Abstract

This paper describes the design, fabrication and testing of the Anti-Coincidence Detector (ACD) for the Gamma-ray Large Area Space Telescope (GLAST) Large Area Telescope (LAT). The ACD is LAT first-level defense against the charged cosmic ray background that outnumbers the gamma rays by 3-5 orders of magnitude. The ACD covers the top and 4 sides of the LAT tracking detector, requiring a total active area of ~8.3 square meters. The ACD detector utilizes plastic scintillator tiles with wave-length shifting fiber readout. In order to suppress self-veto by shower particles at high gamma-ray energies, the ACD is segmented into 89 tiles of different sizes. The overall ACD efficiency for detection of singly charged relativistic particles entering the tracking detector from the top or sides of the LAT exceeds the required 0.9997., Comment: 33 pages, 19 figures

2. The evolution of oral PrEP access: tracking trends in global oral PrEP use over time

Abstract

Background: Since oral pre-exposure prophylaxis (PrEP) was approved for HIV prevention in 2012, 78 countries have begun offering PrEP in some form. Measuring progress against global targets and sharing strategies across countries are critical to driving impact. Since 2014, AVAC has collected data from PrEP programs to track and generate insights on PrEP implementation. Methods: The Global PrEP Tracker is a comprehensive database of PrEP projects and national programs. Data are collected through a quarterly survey to 253 programs, and include PrEP initiation numbers, geography, and other indicators. This analysis examined data from Quarter 3 (Q3) 2016 to Q2 2020 to highlight global and regional PrEP initiation trends. Results: Global PrEP uptake has increased six-fold in approximately four years, from 102,446 initiations in 2016 to 651,586 in 2020. Annual growth has slowed over time, from 104% from 2017 to 2018, to 55% from 2018 to 2019, to 18% from 2019 to 2020. At the regional level, Oceania has the highest rate of change, with total PrEP initiations increasing from 318 to 29,093, driven largely by Australia. Sub-Saharan Africa has substantially expanded PrEP access, from 4154 initiations in 2016 to 290,981 by mid-2020, comprising 44% of the global total. South Africa and Kenya have led this growth, counting 69,876 and 63,000 cumulative initiations, respectively. Brazil is leading PrEP uptake in Latin America and the Caribbean, accounting for two-thirds of initiations, and Thailand comprises 51% of initiations in Asia. The United States has the most cumulative initiations at 203,837, about one-third of the global total, but has comparatively modest growth rates. Shared traits in many of these settings are early adoption, national commitment to scale-up, and programs tailored to populations at high risk offering rights-based services and linkages to social support. Conclusions: While PrEP initiations have grown exponentially in several countries, global uptake falls far short of UNAIDS' target of 3 million users, indicating a need for sustained demand creation where PrEP programs exist, and scale-up where PrEP is provided via demonstration projects with limited reach. 3. SARS-CoV-2 exposures among healthcare workers in New York City.

Abstract

Background Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has had a significant impact on hospitals, including the occupational health departments in charge of handling healthcare worker (HCW) staffing during high rates of exposure and infection of HCWs. HCWs who were exposed to a patient or community member infected with SARS-CoV-2 were required to isolate from work for a minimum of 14 days from the date of exposure. Aims This study was aimed to assess the relative risk of SARS-CoV-2 infection following different types of workplace and community exposures. Methods We analyzed the details of workplace and community exposures of HCWs to SARS-CoV-2 at Montefiore Medical Center in New York between 22 June 2020 and 22 November 2020. Results Of 562 HCW SARS-CoV-2 exposures analyzed, 218 were from the community and 345 were from the workplace. Twenty-nine per cent of community exposures resulted in infection, which was significantly greater than workplace exposure infection (2%). Household community exposures resulted in a larger frequency of infection than non-household community exposures. Of the seven infections after workplace exposures, five had qualifying exposures to a co-worker and two were exposed to an infected patient during a non-aerosolized procedure. Conclusions HCW exposure to SARS-CoV-2 continues to present staffing challenges to healthcare systems. Even with deviations from standard personal protective equipment protocol, workplace exposures resulted in low frequencies of infection. In our study, the primary source of HCW infection was exposure in the community. Our findings support investing in efforts to educate around continued masking and social distancing in the community in addition to interventions targeted at addressing vaccine hesitancy. Abstract

We propose a new type of telescope designed specifically for the lunar environment of high vacuum and low temperature. Large area UV-Visible-IR telescope arrays can be built with ultra-light-weight replica optics. High T(sub c) superconductors provide support, steering, and positioning. Advantages of this approach are light-weight payload compatible with existing launch vehicles, configurable large area optical arrays, no excavation or heavy construction, and frictionless electronically controlled mechanisms. We have built a prototype and will be demonstarting some of its working characteristics.

Abstract

The Vela pulsar is the brightest persistent source in the GeV sky and thus is the traditional first target for new gamma-ray observatories. We report here on initial Fermi Large Area Telescope observations during verification phase pointed exposure and early sky survey scanning. We have used the Vela signal to verify Fermi timing and angular resolution. The high quality pulse profile, with some 32,400 pulsed photons at E>0.03 GeV, shows new features, including pulse structure as fine as 0.3ms and a distinct third peak, which shifts in phase with energy. We examine the high energy behavior of the pulsed emission; initial spectra suggest a phase-averaged power law index of Gamma=1.51{+0.05/-0.04} with an exponential cut-off at E_c=2.9+/-0.1 GeV. Abstract

Botulinum neurotoxin (BoNT) can be injected to achieve therapeutic benefit across a large range of clinical conditions. To assess the efficacy and safety of BoNT injections for the treatment of spasticity associated with the upper motor neuron syndrome (UMNS), an expert panel reviewed evidence from the published literature. Data sources included English-language studies identified via MEDLINE, EMBASE, CINAHL, Current Contents, and the Cochrane Central Register of Controlled Trials. Evidence tables generated in the 2008 Report of the Therapeutics and Technology Assessment Subcommittee of the American Academy of Neurology (AAN) review of the use of BoNT for autonomic disorders were also reviewed and updated. The panel evaluated evidence at several levels, supporting BoNT as a class, the serotypes BoNT-A and BoNT-B, as well as the four individual commercially available formulations: abobotulinumtoxinA (A/Abo), onabotulinumtoxinA (A/Ona), incobotulinumtoxinA (A/Inco), and rimabotulinumtoxinB (B/Rima). The panel ultimately made recommendations on the effectiveness of BoNT for the management of spasticity, based upon the strength of clinical evidence and following the AAN classification scale. While the prior report by the AAN provided recommendations for the use of BoNT as a class of drug, this report provides more detail and includes recommendations for the individual formulations. For the treatment of upper limb spasticity, the evidence supported a Level A recommendation for BoNT-A, A/Abo, and A/Ona, with a Level B recommendation for A/Inco; there was insufficient evidence to support a recommendation for B/Rima. For lower limb spasticity, there was sufficient clinical evidence to support a Level A recommendation for A/Ona individually and BoNT-A in aggregate; the clinical evidence for A/Abo supported a Level C recommendation; and there was insufficient information to recommend A/Inco and B/Rima (Level U). Abstract

The Vela pulsar is the brightest persistent source in the GeV sky and thus is the traditional first target for new gamma-ray observatories. We report here on initial Fermi Large Area Telescope observations during verification phase pointed exposure and early sky survey scanning. We have used the Vela signal to verify Fermi timing and angular resolution. The high-quality pulse profile, with some 32,400 pulsed photons at E >= 0.03 GeV, shows new features, including pulse structure as fine as 0.3 ms and a distinct third peak, which shifts in phase with energy. We examine the high-energy behavior of the pulsed emission; initial spectra suggest a phase-averaged power-law index of Gamma = 1.51(-0.04)(+0.05) with an exponential cutoff at E-c = 2.9 +/- 0.1 GeV. Spectral fits with generalized cutoffs of the form e(-(E/Ec)b) require b <= 1, which is inconsistent with magnetic pair attenuation, and thus favor outer-magnetosphere emission models. Finally, we report on upper limits to any unpulsed component, as might be associated with a surrounding pulsar wind nebula.
