Hepatic Encephalopathy is a major neuropsychiatric syndrome that arises from acute and chronic liver disease-induced cerebral disorders. Chronic hepatic encephalopathy is associated with cirrhosis and stems from progressive liver brosis, thereby inducing portal hypertension and deterioration in liver function. Hepatic encephalopathy is characterized by increased levels of ammonia, named hyperammonemia. Given that hepatic encephalopathy induces disturbances in cerebral osmoregulation, neurotransmission, antioxidant and energy metabolism, 1H magnetic resonance spectroscopy was performed longitudinally on a rat model of Type C chronic hepatic encephalopathy to assess cerebral osmolyte, energy, neurotransmitter and antioxidant metabolite concentrations. This technique was combined with 31P Magnetic resonance spectroscopy with the purpose of measuring additional energy metabolite concentrations. The studies were carried out at 9.4 Tesla. Rats undergone bile-duct ligation and studies were performed at several stages of disease progression: 0, 4, 6 and 8 weeks after surgery. Results regarding brain osmolyte concentration showed a signi cant increase in Gln, a decrease in tChol and Ins as well as trends of decrease in Tau and Cr. These results suggest an osmoregulatory response to the increase of Gln. In what concerns to neurotransmission, a decrease was observed in Asp and Glu suggesting that neurotransmission is a ected by hyperammonemia which may be an evidence of alterations in the out ow of Gln from astrocytes and interfere with Glu synthesis. The reduction of antioxidants Asc and GSH may indicate oxidative stress due to ammonia exposure. Small trends of decrease observed in -ATP and other energy metabolites which may be a sign of energy disturbances but not signi cant to cause brain oedema. Overall, an increase in concentration levels of Gln it is pointed as the main cause of the minimal brain oedema supported by Glutamine Hypothesis. The results of this study are encouraging and relevant for future studies.

A imagem por tensor difusão (DTI) é uma extensão da imagem por ressonância magnética que permite a caraterização microestrutural dos tecidos, sendo um destes, os músculos esqueléticos. No entanto, dadas as grandes dimensões dos mesmos, a aquisição de volumes DTI deste tipo de tecidos é comumente realizada em mais que uma sessão. A não linearidade dos gradientes de codificação espacial e a presença de magnetizações parasitas levam a que ocorram distorções que impossibilitam, quando unidos, a continuação de volumes na sua transição. No mercado já existem sotwares que permitem estas correções, no entanto, o seu objetivo tem como fim estudos populacionais aplicados na neurologia, não estando preparados para o alinhamento e união de volumes contíguos. Ao recorrer a algoritmos aplicados nestes softwares, com a devida preparação, é possível garantir a continuidade de volumes. Um desses algoritmos, demons, foi aplicado em pontos de sobreposição de imagens de volumes contíguos. A transformada resultante deste algoritmo foi aplicado ao volume considerado a transformar e unido ao volume de referência. Mostra-se ser possível a aplicação destes algoritmos para a continuação de volumes, sendo observada a preservação da coerência anatómica das fibras quando comparadas aos grupos de controlo. Numa versão futura, recomenda-se a utilização de algoritmos mais robustos, que tomam partido da informação direcional que a imagem por tensor de difusão fornece.

Unlike injury to the peripheral nervous system (PNS), where injured neurons can trigger a regenerative program that leads to axonal elongation and in some cases proper reinnervation, after injury to the central nervous system (CNS) neurons fail to produce the same response. The regenerative program includes the activation of several injury signals that will lead to the expression of genes associated with axonal regeneration. As a consequence, the spawned somatic response will ensure the supply of molecular components required for axonal elongation. The capacity of some neurons to trigger a regenerative response has led to investigate the mechanisms underlying neuronal regeneration. Thus, non-regenerative models (like injury to the CNS) and regenerative models (such as injury to the PNS) were used to understand the differences underlying those two responses to injury. To do so, the regenerative properties of dorsal root ganglion (DRG) neurons were addressed. This particular type of neurons possesses two branches, a central axon, that has a limited capacity to regenerate; and a peripheral axon, where regeneration can occur over long distances. In the first paradigm used to understand the neuronal regeneration mechanisms, we evaluated the activation of injury signals in a non-regenerative model. Injury signals include the positive injury signals, which are described as being enhancers of axonal regeneration by activating several transcription factors. The currently known positive injury signals are ERK, JNK and STAT3. To evaluate whether the lack of regeneration following injury to the central branch of DRG neurons was due to inactivation of these signals, activation of the transcription factors pELK-1, p-c-jun (downstream targets of ERK and JNK, respectively) and pSTAT3 were examined. Results have shown no impairment in the activation of these signals. As a consequence, we further proceed with evaluation of other candidates that could participate in axonal regeneration failure. By comparing the protein profiles that were triggered following either injury to the central branch of DRG neurons or injury to their peripheral branch, we were able to identify high levels of GSK3-β, ROCKII and HSP-40 after injury to the central branch of DRG neurons. While in vitro knockdown of HSP-40 in DRG neurons showed to be toxic for the cells, evaluation of pCRMP2 (a GSK3-β downstream target) and pMLC (a ROCKII downstream target), which are known to impair axonal regeneration, revealed high levels of both proteins following injury to the central branch when comparing with injury to their peripheral one. Altogether, these results suggest that activation of positive injury signals is not sufficient to elicit axonal regeneration; HSP-40 is likely to participate in the cell survival program; whereas GSK3-β and ROCKII activity may condition the regenerative capacity following injury to the nervous system.(...)

The plasma membrane is the outer limit of the animal cell. As such, it is both a border separating inside from outside and a signaling platform for interactions with the surroundings. Among these interactions are extracellular matrix contacts and adhesion sites. The membrane and its contact sites together with the underlying cytoskeleton undergo constant remodeling, which leads to changes of the cell shape. In addition to spatial information micro-topographical maps provide, information about the z-dimension and describe the position of the plasma membrane with respect to the distance to a given substrate. Here we address how to measure height differences in the plasma membrane and how to create topographical maps of the plasma membrane with nanometer resolution. We address the currently used methodologies along with their advantages and drawbacks. Moreover, we delineate a label-free method to obtain topographic maps of the plasma membrane that are corrected for differences in the refractive index of the membrane utilizing an interferometric approach with multiple wavelengths and a normalization procedure to account for changes in the refractive index in the membrane. Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica O crescimento constante do uso de tecnologia na saúde tem provocado a melhoria e eficiência dos processos de assistência à saúde, porém acresce à complexidade funcional dos sistemas de saúde. A Organização Mundial da Saúde (OMS) tem alertado quanto à necessidade de avaliar adequadamente a escolha e utilização de tecnologia em saúde. Conhecida como Avaliação de Tecnologia em Saúde – ATS (Health Technology Assessment– HTA), essa ferramenta tem sido utilizada para apoiar gestores de saúde na tomada de decisão promovendo a qualidade do processo tecnológico. Diante deste contexto, a Engenharia Clínica necessita aprimorar os seus estudos e metodologias para a gestão de tecnologias em saúde. Baseada na avaliação de evidências e informações desenvolvida para o suporte na tomada de decisão, este trabalho tem por objetivo desenvolver um processo metodológico para a avaliação de equipamentos médico-assistenciais (EMAS) durante a sua fase de utilização do ciclo de vida da tecnologia através da ferramenta de mini-ATS. A metodologia desenvolvida assenta na avaliação de quatro domínios: tecnologia, recursos humanos, infraestrutura e economia. Após todo o processo cada domínio é classificado como adequado, inadequado ou incerto.De acordo com esta classificação é realizada a recomendação final quanto à utilização do EMA em estudo. Essa ferramenta de avaliação EMA desenvolvida foi aplicada em dois casos de estudo para o equipamento de angiografia alocado em dois hospitais distintos. No primeiro caso a utilização do EMA foi considerada inadequada, devido às inconformidades respeitantes à infraestrutura, sendo que esta inadequação influenciou os demais domínios. No segundo caso a utilização foi considerada adequada de acordo com a avaliação de cada domínio. De modo geral, a metodologia desenvolvida permite à engenharia clínica tornar mais objectiva a tomada de decisão no processo tecnológico na fase de utilização dos equipamentos médico-assistenciais.

Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica Introdução & Objectivos: A elastografia é uma área da ultra-sonografia que muito tem beneficiado com os avanços tecnológicos da última década, sendo isto traduzido por uma grande inovação nesta área. A técnica ARFI surge neste panorama como uma nova técnica ultra-sonográfica de quantificação efectiva de rigidez de tecidos em comparação com as técnicas existentes que apenas permitiam a quantificação relativa. A realização deste projecto tem como objectivo avaliar a técnica ARFI na quantificação da rigidez hepática, em particular na caracterização da rigidez hepática em crianças saudáveis. Materiais & Métodos: Foi examinada a rigidez hepática de 150 crianças (81 do sexo feminino, 69 do sexo masculino) através da técnica ARFI. Para cada criança obteve-se um conjunto de velocidades de propagação. Com esta amostra avaliou-se a influência da idade, género, lobo hepático e posição da sonda. Uma amostra mais pequena foi usada para estudar a influência da profundidade, nomeadamente 135 crianças. Resultados: Não foram encontradas variações estatisticamente significativas entre géneros ou entre diferentes formas de posicionamento da sonda. No entanto o lobo hepático provou ser uma fonte de variabilidade na velocidade resultante, bem como a profundidade atingida pelo pulso de radiação. Relativamente ao factor idade foram encontradas diferenças significativas nas velocidades obtidas para crianças com idade inferior a 72 meses face aos restantes grupos etários. Conclusões: Sugere-se que a avaliação hepática na população infantil, utilizando a técnica ARFI, seja feita através da análise do lobo hepático direito a profundidades inferiores a 6 cm. Este estudo estabelece também a velocidade de propagação para crianças e adolescentes saudáveis como 1,07 ± 0,10 m/s, podendo atingir 1,12 m/s no caso de crianças mais novas. Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica Nonalcoholic fatty liver disease (NAFLD) is one of the most common chronic liver conditions in the Western world. It is normally associated with health disorders such as obesity, diabetes and hypertriglyceridemia. The gold standard for the diagnosis and staging of NAFLD is liver biopsy, which is highly invasive and is associated with high morbidity and inherent sampling error. Intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) is able to distinguish between true molecular diffusion and the pseudo-random motion of water molecules inside micro vessels. IVIM-DWI has emerged in the recent years as a possible alternative to probe tissue changes in NAFLD. However few studies have addressed the problem of the dependence of IVIM-DWI parameters on pulse sequence parameters. In order to study this dependence, two studies were carried-out: 1) A simulation study, where we studied the influence of acquisition parameters on the error and bias associated with IVIM-DWI parameters; 2) In-vivo study in order to test the performance of the b-value sequences derived from the simulation studies. Results showed that the parameter which is more affected by the acquisition parameters is D*. Furthermore, it was also shown that the higher the number of b-values used to sample the data, the better the estimation of IVIM-DWI parameters is. However, after a certain number of points and for low SNRs, the effect of noise in extra b-values counteracts the effect of having more data points. It was also shown that the b-value sequence that is used to sample the data greatly influences IVIM-DWI estimations. We concluded that the conventionally used b-value sequence does not provide optimum IVIM-DWI estimations. Furthermore, results show that different weights should be attributed to each IVIM-DWI parameter in order to obtain a better performance of the optimized b-value sequence. Also, it was seen that the influence of T2 relation effects should be accounted for in the IVIM-DWI model. Lastly, our study showed that in the presence of steatosis, the value D* significantly decreased while D only slightly decreased. However, the differences between patients with steatosis and healthy controls were extremely influenced by the number of b-values used, leading to different diagnosis depending on the number of b-values used in the acquisition.

Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica A presente dissertação de mestrado, tem como objectivo o processamento de imagens Blood Oxygen Level Dependent (BOLD), dos músculos da perna, quando submetidos a exercício intenso até ser atingida a fadiga muscular, recorrendo à técnica de processamento Independent Component Analysis (ICA). A ICA é um método estatístico utilizado para descobrir factores escondidos, procura decompor os dados em mapas espaciais independentes e em gráficos da variação no tempo, nos quais idealmente cada um representaria diferentes artefactos ou padrões de activação. Esta técnica foi aplicada com carácter exploratório no presente estudo, pretendendo-se encontrar dados relevantes, que de outro modo não seriam possíveis. As imagens BOLD utilizadas neste estudo foram adquiridas durante a realização de uma dissertação anterior a esta, realizada por Ana Rita Pereira com o tema: "Processamento de Imagens BOLD de Ressonância Magnética do Músculo". O paradigma utilizado foi o salto unipedal, executado em posição ortostática, permitindo atingir a fadiga muscular em poucos minutos. O pré-processamento das imagens foi feito com o software OsiriX e FMRIB Software Library (FSL) e o processamento foi feito com o software FSL. Os resultados permitiram verificar que a análise efectuada não permitiu separar cada componente em diferentes mapas espaciais. Verificou-se que as variáveis (músculos e fluxo sanguíneo nos vasos sanguíneos) não são independentes. Desta forma, não foi possível verificar isoladamente activação nos diferentes músculos a analisar, o gastrocnémio e trícipete sural para cada voluntário. Verificou-se uma elevada activação nos músculos flexores e extensores dos dedos dos pés e uma elevada activação decorrente do fluxo sanguíneo nos vasos sanguíneos.

Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica A presente dissertação foi desenvolvida com a colaboração da Clínica de Ressonância Magnética de Caselas e com a Faculdade de Motricidade Humana da Universidade Técnica de Lisboa (FMH-UTL) O objectivo primordial desta tese é avaliar as alterações fisiológicas de vários músculos da perna, através da análise do parâmetro T2 de IRM. Este estudo baseia-se na medição do parâmetro T2 em diferentes estágios do exercício físico, de forma a concluir quais os músculos mais recrutados durante a realização do mesmo. No final, pretende-se ainda realizar uma análise comparativa entre os resultados obtidos e as conclusões resultantes de outros estudos, elaborados em paralelo, com duas técnicas diferentes de RM: BOLD e DTI. No estudo realizado, participaram sete voluntários saudáveis e atletas. Foram adquiridas imagens ponderadas em DP e em T2 nas fases pré e pós-exercício. O exercício consistiu na realização de saltos unipedais em posição ortostática até à exaustão e à fadiga muscular. A marcação dos ROIs nos músculos de interesse (tibial anterior, solear, gémeo interno e externo) foi executada no programa OsiriX. Através da informação fornecida pelos ROIs é possível calcular o valor de T2 dos músculos, utilizando uma cápsula de óleo de fígado de bacalhau para referência de calibração. Após a análise e tratamento dos dados, observou-se, em geral, um aumento percentual de T2, da fase pré para a fase pós-exercício. Verificou-se um ganho de 6,3±3,3% no gémeo externo, de 3,6±2,5% no gémeo interno e de 4,7±2,6% no solear, em oposição ao tibial anterior que sofreu uma diminuição de 0,1±2,0%. Apesar de ser necessário um estudo mais minucioso, os resultados obtidos parecem indicar uma participação activa dos gémeos, interno e externo, e solear na realização do exercício em questão, sendo a contribuição do tibial anterior pouco evidente. Os resultados obtidos nas técnicas BOLD e DTI corroboram estas conclusões.

Dissertação para obtenção do Grau de Mestre em Engenharia Biomédica Since mercury thermometers were banned due to environmental concerns, hospitals started to use electronic thermometers for measuring body temperature. Body temperature can be measured from different body parts, although the least invasive and quickest is preferred and therefore eardrum measurements are frequently taken. However, lately the staff feels that the taken measurements are not accurate. A new purchasing agreement for the purchase of these devices renders a good opportunity to study further the use of these devices at the wards of the university hospital, study their maintenance process, identify what performance is essential for the clinical usage, the parameters that are essential to measure and also identify ear thermometers in the market that can be used for comparative study. Temperature measurements were taken with the help of an infrared ear thermometer, Covidien Genius2, in its calibration blackbody device at the R&D department of the Huddinge Hospital in order to verify accuracy claims. This data were compared against other studies and measurements of other infrared ear thermometers devices, the Braun ThermoScan Pro 4000 and also a digital contact thermometer, Welch Allyn Suretemp Plus, applied to different body sites. Informal meetings also took place in order to get more information about the devices and to know where they were used and repaired. It was found that Genius2 measured temperature accurately when compared with a blackbody radiator. Regarding the Braun, it showed an accurate estimate of core temperature in comparison to invasive pulmonary artery catheter thermometry. Electronic tympanic thermometers proved to be a good replacement for conventional methods of thermometry. However, preventive maintenance should occur more often, since the devices are very fragile. Tympanic thermometers are generally very accurate instruments. Most likely, problems are not related to the thermometers themselves, they are possibly the result of an inadequate understanding of the limitations of ear thermometry.
