En chirurgie orthopédie il est quotidien d’implanter des matériels inertes : d’ostéosynthèses ou de prothèses : temporaires pour les premiers, définitifs ou supposés tels pour les seconds. De nombreux produits ont ainsi été utilisés : les métaux : acier de type 316L, alliages chrome cobalt, alliage de titane, métaux à mémoire de forme, les plastiques parmi lesquels : le polyéthylène, le ciment ou methyl metacrylate, des phosphates tricalciques sous forme d’hydroxyapatite ou autres, sous forme massive ou granulaires, des céramiques de frottement comme l’alumine, la zircone, des métaux à surface céramisée. La liaison du vivant et du mort est l’objet de ce travail. Aucun matériel n’est véritablement inerte puisque son introduction produira des réactions locales ou générales encore mal connues. L’étude pluridisciplinaire de ces phénomènes, associant des ingénieurs, des biologistes, s’attachant à comprendre le mécanisme intime de dégradation des produits implantés, ainsi que le mécanisme des réactions de l’organisme à ces produits permettent de progresser. Il est possible en 2017 de remplacer une hanche par une prothèse en assurant une articulation normale : la hanche oubliée. Comment cela est-il possible ? Si les matériaux inertes peuvent être testés au laboratoire, l’évolution des tissus vivants au contact font l’objet d’études plus récentes. L’infection, l’allergie, les réactions à corps étrangers sont connues. Le remodelage des tissus vivants en fonction des contraintes aboutissant à une ostéoporose ou à une adaptation sont moins bien étudiés. Le tissu fibreux dense observé autour des couples alumine stabilise l’articulation et permet de comprendre l’absence de luxation à long terme, différant en cela des couples comportant du polyéthylène. L’avenir sera à la compréhension de ces mécanismes : cicatrisation des tissus alentour, mecanotransduction qui résume les processus de cicatrisation en fonction des contraintes locales., ♦ Hanche en céramique ♦ Genou en céramique ♦ Cicatrisation du tissu fibreux, Orthopedic surgeons used many different materials for bone repair or joint replacement; as well as for scoliosis cure or bone replacement following tumor excision. The relationship between inert and living tissues is of major relevance if we want to insure durability and utility of these materials. Regarding fracture repair, plates, bolts, nails, screws are used on a regular basis to treat fracture. But they are useless after bone healing; More difficult are joint replacement materials because they must provide service and clinical good results for the full remaining life of the patients. Stems materials are now usually made of titanium alloy; fixation could be cemented or cementless depending of the country and the habit, sliding materials are of extreme importance; metal or ceramic on polyethylene bring the problem of macrophagic reactions against polyethylene debris. While ceramic on ceramic used since more than 45 years produced a very low debris formation and experienced very good biological acceptance by the body. Fracture and noise are the most common arguments against this material, but in practice these phenomenon are very anecdotical. Advantages are clear especially in more active and young population; recently it has been shown other advantages of this material: lower infection burden, and increase stability; the occurrence of a strong and dense fibrous tissue might explain these occurrence. We must insist on the place of France in these material innovations: Judet brothers, Paul Gramont, Gilles Bousquet, Pierre Boutin, Jean Dubousset and many others did participate in these innovations of inert materials in orthopedics., ♦ Ceramic hip ♦ Ceramic knee ♦ Fibrous tissue healing

International audience; The mechanical properties of cortical bone have been extensively studied at the macrostructural scale. However, knowledge of the macroscopic mechanical properties is not sufficient to predict local phenomena, such as damage or bone remodelling, both of which are dependent on local mechanical behaviour. The objective of this study was to quantify the mechanical properties of human cortical bone inside the microstructure using micro-extensometry technique. At the scale of an osteon, the present study demonstrated that the strain was heterogeneous and its value depended strongly on the applied loading direction.
