Objective: Real-world data for patients with endometrial cancer (EC) are limited, particularly in Latin America. We present treatment pattern findings from ECHOS-A – Endometrial Cancer Health Outcomes Study in Argentina. Materials and methods: A retrospective study using clinical data from privately insured patients with EC diagnosed from 2010 to 2019. Index (diagnosis proxy) was first date of an EC-related health term or treatment. Demographics, clinical characteristics, and FIGO staging were described. Disease progression and survival were assessed until study end, loss to follow-up, or death. Results: Of 805 patients with EC, 77.4 % (n = 623/805) received any treatment and 22.6 % (n = 182/805) received none. Among those treated, 31.8 % (n = 198/623) had first-line (1L) systemic therapy, and 45.5 % (n = 90/198) proceeded to second-line (2L) therapy. Mean follow-up was 33.6 (SD 31.8) months. Of those receiving any treatment, 87.3 % (n = 544/623) had FIGO stage data (I, 62.9 %; II, 18.6 %; III, 13.6 %; IV, 5.0 %). Treatment by class in 1L and 2L, respectively, were platinum chemotherapy, 73.7 %, 36.7 %; non-platinum chemotherapy, 73.7 %, 62.2 %; immunotherapy, 1.0 %, 11.1 %; hormone therapy, 17.7 %, 26.7 %. Carboplatin/paclitaxel was the most frequent 1L (52.5 %) and 2L (14.4 %) regimen. Mean time to progression was 14.1 (SD 16.3) and 8.8 (SD 8.3) months in 1L and 2L, respectively. Adjusted 1- to 5-year risk of progression/death was 46.5–77.5 % and 65.0–86.2 % in 1L and 2L, respectively. Conclusions: Approximately one-quarter of patients with EC received no treatment, and approximately two-thirds were not treated with 1L systemic therapy. Efforts to better understand the reasons for these treatment patterns are crucial for improving patient outcomes.

This study investigates the long-term trends (1992–2022) of nitrogen and phosphorus loadings exported by the River Po to the Adriatic Sea, to better analyse how changes in hydrology are affecting the timing and magnitude of river nutrient loads. We used 30 years of monitoring data in order to (a) identify the main temporal patterns and their interactions at a decadal, annual and seasonal scale, (b) estimate precipitation effects on load formation and evaluate whether and to which extent the hydrological regime affects nutrient export across the years and (c) analyse the nutrient export regime at a monthly scale and the main transport dynamic of N and P chemical species (hydrological vs. biogeochemical control). The long-term analysis shows a general decrease of both P and N loadings, but the trends are different between the elements and their chemical species, as well as undergoing different seasonal variations. We found a statistically significant relationships between precipitation and loads, which demonstrates that precipitation patterns drive the exported load at the intra- and interannual time scales considered in this study. Precipitation-induced load trends trigger seasonal changes in nutrient deliveries to the sea, peaking in spring and autumn. The nitrogen decrease is mainly concentrated in the summer dry period, while total phosphorus diminishes mainly in spring and autumn. This mismatch of N and P results in variable molar N:P ratios within the year. The effects of extreme drought and flood events, along with the progressive decrease in the snowmelt contribution to water fluxes, are expected to exacerbate the variability in the N and P loadings, which in turn is expected to perturbate the biodiversity, food webs and trophic state of the Northern Adriatic Sea.

Summary: Glycoprotein processing along a complex highly compartmentalized pathway is a hallmark of eukaryotic cells. We followed the kinetics of intracellular, site-specific glycan processing of a model protein with five distinct N-glycosylation sites and deduced a mathematical model of the secretory pathway that describes a complex set of processing reactions localized in defined intracellular compartments such as the endoplasmic reticulum the Golgi, or the lysosome. The model was able to accommodate site-specific N-glycan processing and we identified phosphorylated glycan structures of the mannose-6-phosphate pathway responsible for the lysosomal sorting of the glycoprotein. Importantly, our model protein can take different routes of the cellular secretory pathway, resulting in an increased glycan complexity of the secreted protein.

Benthic vegetation at the land-water interface is recognized as a filter for silica fluxes, which represents an important but under-investigated subject. This paper aims to analyze stocks and fluxes of biogenic (BSi) and dissolved (DSi) silica in relation to nitrogen (N) and phosphorus (P) in the littoral zone of a deep lake. Specifically, we evaluated how different primary producers can influence BSi retention and DSi release. The study was performed from April to October in 2017, in three different benthic communities: submerged aquatic vegetation (SAV) and microphytobenthos (MPB), both occurring in soft bottom sediments, and epilithic macro- and microalgae (EA) on rocky substrates. The main result was that SAV and MPB were a DSi source and a N and P sink with the DSi efflux from SAV nearly three times as much as in MPB patches. These findings corroborate the hypothesis that SAV mediates the DSi transport from pore water to the water column. Conversely, EA communities were a DSi sink and a N and P source. Overall, these results highlight the fact that the littoral zone of lakes plays a key role in regulating aquatic Si cycling, which is likely to depend on the health status of SAV communities.

La farmacogenómica es el estudio de las variaciones genéticas que determinan una modificación de la respuesta a los fármacos. Estas variaciones se manifiestan como una diferente capacidad para el metabolismo o el transporte de fármacos, o bien como una diferente expresión de los receptores de drogas. La terapéutica farmacológica en gastroenterología ofrece distintos ejemplos de la utilidad del análisis farmacogenómico para la identificación del fármaco y la dosis más apropiada para cada paciente. El uso de inhibidores de la bomba de protones en el tratamiento de la enfermedad por reflujo gastroesofágico y en la erradicación del Helicobacter pylori puede ser optimizado mediante el análisis de polimorfismos de CYP2C19. Asimismo, el estudio de variantes de IL28 permite identificar a los pacientes con mayores chances de responder al tratamiento de la hepatitis C con interferón y ribavirina. El análisis de polimorfismos del gen codificante de la enzima tiopurina metil transferasa (TPMT) permite ayudar a reducir los riesgos del uso de azatioprina en pacientes con enfermedad inflamatoria intestinal. De este modo, la farmacogenómica se presenta como una herramienta que ya demuestra utilidad en la individualización terapéutica en gastroenterología, pero que presenta una proyección mucho mayor hacia el futuro.

BACKGROUND: Convalescent plasma is frequently administered to patients with Covid-19 and has been reported, largely on the basis of observational data, to improve clinical outcomes. Minimal data are available from adequately powered randomized, controlled trials. METHODS: We randomly assigned hospitalized adult patients with severe Covid-19 pneumonia in a 2:1 ratio to receive convalescent plasma or placebo. The primary outcome was the patient's clinical status 30 days after the intervention, as measured on a six-point ordinal scale ranging from total recovery to death. RESULTS: A total of 228 patients were assigned to receive convalescent plasma and 105 to receive placebo. The median time from the onset of symptoms to enrollment in the trial was 8 days (interquartile range, 5 to 10), and hypoxemia was the most frequent severity criterion for enrollment. The infused convalescent plasma had a median titer of 1:3200 of total SARS-CoV-2 antibodies (interquartile range, 1:800 to 1:3200]. No patients were lost to follow-up. At day 30 day, no significant difference was noted between the convalescent plasma group and the placebo group in the distribution of clinical outcomes according to the ordinal scale (odds ratio, 0.83 (95% confidence interval [CI], 0.52 to 1.35; P=0.46). Overall mortality was 10.96°/o in the convalescent plasma group and 11.43°/o in the placebo group, for a risk difference of-0.46 percentage points (95% CI, -7.8 to 6.8). Total SARS-CoV-2 antibody titers tended to be higher in the convalescent plasma group at day 2 after the intervention. Adverse events and serious adverse events were similar in the two groups. CONCLUSIONS: No significant differences were observed in clinical status or overall mortality between patients treated with convalescent plasma and those who received placebo. [ABSTRACT FROM AUTHOR]
