A need for critical evaluation of the available experimental and clinical data which could be used as guidelines to conduct future studies on scleroderma which could lead to severe dysfunction and failure of almost any internal organ are discussed. The vast research related to targeted inhibition of signaling pathways by tyrosine kinase inhibitors and the identification of biomarkers of disease severity which might pave the way toward the development of disease-specific and stage-specific targeted therapies and well-defined end points for clinical trials are also reviewed.

Abstract

Background: Mesenchymal stem cells can differentiate into endothelial cells and participate in angiogenesis in adults. In experimental models of acute myocardial infarction, mesenchymal stem cells led to the recovery of cardiac function through the formation of a new vascular network. Objective: To describe treatment with intravenous infusions of expanded autologous mesenchymal stern cells in 1 patient with critical limb ischemia due to systemic sclerosis. Design: Case report. Setting: The rheumatology unit at the University of Florence, Florence, Italy. Patient: A woman, aged 34 years, with systemic sclerosis who developed acute gangrene of the upper and lower limbs. Intervention: 3 intravenous pulses of expanded autologous mesenchymal stem cells. Measurements: Angiography, skin histopathology, and immunohistochemistry. Results: Areas of necrotic skin were reduced after the first mesenchymal stem-cell infusion. After the third infusion, angiography showed revascularization of the patient's extremities. Skin section analysis revealed cell clusters with tubelike structures, and angiogenic factors were strongly expressed. Limitation: Causality cannot be established by a single case. Conclusion: In patients with systemic sclerosis who have severe peripheral ischemia, intravenous infusion of expanded autologous mesenchymal stem cells may foster the recovery of the vascular network, restore blood flow, and reduce skin necrosis. Abstract

Capillaroscopy of the fingernail fold skin appears to be an effective tool to identify patients with scleroderma. Capillaroscopy is a microscopic imaging technique to examine small blood vessels called capillaries. Researchers compared the findings of capillaroscopy and tissue sample tests on 24 patients with scleroderma and 10 healthy volunteers (the control group). Most (87.5%) of the capillaroscopy examinations in the patients with scleroderma showed typical scleroderma patterns including enlarged blood vessels and areas of skin with few or no capillaries. Seventy percent of the patients with scleroderma had abnormal findings with all three tissue sample tests. These findings included splits in the membrane beneath the skin surface, excessive amounts of connective tissue, and abnormal amounts of the immune complexes IgG and complement C3. The one person in the control group who had abnormal vessel patterns with capillaroscopy and abnormal results with tissue sample testing later developed a connective tissue disease.
