Glucosamine and chondroitin sulphate are used to treat osteoarthritis, and the multicenter, double-blind, placebo- and celecoxib-controlled Glucosamine/chondroitin Arthiritis Intervention Trial (GAIT) evaluated their efficacy and safety as a treatment for knee pain from osteoarthritis. Exploratory analyzes suggest that the combination of glucosamine and chondroitin sulphate may be effective in the subgroup of patients with moderate-to-severe knee pain.

The most common crystal-related arthropathies--gout, calcium pyrophosphate dihydrate disease or 'pseudogout,' and calcific periarthritis/tendinitis--may be appropriately diagnosed and managed by the primary care physician. Definitive diagnosis via synovial tap is recommended, as the clinical picture may not identify some cases. The acute pain and swelling of attacks, regardless of etiology, generally respond to treatment with nonsteroidal anti-inflammatory drugs and local or occasionally systemic corticosteroids. Once a causative crystal has been identified and a diagnosis established, a plan for long-term management and prevention of recurrences may be devised. Thus, uric-acid-lowering therapy may be indicated in a patient who has experienced recurrent attacks of gout, whereas control of serum phosphate levels might be effective in some individuals with hyperphosphatemia and hydroxyapatite-associated periarthritis or arthritis. Crystal deposits in joints can be destructive as well as painful. Treatment, therefore, has two objectives: To relieve the pain of the acute attack, thus restoring normal function, and to prevent the accumulation of crystals that can lead to degenerative disease. Identification and subsequent treatment of preventable or correctable underlying disorders may be one of the most gratifying aspects of managing crystal-induced arthropathies.

Ehlers-Danlos syndrome (EDS) is an inherited disorder of connective tissue (skin, bone, cartilage, and similar structural tissues) that involves mutations of collagen (the body's major structural protein) and features joint hypermobility, skin hyperelasticity and fragility, and a variety of other problems. One rare and unusual type of EDS, type VIII EDS, is characterized by gum disease and premature loss of teeth. A case of EDS with these features is described; the patient also had bone problems, vasculitis (inflammation of blood vessels), and heart valve disease. The patient developed more or less normally until puberty, except for premature loosening of permanent teeth. Soon after the start of menstruation, widespread hives with underlying small-vessel vasculitis developed. Arthritis in multiple joints then developed, and steroids failed to heal the skin problems. Vasculitis remained confined to the skin, however. Over a 10-year period, since age 15, joint dislocations, erosive arthritis, and severe osteolysis (bone destruction) occurred, as have classic signs involving skin and joints. Insufficiency of several heart valves occurred, with hypertrophy (overgrowth) of heart chambers slowly developing. Modern laboratory techniques were used to analyze the patient's collagen. Two subtypes of type I collagen were present in both a normal and abnormal form, and the abnormal type apparently could not be secreted efficiently by cells for use in structural tissues. The patient had T lymphocytes (immune cells important in fighting foreign organisms) which reacted with the type I collagen; collagen is usually inert to the immune system and a protein made by the T cells which reacted with the collagen was detected. 10. Pseudoseptic arthritis: an unusual presentation of neuropathic arthropathy

Septic arthritis is an arthritic condition that arises from infection of synovial (joint) tissues by bacteria or other agents. A case is described of a 49-year-old man who had a condition that mimicked septic arthritis, but was caused by neuropathic arthropathy, a joint disorder in which nerve damage and loss of sensation lead to overuse and repetitive joint injury. The patient had syringomyelia, a fluid-filled cavity within the spinal cord, which contributed to tingling and numbness in the hand, numbness and weakness in the arm, and muscle atrophy. Neuropathic arthropathy occurs in up to 30 percent of patients with syringomyelia, most often involving the shoulder. Diagnosis of the joint problem was delayed by seven months, as the shoulder swelling, although massive, was painless. Many details of this patient's case are typical. Severe destruction of the shoulder developed rapidly, within one month. Neuropathic arthropathy involving the shoulder is thought to involve nerve loss, which then leads to decreased blood flow to bone, and bone resorption (loss of calcium and bone structure). The case is unusual, however, regarding the high level of inflammation and high numbers of white blood cells that were present in the synovial fluid. Bacterial infections were ruled out. This type of inflammatory phase may be a more common feature of early neuropathic arthropathy than realized. Alternatively, it might have been provoked by high synovial fluid levels of fat particles, possibly generated from red blood cells or the breakdown of bone marrow. 11. Effect of medication on synovial fluid leukocyte differentials in patients with rheumatoid arthritis

Levels of leukocytes (white blood cells) in synovial (joint) fluid increase in most types of arthritis, and the type of cell can help to determine the extent of inflammatory disease that may be present. Numbers of leukocytes can vary, from an average of 15,000 per cubic millimeter of fluid (mm3) to as high as 100,000/mm3 in some cases of rheumatoid arthritis (RA). In addition, the type of leukocyte is important; higher levels of polymorphonuclear neutrophils (PMNs) are often found in cases of severe acute arthritis. The differences in levels of leukocytes and particularly in levels of lymphocytes (immune cells important in defending against infection and which may mediate tissue destruction in arthritis) were studied in relation to medication taken by 45 patients with RA. The patients had detectable joint effusions (extrusion of fluid, suggestive of continued arthritis in spite of medication), necessitating fluid removal, so that synovial fluid was thus available for analysis. Of 22 patients taking nonsteroidal anti-inflammatory drugs (NSAIDs, which decrease inflammation but do not affect the underlying disease process), 17 had a predominance of lymphocytes in synovial fluid. The proportion and total levels of lymphocytes were significantly lower in patients taking a second-line (disease-modifying) drug such as methotrexate (MTX), gold, or hydroxychloroquine (HCQ). Total leukocyte counts were significantly higher in patients taking MTX or gold than those taking HCQ plus NSAIDs. Among these latter patients, five had taken only NSAIDs at one point and had predominantly lymphocytes in synovial fluid. An overview is presented of the types of foreign matter that can cause inflammation within joints together with their diagnosis, how they cause disease, and treatments. Crystals may be deposited within the joint, the most common of which is urate crystals, the cause of gout. Treatment is available to dissolve urate crystals and the crystal deposits that sometimes occur with kidney dialysis but not other types of crystals. Joint inflammation resulting from infection may be difficult to diagnose with without biopsy of the joint membrane and specialized laboratory techniques. The most common entry is via the circulation. Pathogens may cause inflammation by infecting cells, producing toxins, or triggering an immune response. Antibiotics and drainage usually effect cure, although some pathogens are recalcitrant. Artificial joints can introduce foreign bodies through wear. Materials should be used that do not create this problem.

Yoga stretching and relaxation exercises may effectively relieve symptoms of carpal tunnel syndrome. Repetitive motion in the hands and wrist can cause chronic pain. Researchers assigned 42 patients to attend twice-weekly sessions of Iyengar hatha yoga, or wear a wrist splint. The yoga exercises stretched and flexed the joints in the upper body, followed by a period of relaxation. After eight weeks, yoga participants had improved grip strength, and less wrist pain. Patients who simply wore a splint showed no improvement in their symptoms of carpal tunnel syndrome.

Objective: To describe the current pharmacological approach to gout treatment reported by rheumatologists in Brazil., Methods: We performed a cross-sectional survey study using an online questionnaire e-mailed to 395 rheumatologists, randomly selected, from among the members of the Brazilian Society of Rheumatology., Results: Three hundred and nine rheumatologists (78.2%) responded to the survey. For acute gout attacks, combination therapy (NSAIDs or steroid + colchicine) was often used, even in monoarticular involvement, and colchicine was commonly started as monotherapy after 36 hours or more from onset of attack. During an acute attack, urate-lowering therapy (ULT) was withdrawn by approximately a third of rheumatologists. Anti-inflammatory prophylaxis (98% colchicine) was initiated when ULT was started in most cases (92.4%), but its duration was varied. Most (70%) respondents considered the target serum uric acid level to be less than 6 mg/dl. Approximately 50% of rheumatologists reported starting allopurinol at doses of 100 mg daily or less and 42% reported the initial dose to be 300 mg daily in patients with normal renal function. ULT was maintained indefinitely in 76% of gout patients with tophi whereas in gout patients without tophi its use was kept indefinitely in 39.6%., Conclusion: This is the first study evaluating gout treatment in a representative, random sample of Brazilian rheumatologists describing common treatment practices among these specialists. We identified several gaps in reported gout management, mainly concerning the use of colchicine and ULT and the duration of anti-inflammatory prophylaxis and ULT. Since rheumatologists are considered as opinion leaders in this disease, a program for improving quality of care for gout patients should focus on increasing their knowledge in this common disease.

- Abstract
Identifying persons with early rheumatoid arthritis (RA) is a major challenge. The role of the Internet in making decisions about seeking care has not been studied. We developed a method for early diagnosis and referral using the Arthritis Foundation's website. A person with less than 3 months of joint pain symptom who has not yet sought medical attention was screened. Prescreened persons are linked to a self-scoring questionnaire and get a "likelihood" of RA statement. If "likely," the person is offered a free evaluation and biomarker testing performed by Quest Diagnostics. The system available only to Massachusetts's residents yielded a small steady flow of screen-positive individuals. Over 21 months, 43,244 persons took the Arthritis Foundation website prescreening questionnaire; 196 were from Massachusetts and 60 took the self-scoring algorithm. Of the 48 who screened positive, 29 set up an appointment for a free evaluation, but six never came in. Twenty-four subjects were evaluated and diagnosed independently by three rheumatologists. One met the 1987 American College of Rheumatology (ACR) criteria for RA and two met the 2010 ACR/EULAR RA criteria. The 24 examined individuals were contacted at a minimum of 1 year and asked to redo the case-finding questionnaire and asked about their health resource utilization during the interval. Seventeen of the 24 subjects responded, and 10 had seen a health professional. Three of the 17 had a diagnosis of RA; all were on at least methotrexate. Internet case finding was useful in identifying new potential RA cases. The system's performance characteristics are theoretically limited only by the number of study sites available. However, the major barrier may be that seeing a health professional is not a priority for many individuals with early symptoms.

Objectives: Traditionally, differences in absolute numbers of cells expressing a certain marker (e.g., positive staining cells per mm²) have been used in immunohistological synovial tissue classification. We have begun to evaluate the relative composition of the inflammatory infiltrates, i.e. percentages of inflammatory cell types in inflammatory infiltrates, as an alternate classification tool that may potentially improve tissue diagnostics, subgrouping in clinical trials, and understanding of pathogenesis of inflammatory and noninflammatory arthropathies., Methods: Synovial tissue specimens (normal synovium, n=15; orthopedic arthropathies, n=6; osteoarthritis, n=26; early undifferentiated arthritis, n=10; rheumatoid arthritis, n=26; chronic septic arthritis, n=11) were stained for CD15, CD68, CD3, CD20, and CD38. Densities of cells expressing a given marker were determined in the superficial subintima. Binary and multicategory receiver operating characteristic (ROC) analysis and naïve Bayes classifier were used to compare the abilities of (1) the absolute densities of cells expressing a given marker (absolute method) with (2) the percentages of these cells in the inflammatory cell population (relative method) to differentiate among the six tissue classes., Results: The inflammatory infiltrates in normal synovium and the orthopedic arthropathies consisted almost exclusively of CD68+ and CD3+ cells. Notable fractions of CD20+ and CD38+ cells appeared in a subset of osteoarthritis samples, and increased further in early, rheumatoid and chronic septic arthritis. ROC analyses and naïve Bayes classifier ranked the absolute method above the relative method in terms of overall discriminatory ability. The relative method became slightly superior when the samples were also stratified according to the total number of inflammatory cells/mm²., Conclusions: This exploratory investigation featuring a variety of joint disorders revealed that measuring the relative proportions of inflammatory cell types may aid in synovial tissue classification if the samples are also stratified according to the intensity of inflammation.

Patient education is an important aspect of gout management, but there is evidence that many patients lack adequate knowledge of their condition. Our aim was to examine the characteristics of gout patient education resources. Ten gout patient information resources were examined for readability (Flesch-Kincaid reading level, the Simple Measure of Gobbledygook measure and the Flesch Reading Ease Score), qualitative characteristics such as figure and jargon use and whether they included information on the major points of gout. The median readability grade level of the examined resources was 8.5. The difference in readability grade level between the highest and the lowest education resource was 6.3 grade levels. The information content of the resources was high with an average of only 3.9 proposed criteria of 19 (19 %) absent from the resources. Jargon use was low and concepts were usually explained. However, important information regarding acute flare prophylaxis during urate-lowering therapy initiation and titration and treating serum uric acid to target was absent from 60 % of the patient education resources. There was poor use of key messages at the start. Gout patient resources have a wide range of readability. Thirty percent of resources were above the average reading level of rheumatology outpatients reported in previous studies. Sixty percent of gout patient resources omit education items that could impact on patient adherence and in turn patient outcomes. Further research is needed into the literacy levels and education requirements of patients with gout.
