universities and turn highlight the importance of the follow-up to graduates for higher education institutions as an evaluation and feedback strategy. The research was quantitative-descriptive; surveys were carried out among groups of graduates of the Technical University of Babahoyo. Among the results, it is highlighted that a large percentage of respondents are doing work activities directly related to their graduation career are in constant relationship with the university through their programs of study and professional improvement, and are always ready to engage in activities of the institution. In this way, it is possible to understand more clearly the role that higher education institutions play in the training of graduates and what are the requirements that society needs to meet, mainly through the demands of the labor market, as well as the interaction that occurs between the actors that are part of a system of follow- up to graduates. Introduction: a common concern in presurgical medical appointment of total knee replacement medical appointment is return to exercise. The purpose of this study was to analyze functional results and return to sport in patients under 60 years of age after this surgery. Material and methods: we retrospectively analyzed 41 total knee replacements in 36 athletic patients (average age: 53 years [46-60]). Average follow-up of two years (6 months-5 years). Diagnoses: 37 osteoarthritis, three sequelae of Rheumatoid Arthritis, 1 extra-articular deformity. Functional and radiographic outcomes assessed using modified Knee Society and High Activity Arthroplasty Scores. Results: average improvement from 31.95 to 91.61 in KSS and average from 7.95 to 13.73 in HAAS. Return to sport in 3.5 months average (range 2-6 months). Three patients did not return to sport. Conclusions: we consider that delaying surgery in these patients will cause progression in their osteoarthritis pathology and cessation of their sports activities. This makes the surgical technique difficult in addition to reducing the patient's physical performance. Analyzing the survival rate of implants in young patients, more than 80% is reported in a 25-year follow-up. [ABSTRACT FROM AUTHOR]

Introducción: la esclerosis múltiple (EM) es una enfermedad crónica que afecta principalmente a la población joven, además que su diagnóstico y tratamiento tardío generan una alta discapacidad y deterioro en la calidad de vida. La evidencia sobre el uso y la utilidad de las herramientas de salud móvil (mHealth) para el diagnóstico y seguimiento de la EM no ha sido ampliamente explorada, generando un vacío de conocimiento. Por ello, el objetivo de este estudio fue analizar la utilidad de estas herramientas como apoyo en la sospecha diagnóstica y el seguimiento de pacientes con EM. Materiales y métodos: esta fue una revisión de la literatura con una estrategia de búsqueda en: Pubmed, Scopus, ScienceDirect y Web of Science, utilizando términos MeSH. Además, se realizó la selección de artículos siguiendo el flujograma Prisma, y como criterio de inclusión, se revisaron artículos sobre el uso y la aplicación de herramientas tecnológicas en el diagnóstico y seguimiento de pacientes con EM. Resultados: se incluyeron 10 artículos a nivel global que evaluaron a 10?581 participantes, de los cuales 9110 tuvieron un diagnóstico de EM, provenientes de Estados Unidos y Europa, donde el desenlace fue la oportunidad diagnóstica y el seguimiento en calidad de vida. Las mHealth incorporadas evaluaron subjetiva y objetivamente datos clínicos, permitiendo caracterizar factores de riesgo, signos y síntomas, características clínicas y sociodemográficas, entre otras, estableciendo parámetros de diagnóstico temprano. Discusión: las mHealth han demostrado en países desarrollados su utilidad para el diagnóstico temprano, tratamiento y pronóstico de la EM, además de ser innovadoras y efectivas para mejorar la gestión, dada su capacidad de monitorización y seguimiento de datos clínicos. Esta utilidad podría representar enormes beneficios para poblaciones de países en vías de desarrollo. Conclusiones: las mHealth en EM son útiles como apoyo diagnóstico, ya que reducen el tiempo diagnóstico, son aplicables en monitorización e identificación de la sintomatología, orientando a mejores tratamientos y seguimiento.

La presencia de quebrantahuesos Gypaetus barbatus en el macizo de Aralar viene observándose regularmente desde al menos la década de 1990. Previamente, al menos desde la década de 1960, ya se registraban citas ocasionales. Su nidificación en la sierra, no obstante, es aún más reciente (los primeros intentos son de 2017, cuando se construye un nido por primera vez) y no ha sido hasta 2024 cuando del único territorio existente en el macizo ha criado con éxito un pollo. Esto constituye, así, el primer caso de cría con éxito del quebrantahuesos en Aralar en la historia reciente, tras su extinción de Gipuzkoa en torno al año 1920. El nido se localizó en el Aralar navarro, aunque el área de campeo de la pareja se extiende, principalmente, por el Aralar guipuzcoano. Desde que se formara la primera pareja en 2016, han tenido que transcurrir nueve años hasta que la actual pareja haya sido capaz de criar con éxito. ABSTRACT: Presence of the lammergeier Gypaetus barbatus in the Aralar massif has been observed regularly since at least the 1990s. Previously, at least since the 1960s, occasional sightings had already been recorded. Evidence of breeding attempts in this mountain massif, however, is even more recent (the first attempts were in 2017, when a nest was built for the first time) and it was not until 2024 that it successfully raised a chick in the only territory existing within the massif. Therefore, this constitutes the first case of successful breeding of the lammergeier in Aralar in recent times, after its extinction from the zone around 1920. Even though the nest was situated in the Navarran part of Aralar, the main foraging area of this pair extends mainly through the Gipuzkoan part of this massif. Since the first pair was formed in 2016, nine years passed before the current pair was able to successfully breed.

Introduction: Crohn’s disease (CD) is a subtype of chronic and incurable inflammatory bowel disease. It can affect the entire gastrointestinal tract and its etiology is unknown. Objective: The aim of this consensus was to establish the most relevant aspects related to definitions, diagnosis, follow-up, medical treatment, and surgical treatment of Crohn’s disease in Mexico. Material and methods: Mexican specialists in the areas of gastroenterology and inflammatory bowel disease were summoned. The consensus was divided into five modules, with 69 statements. Applying the Delphi panel method, the pre-meeting questions were sent to the participants, to be edited and weighted. At the face-to-face meeting, all the selected articles were shown, underlining their level of clinical evidence; all the statements were discussed, and a final vote was carried out, determining the percentage of agreement for each statement. Results: The first Mexican consensus on Crohn’s disease was produced, in which recommendations for definitions, classifications, diagnostic aspects, follow-up, medical treatment, and surgical treatment were established. Conclusions: Updated recommendations are provided that focus on definitions, classifications, diagnostic criteria, follow-up, and guidelines for conventional medical treatment, biologic therapy, and small molecule treatment, as well as surgical management. Resumen: Antecedentes: La enfermedad de Crohn (EC) es un subtipo de la enfermedad inflamatoria intestinal crónica e incurable que puede afectar a todo el tracto gastrointestinal y cuya etiología es desconocida. Objetivo: Establecer los aspectos más relevantes relacionados a las definiciones, diagnóstico, seguimiento, tratamiento médico y quirúrgico de la enfermedad de Crohn en nuestro país. Material y métodos: Se invitaron a especialistas de la República Mexicana de las áreas de Gastroenterología y Enfermedad Inflamatoria Intestinal. Se dividió el consenso en 5 módulos, con 69 enunciados. Se aplicó el método de panel Delphi, para ello se envió las preguntas previó a la reunión a todos los participantes para que fueran editadas y ponderadas. Durante la reunión presencial se mostraron los artículos seleccionados al nivel de evidencia clínica y se llevó a cabo la discusión y votación final del grado de acuerdo en todos los enunciados. Resultados: Es el primer consenso mexicano de la enfermedad de Crohn en donde se establecen las recomendaciones para las definiciones, clasificaciones, aspectos diagnósticos, seguimiento, tratamiento médico y quirúrgico. Conclusiones: Se dan recomendaciones actualizadas enfocadas a las definiciones, clasificaciones, los criterios diagnósticos, seguimiento y pautas del tratamiento médico convencional, biológico y moléculas pequeñas, así como el manejo quirúrgico.

13. Approach to interstitial lung disease associated with systemic sclerosis—A survey to pulmonologists and rheumatologists in Colombia.

Interstitial lung disease is a leading cause of mortality in patients with systemic sclerosis. Currently, there is a lack of consensus regarding screening, rescreening, diagnosis, and follow-up practices in interstitial lung disease associated with systemic sclerosis (SSc-ILD) in Colombia. A structured survey focused on clinical practices in patients with SSc-ILD was conducted. Members of the Asociación Colombiana de Neumología y Cirugía de Tórax (Asoneumocito) and the Asociación Colombiana de Reumatología (Asoreuma) were invited to participate from March 2023 to May 2023. We surveyed 51 pulmonologists and 44 rheumatologists. Overall, 51.6% reported having access to multidisciplinary team discussion in ILD. Among the 95 participants, 78.9% would routinely perform a high-resolution computed tomography scan of the chest once a diagnosis of systemic sclerosis was established. This practice is more frequent among rheumatologists (84.1%) than among pulmonologists (74.5%). Approximately half of the participants would rescreen patients annually with computed tomography scan (56.8%) if baseline images were negative. Spirometry (81.1%), diffusing capacity of the lung for carbon monoxide (80.0%), and 6-min walk test (55.8%) were the most frequently performed tests upon diagnosis of systemic sclerosis. During follow-up, participants would consider repeating pulmonary function tests mostly every 6 months. Screening of SSc-ILD is high among pulmonologists and rheumatologists. Decision-making on diagnosis and follow-up is similar between specialties, but there are variations in their frequency and indications. Further research is needed to evaluate how to adapt recommendations for assessing SSc-ILD in different settings. [ABSTRACT FROM AUTHOR]

El objetivo de esta investigación, es dar a conocer los resultados del seguimiento, evaluación del impacto y alcance del programa Banco de las Oportunidades -BOA- que se promocionó con el fin de dar oportunidad de acceso al crédito, a aquellas personas vulnerables del departamento que desempeñaban actividades productivas y, que por algún u otro motivo no cumplían con los requisitos de la banca tradicional para desempeñar su actividad económica. Con el resultado positivo que se tuvo de la implementación del programa, se propuso la necesidad de la estructuración de un modelo de seguimiento y evaluación, mediante un estudio de tipología descriptiva de las variables para el modelo, que arrojaran datos estadísticos tanto cuantitativos como cualitativos, con el propósito de validar el impacto del programa en la población objetivo. Para tal fin, fue necesario indagar sobre información de la política e implementación del programa en el departamento y la población beneficiaria. Finalmente, se presentan los resultados y un análisis del comportamiento de la usabilidad de los recursos y la generación de información, que facilitó realizar el seguimiento al proceso de colocación de los recursos ejecutados por el programa BOA en el departamento de Antioquia.

El objetivo de este artículo es plantear una política pública destinada a garantizar un uso adecuado de las medidas cautelares, las unidades de servicios previos al juicio. Estas estructuras administrativas, por un lado, potencian la aplicación de las salidas alternativas a la prisión preventiva, así como, por otro, se ocupan de que su aplicación no sea sinónimo de impunidad e inseguridad. Esta estrategia enfrenta dos problemas centrales de la justicia tradicional, la ausencia de un diagnóstico adecuado para escoger la opción cautelar más adecuada, datos del arraigo social, y el seguimiento al régimen impuesto. La ventaja práctica es que esta propuesta se fundamenta en una metodología probada en el contexto internacional, en el Ecuador y recomendada como una buena práctica por la Comisión Interamericana de Derechos Humanos. De hecho, en Latinoamérica las distintas experiencias cuyos resultados fueron medidos revelan que la aplicación de esta estrategia en diversos momentos alcanzó una efectividad sin precedentes para potenciar medidas sustitutivas y para conseguir la presentación de las personas al juicio. Inclusive, en un momento en el que la prensa y la ciudadanía ejercen presiones para aumentar el encarcelamiento provisional son una respuesta válida, pues el informe emitido transparenta las causas de la resolución judicial adoptada.

20. Optimizing a Novel Tracking System for Living Beings and Objects through Advanced Mathematical Modeling and Graph Theory.

- Abstract
This study extends the formulation of a tracking system for both live items and living persons, and gives a thorough theoretical framework for an advanced tracking system. A large number of tracking systems in use today were created inside certain frameworks and designed to monitor in either infinite or restricted spatial contexts. The latter typically makes use of specialized technological instruments designed with tracking objects or living things in mind. Our contribution to this topic is the formulation of a system theory that both formulates and innovates the challenge of monitoring objects and living things. Graphical modeling is widely used in tracking, which is interesting because it makes it easier to create precise relationships between the objects that need to be tracked and other parts of the system. But our study argues that the best way to achieve a high-performing, contextually relevant, and flexible system in a range of scenarios is still to build a tracking system around graphs, both theoretically and practically. We provide a unique tracking method to further the discipline, based on the ideas of hypergraphs and graph learning. This method carefully examines the order between various linkages inside the system, allowing the system to fully use both direct and indirect relations. The way we formulate tracking is as a complex search problem on graphs and hypergraphs. In this case, the system’s components—living things or objects—are represented by vertices, and the kinds of relationships that exist between them are indicated by edges. We present a governing law that facilitates different processing tasks, manages shared data across system parts, and defines the connections between vertices. Additionally, we provide illustrated examples covering single and multi-context tracking scenarios to support our work. These illustrations highlight how, in comparison to current tracking technologies, the suggested approach performs better theoretically. This paper presents a theoretical framework for a tracking system, wherein we generalize the formulation of a tracking system designed for living beings and objects. Many tracking systems are typically developed within specific frameworks, either for tracking in limited or unlimited space. The latter often relies on technical tools dedicated to tracking living beings or objects. In this study, we propose a system theory that formulates the task of tracking both living beings and objects. Graphical modeling is widely employed in tracking to establish correct connections between the elements to be tracked and other components in the system. However, basing a tracking system on graphs in both its theoretical and practical aspects remains the optimal method for achieving a high-performing, relevant, and adaptable system in various situations. This paper introduces a tracking system based on graph learning and hypergraphs, fully leveraging direct and indirect relations while considering the order between multiple system links. Tracking is thus formulated as a search problem on graphs and hypergraphs, with vertices representing the elements of the system (living beings or objects), and edges representing the types of connections between these elements. We define a law governing the relationships between the vertices, managing the shared data between the elements of the system and other processes. Furthermore, examples of single and multi-context tracking situations demonstrate that the proposed system, in its theoretical foundation, outperforms existing systems. [ABSTRACT FROM AUTHOR]

22. Veterans’ 12-month PTSD and depression outcomes following 2- and 3-week intensive cognitive processing therapy-based treatment.

Background: Growing evidence indicates that daily delivery of evidence-based PTSD treatments (e.g. Cognitive Processing Therapy (CPT)), as part of intensive PTSD treatment programmes (ITPs), is feasible and effective. Research has demonstrated that a 2-week CPTbased ITP can produce equivalent outcomes to a 3-week ITP, suggesting shorter treatment can also be highly effective. However, the extent to which ITP length and composition impact longer-term outcomes needs further study. Objective: We examined whether PTSD and depression symptoms 3-, 6-, and 12-months following completion of a 2-week ITP could be considered non-inferior, or equivalent, to those of a 3-week ITP. Method: Data from 638 veterans who participated in a 2-week CPT-based ITP were evaluated against 496 veterans who participated in a 3-week CPT-based ITP. A Bayes factor approach was used to examine whether PTSD and depression severity outcomes of the 2-week ITP could be considered equivalent to the 3-week ITP. Results: Participants across both ITPs reported large PTSD (d = 0.98) and moderate to large depression symptom reductions (d = 0.69) from baseline to 12-month follow-up. The PTSD and depression symptom reductions seen in the 2-week ITP were determined to be equivalent to those of the 3-week ITP. Conclusions: Low follow-up completion was a limitation. Future research might replicate the present findings using samples with greater follow-up rates and explore whether adjunctive services impact other relevant constructs, such as quality of life and functioning. 27. Delphi consensus on the transition from paediatric to adult epilepsy care

Introduction: The transition from paediatric to adult neurology consultations in patients with epilepsy must be understood as a process in which the patient acquires independence in the management of their disease and faces new issues related to adulthood. The aim of this study is to gather the opinion of experts in epilepsy on this transition in order to issue recommendations. Methods: Using a Delphi consensus process, 54 paediatric and adult neurologists with expertise in epilepsy expressed their degree of agreement on 38 statements on about the transition from paediatric to adult consultations, grouped into the following sections: (1) preparation and moments prior to transition; (2) transition process; and (3) moments after the transition and follow-up. Results: After 2 rounds, consensus was reached for 33 statements (86.8%). For the remaining 5 statements (13.2%), there was neither agreement nor disagreement. Among the subjects for which consensus was strongest was the involvement of the patient and family, who must be provided sufficient information to resolve their doubts and concerns. Before referral, the clinical report must be reviewed to ensure that it contains all relevant information on the disease. During the first follow-up visits after the transition, the patient's knowledge should be brought up to date in order to promote their autonomy. Conclusion: The clinical report, early preparation, and complete case review are considered fundamental in the process of transition from paediatric to adult neurology care. Resumen: Introducción: La transición desde neuropediatría a las consultas de neurología para adultos en pacientes con epilepsia ha de entenderse como un proceso en el que el paciente adquiere una independencia en el manejo de la enfermedad y afronta nuevos aspectos relacionados con la edad adulta. El objetivo de este estudio es conocer la opinión de expertos en epilepsia sobre esta transición para poder ofrecer recomendaciones. Métodos: Mediante un consenso Delphi, 54 neuropediatras y neurólogos expertos en epilepsia mostraron su grado de acuerdo en 38 aseveraciones sobre la transición de pediatría a la consulta de adultos agrupadas en los siguientes bloques: 1) preparación y momento previo a la transición; 2) durante la transición; y 3) momento posterior a la transición y seguimiento. Resultados: Tras dos rondas, se alcanzó un consenso en el acuerdo en 33 aseveraciones (86,8%). En las 5 aseveraciones restantes (13,2%) no hubo ni acuerdo ni desacuerdo. Entre los acuerdos más importantes destaca la implicación del paciente y la familia a los que es necesario aportar información suficiente para disipar sus dudas y miedos. Antes de la derivación, se ha de comprobar que el informe clínico contiene toda la información relevante de la enfermedad. Durante las primeras visitas de seguimiento tras la transición, se han de actualizar los conocimientos de los pacientes para lograr su autonomía. Conclusión: El informe clínico, la preparación anticipada y la revisión completa del caso se consideran fundamentales en el proceso de transición de pediatría a la consulta de adultos.

Unmanned Aerial Vehicles (UAV) has evolved in recent years, their features have changed to be more useful to the society, although some years ago the drones had been thought to be teleoperated by humans and to take some pictures from above, which is useful; nevertheless, nowadays the drones are capable of developing autonomous tasks like tracking a dynamic target or even grasping different kind of objects. Some task like transporting heavy loads or manipulating complex shapes are more challenging for a single UAV, but for a fleet of them might be easier. This brief survey presents a compilation of relevant works related to tracking and grasping with aerial robotic manipulators, as well as cooperation among them. Moreover, challenges and limitations are presented in order to contribute with new areas of research. Contexto: el hipotiroidismo se define como un síndrome caracterizado por múltiples manifestaciones sistémicas, donde cualquiera de los componentes del eje hipotálamo-hipófisis-tiroides puede estar comprometido; es una entidad altamente prevalente que puede afectar a diferentes poblaciones, independiente de su grupo etario y de sus condiciones biológicas; no obstante, la inespecificidad de sus síntomas y evolución clínica insidiosa generan un reto en la práctica clínica diaria para el oportuno y adecuado diagnóstico, tratamiento o seguimiento de la enfermedad. Independientemente de su etiología y clasificación, actualmente el tratamiento de elección es la levotiroxina (con el objetivo de alcanzar el control bioquímico). Finalmente, el seguimiento del paciente con hipotiroidismo es fundamental para garantizar el control adecuado, el cual permitirá detectar oportunamente las posibles comorbilidades asociadas. Objetivo: definir los criterios para el diagnóstico, el tratamiento y el seguimiento del hipotiroidismo en Colombia. Metodología: se realizó un consenso tipo Delphi modificado, para ello se escogió un panel de médicos endocrinólogos colombianos, expertos en el manejo de las patologías tiroideas. Los resultados fueron analizados y discutidos a partir en consenso y se resumieron las conclusiones y las recomendaciones en los diferentes aspectos relacionados con el hipotiroidismo. Resultados: se obtuvieron las recomendaciones para el abordaje del paciente con hipotiroidismo en Colombia, incluyendo desde la sospecha clínica, el diagnóstico y el tratamiento, hasta el seguimiento del paciente con la enfermedad, además se generaron consideraciones en poblaciones especiales desde la perspectiva del sistema de salud nacional. Conclusiones: en Colombia es importante disminuir la variabilidad en la práctica clínica diaria, así como fortalecer el conocimiento del personal médico, desde el escenario de atención primaria hasta la atención de alta complejidad, respecto al diagnóstico, el tratamiento y el seguimiento del paciente con hipotiroidismo, con el objetivo de brindar un manejo oportuno y adecuado de la enfermedad.

32. Global positioning system (GPS) and platform transmitter terminal (PTT) tags reveal fine-scale migratory movements of small birds: A review highlights further opportunities for hypothesis-driven research.

Studying migration is critically important for understanding the full-annual cycle of migrating birds and aids in management and conservation. In recent years, global positioning system (GPS) and platform transmitting terminal (PTT) tags have become miniaturized, revolutionizing migration research by allowing for higher location accuracy and global coverage for smaller species. We reviewed 116 primary research papers from 2006 to 2021 that reported on GPS- or PTT-tagged small birds (<500 g) during migration and assessed research aims, tag efficacy, ecological findings, and future opportunities. Reviewed papers covered 58 unique species (nearly half of which were shorebirds) and 65% of studies tagged birds at breeding grounds. Tags that did not require retrieval had higher success obtaining data, with PTT tags having the greatest success (85% of deployed tags had data). Of studies that reported tag load information, most (70%) reported tag loads -3% body mass, which is a potential concern given that 3% is considered the preferable upper limit. Most papers (69%) focused on presenting spatial information such as migratory routes, stopover sites, and habitat use, and only 33% of papers tested explicit hypotheses, demonstrating that we are in the early stages of understanding small bird migration at fine scales. Almost all tag deployments (93%) occurred in the northern hemisphere, most often in the United States (24%), indicating a deficiency in GPS and PTT research on small bird migration from the southern hemisphere. GPS and PTT tags have revealed new biological insights not possible with other technologies by providing fine-scale movement data and global coverage. There are exciting opportunities for researchers to move beyond descriptive spatial studies and explore hypothesis-driven migratory research, into topics such as intraspecific variation, carry-over effects, and climate-driven movements for irruptive species. 33. Características endosonográficas de las lesiones subepiteliales del tracto digestivo superior: experiencia de un centro de referencia en Colombia.

Introduction: Subepithelial lesions (SELs), described as bulges or masses covered by healthy-looking mucosa, are usually found incidentally during endoscopic studies. They are typically asymptomatic and are estimated to be identified in 1% of esophagogastroduodenoscopies performed. Materials and methods: A descriptive study was conducted with retrospective data collection. We included all patients treated at the Unión de Cirujanos, a referral gastroenterology unit of the Coffee Region in Manizales, between January 2020 and January 2022, who underwent endoscopic ultrasonography to study subepithelial-looking lesions located in the esophagus, stomach, and duodenum. Results: 152 endoscopic ultrasounds were performed, finding 108 SELs; 66.6% of the patients were women, and the average age was 58. Most SELs were located in the stomach (78.7%), the antrum being the most frequent location. The average diameter of the gastric SELs was 14.6 mm, and 47% of the lesions depended on the fourth echolayer; the most frequent presumptive diagnoses were gastrointestinal stromal tumor (GIST; 65.8%) and lipoma (11.7%). Conclusions: SELs of the GI tract originate in the muscularis mucosae, submucosa, or muscularis propria. They are most frequently located in the stomach, and their characterization usually requires endoscopic ultrasonography and histopathology. Treatment of these lesions remains controversial due to their low frequency, histological variety, and low malignant potential. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: Atopic dermatitis is a chronic, systemic, relapsing disease with dermatological manifestations, which imposes a high burden on patients, families and the health care system and has a high psychological, social, and economic impact and on the quality of life of patients. It mainly affects the pediatric population and, to a lesser extent, the adult population. The clinical presentation varies according to the age and evolution of the disease, and currently there are multiple pharmacological and non-pharmacological therapies available for the symptomatic management of patients. Objective: To present an algorithm for the management of atopic dermatitis, proposed as a series of recommendations on the management, diagnosis, education, and follow-up of these patients. Methods: A consensus was reached using the nominal group technique. The methodology was developed in 7 phases, including: posing the research questions, literature search, an initial proposal of recommendations, elaboration of the final recommendations and the management algorithm with three voting cycles, consensus was established with 80% favorability. Results and conclusions: The result of the consensus process is a management algorithm for patients with mild, moderate/severe atopic dermatitis derived from expert recommendations. The algorithm establishes diagnostic and treatment criteria and provides updated recommendations, including all therapeutic alternatives available in Peru for the management of patients with mild, moderate, and severe atopic dermatitis. Cancer and its treatment can be related to sequelae and late effects that impact the quality of life at survivorship. These symptoms can be physical, psychological and social and can affect up to 80% of the cancer survivors. Method: Participants were 183 adult Mexican cancer survivors who answered the Cancer Survivor's Unmet Needs (CaSUN-Mx) Measure using the open question, "Describe any unmet need you have had in the last month that has not been asked previously". The qualitative BOTTOM-UP analysis was performed to process the answers. Results: There were 241 identified units excluding the category "none". The result was 143 units with reported needs divided into nine categories; the most frequent category was Comprehensive care services (nutrition, psychology, physical therapy, and medical oncology), followed by follow-up/sequelae needs, information and communication with the medical team, access to health services, group and social support, followed by readaptation and work issues, legal needs, aesthetic care needs and COVID-19 pandemic needs. Conclusions: Patients do not perceive adequate attention to comprehensive care issues. They present difficulties in adapting to life after cancer treatment, presenting physical, emotional, social, and legal issues and needs because of the COVID-19 pandemic. [ABSTRACT FROM AUTHOR]

This guideline will also inform interested patients requiring HEN. Home parenteral nutrition is not included but will be addressed in a separate ESPEN guideline. The guideline is based on the ESPEN scientific guideline published before, which consists of 61 recommendations that have been reproduced and renumbered, along with the associated commentaries that have been shorted compared to the scientific guideline. Evidence grades and consensus levels are indicated. The guideline was commissioned and financially supported by ESPEN and the members of the guideline group were selected by ESPEN. Background: Technological advances in diagnostic imaging tests have increased the incidental detection of small pulmonary nodules, however, the lack of scientific studies limits the knowledge of the prevalence and real characteristics. Objective: To know the evolution of pulmonary nodules from 6 to 30 mm in a population endemic for tuberculosis. Method: An analytical study of ambidirectional cohort type was carried out with a sam ple of 10 patients, with incidental nodules in computed tomography from 6 to 30 mm, > 35 years old. Results: Of the 378 tomographic studies reviewed, 34 patients (9.7%) presented pulmonary nodules from 6 to 30 mm, of which 79.3% were single nodules and 20.6% multiple, 61.7% were presented in men and 38.2% women. The presence of nodules is greater in the right lung (59.9%); 86.6% of the nodules were solid with smooth edges (93.3%), without calcifications (64.4%), while 8.8% were subsolid and 4.4% mixed. In the tomographic control of the nodules from 6 to 30 mm, 35.3% reduced their size, 29.4% remained constant, 23.5% apparently increased their size, and 11.8% disappeared. [ABSTRACT FROM AUTHOR]

El acompañamiento pedagógico mediante el clown educador, se desarrolló en el marco de un proyecto de extensión y proyección social de la Universidad Católica de Manizales, en asocio con la Facultad de Educación y la Facultad de Salud en el año 2021. El proyecto tuvo como objetivo fortalecer aspectos de la salud y aprendizaje de los niños, adolescentes y sus familias, que asisten al Aula Hospital de la Fundación Divina Misericordia, mediante el clown educador y los programas de licenciatura de la Universidad Católica de Manizales (UCM). La metodología se realizó con una perspectiva del enfoque cualitativo, con un tipo de investigación-reflexión-acción con el propósito de analizar y reconocer la realidad del contexto en el cual se lleva a cabo la intervención pedagógica. Los resultados obtenidos reflejan la relación entre las necesidades en salud y los procesos académicos de los participantes y, también, permitió el trabajo interdisciplinario, en favor de las personas consolidando un proceso de aprendizaje integral entre los docentes en formación de la UCM y la implementación de estrategias clown en salud. Se pudo concluir que los espacios de aprendizaje requieren de un acompañamiento permanente donde tanto docentes, directivos y familia puedan articular en pro de la formación de los estudiantes en cualquier contexto o dificultad educativa.

El avance tecnológico en los últimos años es cada vez mayor, su uso repercute de manera directa e indirecta en la vida cotidiana de las personas. Haciendo uso de este progreso se propone una aplicación que permita a personas con cierto grado de discapacidad interactuar mediante la visión computacional, en particular la detección de movimiento con entornos que simulen la realidad. En primer lugar, se realiza una especificación del grupo a quien va dirigida esta propuesta, esto mediante la clasificación internacional del funcionamiento de la discapacidad y de la salud, con el fin de delimitar el público objetivo y poder seleccionar los algoritmos óptimos para esta situación. A continuación, se realiza un análisis de los métodos utilizados para el análisis de video que permitan detectar objetos y realizar un tracking de movimiento, proceso que dará como resultado la interacción de la persona con el software sin necesidad de periféricos extra.

El presente artículo de revisión pretende analizar el tema de los riesgos financieros que suelen presentarse en las empresas, este es un tema muy común debido a que los riesgos se los puede encontrar en cualquier institución, por ello es importante identificar como esos riesgos afectan a la institución y como se puede crear alternativas de solución que permitan minimizar ese riesgo, por ello se ha propuesto como objetivo Identificar el nivel de riesgo operativo mediante una matriz de riesgo empresarial que permita analizarlas y establecer estrategias de mitigación del riesgo, es importante rescatar que las empresas deben contar con matrices de riesgo que ayuden a su identificación de cada proceso y observar que proceso se le puede generar un plan de mejoramiento que aporte en la generación de valor y reduzca los niveles de riesgo. Es importante tener presente cuáles son las amenazas, los riesgos para las entidades puesto que son sucesos que se pueden presentar frecuentemente en diferente área. Por ende, es necesario realizar monitoreo y seguimiento continuo para verificar que las acciones de mitigación se están realizando de la mejor manera para conseguir los resultados esperado. Así que se debe realizar una frecuencia de seguimiento trimestral, que ayude a mitigar los riesgos y dar continuidad a los siguientes riesgos que también se encuentren en catastróficos.

43. Características endosonográficas de las lesiones subepiteliales del tracto digestivo superior: experiencia de un centro de referencia en Colombia

Introducción: las lesiones subepiteliales (LSE), descritas como abultamientos o masas cubiertas por mucosa de aspecto sano, se encuentran usualmente de manera incidental durante estudios endoscópicos; suelen ser asintomáticas y se estima que se identifican en el 1% de las esofagogastroduodenoscopias realizadas. Métodos: se realizó un estudio descriptivo con recolección retrospectiva de la información. Se incluyeron todos los pacientes atendidos en Unión de Cirujanos, unidad de gastroenterología de referencia del Eje Cafetero ubicada en la ciudad de Manizales, entre enero de 2020 y enero de 2022, a quienes se les realizó ultrasonografía endoscópica como parte del estudio de lesiones de aspecto subepitelial localizadas en el esófago, estómago y duodeno. Resultados: se realizaron 152 endosonografías y se encontraron 108 lesiones subepiteliales, 66,6% de los pacientes eran mujeres y el promedio de edad fue 58 años. La mayoría de las LSE se localizaron en el estómago (78,7%) y, de estas, la localización más frecuente fue el antro; el diámetro promedio de las LSE gástricas fue de 14,6 mm y el 47% de las lesiones eran dependientes de la cuarta ecocapa; los diagnósticos presuntivos más frecuentes fueron el tumor del estroma gastrointestinal (GIST; 65,8%) y lipoma (11,7%). Conclusiones: las LSE del tracto gastrointestinal se originan en la muscular de la mucosa, submucosa o muscular propia, de manera más frecuente se localizan en el estómago y su caracterización suele requerir la realización de ultrasonografía endoscópica y estudio histopatológico. El tratamiento de estas lesiones sigue siendo controversial debido a su baja frecuencia, variedad histológica y bajo potencial maligno.

Introducción: La dermatitis atópica es una enfermedad crónica, sistémica, reincidente, con manifestaciones dermatológicas, que impone una alta carga a los pacientes, las familias y los sistemas de salud, y tiene repercusión psicológica, social y económica, y en la calidad de vida de los pacientes. Afecta principalmente a la población pediátrica y con menor frecuencia a la adulta. Las manifestaciones clínicas varían según la edad y evolución de la enfermedad, y en la actualidad se dispone de múltiples opciones farmacológicas y no farmacológicas para el tratamiento de los pacientes. Objetivo: Presentar un algoritmo de tratamiento de la dermatitis atópica, propuesto con una serie de recomendaciones acerca del diagnóstico, tratamiento y seguimiento de los pacientes. Métodos: Se llevó a cabo un consenso de expertos, utilizando la técnica del grupo nominal. La metodología se desarrolló en 7 fases que incluyeron: planteamiento de las preguntas de investigación, búsqueda de la bibliografía, propuesta inicial de las recomendaciones, elaboración de las recomendaciones finales y del algoritmo de tratamiento con tres ciclos de votación. Se estableció el consenso con un 80% de favorabilidad. Resultados y conclusiones: El resultado del consenso fue un algoritmo de tratamiento de pacientes con dermatitis atópica leve, moderada-grave, derivado de las recomendaciones de expertos. En el algoritmo se establecen criterios diagnósticos y de tratamiento, y se aportan recomendaciones actualizadas que incluyen las alternativas disponibles en Perú.

La artritis psoriásica (APs) es una enfermedad inflamatoria crónica mediada por el sistema inmune que afecta al sistema musculoesquelético y la piel, y se manifiesta de forma heterogénea y con un curso variable. En la práctica clínica habitual se ha observado variabilidad y limitaciones en su seguimiento. El objetivo del proyecto CREA fue consensuar estrategias de mejora para la valoración inicial y el seguimiento de los pacientes con APs en España. Se realizó una encuesta a una muestra representativa de reumatólogos expertos del territorio español, que contenía 33 preguntas sobre la práctica clínica habitual, los recursos disponibles y las limitaciones actuales en el seguimiento de los pacientes con APs. Se discutieron los resultados en reuniones regionales y se propusieron 105 estrategias que, finalmente, fueron valoradas por 85 expertos en un consenso Delphi. Las limitaciones destacadas en el seguimiento de la APs fueron la falta de tiempo en consulta, de personal de enfermería, y el retraso en la realización de pruebas de imagen. Se propusieron 108 estrategias relacionadas con la evaluación de los índices de calidad de vida e impacto de la enfermedad; las comorbilidades y las manifestaciones extraarticulares; las pruebas de laboratorio; las pruebas de imagen; la exploración física y metrología y los índices de actividad y función. Entre todas, 53 se consideraron altamente aconsejables, sin diferencias regionales en los valores de consenso. Las propuestas ofrecidas en el estudio actual son aplicables a todo el territorio nacional, responden a las necesidades no cubiertas detectadas en la encuesta inicial, conforman un cuadro de actuación mínimo y aseguran un seguimiento óptimo de los pacientes con APs. Psoriatic arthritis (PsA) is a chronic immune-mediated inflammatory disease that affects the musculoskeletal system and skin, and manifests heterogeneously, with a variable course. In current clinical practice, variability and limitations in its follow-up have been observed. The aim of the CREA project was to agree on strategies to improve the initial assessment and follow-up of patients with PsA in Spain. A survey was conducted among a representative sample of expert rheumatologists in Spain, containing 33 questions on current clinical practice, available resources, and current limitations in the follow-up of patients with PsA. The results were discussed in regional meetings and 105 strategies were proposed and finally evaluated by 85 experts in a Delphi consensus. The most important limitations in the follow-up of PsA were lack of consultation time, lack of nursing staff, and delays in performing imaging tests. A total of 108 strategies were proposed related to the assessment of quality of life and disease-impact indices; comorbidities and extra-articular manifestations; laboratory tests; imaging tests; physical examination and metrology; and activity and function indices. Of the total, 53 were considered highly advisable, with no regional differences in consensus values. The proposals offered in the current study are applicable to the entire country, respond to the unmet needs detected in the initial survey, form a minimum action framework, and ensure optimal follow-up of patients with PsA. La espondiloartritis axial (EspAax) es una enfermedad musculoesquelética con manifestaciones diversas. En la práctica clínica se ha observado variabilidad y limitaciones en la recogida de las variables necesarias para su seguimiento. El objetivo del proyecto CREA fue consensuar estrategias de mejora para la valoración inicial y el seguimiento de los pacientes con EspAax en España. Se realizó una encuesta con 33 preguntas a una muestra representativa de reumatólogos expertos del territorio español sobre la práctica clínica, los recursos y las limitaciones actuales en el seguimiento de los pacientes con EspAax. En 10 reuniones regionales se discutieron los resultados de la encuesta y se propusieron 107 estrategias que fueron valoradas mediante un consenso Delphi en el que participaron 85 expertos. La falta de tiempo en consulta, de personal de enfermería y/o de apoyo, y el retraso en la realización de pruebas de imagen fueron las limitaciones más destacadas en el seguimiento de los pacientes con EspAax. Se propusieron 202 estrategias relacionadas con la evaluación de los índices de calidad de vida e impacto de la enfermedad; las comorbilidades y manifestaciones extraarticulares; las pruebas de laboratorio; las pruebas de imagen; la exploración física y metrología; y los índices de actividad y función. De todas, 54 se consideraron altamente aconsejables. No se encontraron diferencias regionales en los valores de consenso. Las propuestas consensuadas como altamente aconsejables en el estudio actual son aplicables a todo el territorio nacional, permiten realizar un seguimiento y control más estrecho y homogéneo de los pacientes con EspAax, facilitar un manejo integral y responden a las necesidades no cubiertas detectadas en la encuesta inicial. Axial spondyloarthritis (axSpA) are musculoskeletal diseases with different manifestations. In clinical practice, variability, and limitations in the collection of the outcomes required for follow-up have been observed. The objective of the CREA project was to agree on improvement strategies for the initial assessment and follow-up of patients with axSpA in Spain. A survey with 33 questions was conducted by a representative sample of rheumatologists on clinical practice, resources, and present limitations in the follow-up of patients with axSpA. The results of the survey were discussed in 10 regional meetings, and 105 strategies were proposed and evaluated through a Delphi consensus in which 85 experts participated. The lack of time for clinical visits, the lack of nurses and/or support staff and the delay in performing the imaging tests were the most prominent limitations in the follow-up of patients with axSpA. One hundred and five strategies were proposed related to the evaluation of disease activity, physical function, quality of life and disease impact, to the evaluation of comorbidities and extra-articular manifestations, laboratory tests; imaging tests, physical examination and metrology. Of the total, 85 were considered highly advisable. No regional differences were found. The proposals agreed upon as highly advisable in the present study are applicable to the entire national territory, allow tighter and more homogeneous monitoring of the patients with axSpA, facilitate more comprehensive management of the disease, and respond to the unmet needs detected in the initial survey.
