Abstract

This study presents a novel methodology for stratifying post-myocardial infarction patients at risk of ventricular arrhythmias using patient-specific 3D cardiac models derived from late gadolinium enhancement cardiovascular magnetic resonance (LGE-CMR) images. The method integrates imaging and computational simulation with a simplified cellular automaton model, Arrhythmic3D, enabling rapid and accurate VA risk assessment in clinical timeframes. Applied to 51 patients, the model generated thousands of personalized simulations to evaluate arrhythmia inducibility and predict VA risk. Key findings include the identification of slow conduction channels (SCCs) within scar tissue as critical to reentrant arrhythmias and the localization of high-risk zones for potential intervention. The Arrhythmic Risk Score (ARRISK), developed from simulation results, demonstrated strong concordance with clinical outcomes and outperformed traditional imaging-based risk stratification. The methodology is fully automated, requiring minimal user intervention, and offers a promising tool for improving precision medicine in cardiac care by enhancing patient-specific arrhythmia risk assessment and guiding treatment strategies.

2. A robust shape model for blood vessels analysis

Abstract

The availability of digital twins for the cardiovascular system will enable insightful computational tools both for research and clinical practice. This, however, demands robust and well defined models and methods for the different steps involved in the process. We present a vessel coordinate system (VCS) that enables the unanbiguous definition of locations in a vessel section, by adapting the idea of cylindrical coordinates to the vessel geometry. Using the VCS model, point correspondence can be defined among different samples of a cohort, allowing data transfer, quantitative comparison, shape coregistration or population analysis. Furthermore, the VCS model allows for the generation of specific meshes (e.g. cylindrical grids, ogrids) necessary for an accurate reconstruction of the geometries used in fluid simulations. We provide the technical details for coordinates computation and discuss the assumptions taken to guarantee that they are well defined. The VCS model is tested in a series of applications. We present a robust, low dimensional, patient specific vascular model and use it to study phenotype variability analysis of the thoracic aorta within a cohort of patients. Point correspondence is exploited to build an haemodynamics atlas of the aorta for the same cohort. The atlas originates from fluid simulations (Navier-Stokes with Finite Volume Method) conducted using OpenFOAMv10. We finally present a relevant discussion on the VCS model, which covers its impact in important areas such as shape modeling and computer fluids dynamics (CFD).

Abstract

Cardiac in silico clinical trials can virtually assess the safety and efficacy of therapies using human-based modelling and simulation. These technologies can provide mechanistic explanations for clinically observed pathological behaviour. Designing virtual cohorts for in silico trials requires exploiting clinical data to capture the physiological variability in the human population. The clinical characterisation of ventricular activation and the Purkinje network is challenging, especially non-invasively. Our study aims to present a novel digital twinning pipeline that can efficiently generate and integrate Purkinje networks into human multiscale biventricular models based on subject-specific clinical 12-lead electrocardiogram and magnetic resonance recordings. Essential novel features of the pipeline are the human-based Purkinje network generation method, personalisation considering ECG R wave progression as well as QRS morphology, and translation from reduced-order Eikonal models to equivalent biophysically-detailed monodomain ones. We demonstrate ECG simulations in line with clinical data with clinical image-based multiscale models with Purkinje in four control subjects and two hypertrophic cardiomyopathy patients (simulated and clinical QRS complexes with Pearson's correlation coefficients > 0.7). Our methods also considered possible differences in the density of Purkinje myocardial junctions in the Eikonal-based inference as regional conduction velocities. These differences translated into regional coupling effects between Purkinje and myocardial models in the monodomain formulation. In summary, we demonstrate a digital twin pipeline enabling simulations yielding clinically-consistent ECGs with clinical CMR image-based biventricular multiscale models, including personalised Purkinje in healthy and cardiac disease conditions., Comment: Paper under revision

Abstract

El potencial humano es el capital más valioso en las micro y pequeñas empresas, con sus actividades desarrolladas de manera efectiva permiten el cumplimiento de las metas propuestas, para lograr estos resultados es necesario desarrollar estrategias que permitan motivarlos, para lograrlo se cuenta con el Endomarketing. La investigación está enfocada en conocer cómo la aplicación de las estrategias de endomarketing generan la motivación de los colaboradores de las micro y pequeñas empresas, en el distrito de Barranca-Perú. El enfoque aplicado fue cuantitativo, de tipo descriptivo – correlacional, se utilizó la técnica de la encuesta y como instrumento el cuestionario, la muestra estuvo conformada por 120 colaboradores de las micro y pequeñas empresas. Los resultados muestran que la aplicación del Endomarketing y la motivación es de nivel medio; asimismo, se evidencia que el desarrollo de personal y la adecuación en el trabajo, son estrategias importantes para generar que los trabajadores desarrollen su trabajo de manera intensa, y con persistencia. En conclusión, los dueños de las micro y pequeñas empresas, deben prestar atención a estas estrategias para fortalecerlas y replantear aquellas de retención de personal y comunicación interna, para fomentar la identificación con la empresa y el adecuado desempeño de los trabajadores.

Abstract

Rule-based methods are often used for assigning fiber orientation to cardiac anatomical models. However, existing methods have been developed using data mostly from the left ventricle. As a consequence, fiber information obtained from rule-based methods often does not match histological data in other areas of the heart such as the right ventricle, having a negative impact in cardiac simulations beyond the left ventricle. In this work, we present a rule-based method where fiber orientation is separately modeled in each ventricle following observations from histology. This allows to create detailed fiber orientation in specific regions such as the endocardium of the right ventricle, the interventricular septum and the outflow tracts. We also carried out electrophysiological simulations involving these structures and with different fiber configurations. In particular, we built a modelling pipeline for creating patient-specific volumetric meshes of biventricular geometries, including the outflow tracts, and subsequently simulate the electrical wavefront propagation in outflow tract ventricular arrhythmias with different origins for the ectopic focus. The resulting simulations with the proposed rule-based method showed a very good agreement with clinical parameters such as the 10 ms isochrone ratio in a cohort of nine patients suffering from this type of arrhythmia. The developed modelling pipeline confirms its potential for an in silico identification of the site of origin in outflow tract ventricular arrhythmias before clinical intervention.

Abstract

The building systems are being projected in the improvement of the efficiency of the total energy consumption and the integration of renewable energies. However, buildings have become increasingly complex because they integrate different types of local energy generation, distribution, consumption, and storage elements. That is why energy management is considered an optimization problem. Therefore, the objective of this review work was to establish a scientific map of building energy management systems (BEMS) and to identify the scientific production that identifies future research projections. In this sense, the bibliometric review methodology was used to quantify the performance, quality, and impact of the research field. The scientific mapping approach analyzes the co-occurrence of terms through clustering techniques and performance analysis to detect and visualize conceptual subdomains of BEMS. The results show five thematic areas in two evaluated periods: Wireless sensor networks (WSN) and office buildings as leading themes in the period 2000 – 2015. While photovoltaic systems (PVS) and internet of things (IoT) are identified as the important issues for the period 2016 – 2022. The most challenging issues of the BEMS is to maximize energy efficiency by minimizing energy losses and consumption. These concepts have given rise to the IoT, which is shown as a motor theme in the strategic map of bibliometric analysis. However, the thematic concept of the Internet of Energy (IoE) in energy monitoring and control in buildings through bidirectional communication between the smart grid and BEMS is appreciated. Abstract

Understanding the process of speciation, from incipient population divergence to the formation of reproductively isolated lineages, has been one the main goals of evolutionary biology. Recent advances in high-throughput DNA sequencing technologies have allowed studying the different stages of the speciation process in non-model organisms, providing affordable access to genome-wide data and information on neutral and adaptive population divergence and the genetic basis of relevant fitness traits. In order to contribute to the understanding of the speciation process, the aims of this thesis are: (1) to detect phenotypic divergence and the regions under selection that contribute to avian adaptation to insular environments by comparing the genomic landscapes of four different passerines that have colonized oceanic islands, including the red billed chough (Pyrrhocorax pyrrhocorax) and the common chaffinch (Fringilla coelebs) of La Palma in the Atlantic Ocean, and the dark-eyed junco (Junco hyemalis/insularis) and the house finch (Haemorhous mexicanus) of Guadalupe Island in the Pacific Ocean; (2) to study the mechanisms driving the diversification of the common chaffinch as it colonized different archipelagos in the Macaronesian region, and (3) to explore the genetic basis of local adaptation of the common chaffinch in La Palma, and its role in driving evolutionary divergence. The island-mainland four-species comparison shows that even though species have evolved parallel phenotypic changes upon island colonization that are consistent with the island rule, the genomic processes underlying these changes are lineage-specific. In Macaronesia, the common chaffinch has colonized the Atlantic archipelagos sequentially, starting from the continent to Azores, then Madeira and finally the Canary Islands, diverging in phenotype and genotype, and generating a species-level radiation. Abstract

[EN] Cardiovascular diseases currently have a major social and economic impact, constituting one of the leading causes of mortality and morbidity. Personalized computational models of the heart are demonstrating their usefulness both to help understand the mechanisms underlying cardiac disease, and to optimize their treatment and predict the patient's response. Within this framework, the Spanish Research Network for Cardiac Computational Modelling (VHeart-SN) has been launched. The general objective of the VHeart-SN network is the development of an integrated, modular and multiscale multiphysical computational model of the heart. This general objective is addressed through the following specific objectives: a) to integrate the different numerical methods and models taking into account the specificity of patients; b) to assist in advancing knowledge of the mechanisms associated with cardiac and vascular diseases; and c) to support the application of different personalized therapies. This article presents the current state of cardiac computational modelling and different scientific works conducted by the members of the network to gain greater understanding of the characteristics and usefulness of these models., [ES] Las enfermedades cardiovasculares tienen en la actualidad un gran impacto social y económico y constituyen una de las principales causas de mortalidad y morbilidad. Los modelos computacionales personalizados del corazón están demostrando ser útiles tanto para ayudar a comprender los mecanismos subyacentes a las patologías cardiacas como para optimizar su tratamiento prediciendo la respuesta del paciente. En este contexto, se ha puesto en marcha la Red Española de Investigación en Modelización Computacional Cardiaca (V-Heart SN). El objetivo general de V-Heart SN es el desarrollo de un modelo computacional multifísico y multiescala integrado del corazón. Este objetivo general se aborda a través de los siguientes objetivos específicos: a) integrar los diferentes modelos numéricos teniendo en cuenta la especificidad de los pacientes; b) ayudar a avanzar en el conocimiento de los mecanismos asociados a las diferentes patologías cardiacas y vasculares; y c) apoyar la aplicación de terapias personalizadas. Este artículo presenta el estado actual de la modelización computacional cardiaca y los diferentes trabajos científicos desarrollados por los miembros de la red para favorecer una mayor comprensión de las características y utilidad de los modelos.
