1. La inducción del periodo ovulatorio

Abstract

Two powerful instruments -the human gonadotropin and clomiphene-- citrate are at our disposal for the induction of ovulation period in all patients who show an anovulatory period of central origin: psychogenic, neurogenic and pituitary. These drugs must not be used without a prior study of the etiology excluding not only the peripheral fault OVO rich but also the end-organ failure. They are not used in cases of amenorrhea due to intermediary causes as chronic diseases, metabolic diseases -tiroides, adrenol, pancreas and nutritional conditions factors. To avoid complications that are polycystic ovaries and multiple pregnancies, one has to use the minimum effective dose. This dose is, to some extent, based on a single criterion. If these precautions are taken properly, one can expect to improve the outcome of difficult cases such as amenorrhea caused by neurogenic factors and pituitary, hypothalamic amenorrhea and perhaps eradicate the surgical treatment of Stein-Leventhal syndrome. Dos instrumentos potentes -la gonadotropina humana y el citrato del clomiphene-- están a nuestra disposición para la inducción del período ovulatorio en todos las pacientes que demuestran un período anovulatorio de origen central: psicogénico, neurogénico e hipofisario. Estos drogas no han de emplearse sin un previo estudio sobre el diagnóstico etiológico excluyendo no solamente el fallo periférico ovó rico sino también el fallo de los órganos terminales. No han de emplearse en casos de amenorrea debida a causas intermediarias como enfermedades crónicas, enfermedades del metabolismo -tiroides, adrenol, páncreas y factores nutricionales-. Para obviar complicaciones que son ovarios poliquísticos y embarazos múltiples, uno ha de emplear la dosis mínima y efectiva. Esta dosis es, hasta cierto punto, a base de un criterio individual. Si estas precauciones son debidamente llevadas, uno puede esperar mejorar el pronóstico de los casos difíciles como la amenorrea causada por factores neurogénicos e hipofisarios, la amenorrea hipotalámica y quizás erradicar el tratamiento quirúrgico del síndrome de Stein-Leventhal.

2. Variations in Plasma Levels of Substance P and Effects of a Specific Substance P Antagonist of the NK[sub 1] Receptor on Preovulatory LH and FSH Surges and Progesterone Secretion in the Cycling Cynomolgus Monkey.

Abstract

These studies investigated the role of substance P (SP) in the regulation of the hypothalamic-pituitary-ovarian axis in cynomolgus monkeys with normal menstrual cycles. Plasma concentrations of SP were determined in blood samples taken every morning in normally menstruating cynomolgus monkeys throughout the menstrual cycle. There was a significant decreasing linear trend of SP during the follicular phase (cycle day –13 to day 0) and a significant inverse relationship between SP plasma values and plasma 17β-estradiol (E[sub 2] ) values from day –13 to day 0 of the adjusted cycle. Correspondingly, SP area under the curve was significantly greater during the follicular phase than the luteal phase. In a second experiment, plasma concentrations of E[sub 2] , luteinizing hormone (LH), follicle-stimulating hormone (FSH) and progesterone and length of cycles were measured after five daily intragastric administrations (10 mg/kg) of an NK[sub 1] receptor (SP receptor) antagonist (RPR 100893; 10 mg/kg) initiated after serum E[sub 2] concentrations had exceeded 125 pg/ml. There was a statistically significant reduction in the amplitude (41% of control) and the area under the curve (37% of control) of the preovulatory LH surge. In addition, there was a reduction of the duration of the LH surge (3 ± 0.1 days in controls vs. 2.1 ± 0.2 days in treated animals). The present results show for the first time that there are significant variations in plasma levels of SP, with a strong negative correlation with serum levels of E[sub 2] during the follicular phase of the cynomolgus monkey, and that endogenous SP has a potentiating role in the interactive hypothalamo-anterior-pituitary mechanisms which lead to the preovulatory LH and FSH surges during the menstrual cycle in the monkey.Copyright © 2000 S. Abstract

Prediction of ovulation was established by correlation of clinical parameters, follicular development by ultrasound, and estradiol, progesterone, and luteinizing hormone (LH) determination in 71 menstrual cycles. Laparoscopic follicular aspiration was accomplished in 41 of those cycles. A 28-hour interval from the ascending limb of the LH seems to be the “ideal time” for retrieval of a preovulatory oocyte. The variability in the amount of LH to which the follicle is exposed during the LH surge seems to indicate that there is a relatively low specific value necessary for ovulation. Ovulation occurs approximately 10 ± 5 hours from the LH peak. Progesterone occurs in relation to the LH surge and is helpful for the retrospective analysis of the menstrual cycle.

4. Fertility Following Bilateral Ovarian Wedge Resection: A Critical Analysis of 90 Consecutive Cases of the Polycystic Ovary Syndrome

Abstract

Fertility following bilateral ovarian wedge resection (BOWR) was evaluated in a retrospective cohort study of 90 consecutive cases of the polycystic ovary syndrome. Post-BOWR follow-up was available for varying time spans of up to 10 years. BOWR resulted in the resumption of menstrual cyclicity in 91.1% (82/90) of the cases. However, within this ovulatory group, 26 patients were characterized by oligo-ovulation and a significantly reduced conception rate (292%), as compared with that of 56 normo-ovulatory counterparts (60.3%). Although the crude overall conception rate for this series was 47.8%, the overall cumulative probability of conception at the end of follow-up as determined by life table analysis was 73%. The likelihood of conception at any given point in time was estimated by a monthly fecundability rate of 1.34%. Our findings also indicate that the probability of post-BOWR conception was unaffected by age, race, ward status, or duration of infertility. In contrast, persistent post-BOWR oligo- or anovulation and the presence of concurrent tuboperitoneal disease were reaffirmed as the most important determinants of the likelihood of post-BOWR conception. A minimum incidence of 7.8% was documented for acquired post-BOWR pelvic disease.
