Abstract

We implemented a controlled study of computerized ambulatory records compared with traditional paper charts. This paper reports the effect of a computer health care maintenance reminder system on compliance with recommended preventive health care maneuvers. Medical residents who had access to computerized records and reminders did significantly more hemoccult tests, proctosigmoidoscopy and influenza vaccinations than residents using paper records. The attending physician staffing the resident also had a profound but independent effect on whether preventive care was delivered. Abstract

Controlled studies of computerized ambulatory information systems are rare. As part of an evaluation of the effects of COSTAR on clinic function, we divided our resident teaching clinic into a study group with access to COSTAR and a control group allowed access only to conventional medical records. We sampled staff attitudes toward use of the computer and did detailed time studies of clinic patient flow. Staff attitudes reflected a high degree of acceptance, favoring COSTAR over conventional records. This was primarily related to improvement in telephone management and demand care. House staff never became facile users of COSTAR because of infrequent clinic sessions. Clinics assigned to COSTAR experienced somewhat longer waiting times due to an increased workload and training effects. 20. The Lipase to Amylase Ratio in Acute Pancreatitis.

Abstract

Objectives: The ratio of serum lipase to serum amylase has been proposed to distinguish acute episodes of alcoholic from nonalcoholic pancreatitis. We evaluated the efficacy of this test in a community hospital setting. Methods: Charts of all patients discharged with a diagnosis of acute pancreatitis over 19 months were retrospectively reviewed. Patients were excluded if their creatinine was greater than 3.0 mg/dl, if the amylase and lipase were not measured within 72 h of the onset of symptoms, or if the cause of pancreatitis was not known by the time of discharge. Results: Of the 56 patients, 31 had alcoholic pancreatitis. The lipase to amylase ratio did not differ significantly between patients with alcoholic and nonalcoholic pancreatitis. Median amylase and lipase were significantly higher in nonalcoholic pancreatitis; however, the wide ranges of both meant that neither amylase nor lipase accurately determined the cause of pancreatitis.
