Objective: to address aspects related to gender dysphoria and speech therapy, by organizing and providing information about therapeutic principles. Methods: this is a literature review through research in scientific articles in the database Pubmed and Science Direct on the theme of gender dysphoria and voice, in the last 20 years. The research was conduct, by the study authors, from January 2018 to September 2021, with the keywords: (Transgender Persons) OR (Transsexualism) OR (Transsexual) OR (Gender Identity)) AND ((voice training) OR (voice) OR (voice quality)). Results: 31 scientific articles were included in the study. Conclusion: there is a need for multi-professional follow-up and ratifies the importance of speech therapy, since hormonal treatment and laryngeal surgeries do not include all the desired changes in voice and speech in transsexual women. In this way, the speech therapist, besides seeking the elevation of f0, should stimulate the feminine mannerisms, minutiae of the voice and delicacies of the speech. The importance of performing laryngeal surgery combined with speech therapy for voice modification of transgender women was verify. Objetivo: abordar aspectos relacionados con la disforia de género y logopedia, organizando y proporcionando información sobre principios terapéuticos. Métodos: se trata de una revisión de la literatura a través de la investigación en artículos científicos en las bases de datos Pubmed y Science Direct sobre el tema de la disforia de género y la voz, en los últimos 20 años. La investigación fue realizada por los autores del estudio desde enero de 2018 hasta septiembre de 2021, con las palabras clave: (personas trans) OR (transexualismo) OR (transexual) OR (identidad de género)) Y ((formación vocal) OR (voz) OR (voz calidad)). Resultados: se incluyeron 30 artículos científicos en el estudio. Conclusión: existe la necesidad de un seguimiento multidisciplinario y se ratifica la importancia de la logopedia, ya que el tratamiento hormonal y las cirugías laríngeas no incluyen todos los cambios de voz y habla deseados en las mujeres transgénero. Por lo tanto, el logopeda, además de buscar elevar f0, debe fomentar los gestos femeninos, los detalles de la voz y las delicadezas del habla. Se verificó la importancia de realizar cirugía laríngea asociada a logopedia para la modificación vocal de mujeres transgénero. Objetivo: abordar aspectos relacionados à disforia de gênero e à terapia fonoaudiológica, organizando e fornecendo informações sobre os princípios terapêuticos. Métodos: trata-se de uma revisão de literatura por meio de pesquisa em artigos científicos nas bases de dados Pubmed e Science Direct sobre o tema disforia de gênero e voz, nos últimos 20 anos. A pesquisa foi realizada, pelos autores do estudo, de janeiro de 2018 a setembro de 2021, com as palavras-chave: (Pessoas trans) OR (Transexualismo) OR (Transexual) OR (Identidade de Gênero)) AND ((treinamento vocal) OR (voz) OR (qualidade de voz)). Resultados: 31 artigos científicos foram incluídos no estudo. Conclusão: há necessidade de acompanhamento multiprofissional e ratifica-se a importância da fonoterapia, uma vez que o tratamento hormonal e as cirurgias laríngeas não contemplam todas as alterações desejadas na voz e na fala em mulheres transgênero. Dessa forma, o fonoaudiólogo, além de buscar a elevação da f0, deve estimular os maneirismos femininos, minúcias da voz e delicadezas da fala. Verificou-se a importância da realização de cirurgia laríngea associada à fonoterapia para modificação vocal de mulheres transgênero.

Introduction The understanding of normal vocal production is essential to guide any voice professional as it is fundamental to understand the effects of the posterior glottal gap on the vocal quality. Objective The aim of the present study was to verify the association between glottic closure, acoustic parameters, and some characteristics of the videolaryngostroboscopy of young women without vocal complaints nor laryngeal disorders. Methods This is a cross-sectional study with 56 women between 20 and 30 years old who underwent videolaryngostroboscopy. The acoustic parameters of the vowel /a:/ were analyzed using the Praat software, Release 4.6.10 (Paul Boersman and David Weenik, Amsterdam, Netherlands). Statistical Analysis The chi-squared, Fischer, and Kruskall-Wallis tests were applied, with 5% significance. Results Significant occurrence of posterior glottal gap (85.71%, p < 0.001), of normal vocal folds vibration amplitude (82.14%, p < 0.001), and of absence of significant constriction of the laryngeal vestibule (98.21%, p < 0.001); no significant association of the glottic closure with the vocal acoustic parameters; no significant association of glottic closure, vocal folds vibration amplitude, and constriction of the laryngeal vestibule. Conclusion There was a predominance of posterior glottal gap, normal vocal folds vibration amplitude, and absence of laryngeal vestibule constriction, and no relation with the acoustic parameters, suggesting that the posterior glottal gap did not generate impact on the vocal production of the young adult women studied.

An extreme incongruence between sex and gender identity leads individuals with gender dysphoria (GD) to seek cross-sex hormone therapy (CSHT), and gender-affirming surgery (GAS). Although few studies have investigated the effects of CSHT on the brain prior to GAS, no studies in the extant literature have evaluated its impact during hypogonadism in post-GAS individuals. Here, we aimed to evaluate the effects of estradiol on resting-state functional connectivity (rs-FC) of the sensorimotor cortex (SMC) and basal ganglia following surgical hypogonadism. Eighteen post-GAS (male-to-female) participants underwent functional magnetic resonance imaging (fMRI) and neuropsychiatric and hormonal assessment at two time points (t1, hormonal washout; t2, CSHT reintroduction). Based on the literature, the thalamus was selected as a seed, while the SMC and the dorsolateral striatum were targets for seed-based functional connectivity (sbFC). A second sbFC investigation consisted of a whole-brain voxel exploratory analysis again using the thalamus as a seed. A final complementary data-driven approach using multivoxel pattern analysis (MVPA) was conducted to identify a potential seed for further sbFC analyses. An increase in the rs-FC between the left thalamus and the left SCM/putamen followed CSHT. MVPA identified a cluster within the subcallosal cortex (SubCalC) representing the highest variation in peak activation between time points. Setting the SubCalC as a seed, whole-brain analysis showed a decoupling between the SubCalC and the medial frontal cortex during CSHT. These results indicate that CSHT with estradiol post-GAS, modulates rs-FC in regions engaged in cognitive, emotional, and sensorimotor processes.

Introduction: Transsexualism (ICD-10) is a condition characterized by a strong and persistent dissociation with one's assigned gender. Sex reassignment surgery (SRS) and hormone therapy provide a means of allowing transsexual individuals to feel more congruent with their gender and have played a major role in treatment over the past 70 years. Brain-derived neurotrophic factor (BDNF) appears to play a key role in recovery from acute surgical trauma and environmentally mediated vulnerability to psychopathology. We hypothesize that BDNF may be a biomarker of alleviation of gender incongruence suffering. Objectives: To measure preoperative and postoperative serum BDNF levels in transsexual individuals as a biomarker of alleviation of stress related to gender incongruence after SRS. Methods: Thirty-two male-to-female transsexual people who underwent both surgery and hormonal treatment were selected from our initial sample. BDNF serum levels were assessed before and after SRS with sandwich enzyme linked immunosorbent assay (ELISA). The time elapsed between the pre-SRS and post-SRS blood collections was also measured. Results: No significant difference was found in pre-SRS or post-SRS BDNF levels or with relation to the time elapsed after SRS when BDNF levels were measured. Conclusion: Alleviation of the suffering related to gender incongruence after SRS cannot be assessed by BDNF alone. Surgical solutions may not provide a quick fix for psychological distress associated with transsexualism and SRS may serve as one step toward, rather than as the conclusion of, construction of a person's gender identity. Resumo Introdução: O transexualismo (CID-10) é uma condição caracterizada por forte e persistente dissociação com o gênero atribuído. A cirurgia de redesignação sexual (CRS) e a terapia hormonal (TH) permitem que indivíduos transexuais se sintam mais congruentes com seu gênero e, por isso, têm desempenhado papel importante nos últimos 70 anos. O fator neurotrófico derivado do cérebro (BDNF) parece desempenhar um papel fundamental na recuperação do trauma cirúrgico agudo e vulnerabilidade ambiental à psicopatologia. Nós hipotetizamos que o BDNF pode ser um biomarcador de alívio do sofrimento de incongruência de gênero pós-CRS. Objetivos: Mensurar os níveis séricos de BDNF no pré e pós-operatório em indivíduos transexuais como biomarcador de alívio de estresse relacionado à incongruência de gênero após a CRS. Métodos: Trinta e duas pessoas transexuais masculino para feminino submetidas a cirurgia e tratamento hormonal foram selecionadas de nossa amostra inicial. O nível sérico de BDNF foi avaliado antes e depois da CRS pela técnica ELISA. O tempo decorrido entre as coletas de sangue pré e pós-CRS foi medido. Resultados: Não houve diferença significativa nos níveis de BDNF pré e pós-CRS ou em relação ao tempo decorrido entre a CRS e a coleta. Conclusão: O alívio do sofrimento relacionado à incongruência de gênero pós-CRS não pode ser avaliado apenas pelo BDNF. Soluções cirúrgicas podem não fornecer uma solução rápida para o sofrimento associado ao transexualismo, e a CRS pode servir como um passo em direção à, em vez de conclusão da, construção da identidade de gênero de uma pessoa.

RESUMO Buscou-se verificar os efeitos de um programa fonoterapêutico que incluiu orientação vocal e postural, adequação da função respiratória e a técnica de sons nasais em disfonias hiperfuncionais. Foi realizado um estudo de casos clínicos, observacional, longitudinal, não controlado, de caráter quantitativo que analisou três sujeitos do sexo feminino com idade média de 31,33 anos que apresentavam disfonias hiperfuncionais. Os sujeitos foram submetidos a: videolaringoscopia, avaliação vocal perceptivoauditiva e acústica, coleta de tempos máximos de fonação, triagem postural e determinação do tipo respiratório durante a fala, antes e após um programa terapêutico composto por orientação, conscientização e treinamento vocal com sons nasais durante 16 sessões de fonoterapia, uma vez na semana com treinamento em domicílio. Os dados foram analisados por meio dos testes não-paramétricos Mann-Whitney e Qui-quadrado, com nível de significância de 5%. Pós-terapia, observou-se que a postura corporal passou de desalinhada para alinhada e o tipo respiratório de superior para costodiafragmático abdominal; houve diminuição das medidas acústicas em relação ao grau e número de subharmônicos na maioria significante dos sujeitos, além de melhoras teciduais e diminuição do edema na mucosa das pregas vocais e na região aritenoide, e melhora da coaptação glótica. Após a execução de um programa fonoterapêutico com orientação vocal e postural, adequação da função respiratória e uso da técnica de sons nasais em disfonias hiperfuncionais, observou-se melhora significante da postura corporal, do tipo respiratório, das medidas acústicas sugestivas de ruído à emissão vocal, e efeitos positivos sobre o tecido e o fechamento das pregas vocais.

ABSTRACT We sought to verify the effects of a phonotherapy program that included vocal and postural orientation, adequacy of respiratory function and the technique of nasal sounds in hyperfunctional dysphonia. We carried out a clinical case, observational, longitudinal, non-controlled study with quantitative approach that analyzed three female subjects with mean age of 31.33 years who presented hyperfunctional dysphonia. The subjects were submitted to: laryngoscopy, vocal, perceptive-auditory and acoustic assessments, collection of maximum phonation time, postural screening and determination of the respiratory tract during speech before and after a therapeutic program which consisted of orientation, awareness and vocal training with nasal sounds during 16 speech therapy sessions, once a week with training at home. The data were analyzed by using the non-parametric Mann-Whitney and Chi-square, with 5% significance level. Post-therapy, we observed that the posture passed from aligned and misaligned and the respiratory type from the upper to the costodiaphragmatic-abdominal; there was a decrease of the acoustic measurements in relation to the degree and to the number of subharmonics in most subjects, besides tissue improvements and reduction of edema in the mucosa of the vocal folds and arytenoid region, as well as glottal closure improvement. After performing the phonotherapy program with vocal and postural orientation, adequacy of respiratory function and use of the technique of nasal sounds in hyperfunctional dysphonia, we observed significant improvement in body posture, the respiratory tract, the acoustic measurements suggestive of noise to vocal emission and positive effects on the tissue and the closure of the vocal folds.
